INTRODUCTION AND IMPORTANCE: Wernicke encephalopathy (WE) is an acute neurological emergency caused by thiamine deficiency. It is uncommon after pancreaticoduodenectomy and may be overlooked when postoperative metabolic disturbances coexist.
PRESENTATION OF CASE: A 66-year-old woman underwent pancreaticoduodenectomy for a duodenal neoplasm. Her postoperative course was complicated by pancreatic fistula, extended-spectrum beta-lactamase-producing Escherichia coli infection, impaired oral intake, and prolonged nutritional support. During the third to fourth postoperative weeks, she developed fl

BACKGROUND: Ataxias are a group of rare diseases. In the European Union, a disease is defined as rare if its prevalence is less than 5 per 10 000 people. Ataxias are rarer than this threshold, although reliable prevalence estimates are largely lacking. The worldwide mean prevalence of autosomal dominant hereditary ataxias is approximately 2.7 per 100 000 people (95% confidence interval [1.5; 4.0/100 000]), and that of autosomal-recessive ataxias is approximately 3.3 per 100 000 people [1.8; 4.9/100 000]. Ataxias include hereditary (genetic), sporadic degenerative, and acquired types.

The "Face of the Giant Panda" sign is a characteristic magnetic resonance imaging (MRI) appearance classically associated with Wilson disease. Although traditionally considered highly suggestive of Wilson disease, increasing evidence shows that this pattern is not pathognomonic and may occur across a wide range of neurological disorders. This review provides a comprehensive account of non-Wilsonian causes of the giant panda sign, with emphasis on pathophysiological mechanisms, characteristic imaging features, and, critically, the additional MRI findings beyond the panda configuration itself th

Wernicke encephalopathy (WE) is a severe neurological condition caused by thiamine deficiency that remains underreported in pregnancy. We report a clinical diagnosis of WE in a 41-year-old woman, gravida 2, para 1, at 16 weeks of gestation. She presented with acute, painless visual impairment, severe dietary restriction, and marked weight loss secondary to severe hyperemesis gravidarum (HG). Examination revealed reduced visual acuity, horizontal nystagmus, psychomotor retardation, and an ataxic gait. The initial diagnosis was delayed due to her atypical presentation and the absence of MRI or b

Bariatric surgery is one of the causes of Wernicke's encephalopathy. It is more frequent in techniques with malabsorption and less common in surgeries such as sleeve gastrectomy. We present the case of a 48-year-old patient with a body mass index of 55 who, after a sleeve gastrectomy with cholecystectomy, suffered an episode of prolonged vomiting and was hospitalised with confusion, amnesia and limb weakness associated with low thiamine levels. After a diagnosis based on clinical manifestations and low blood thiamine levels, treatment with intravenous thiamine was started with an adequate evol

BACKGROUND: Evidence suggests metabolic health plays a significant role in long-term cognitive outcomes. Whether this association stems from weight loss operations or other factors, remains controversial. Although a link between metabolic and bariatric surgery (MBS) and reduced likelihood of cognitive impairment has also been reported, a large-scale analysis of this association is lacking.
OBJECTIVE: To evaluate the association of prior metabolic surgery and the odds of cognitive impairment.
SETTING: Academic, university-affiliated; US.
METHODS: All adult (≥18 years) hospitalizations with a

A 40-year-old female with no history of alcohol use presented with altered mental status and generalized weakness ten days after dilation and curettage for missed abortion, complicated by persistent vomiting and electrolyte imbalances. Examination revealed confusion and disorientation, without nystagmus, ophthalmoplegia, or ataxia. Brain MRI showed characteristic symmetrical periventricular signal abnormalities consistent with Wernicke's encephalopathy. She was treated with intravenous thiamine (500 mg three times daily for five days), followed by oral supplementation, with gradual resolution

INTRODUCTION: Different observational studies have analyzed the effects of glucagon-like peptide-1 receptor agonists (GLP-1 RAs) on the risk of ocular adverse events (AEs). The present meta-analysis aimed to assess the effects of GLP-1 RAs on the risk of ocular AEs, including retinopathy, glaucoma, and non-arteritic anterior ischemic optic neuropathy (NAION).
METHODS: A systematic review and meta-analysis of observational studies was conducted in PubMed, Embase, and Web of Science from 2006 to 2025. Studies involving individuals diagnosed with diabetes and/or obesity and overweight, receiving

Thiamine, or vitamin B1, is a water-soluble vitamin necessary for multiple metabolic processes throughout the human body. Thiamine is primarily obtained from our diet and is found in sources such as whole grains, legumes, and pork. Short bowel syndrome, severe malnutrition, alcohol use disorder, or diuretic use can all lead to thiamine deficiency. Thiamine deficiency may contribute to significant morbidity if not promptly identified and treated. Thiamine supplementation has been established as the primary treatment for patients with Wernicke's encephalopathy. Supplementation of thiamine is als

Wernicke's encephalopathy (WE) is a neurological emergency caused by thiamine deficiency and is a recognised stroke mimic. While prior reports often describe subacute presentations, we present a uniquely instructive case characterised by abrupt neurological collapse, with a rapid decline in Glasgow Coma Scale from 14 to 3 within 48 hours, closely simulating catastrophic cerebrovascular disease. Initial computed tomography was unremarkable; however, magnetic resonance imaging demonstrated classic, symmetrical involvement of the medial thalami, periaqueductal grey matter, tectal plate, mammillar

BACKGROUND: Wernicke's encephalopathy (WE) is an acute neurologic emergency resulting from thiamine deficiency that can cause irreversible injury if treatment is delayed. Although often associated with alcohol use, WE also occurs in the setting of malnutrition, dysphagia, chronic illness, and malignancy. Dysphagia-related WE is rare and typically linked to impaired oral intake. We report a case of a 26-year-old woman with 3 months of dysphagia and subsequent poor oral intake who developed WE.
CASE REPORT: A 26-year-old woman with type 2 diabetes and hypothyroidism presented for confusion, spe

Hyperemesis gravidarum (HG) is a severe and potentially life-threatening form of nausea and vomiting in pregnancy, characterized by intractable vomiting, dehydration, electrolyte imbalance, nutritional deficiencies, and significant weight loss. Despite being a leading cause of early pregnancy hospitalization, its management remains inconsistent and often empirical. Emerging evidence suggests a paradigm shift in the understanding of HG pathophysiology, moving beyond traditional hormonal theories toward an integrated model involving genetic susceptibility and metabolic signaling pathways, partic

Wernicke's encephalopathy (WE) is an acute neurological disorder precipitated by thiamine deficiency, most commonly associated with chronic alcohol misuse. WE is underdiagnosed and can lead to significant neurological morbidity if treatment is delayed. We report the case of a 35-year-old man with alcohol dependence and a history of seizures who was admitted for the management of status epilepticus requiring intubation and intensive care. During his sedation, his regular thiamine supplementation was inadvertently withheld for four days. Following extubation and seizure stabilization, the patien

Wernicke's Encephalopathy, although classically associated with chronic alcoholism, can also occur during pregnancy as a complication of hyperemesis gravidarum and is often underdiagnosed. We report a case of a 34-year-old pregnant woman at 16 weeks of gestation who presented with prolonged vomiting, altered mental status, nystagmus, and gait ataxia. Magnetic Resonance Imaging of the brain revealed characteristic bilateral thalamic and periaqueductal hyperintensities. Pregnancy increases thiamine requirements, and persistent vomiting in hyperemesis gravidarum can rapidly precipitate thiamine d

Wernicke's encephalopathy (WE) is a life-threatening neuropsychiatric syndrome resulting from thiamine deficiency. Alcohol misuse is the main cause. However, non-alcoholic cases, including those associated with hyperemesis gravidarum and cancer, have also been reported. We herein report a case of WE in a patient with G4 stage, non-dialysis-dependent chronic kidney disease (CKD). The patient strongly believed that an extremely-low-protein diet would prevent the development of CKD. His estimated daily thiamine intake (0.37 mg) was less than one-third of the recommended level (1.3 mg). This case

INTRODUCTION: Pregnancy is a metabolically vulnerable state in which electrolyte disturbances, nutritional deficiencies, and hypoxic events can rapidly lead to severe neurological complications. Wernicke's encephalopathy and hypoxic-ischemic brain injury are uncommon but potentially devastating conditions that are frequently underdiagnosed, particularly in resource-limited intensive care settings. To our knowledge, the coexistence of profound hypokalemia, Wernicke's encephalopathy, and hypoxic-ischemic brain injury during pregnancy has been rarely reported, particularly from resource-limited i
