Recurrent anorexia because of gastrointestinal beriberi in a home care patient.

An elderly man in his 80s receiving home medical care presented with recurrent loss of appetite. Although the physical examination was normal, thiamine deficiency was suspected based on his history of chronic alcohol use, prior unexplained anorexia, and ongoing cancer treatment. Oral thiamine supplementation resulted in marked improvement within days. Subsequent testing confirmed thiamine deficiency, leading to the diagnosis of gastrointestinal beriberi. This case underscores the importance of considering gastrointestinal beriberi in patients with nonspecific gastrointestinal symptoms in the h

Cardiac Manifestations of Nutritional Deficiencies.

Nutritional deficiencies remain an important yet underrecognized cause of cardiovascular morbidity worldwide. Our review examines how deficiencies in key micronutrients and macronutrients lead to distinct cardiac manifestations, ranging from structural alterations to functional impairments. We begin with water-soluble vitamins, highlighting classic manifestations like thiamin deficiency and wet beriberi, later pyridoxine, folate, cobalamin, and ascorbic acid. Next, we discuss fat-soluble vitamins A, D, E, and K. We also address the role of minerals, including calcium, iodine, magnesium, potass

Subclinical thiamine deficiency is associated with traditional dietary practices among lactating women: A community-based cross-sectional study.

BackgroundThiamine deficiency is an important cause of adverse maternal and neonatal outcomes. It is especially common among women who follow customary dietary restrictions.AimTo estimate the prevalence of subclinical thiamine deficiency among lactating mothers following customary dietary restrictions and to compare it with age-matched controls.MethodA community-based cross-sectional study, conducted on healthy lactating mothers. Study subjects consisted of two groups namely "Exposure group" (mothers on customary dietary restrictions and "Control group" (mothers consuming more diverse diet). R

Overestimation of Aortic Stenosis in Thiamine Deficiency: Two Cases With Distinct Underlying Conditions.

The severity of aortic stenosis (AS) can be overestimated in high-flow states, and thiamine deficiency is an underrecognized cause of high-output circulatory states. We report two elderly females who developed thiamine deficiency-related high-output circulatory states, initially leading to an overestimation of AS severity. Case 1 was assessed as severe but was downgraded to moderate after thiamine replacement, and case 2 was initially assessed as moderate but was reclassified as mild after treatment. Case 1 had a history of gastrectomy, predisposing her to thiamine deficiency due to limited po

Clinical and Etiopathological Perspective of Vitamin B1 Hypersensitivity and an Example of a Desensitization Protocol.

Vitamin B1 (thiamine) is a water-soluble B vitamin. As a cofactor of many enzymes, it is essential for the proper functioning of many body systems and organs, including metabolic and energy metabolism. In extreme cases, vitamin B1 deficiency causes neurodegenerative disorders, including beri-beri, or cognitive impairment resulting from encephalopathy. B1 avitaminosis may result from increased demand, dietary errors, malabsorption, or excessive loss. Thiamine supplementation is used in cases of vitamin B1 deficiency or for preventative measures in situations of increased demand. Vitamin B1 can

Nutritional Myeloneuropathy Secondary to Thiamine Deficiency: A Case Report.

Thiamine (vitamin B1) deficiency can occur secondary to malnutrition, which is associated with multiple etiologies, from imbalanced dietary habits to significant gastrointestinal nutritional losses such as recurrent vomiting or diarrhea. Thiamine deficiency manifests as dry or wet beriberi, Wernicke encephalopathy, or Korsakoff syndrome. We present a 26-year-old lady, three months post-laparoscopic sleeve gastrectomy with a complicated postoperative course due to poor oral intake and recurrent vomiting, who presented with bilateral limb weakness and vision loss of five days duration. Physical

Wernicke Encephalopathy Associated with Crohn's Disease.

Poorly controlled Crohn's disease is associated with several complications, including malabsorption of vitamins and minerals such as vitamin D, vitamin B12, and vitamin B1 (thiamine). Thiamine, among other vitamins, plays a crucial role in Adenosine Triphosphate formation as well as nerve propagation. Thiamine deficiency initially manifests as wet or dry beriberi but may lead to the devastating Wernicke-Korsakoff syndrome. In this report, we present a case of full-blown Wernicke encephalopathy associated with uncontrolled Crohn's disease. An 18-year-old young man was admitted with a chief comp

An Overview of Beriberi.

<p>Beriberi is a nutritional disorder caused by thiamine deficiency. Classically, Beriberi presents in two primary clinical forms: "wet" Beriberi, which features heart and circulatory system impairment, and "dry" Beriberi, which causes polyneuropathy. Although it is an easily treatable condition, it is often misdiagnosed and can be life-threatening if not promptly recognized and managed. The diagnosis of Beriberi is performed by the signs and symptoms of the disease and can be confirmed by thiamine deficiency identification or by therapeutic testing. However, considering the costs and the limi

Cardiac Beriberi After Gastrectomy Presenting With Peripheral Neuropathy and Mimicking Tachycardia-Induced Cardiomyopathy: A Case Report.

Cardiac beriberi, a manifestation of thiamine deficiency, is an uncommon but reversible cause of heart failure that can mimic other forms of cardiomyopathy, such as dilated or tachycardia-induced cardiomyopathies. It is likely underrecognized or misdiagnosed. We report an elderly man with neurological symptoms and globally reduced left ventricular (LV) function in the context of prior gastrectomy, chronic alcohol consumption, and prolonged malnutrition. Early empiric thiamine administration led to rapid improvement in pulse rate, left ventricular function, and neurological symptoms, and the di

Gastrointestinal Beriberi Presenting With Chronic Diarrhea: A Case Report.

Gastrointestinal beriberi is an underrecognized yet important manifestation of thiamine deficiency that may occur even in individuals without a history of chronic alcohol use. It often presents with nonspecific upper gastrointestinal symptoms, leading to diagnostic delay. The case of a woman in her 60s with a history of distal gastrectomy who developed persistent anorexia and intractable diarrhea over four months, resulting in multiple hospitalizations, is presented. Despite comprehensive evaluations, including blood tests, CT scans, stool analyses, endoscopy, and biopsies, no definitive cause

Clinical Features and Outcomes of Shoshin Beriberi.

Shoshin beriberi is a fulminant form of wet beriberi, but there are no large-scale studies detailing the clinical features of this disease. We investigated the clinical features and outcomes of Shoshin beriberi using data from a nationwide database in Japan.Using the Diagnosis Procedure Combination database, we identified patients with Shoshin beriberi between July 2010 and March 2021. We retrospectively investigated the characteristics, comorbidities, treatment, and in-hospital mortality of patients with Shoshin beriberi. The chi-square test or Fisher's exact test was used for categorical var

From Collapse to Recovery: Thiamine Intervention in Cardiac Beriberi.

This case report details the challenging presentation of a 68-year-old patient of cardio-circulatory collapse with pericardial effusion and recurrent pleural effusions. Hypotension did not respond to conventional intensive care measures. Despite comprehensive investigations, the underlying cause remained unidentified until intravenous thiamine replacement therapy was administered, restoring normotension and preventing pericardial or pleural effusion recurrence. The case underscores the importance of early recognition of thiamine deficiency in patients with pericardial and pleural effusions or

Thiamine deficiency as a cause of anorexia in terminally ill cancer patients: A case of possible gastrointestinal beriberi.

OBJECTIVES: Anorexia in cancer patients has a variety of causes and impairs patients' quality of life. However, there have been few reports of thiamine deficiency (TD) playing a role in anorexia during palliative care. The objective of this report was to describe such a case. METHODS: An 82-year-old woman with advanced cervical cancer was admitted for palliative care because of progressive disease. One month after admission, she developed myoclonus on the left side, difficulty speaking, slurred speech, and drooping of the left corner of the mouth, with no consciousness disturbance. She also d
