Wernicke's Encephalopathy in a Young Non-alcoholic Woman Secondary to Prolonged Reduced Oral Intake: A Case Report.

Wernicke's encephalopathy (WE) is an acute neurological disorder resulting from thiamine deficiency and is classically associated with chronic alcohol misuse. However, it may also occur in non-alcoholic patients with prolonged malnutrition, persistent vomiting, or impaired nutritional intake, making diagnosis particularly challenging when the classical triad of ophthalmoplegia, ataxia, and altered mental status is incomplete or develops gradually. Delayed recognition can result in irreversible neurological injury, whereas early treatment with thiamine is associated with promising outcomes. We

Case Report: a case of prolonged post-traumatic duodenal obstruction complicated by traumatic pancreatitis and Wernicke's encephalopathy: a multidisciplinary challenge.

BACKGROUND: Post-traumatic gastrointestinal injuries, especially those involving the duodenum and proximal jejunum, present significant management challenges. Complications such as retroperitoneal abscess, high intestinal obstruction, and secondary pancreatitis can lead to prolonged fasting and severe nutritional deficiencies. We report a complex case where a patient developed a rare triad of post-operative combined abscess (retroperitoneal abscess and intra-abdominal abscess)-induced high intestinal obstruction, traumatic pancreatitis, and subsequent Wernicke's encephalopathy (WE), highlighti

Atypical MRI presentation of symmetrical thalamic and basal ganglia lesions in adult-onset MOG-IgG-associated disease: a case report.

Myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) is a rare autoimmune neurological condition characterized by clinical phenotypes that include acute disseminated encephalomyelitis, optic neuritis, and transverse myelitis. Neuroimaging features differ markedly between adults and children, and brain lesions are relatively uncommon in adult MOGAD patients. Atypical imaging patterns can complicate the differentiation of MOGAD from other diseases. We report a 21-year-old female presenting with dysarthria, gait deviation, blurred vision, and slow response. Brain MRI revealed m

miR-320d Is Associated with Reduced Nasopharyngeal Carcinoma Progression, Potentially through the NF-κB/IL-8 Axis-Mediated Inhibition of Neutrophil Extracellular Trap Formation.

Objectives: Nasopharyngeal carcinoma (NPC) is an aggressive head and neck malignancy in which post-treatment recurrence and distant metastasis remain major contributors to poor clinical outcomes. Although microRNAs are important post-transcriptional regulators of tumor progression, the role of miR-320d in NPC remains incompletely understood. This study evaluated the biological role of miR-320d and explored whether it is involved in regulating neutrophil extracellular trap (NET) formation through the nuclear factor kappa-B (NF-κB)/interleukin-8 (IL-8) signaling axis. Methods: miR-320d was overe

Wernicke's encephalopathy unmasking progressive supranuclear palsy and Alzheimer's pathology: a diagnostic challenge in an older adult.

This case highlights the rare coexistence of Wernicke's encephalopathy, probable progressive supranuclear palsy and probable Alzheimer's disease. The patient presented with acute confusion, repeated falls, postural instability, bradyphrenia, apathy and vertical gaze palsy. Clinical examination raised suspicion of overlapping neurological syndromes. Complementary investigations included brain MRI which revealed periventricular hyperintense regions on T2 and on fluid-attenuated inversion recovery (FLAIR) sequences, suggestive of Wernicke's encephalopathy. A subsequent ioflupane (123I) single-pho

[Wernicke Encephalopathy In Refeeding Syndrome].

This report describes a patient with a history of recurrent depressive disorder and restrictive anorexia nervosa. In the context of a depressive episode, increased appetite led to refeeding syndrome with consecutive Wernicke encephalopathy following previous food deprivation due to anorexia. The pathophysiology is based on increased thiamine consumption during a sudden transition to an anabolic metabolic state. Wernicke's encephalopathy should be considered even in cases of nonspecific symptoms associated with any type of malnutrition, especially since the encephalopathy can be triggered by re

Alcohol-Related Metabolic Emergencies.

Patients with alcohol use disorders are commonly identified and managed in the emergency department. Although the alcohol-intoxicated patient has a high risk for significant injury and diseases, the majority will be allowed to sober in the emergency department and can be discharged without incident. However, there are metabolic derangements in these patients, such as alcoholic ketoacidosis, Wernicke-Korsakoff, and potomania that very commonly present similar to intoxication and can be misdiagnosed by emergency clinicians.

Thiamine for altered mental status: a high-value strategy beyond alcohol.

BACKGROUND: Altered mental status (AMS) prompts rapid diagnostic evaluation in hospitalized patients, yet a low-cost, reversible cause - thiamine deficiency - often receives inconsistent attention outside alcohol-associated contexts. Although classically linked to alcohol misuse, contemporary evidence links thiamine depletion to a broad range of medical, surgical, oncologic, obstetric, and critically ill populations. DISCUSSION: In these settings, AMS may be the earliest clinical manifestation, while standard diagnostic tools such as MRI and serum thiamine levels frequently lack sensitivity e

Wernicke Encephalopathy Associated with Malabsorption in Degos Disease.

A 40-year-old female with systemic Degos disease presented with 4 days of confusion, somnolence, and ataxia. She was dependent on total parenteral nutrition (TPN) following ileocecectomy for Degos-related gastrointestinal complications and had recently transitioned from intravenous (IV) to oral thiamine supplementation. Although central nervous system involvement of Degos was initially suspected, magnetic resonance imaging findings, low thiamine levels, and rapid improvement with IV thiamine confirmed Wernicke's encephalopathy. This case highlights the importance of recognizing nutritional def

Increasing Hospitalizations for Wernicke Encephalopathy in Spain: A Nationwide Population-Based Study.

Background/Objectives: Wernicke encephalopathy (WE) is an acute neurological syndrome caused by severe thiamine deficiency. Early detection is challenging due to the low sensitivity of the classic triad. Methods: This retrospective observational study used the Spanish Minimum Basic Data Set, including hospital admissions with a primary diagnosis of WE (2016-2022). Demographic, clinical, and economic variables were also analyzed. Severity of illness (SOI) and risk of mortality (ROM) were assessed using the All Patient Refined Diagnosis-Related Groups (APR-DRG) system. Results: A total of 2477 W

Nonalcoholic Wernicke Encephalopathy Following Prolonged Intermittent Fasting: A Case Report.

Wernicke encephalopathy (WE) is an acute and potentially reversible neurological disorder caused by thiamine deficiency and is most commonly associated with chronic alcohol use. However, it can also occur in nonalcoholic individuals with nutritional deprivation, where diagnosis is often delayed because of low clinical suspicion. We report a case of WE occurring in the setting of prolonged intermittent fasting for weight loss. Wernicke Encephalopathy Complicating a Distinctive POLG Phenotype With MNGIE-Like Features.

BACKGROUND: Mitochondrial neurogastrointestinal encephalomyopathy (MNGIE) is an extremely rare autosomal recessive disease caused by variants in the thymidine phosphorylase gene (TYMP), primarily characterized by severe gastrointestinal and neurological symptoms. The complete phenotype of MNGIE has not been linked to any gene other than TYMP. METHODS: We describe two identical twins who exhibited delayed psychomotor development, infantile bilateral cataract, congenital demyelinating polyneuropathy, and severe progressive gastrointestinal dysmotility with recurrent pseudo-obstruction episodes,

A Case of Wernicke's Encephalopathy in the Second Trimester of a Woman With Super-Super Obesity and a Monochorionic Diamniotic Twin Pregnancy.

Super super obesity increases perinatal risk, and monochorionic diamniotic twin gestation is uncommon. Although Wernicke's encephalopathy is linked to hyperemesis gravidarum in underweight women, severe obesity does not preclude micronutrient depletion. A woman with body mass index 65 kg/m2 carrying monochorionic diamniotic twins developed Wernicke's encephalopathy at 25 weeks after prolonged poor diet with restrictions and persistent nausea and vomiting. Severe obesity may create a false impression of nutritional adequacy. Furthermore, twin pregnancy increases metabolic demand and complicates

Intractable Nausea as the Initial Presentation of Neuromyelitis Optica.

Neuromyelitis optica spectrum disorder (NMOSD) is an autoimmune condition of the central nervous system, primarily involving the optic nerves and spinal cord. Patients typically present with visual disturbance, limb weakness or sensory deficits, and bladder dysfunction. However, atypical and nonspecific presentations may occur, including persistent nausea and vomiting due to involvement of the area postrema. We report the case of a 26-year-old female patient who presented with a one-month history of intractable vomiting, associated with epigastric discomfort (worse prior to vomiting), reduced

Psychiatric-Predominant Wernicke Encephalopathy With Normal Blood Thiamine Levels: A Case Report.

Wernicke encephalopathy (WE), caused by thiamine deficiency in patients with chronic alcohol use, may progress to Korsakoff syndrome if untreated. Diagnosis is challenging and often missed because the classic triad can be incomplete and blood thiamine levels can be normal. A 56-year-old woman with 40-60 g of pure alcohol consumption daily since her 20s and a previous psychiatric hospitalization for alcohol dependence at the age of 50 years developed hallucinations, delusions, agitation, and cognitive decline. One month later, she fractured her left femoral neck at home, for which she underwent

Astrocytopathy in Wernicke Encephalopathy and Neuromyelitis Optica Spectrum Disorder. Pathogenic Differences With Occasional Clinical and Neuroimaging Overlap. A Review.

PURPOSE OF THE REVIEW: There is an increasing number of single case reports describing patients with findings compatible with thiamine deficiency, in whom rapid replacement of thiamine fails to induce improvement. Further work-up identifies positive aquaporin-4 antibodies (AQP-4) leading to a final diagnosis of neuromyelitis optica spectrum disorder (NMOSD). Occasionally, NMOSD is the initial diagnosis, but it turns out that the patient has thiamine deficiency. Similarly, these two diagnoses may overlap, particularly with area postrema lesions and in other cases of protracted vomiting. RECENT

The endoplasmic reticulum autophagy receptor TEX264 drives epidermal differentiation and is dysregulated in Darier disease.

Differentiating keratinocytes break down their organelles and nuclei to become the compacted cornified layers of the epidermal barrier in a poorly understood catabolic process. Live confocal imaging of stratified human organotypic epidermis revealed endoplasmic reticulum (ER) fragmentation and lysosomal engulfment in the cornifying layers, where we found up-regulation of TEX264, a receptor that mediates selective autophagy of the ER (reticulophagy). TEX264 expression was increased by ER stress, which caused precocious cornification of organotypic epidermis. In undifferentiated keratinocytes, e
