Epithelial abrasion precipitates stromal ulceration in the vitamin A--deficient rat cornea.

Although the role of vitamin A deficiency in the development of xerophthalmia is well established, there is still some question as to whether the deficiency alone is sufficient cause for the development of keratomalacia. This article describes the clinical, histologic, and microbiologic changes occurring in eyes of vitamin A-deficient rats when keratomalacia-like stromal ulceration is induced by epithelial injury alone. The corneal epithelia of 21 severely vitamin A-deficient rats and 11 pair-fed controls were totally removed either by scraping or by n-heptanol. At 96 hr after epithelial remov

Nutritional factors in corneal xerophthalmia and keratomalacia.

Six measures of nutritional status were studied in 162 consecutive cases of presumed nutritional keratopathy ranging from mild xerosis through full-thickness necrosis (keratomalacia) and in a variety of control subjects. The severity of corneal involvement was related to the severity of wasting, prevalence of edema, and serum levels of albumin, transferrin, and vitamin A. Even the most severe corneal alterations were compatible with normal indices of protein and anthropometric status, but not with normal serum vitamin A levels. Analysis suggests that interaction between vitamin A and protein s

Sequelae of severe xerophthalmia--a follow-up study.

Two-hundred sixteen children with vitamin A deficiency were hospitalized in Hyderabad, India, between 1970 and 1977. Out of these, 22 died during hospitalization. Fifty-six children could be followed-up, of whom 32 had keratomalacia and 24 corneal xerosis. In the keratomalacia group, nine died of reasons attributable to severe protein-energy malnutrition within 3 to 4 months after discharge from the hospital, five became totally blind, nine monocularily blind, and nine had adequate vision, although four of these had leucomas. Of the blind children, no change in condition was noted in the 11 lo

The effect of thermal burns on the release of collagenase from corneas of vitamin A--deficient and controls rats.

A mild trauma in the form of a thermal burn was applied to corneas of vitamin A--deficient rats and their pair-fed controls. The control corneas routinely showed rapid re-epithelialization without stromal changes. The corneas of deficient rats recovered more slowly, frequently exhibiting stromal edema, leukoma, and sometimes ulceration. Because collagenase is thought to initiate collagen destruction in corneal ulceration, the relationships among vitamin A status, severity of trauma, and collagenase levels were determine. Mild thermal burns were found to cause corneas from less severely deficie

Serum retinol-binding protein and vitamin A levels in malnourished children.

Serum levels of vitamin A and retinol-binding protein (RBP) were measured in children with vitamin A deficiency, in children with protein-energy malnutrition (PEM) and in normal children, before and after administration of 100 000 IU of water-miscible vitamin A. Serum vitamin A and RBP levels were significantly low in children with vitamin A deficiency and in children with severe PEM, whereas the values in milder grades of PEM were similar to those of normal subjects. In severely malnourished children with corneal lesions, serum vitamin A concentration was reduced to a much greater extent than

[Keratomalacia in children with malnutrition].

Twenty-nine malnourished preschoolers in third grade, with a history of globally deficient feeding and very poor individual and environmental hygiene, who presented ocular lesions attributed to avitaminosis A, mainly keratomalacia, were studied. Fifteen patients had keratomalacia with superficial corneal ulceration and fourteen had severe complications such as corneal perforation, descemetocele, expulsion of ocular contents and leukomas adherent to the rest of the eye structures, with final amaurosis. The pathophysiological alterations caused by vitamin A deficiency in ocular structures are sy

[Xerophthalmia].

Xerophthalmia results from an insufficient supply of vitamin A to the eye. Primary signs of xerophthalmia are conjunctival xerosis, Bitôt's spots with conjunctival xerosis, corneal xerosis, corneal ulceration with xerosis, keratomalacia. Secondary signs of xerophthalmia are night blindness, xerophthalmia fundus and corneal scars. According to the WHO 20 000--100 000 children in developing countries are struck annually with blindness due to xerophthalmia. With regard to the treatment immediate therapy with 100 000 I.U. vitamin A intramuscularly and 200 000 I.U. orally on the following day are a

Hypovitaminosis A and its control.

Hypovitaminosis A is considered to be the most common cause of blindness in the developing countries but it is not possible to estimate the prevalence of keratomalacia directly attributable to it. Subclinical hypovitaminosis A is not measurable at present in human subjects, but studies in animals indicate that the possibility of subclinical effects should not be ignored. The recommended procedure for identifying the "at risk" population involves a three-part survey to evaluate dietary intake, biochemical indices, and clinical signs. This article examines all three approaches in some detail, bu

Keratomalacia in the cachectic hospitalized patient.

Keratomalacia occurred in two cachectic hospitalized patients, each with severe vitamin A-protein deficiency secondary to a lethal disease. Prompt therapy with parenteral and topical vitamin A, protein supplementation, and a soft contact lens restored corneal integrity and prevented visual loss in one patient. Keratinization of the conjunctiva did not occur in either patient. Patients with severe protein deficiency may develop keratomalacia in the absence of a severely decreased serum level of vitamin A.

Keratomalacia.

Xerophthalmia and keratomalacia are public health problems of great magnitude which are usually associated with multiple vitamin and protein deficiencies. The authors report the case of a 27-year-old commune member who subjected herself to a bizarre protein and vitamin deficient diet for many months. This ultimately produced nyctalopia, xerophthalmia and keratomalacia with bilateral corneal perforation. Despite therapy, she remained comatose and expired shortly after admission. Ocular pathological changes included bilateral corneal melting with prolapse of intraocular contents, conjunctival ep

Vitamin A nutritional status of rural community of Khol block in Haryana, North India. A collaborative study.

A nutritional and diet survey was carried out in 2,192 persons belonging to all age groups in a rural community. Vitamin A consumption was less than the recommended daily allowance in persons of all age groups. Bitot's spots and night blindness were detected in 7.1% and 7.2% of the population, respectively. Ocular signs of vitamin A deficiency were more frequently observed in age groups above 5 years as compared to the preschool children. There was no association of vitamin A deficiency with different grades of protein-calorie malnutrition in preschool children. Kerătomalacia was extremely rar

Xerophthalmia and anterior-segment blindness among preschool-age children in El Salvador.

A countrywide prevalence survey of 9,508 children was conducted in El Salvador to determine the prevalence of anterior-segment abnormalities and magnitude of clinical vitamin A deficiency. Thirty-six children had corneal opacities, 56% of them secondary to trauma. Such traumatic corneal opacities were 19 times more frequent among urban than rural children, 57.6 vs. 3.1 per 1,000. Keratomalacia accounted for only 8% of all corneal opacities, and for one (and possibly two) of the five cases of bilateral anterior-segment blindness encountered. The prevalance of Bitot spots and vitamin-A-related c

Mass distribution of vitamin A and the prevention of keratomalacia.

A retrospective review of 3,490 adminissions to the major pediatric facility in El Salvador was undertaken to determine whether biannual administration of massive doses of vitamin A (200,000 international units) to all availabel 1- to 4-year-old children was effective in preventing keratomalacia. During the 12 months preceding and following initiation of the program, the number of children admitted with presumed vitamin-A-related corneal destruction (33 vs. 31) and proportion of all malnourished admissions with such destruction (26 vs. 25 per 1,000) were similar. The seasonal distribution of t
