Vitamin A supplementation during pregnancy for maternal and newborn outcomes.

BACKGROUND: The World Health Organization recommends routine vitamin A supplementation during pregnancy or lactation in areas with endemic vitamin A deficiency (where night blindness occurs), based on the expectation that supplementation will improve maternal and newborn outcomes including mortality, morbidity and prevention of anaemia or infection. OBJECTIVES: To review the effects of supplementation of vitamin A, or one of its derivatives, during pregnancy, alone or in combination with other vitamins and micronutrients, on maternal and newborn clinical outcomes. SEARCH STRATEGY: We searche

Assessment of vitamin A status of preschool children in a sub-Saharan African setting: comparative advantage of modified relative-dose response test.

A nationally-representative sample of 2,696 preschool children living in Congo was examined during Au gust-September 2003 to determine the rates of vitamin A deficiency. Ninety clusters of 30 children, aged six months to six years, were selected, using a randomized two-level cluster-sampling method. Vitamin A deficiency was determined by assessing the prevalence of active xerophthalmia (nightblindness and/or Bitot spots) in the cross-over sample of 2,696 individuals. A semi-quantitative seven-day dietary questionnaire was concurrently applied to the mothers of children enrolled to estimate the

Incidence of low vitamin A levels and ocular symptoms after Roux-en-Y gastric bypass.

BACKGROUND: Previous reports have demonstrated a significant incidence of fat-soluble vitamin deficiency after bariatric surgery. The purpose of the present study was to determine the incidence of vitamin A deficiency after Roux-en-Y gastric bypass and to correlate the laboratory findings with ocular symptoms potentially related to vitamin A deficiency. METHODS: All patients who had undergone Roux-en-Y gastric bypass were invited to participate in a nutritional screening. The patients completed a detailed survey concerning ocular symptoms and had their vitamin A level evaluated. RESULTS: A l

Chapter 29: historical aspects of the major neurological vitamin deficiency disorders: overview and fat-soluble vitamin A.

The vitamine doctrine: Although diseases resulting from vitamin deficiencies have been known for millennia, such disorders were generally attributed to toxic or infectious causes until the "vitamin doctrine" was developed in the early 20th century. In the late-19th century, a physiologically complete diet was believed to require only sufficient proteins, carbohydrates, fats, inorganic salts, and water. From 1880-1912, Lunin, Pekelharing, and Hopkins found that animals fed purified mixtures of known food components failed to grow or even lost weight and died, unless the diet was supplemented wi

Ocular malformation in a newborn secondary to maternal hypovitaminosis A.

We report a case of microphthalmia, inferior adherent leukoma, and optic nerve hypoplasia in an infant whose mother underwent biliopancreatic diversion surgery for obesity 7 years before his birth. The pregnancy was complicated by severe, maternal hypovitaminosis A despite oral supplementation. The infant was found to have undetectable serum vitamin A levels in the perinatal period. At 8 weeks of age, the infant underwent sector iridectomies. At 9 months of age, electroretinography suggested rod dysfunction. His visual performance is poor.

Indonesian women of childbearing age are at greater risk of clinical vitamin A deficiency in families that spend more on rice and less on fruits/vegetables and animal-based foods.

Clinical vitamin A deficiency is characterized by night blindness and greater morbidity and mortality. The aim of this study was to examine the relationships between household food expenditures and night blindness among nonpregnant women of childbearing age among families in the slums of Jakarta, Indonesia. In a cross-sectional study of 42 974 households in the Indonesian Nutrition Surveillance System, 1998 to 2003, night blindness was assessed in nonpregnant women. Food expenditures were divided into 5 major categories as follows: plant-based foods (fruits and vegetables), animal-based foods,

Fundus white spots and acquired night blindness due to vitamin A deficiency.

To report a successfully treated case of acquired night blindness associated with fundus white spots secondary to vitamin A deficiency. An ocular examination, electrophysiologic testing, as well as visual field and OCT examinations were obtained on a 61-year-old man with vitamin A deficiency who had previously undergone gastric bypass surgery. The patient had a re-evaluation after treatment with high doses of oral vitamin A. The patient was observed to have numerous white spots in the retina of each eye. Best-corrected visual acuity was initially 20/80 in each eye, which improved to 20/40-1 OU

Vitamin A deficiency (VAD) after a duodenal switch procedure: a case report.

Vitamin A deficiency (VAD) after bariatric surgery is a well-documented complication. Clinical signs of VAD deficiency are significant if the vitamin A level is below 100 mug/L (4 mumol/L). Main symptom of VAD is nyctalopia. It has been described in several reports after malabsorptive bariatric procedures. We report a 52-year-old man with VAD complained of night blindness and optic neuropathy after undergoing a duodenal switch procedure. The case illustrates the necessity of lifelong vitamin supplementation after malabsorptive bariatric operations.

Vitamin A intake and status in populations facing economic stress.

Dietary quality and diversity reflect adequacy of vitamin A. Both can deteriorate in response to economic crises. Although the nutritional consequences of the 2008 world food price crisis remain unclear, past studies of diet, status, and socioeconomic standing under usual (deprived) and unusually disruptive times suggest dietary quality and vitamin A status decline in mothers and young children. This is presumably the result of shifting diets to include less preformed vitamin A-rich animal source foods and, to a lesser extent, vegetables and fruits. Cross-sectional assessments of diet, deficie

Prevalence and determinants of xerophthalmia in preschool children in urban slums, Pune, India--a preliminary assessment.

PURPOSE: International and national programs to control vitamin A deficiency disorders (VADD) among children in developing countries are reducing the magnitude but VADD still occurs in deprived populations. The purpose of the study was to estimate the prevalence of and identify risk factors for xerophthalmia in children aged 6-71 months living in slums in Pune, India in 2003. METHODS: Children were enrolled into a cross sectional study from randomly selected Anganwadis (kindergartens) in selected slums using proportional to size sampling. Parents/caregivers were interviewed by social workers

Serum retinol binding protein as an indicator of vitamin A status in cirrhotic patients with night blindness.

BACKGROUND/AIM: Vitamin A deficiency is known to be associated with night blindness. Plasma retinol binding protein (RBP) estimation highly correlates with plasma retinol concentration to predict vitamin A status. Serum RBP estimation is reasonably simple, inexpensive, and highly applicable in less technologically developed settings. We studied the correlation of plasma vitamin A levels (by RBP estimation) and ocular manifestation in patients with liver cirrhosis. MATERIALS AND METHODS: This prospective, cohort study included 137 patients with liver cirrhosis. Ocular manifestations in these p

Multilevel modeling of geographically distributed vitamin A deficiency.

Vitamin A deficiency is a common health problem in developing countries like India. Present study involves data on children aged between 6-36 months from northern part of India collected geographically, to find prevalence and important factors for risk of nightblindness. Both traditional logistic models and multilevel logistic models were applied to achieve our aim. All Individual level variables vitamin A diet intake, age, vitamin A capsule intake and awareness about vitamin A were found significant for risk of night-blindness (p < 0.05) in individual level analysis. The effect of risk factor

Zinc therapy for night blindness in cystic fibrosis.

This is the first report of a supplemented CF patient presenting with clinical vitamin A deficiency to be successfully treated with zinc therapy alone. Therefore in addition to retinol supplementation, normalizing serum zinc levels may be important in maintaining the vitamin A status of CF patients. The interactions and synergistic effects between the two micronutrients are discussed.

Maternal night blindness during pregnancy is associated with low birthweight, morbidity, and poor growth in South India.

Maternal night blindness is common during pregnancy in many developing countries. Previous studies have demonstrated important consequences of maternal night blindness during pregnancy on the health of the mother and newborn infant. We compared birthweight, 6-mo infant mortality, morbidity, and growth among infants of women who did and did not report a history of night blindness from a community-based, randomized trial of newborn vitamin A supplementation in south India. Birthweight was measured within 72 h of delivery. Infants were followed until 6 mo of age for mortality and morbidity was as

[Night blindness and xerophthalmia after surgery for morbid obesity].

CASE REPORT: We describe ocular findings due to a vitamin A deficiency in a 50-year-old man. The patient had undergone intestinal bypass surgery two years before. After therapy with oral vitamin A the symptoms improved. DISCUSSION: The incidence of morbid obesity is increasing throughout much of the developed world, with intestinal bypass surgery the treatment of choice for most people with the condition. This type of surgery can lead to a vitamin A deficiency, with remarkable ophthalmological consequences, which without correct treatment may ultimately cause blindness. For this reason, the p
