Heart failure in Somalia: a scoping review of the epidemiology, etiology, clinical characteristics, management, outcomes, and evidence gaps.

BACKGROUND: Heart failure is an important cause of morbidity, hospitalisation, disability, and mortality, but evidence from Somalia remains fragmented and largely confined to individual hospital-based studies. This scoping review mapped the available evidence on the epidemiology, aetiologies, clinical and diagnostic characteristics, management, outcomes, and research gaps relating to heart failure in Somalia. METHODS: PubMed/MEDLINE, Scopus, and Embase were searched from inception to 27 July 2026. Backward citation searching was conducted on 3 August 2026. Original studies reporting heart-fai

Functional thiamine deficiency in patients with chronic kidney disease in Northeast Thailand.

BACKGROUND: Patients with chronic kidney disease (CKD) may be vulnerable to thiamine insufficiency due to dietary restrictions, altered metabolism, and renal replacement therapy. We aimed to determine the prevalence of functional thiamine deficiency, its distribution across CKD stages, and the associated clinical factors in a Thai population. AREAS COVERED: Thiamine status was assessed in red blood cells using the erythrocyte transketolase activity coefficient (ETKAC) assay. Participants (n = 605) were categorized as having normal thiamine status (ETKAC ≤1.25) or functional thiamine deficienc

Non-alcoholic Wernicke encephalopathy following pancreaticoduodenectomy (Whipple procedure): a case report and review of the literature.

INTRODUCTION AND IMPORTANCE: Wernicke encephalopathy (WE) is an acute neurological emergency caused by thiamine deficiency. It is uncommon after pancreaticoduodenectomy and may be overlooked when postoperative metabolic disturbances coexist. PRESENTATION OF CASE: A 66-year-old woman underwent pancreaticoduodenectomy for a duodenal neoplasm. Her postoperative course was complicated by pancreatic fistula, extended-spectrum beta-lactamase-producing Escherichia coli infection, impaired oral intake, and prolonged nutritional support. During the third to fourth postoperative weeks, she developed fl

Neuropathic symptoms after sleeve gastrectomy: two cases illustrating diagnostic pitfalls and discordant nerve conduction findings - a case report.

INTRODUCTION: Peripheral neuropathy is a recognized but often underappreciated complication after sleeve gastrectomy, particularly in patients with rapid weight loss, recurrent vomiting, poor oral intake, and inadequate adherence to postoperative supplementation. PRESENTATION OF CASE: We describe two young women who developed neuropathic symptoms within 3-4 months of laparoscopic sleeve gastrectomy. Both experienced substantial early weight loss, poor oral intake, recurrent vomiting or regurgitation, and poor adherence to prescribed supplements. Serum vitamin B12 and thiamine levels were elev

Addressing micronutrient deficiencies and vitamin metabolism disorders in pediatric pulmonary hypertension: a strategic proposal for low- and middle-income countries.

BACKGROUND: Pediatric pulmonary hypertension (PH) is traditionally managed under a strict hemodynamic paradigm, often overlooking systemic metabolic and nutritional drivers. This review introduces the "Cellular Deprivation Model" to evaluate how specific micronutrient deficiencies and metabolic blocks accelerate pulmonary vasculopathy. METHODS: A systematic, PRISMA-compliant search was conducted in PubMed/MEDLINE for literature published between January 2000 and April 2026. Keywords intersected pediatric PH with malnutrition, avitaminosis, iron deficiency, and inborn errors of metabolism. Out

The Ataxias as an Example of Rare Diseases: Diagnosis, Genetics, and Treatment.

BACKGROUND: Ataxias are a group of rare diseases. In the European Union, a disease is defined as rare if its prevalence is less than 5 per 10 000 people. Ataxias are rarer than this threshold, although reliable prevalence estimates are largely lacking. The worldwide mean prevalence of autosomal dominant hereditary ataxias is approximately 2.7 per 100 000 people (95% confidence interval [1.5; 4.0/100 000]), and that of autosomal-recessive ataxias is approximately 3.3 per 100 000 people [1.8; 4.9/100 000]. Ataxias include hereditary (genetic), sporadic degenerative, and acquired types. METHODS:

Beyond Wilson Disease: a comprehensive review of the Non-Wilsonian Causes and differentiating imaging features of the "Face of the Giant Panda" sign.

The "Face of the Giant Panda" sign is a characteristic magnetic resonance imaging (MRI) appearance classically associated with Wilson disease. Although traditionally considered highly suggestive of Wilson disease, increasing evidence shows that this pattern is not pathognomonic and may occur across a wide range of neurological disorders. This review provides a comprehensive account of non-Wilsonian causes of the giant panda sign, with emphasis on pathophysiological mechanisms, characteristic imaging features, and, critically, the additional MRI findings beyond the panda configuration itself th

Acute Bilateral Visual Loss as an Atypical Manifestation of Wernicke Encephalopathy Secondary to Hyperemesis Gravidarum: A Case Report and Narrative Literature Review.

Wernicke encephalopathy (WE) is a severe neurological condition caused by thiamine deficiency that remains underreported in pregnancy. We report a clinical diagnosis of WE in a 41-year-old woman, gravida 2, para 1, at 16 weeks of gestation. She presented with acute, painless visual impairment, severe dietary restriction, and marked weight loss secondary to severe hyperemesis gravidarum (HG). Examination revealed reduced visual acuity, horizontal nystagmus, psychomotor retardation, and an ataxic gait. The initial diagnosis was delayed due to her atypical presentation and the absence of MRI or b

Wernicke's encephalopathy after laparoscopic sleeve gastrectomy.

Bariatric surgery is one of the causes of Wernicke's encephalopathy. It is more frequent in techniques with malabsorption and less common in surgeries such as sleeve gastrectomy. We present the case of a 48-year-old patient with a body mass index of 55 who, after a sleeve gastrectomy with cholecystectomy, suffered an episode of prolonged vomiting and was hospitalised with confusion, amnesia and limb weakness associated with low thiamine levels. After a diagnosis based on clinical manifestations and low blood thiamine levels, treatment with intravenous thiamine was started with an adequate evol

Association of metabolic and bariatric surgery and risk of cognitive impairment among U.S. adults.

BACKGROUND: Evidence suggests metabolic health plays a significant role in long-term cognitive outcomes. Whether this association stems from weight loss operations or other factors, remains controversial. Although a link between metabolic and bariatric surgery (MBS) and reduced likelihood of cognitive impairment has also been reported, a large-scale analysis of this association is lacking. OBJECTIVE: To evaluate the association of prior metabolic surgery and the odds of cognitive impairment. SETTING: Academic, university-affiliated; US. METHODS: All adult (≥18 years) hospitalizations with a

Wernicke's Encephalopathy Following Pregnancy Loss: A Case Report and Literature Review.

A 40-year-old female with no history of alcohol use presented with altered mental status and generalized weakness ten days after dilation and curettage for missed abortion, complicated by persistent vomiting and electrolyte imbalances. Examination revealed confusion and disorientation, without nystagmus, ophthalmoplegia, or ataxia. Brain MRI showed characteristic symmetrical periventricular signal abnormalities consistent with Wernicke's encephalopathy. She was treated with intravenous thiamine (500 mg three times daily for five days), followed by oral supplementation, with gradual resolution

Genome-wide gene by sleepiness interaction analysis for sleep apnea.

STUDY OBJECTIVES: Excessive daytime sleepiness (EDS), influenced by environmental and social-behavioral factors, is reported by a subset of patients with sleep apnea-a group that may be at elevated cardiovascular risk. However, it is unclear whether sleep apnea with and without EDS have distinct genetic underpinnings. In this study, we perform gene-by-EDS interaction analyses for apnea hypopnea index, a diagnostic marker of sleep apnea severity, to understand EDS's influence on its underlying genetic risk. METHODS: Discovery interaction analyses for common variants and gene-based rare variant

Post-traumatic stress disorder impairs CD8+ T cell-dependent anti-tumor immune responses by downregulating thiamine.

Post-traumatic stress disorder (PTSD) is a severe and chronic psychiatric condition. However, whether PTSD influences tumor immunity remains poorly understood. In this study, we demonstrated that PTSD promotes the growth of subcutaneous transplant tumors in mice, an effect that can be reversed by anti-CD8 antibody treatment. Furthermore, we found that PTSD downregulates serum thiamine levels, which mediates the tumor-promoting effects observed in PTSD mice. In vitro, thiamine enhances CD8+ T cell activation and cytotoxic function. Mechanistically, thiamine augments tricarboxylic acid (TCA) cyc

Astrocytic TPK1 mitigates amyloid pathology via TFEB-mediated endocytosis.

Alzheimer's disease (AD), the leading cause of dementia, is characterized by amyloid-beta (Aβ) plaques, neurofibrillary tangles, and progressive neurodegeneration. Deregulation of glial cell activity plays an important role in the amyloid pathology. However, it is still unclear how changes in astrocytes contribute to Aβ deposition and clearance in AD. Here, we showed that deficiency of astrocytic thiamine pyrophosphokinase 1 (Tpk1), exacerbated Aβ burden leading to exacerbated spatial memory deficits in a mouse model of AD. While selective overexpression of Tpk1 in astrocytes ameliorated cogni
