Wernicke Encephalopathy Following Severe Hyperemesis Gravidarum: A Missed Opportunity for Timely Thiamine Replacement.

Wernicke's encephalopathy (WE) is a rare but potentially fatal complication of hyperemesis gravidarum (HG) caused by thiamine (vitamin B1) deficiency. WE is a medical emergency with complete recovery occurring in only a few cases associated with HG in pregnancy. Classical clinical features include the triad of ophthalmoplegia, altered mental status, and ataxia.  We report a case of a 42-year-old gravida 2 para 1 female patient at 16 weeks and 3-day period of gestation (POG) who presented to primary care during routine antenatal follow-up with a two-week history of sudden visual loss. She had e

Immune checkpoint inhibitor-induced encephalitis mimicking Wernicke encephalopathy - A case report.

Immune checkpoint inhibitors have revolutionized the treatment of many cancers since their introduction in the early 2010s. Consequently, immune checkpoint inhibitor-induced encephalitis (ICI-iE) is increasingly reported in scientific literature, but its diagnosis can be challenging due to the non-specific clinico-radiological features. Herein, we report the case of an elderly man with metastatic renal cell carcinoma, who developed ICI-iE slightly over a year into his pembrolizumab treatment. However, radiological abnormalities involving bilateral mamillary bodies, medial thalami, and the peri

Dynamic progression of Wernicke encephalopathy in a gastric cancer patient: Multimodal MRI insights with arterial spin labeling perfusion imaging.

RATIONALE: Wernicke encephalopathy (WE), a neurological emergency caused by thiamin deficiency, is traditionally diagnosed based on the triad of ophthalmoplegia, ataxia, and confusion. However, this classic presentation occurs in fewer than 10% of cases, complicating early recognition. Untreated cases risk irreversible brain damage or progression to Korsakoff syndrome. While magnetic resonance imaging (MRI) aids diagnosis, early-stage structural abnormalities may be subtle or absent. Arterial spin labeling (ASL), a noninvasive perfusion imaging technique, offers potential for detecting microci

Prevalence of Wernicke's Encephalopathy When Receiving Dextrose Before Thiamine: A National Study of Veterans.

BACKGROUND/OBJECTIVES: A commonly taught principle when treating emergency department (ED) patients with acute alcohol intoxication is to give thiamine before dextrose to avoid precipitating Wernicke's encephalopathy (WE). We sought to evaluate the prevalence of WE among a national sample of veterans who presented to the ED with alcohol intoxication and who then received dextrose before thiamine. METHODS: This is a retrospective, cross-sectional study of patients presenting to all Veterans Affairs (VA) Emergency Departments (ED) over a 10-year period (2010-2019). Data was obtained through a M

Semaglutide-induced Wernicke encephalopathy: a comprehensive analysis.

Although GLP-1 receptors analogues (RAs) benefits-risks profile has been largely documented in diabetes, higher dosages recently approved in obesity still require further assessment. We describe here the case of a 49-year-old female patient treated with semaglutide for obesity, who presented with Wernicke encephalopathy in a context of iterative vomiting and reduced food intake. Eighteen other cases of Wernicke encephalopathy were reported in literature and in the WHO global safety database (VigiBase). A context of nausea/vomiting or reduced food intake is described in 68% of cases, with weigh

Alcohol-Related and Non-Alcohol-Related Wernicke Encephalopathy: A Systematic Review and Meta-Analysis of Epidemiology and Clinical Features.

INTRODUCTION: The aim of this study was to characterize the epidemiology, risk factors, and clinical presentation of Wernicke encephalopathy (WE) and analyze differences between cases with and without excessive alcohol consumption. METHODS: A systematic search was conducted in PubMed, Scopus, and Web of Science up to April 1, 2025. The included studies provided data on prevalence, risk factors, clinical and radiological findings, mortality, and prognosis in patients with WE. Pooled proportions and weighted means were calculated using random-effect models with Freeman-Tukey transformation. Het

Concurrent Wernicke Encephalopathy and Posterior Reversible Encephalopathy Syndrome Following Gastric Sleeve Surgery.

BACKGROUND: Wernicke encephalopathy-an uncommon and severe condition caused by thiamine deficiency-is most often associated with alcohol use but can occur in other settings of nutrient deficiency. Posterior reversible encephalopathy syndrome is an acute neurologic condition characterized by reversible subcortical vasogenic edema that is often associated with hypertension. We present the case of a patient with concurrent Wernicke encephalopathy and posterior reversible encephalopathy syndrome. CASE REPORT: A 26-year-old female with a surgical history of laparoscopic sleeve gastrectomy performe

Wernicke encephalopathy revealing occult gastric adenocarcinoma: Radiological and clinical insights.

Wernicke's encephalopathy (WE) is an acute neuropsychiatric disorder caused by thiamine deficiency, often underdiagnosed in non-alcoholic patients. We report a rare case of Wernicke encephalopathy in a 60-year-old male presenting with diplopia, dizziness, and gait instability secondary to prolonged vomiting and malnutrition. Contrast-enhanced thoracoabdominal CT revealed irregular wall thickening of the antropyloric region of the stomach with heterogeneous soft-tissue density and multiple hepatic hypodense lesions showing peripheral enhancement, consistent with metastatic spread. Brain MRI dem

Non-alcoholic Wernicke Encephalopathy During Orthopedic Rehabilitation in a Patient Following Total Gastrectomy: A Case Report.

An 82-year-old woman, who had undergone total gastrectomy for gastric cancer 22 years earlier, fell at home three months prior and underwent surgical repair for a femoral neck fracture. She had been receiving rehabilitation at a convalescent hospital. One week before presentation to our hospital, she experienced another fall, after which she developed decreased oral intake, generalized weakness, and altered mental status. Due to these worsening symptoms, she was transferred from the rehabilitation hospital to our facility. After admission, brain magnetic resonance imaging was performed, which

Wernicke encephalopathy observed in a cancer patient receiving terminal-stage home care.

We report an 80 year old man in the terminal stage of cancer receiving home care. On the 18th day after starting home care, he experienced a sudden onset of impaired consciousness, unsteadiness of gait, and ophthalmoplegia. Based on the clinical symptoms and a prolonged loss of appetite, Wernicke encephalopathy (WE) was suspected. Thiamine 100 mg was administered intravenously, and 20 minutes later, the patient responded to verbal stimuli. After 1 h, he was able to engage in conversation. When impaired consciousness occurs in cancer patients receiving home care, it is important to consider WE

Association between visual hallucinations and α-synuclein oligomers in patients with dementia with Lewy bodies.

INTRODUCTION: Visual hallucinations (VHs) represent one of the core clinical features of dementia with Lewy bodies (DLB); however, their underlying pathology remains unclear. METHODS: We employed proximity ligation assay (PLA) and phosphorylated α-synuclein (αSYN) immunohistochemistry to compare αSYN oligomers and Lewy-related pathology across brain regions along the ventral visual pathway in patients with and without VHs (five patients each). RESULTS: Greater αSYN oligomer burden in the parahippocampal cortex was observed in patients with VHs compared to those without (p = 0.041), whereas t

A Case of Successful Neurological Critical Care for the Very-High-Risk Refeeding Syndrome With Central Nervous System Disorders.

Refeeding syndrome potentially causes death from refeeding-induced central nervous system disorders, such as hypoglycemic encephalopathy, Wernicke encephalopathy, and hypophosphatemic encephalopathy. However, there is no established management for malnourished patients with this refeeding-induced central nervous system syndrome. We report a case of successful neurological critical care for refeeding-induced central nervous system syndrome. A 15-year-old female with anorexia nervosa presented to our hospital after fainting. Her body mass index (BMI) was 8.8 kg/m². Upon arrival, her Glasgow Coma

A Case of Postoperative Wernicke Encephalopathy Mimicking Cerebellar Infarction Complicated by Postoperative Nausea and Vomiting: A Critical Diagnostic Pitfall for Thoracic Surgeons.

INTRODUCTION: Wernicke encephalopathy (WE) is a potentially life-threatening neurological disorder caused by a thiamine deficiency, most commonly associated with alcoholism or malnutrition. Although its occurrence after gastrointestinal surgery has been increasingly recognized, WE following thoracic surgery remains extremely rare and is often underrecognized by thoracic surgeons. Given that neurological symptoms of WE can mimic those of cerebral infarction, a timely diagnosis is challenging, especially when complicated by postoperative nausea and vomiting (PONV). CASE PRESENTATION: We report

Neurological complications associated with rapid weight loss and nutritional deficiencies following GLP-1 agonist use: a case report.

GLP-1 agonists have been a welcome addition to the armamentarium in the treatment of diabetes and obesity. While this class of medications is generally considered safe and effective, potentially severe neurological complications may be associated with rapid weight loss and nutritional deficiencies following GLP-1 agonist use. We present a 37-year-old woman who was prescribed semaglutide for diabetes and weight loss who subsequently experienced significant weight reduction and rapid glycemic control over three months. Thereafter, she developed progressive right leg numbness and weakness, follow

A Simple Restriction, a Complex Collapse: Multisystem Wernicke Encephalopathy Triggered by Self-Starvation.

Wernicke encephalopathy (WE) is an acute neuropsychiatric emergency caused by thiamine deficiency, most commonly associated with chronic alcoholism but increasingly recognized in non-alcoholic contexts. This report describes a 23-year-old woman who developed WE secondary to prolonged self-imposed starvation driven by distorted body image. She had deliberately restricted food intake for several weeks, believing she was overweight, which led to severe malnutrition, weight loss, and profound weakness. On admission, she presented with bilateral ptosis, nystagmus, and lower limb weakness but withou

Thyroid storm complicated by metabolic encephalopathy in a young patient: A case report and literature review.

RATIONALE: Metabolic encephalopathy is a rare but potentially devastating complication of thyroid storm. Its etiology is complex, clinical manifestations are diverse, there is no clear diagnostic criteria, so it is easy to lead to missed diagnosis, misdiagnosis, and mistreatment. We report a case of thyroid storm complicated by metabolic encephalopathy. PATIENT CONCERNS: A 20-year-old male patient with a history of hyperthyroidism was admitted to the hospital because of limb twitching, foaming at the mouth and coma. The laboratory test results confirmed the diagnosis of thyroid storm. After t
