Wernicke's encephalopathy (WE) is a neurological disorder caused by thiamine deficiency. This condition is characterized by impaired consciousness, eye movement disorders, and gait ataxia. Early diagnosis and thiamine treatment are critical owing to the condition's high mortality rate and risk of progression to Korsakoff syndrome. However, WE is often underdiagnosed, and treatment guidelines remain limited. In this case study, we report the case of a patient diagnosed with WE shortly after improvement from alcohol withdrawal delirium. High-dose thiamine administration for approximately nine we

Wernicke's encephalopathy (WE), a neuropsychiatric emergency caused by thiamine deficiency, is increasingly recognized in nonalcoholic populations. We present a 64-year-old male with pyloric obstruction from gastric cancer (stage IIIA) who developed WE 20 days after gastrectomy. Prolonged thiamine-free total parenteral nutrition (TPN), delayed enteral intake, and cancer-related hypermetabolism jointly precipitated a thiamine deficiency. Undisclosed psychiatric comorbidity exacerbated diagnostic challenges and potential risk. Despite initial diagnostic challenges, timely neurological assessment

INTRODUCTION AND IMPORTANCE: Wernicke's encephalopathy (WE) is an acute central nervous system disorder resulting from thiamine deficiency. It manifests with symptoms such as ophthalmoplegia, mental confusion, and ataxia. WE is frequently linked with alcoholism but can also arise from nonalcoholic factors like gastrointestinal surgery. In cases where WE is strongly suspected, empirical thiamine supplementation often yields favorable outcomes.
CASE PRESENTATION: We present the case of a 46-year-old female who underwent total parenteral nutrition (TPN) following jejunotomy and left arm open red

BACKGROUND: Delirium in patients with ulcerative colitis may be seen, especially in the elderly and in patients hospitalized for a long time. In children, Wernicke's encephalopathy may occur due to thiamine deficiency in both ulcerative colitis and Crohn's disease. We present a patient with ulcerative colitis who presented with delirium as the first symptom, did not respond to steroid treatment and improved with anti-tumor necrosis factor-alpha treatment.
CASE PRESENTATION: A 14-year-old male child presented with complaints of nonsensical speech and hallucinations for two days. He also had 2-

Thiamine deficiency triggers an acute neurological emergency, Wernicke's encephalopathy, which, if left untreated, can progress to the chronic amnestic Korsakoff syndrome. While chronic alcohol misuse remains the classic risk factor, thiamine deficiency is increasingly documented in malnutrition, hyperemesis gravidarum, post-bariatric surgery, chronic gastrointestinal disease, and malignancy. This expanded etiological spectrum underscores the need for a broader clinical perspective. This review included studies indexed in EMBASE, PubMed, Scopus, Web of Science, and Google Scholar databases. Wh

Background Patients with alcohol use disorders (AUD) are at increased risk of Wernicke's encephalopathy (WE) due to thiamine deficiency. At our centre all admitted patients with AUD are assessed for signs, symptoms and risk factors for WE. Such patients are prescribed oral or intravenous (IV) thiamine replacement in accordance with local guidelines; choice of dose and route of administration is determined based on the clinical features present at presentation. Failure to appropriately treat patients with WE may lead to potentially avoidable progression to Korsakoff syndrome. However, unnecess

INTRODUCTION: Wernicke's encephalopathy (WE) is a neurological emergency caused by thiamine deficiency, most often associated with chronic alcoholism, but it can also occur in non-alcoholic settings such as hyperemesis gravidarum.
CASE PRESENTATION: We report the case of a 37-year-old pregnant woman with intractable vomiting who developed blurred vision, ophthalmoplegia, and subsequently acute flaccid paraparesis. Brain MRI showed typical lesions of WE, and laboratory tests revealed profound hypokalemia. Clinical improvement followed intravenous thiamine and potassium supplementation.

Wernicke's encephalopathy is an acute neurologic disorder caused by thiamine (vitamin B1) deficiency, which is most commonly associated with alcoholism. Rare cases of Wernicke's encephalopathy have been described in cancer patients, mostly with gastrointestinal and hematologic malignancies. Head and neck cancer patients frequently have reduced oral intake as a direct result of their tumor or from chemoradiation treatments. We report a case of Wernicke's encephalopathy in a 44-year-old woman with adenoid cystic carcinoma following chemoradiotherapy. Through a literature review we identified add

Wernicke's encephalopathy (WE) is an acute neuro-metabolic disorder caused by thiamine deficiency. In postoperative and critically ill patients, subtle neurological signs are often masked by sedation, delaying diagnosis. A 59-year-old woman underwent emergent cholecystectomy for acute cholecystitis complicated by multiple bilomas. Postoperative hemorrhage required superselective hepatic artery embolization. After initial stabilization and discharge, she was readmitted with respiratory failure and reduced consciousness, requiring intubation and mechanical ventilation. Despite cessation of sedat

Wernicke's encephalopathy (WE) is caused by thiamine deficiency (TD) whose main risk factor is alcohol use disorder. Pathogenic mechanisms associated with WE include mitochondrial dysfunction, oxidative stress and neuroinflammation. This study aims to explore the gene expression signature of certain candidate genes related to neuroinflammation, mitochondrial dysfunction and thiamine metabolism in the hippocampus from animals exposed to chronic alcohol consumption, thiamine deficiency or the combination of both. Male Wistar rats (n=42) were randomly assigned to 4 experimental groups: control (C

Bariatric surgery is a recognised intervention for severe obesity, a global epidemic associated with numerous comorbidities like diabetes, hypertension, and cardiovascular diseases. Procedures such as Roux-en-Y gastric bypass facilitate significant weight loss, improving metabolic health and quality of life. However, these surgeries carry hidden costs, particularly concerning nutritional deficiencies that are often overlooked in preoperative assessments. This case report presents a 27-year-old patient who, after undergoing gastric sleeve surgery, developed neurological symptoms due to thiamine

Anti-dipeptidyl-peptidase-like protein 6 antibody-mediated disease is a rare autoimmune encephalitis typically presenting with diarrhoea and/or weight loss, central nervous system hyperexcitability and cognitive dysfunction. We present a case of a young woman with 10 days of diplopia and unsteadiness in the context of dysthymia and significant weight loss over 2 months. Initial examination demonstrated mixed dysconjugate nystagmus and ataxic gait. Presumed diagnosis was Wernicke's encephalopathy and intravenous thiamine was given, resulting in partial improvement. Subsequent examination reveal

OBJECTIVES: Wernicke encephalopathy (WE) is an acute neuropsychiatric disorder caused by thiamine deficiency. The classical triad of symptoms for WE include mental status changes, ataxia, and ophthalmoplegia. In contrast, more uncommon symptoms include hallucinations. Known risk factors include alcoholism, malignancies, and chronic kidney disease, particularly hemodialysis. However, WE in nonalcoholic adults is often overlooked.
METHODS: We report a WE patient with lung cancer undergoing hemodialysis who presented with the uncommon symptom of active hallucinations, which were improved by thia

We present a case of a male in his early 50s assessed in the emergency department with a seemingly clear alcohol history but with classic symptoms of Wernicke's encephalopathy (WE): disorientation, gait ataxia and vertical nystagmus. He also had significant bilateral hearing loss and profound anterograde amnesia. Neuroimaging revealed hallmark signs of WE, including symmetrical T2/fluid-attenuated inversion recovery hyperintensity in the medial thalami. Here, we consider important differentials beyond WE, discuss the importance of revisiting the patient's history and explore the significance o

INTRODUCTION: Wernicke encephalopathy is a metabolic disease mainly associated with vitamin B1 deficiency, which is common in chronic alcoholism. Non-alcoholic Wernicke encephalopathy is difficult for early diagnosis.
CASE PRESENTATION: One case involved a 62-year-old man who was admitted to hospital with drug-induced liver failure. He presented lower extremity weakness and progressive worsening of consciousness disturbance post-admission and was eventually identified as Wernicke encephalopathy by magnetic resonance imaging scan and deficiency in vitamin B1. The classic symmetric hyperintense

Wernicke's encephalopathy (WE) is a severe neurological condition caused by the deficiency of thiamine, which is a vitamin B1 molecule. Herein, we present the case of a 3-year-old girl with steroid-resistant nephrotic syndrome (SRNS) who did not achieve remission despite steroid pulse therapy (MPT) and rituximab. She had frequent vomiting and decreased oral intake on the 61st day. High-calorie total parenteral nutrition was initiated on the 86th day, and levothyroxine was administered for suspected hypothyroidism. On the 88th day, she experienced altered consciousness with ocular deviation and

A 34-year-old man with a history of alcoholism experienced progressive vision loss in both eyes over two months. His best corrected visual acuity was 0.1 OD and 0.2 OS, with visual field tests showing central scotoma bilaterally. Fundus examination revealed reddish optic discs with peripapillary telangiectasia in both eyes. Brain MRI showed bilateral high-intensity lesions in the substantia nigra on T2-weighted/Fluid-Attenuated Inversion Recovery (FLAIR) and diffusion-weighted images. Mitochondrial genetic analysis confirmed the m.11778G>A variant. After the patient stopped consuming alcohol a

Wernicke encephalopathy, also known as thiamine deficiency, is characterized by a triad of symptoms: ophthalmoplegia, ataxia, and confusion. However, additional symptoms may manifest depending on the area affected by the lesion. Although multiple cranial neuropathies are possible, sudden onset bilateral hearing loss has been described in rare situations as the first manifestation of Wernicke encephalopathy. Here, we present a case report detailing the clinical experience of a patient diagnosed with Wernicke encephalopathy, whose initial presentation included sudden bilateral deafness. The pati

Wernicke encephalopathy is a nutritionally disabling encephalopathy resulting from thiamine deficiency. The typical triad of symptoms in Wernicke's encephalopathy includes impaired consciousness, ocular motility disturbances, and ataxic gait. Although constricted pupils and diminished light reflexes are recognized features of Wernicke's encephalopathy, few reports have specifically focused on these findings. Here, we highlight the pupil diameter and light reflex abnormalities observed in this case. A 73-year-old man was admitted to our hospital because of dizziness and then referred to our dep

This case report introduces Professor ZOU Wei 's experience of treating a patient with Wernicke encephalopathy using the "regulating spirit and promoting yang acupuncture method". The patient was diagnosed as spleen and stomach deficiency with internal liver wind. The treatment principle focused on regulating spirit and awakening the brain, strengthening the spleen, calming wind, and relaxing the tendons. Three groups of acupoints were selected: ①acupoints for awakening the brain by regulating spirit and unblocking meridians (Baihui [GV20], Qianshencong [EX-HN1] and bilateral Taiyang [EX-HN5],
