Understanding the impact of Klebsiella pneumoniae K-Antigen based MAPS vaccine design on the immune response in animal models.

MAPS technology represents an innovative approach for the development of polysaccharide-based vaccines, relying on the affinity interaction between biotin and rhizavidin (rhavi), with potential for enhanced coverage through the incorporation of pathogen-specific proteins. Due to its flexibility, this platform is particularly attractive for the development of multivalent vaccines, as it is required for Klebsiella pneumoniae, a multidrug-resistant bacterium highlighted as prevalent cause of neonatal sepsis in low- and middle-income countries, against which no vaccines are currently available. Sorafenib, a clinical kinase inhibitor, attenuates Streptococcus pneumoniae pathogenesis and reduces disease progression in vivo.

UNLABELLED: Streptococcus pneumoniae is an opportunistic respiratory commensal bacterium and leading cause of pneumonia, meningitis, and sepsis, resulting in over a million deaths annually, particularly in children aged under 5. The rapid spread of macrolide-resistant strains led the WHO to designate S. pneumoniae as a priority pathogen that urgently requires alternative therapeutic strategies. Here, we identified the FDA-approved cancer drug sorafenib to show a dose-dependent, broad-spectrum efficacy against many pneumococcal serotypes, including multidrug-resistant clinical strains. In silic

Engineered antibodies bypass bacterial immune evasion to drive complement-mediated protection against lethal infections.

The expanding global crisis of bacterial infections caused by antimicrobial-resistant pathogens has resulted in an urgent need for therapeutics. Previous efforts to target pathogen surface antigens with monoclonal antibodies (mAbs) have sought to activate the complement system, primarily through the antibody-dependent classical pathway. However, most mAbs have induced insufficient complement activation because pathogen evasion strategies disrupt the complex spatiotemporal requirements for activation of the classical pathway initiation complex C1. To address this, we developed a targeted comple

Overwhelming Post-Splenectomy Infection: A Case Report With Cause and Manner of Death Considerations.

INTRODUCTION: Overwhelming post-splenectomy infection (OPSI), formerly known as post-splenectomy sepsis, is a rare clinical syndrome occurring following splenectomy. The clinical course is fulminant and frequently fatal, often characterized by a viral-like prodrome followed by acute sepsis and rapid clinical decline. Although OPSI most commonly occurs within the first several years after splenectomy, delayed presentations have been reported decades later. MATERIALS AND METHODS: Clinical records, laboratory studies, imaging findings, and medicolegal investigative records were reviewed in the e

Invasive pneumococcal disease with widespread metastatic involvement in a healthy adult: a case report.

BACKGROUND: This was an unusual case of invasive pneumococcal disease in an immunocompetent adult without serious underlying medical conditions. Streptococcus pneumoniae bacteraemia was found with multiple distant sites of infection, including polyarticular septic arthritis, osteomyelitis, multi-level spondylodiscitis, and paravertebral, psoas and pelvic abscesses. CASE PRESENTATION: A fit and well 56 year old woman presented with a 10 day history of fevers, rigors, and multiple hot, swollen, tender joints. Septic arthritis was initially thought unlikely, but when blood tests showed marked in

Investigation of Intracellular Clearing of Streptococcus pneumoniae by mRNA-Encoded Cpl-1 Bacteriophage Endolysin in Human Macrophages.

Streptococcus pneumoniae remains a major global health threat and is listed by the World Health Organization as a pathogen in urgent need of new antimicrobial strategies. While primarily considered an extracellular pathogen, S. pneumoniae can persist within splenic macrophages in severe disease, creating a protected intracellular niche that may contribute to fulminant sepsis. We recently demonstrated the concept of an mRNA-based therapeutic approach in which host cells produce the pneumococcal bacteriophage endolysin Cpl-1. Here, we investigated whether expression of Cpl-1 in macrophages can t

Fulminant septic shock due to Streptococcus pneumoniae bacteremia without initial radiographic pneumonia.

Streptococcus pneumoniae remains a leading cause of invasive bacterial infection worldwide and is classically associated with community-acquired pneumonia. However, fulminant pneumococcal septic shock in the absence of radiographic pneumonia is uncommon and may delay diagnosis and treatment. We present the case of a 48-year-old immunocompetent female who developed severe septic shock secondary to Streptococcus pneumoniae bacteremia despite initially negative chest imaging for focal pneumonia. The patient presented with profound hypotension, lactic acidosis, thrombocytopenia, acute kidney injur

A "three-in-one" nose-to-brain delivery strategy: intranasal vancomycin spray achieves simultaneous clearance of pneumococcal colonization, bacteremia, and meningitis.

Bacterial meningitis caused by Streptococcus pneumoniae is a lethal central nervous system infection, yet conventional intravenous vancomycin struggles to cross the blood-brain barrier effectively. Interestingly, the natural pathology of this pathogen originates from nasopharyngeal colonization, disseminates into systemic bacteremia, and ultimately breaches the meninges. Inspired by this sequential invasion, we hypothesized that administering vancomycin directly at the exact starting point via a nasal spray could achieve a simultaneous "three-in-one" eradication of all infection stages. Childhood sepsis burden: pathogens/antimicrobial-resistant bacteria, 1990-2021 and 2050 forecasts.

BACKGROUND: To assess the global burden and trends of childhood sepsis, and to evaluate whether clarifying these aspects can provide a basis for formulating targeted prevention and treatment strategies for the condition. METHODS: Using the Global Burden of Disease (GBD) 2021 database, we analyzed sepsis in 0-14-year-olds, covering 62 pathogens (21 drug-resistant), 84 "pathogen-drug" combinations, 12 infection syndromes, and indicators (deaths, disability-adjusted life years [DALYs], age-standardized rates). Bacterial pathogens and antimicrobial resistance patterns in infections associated with stillbirths and under-five deaths in Ethiopia: a post-mortem surveillance study.

BACKGROUND: Bacterial infections remain major contributors to stillbirth, neonatal, and under-five mortality, particularly in low-resource settings where antimicrobial resistance (AMR) increasingly threatens the management of severe infections. Despite the growing burden of AMR in Ethiopia, data on bacterial pathogens and resistance patterns contributing to child mortality remain limited. This study aimed to characterize bacterial pathogens and antimicrobial resistance patterns among isolates obtained from stillbirths and under-five deaths in Ethiopia. METHODS: We analyzed microbiological dat

INTRODUCTION: Secondary hemophagocytic lymphohistiocytosis (HLH) is a rare hyperinflammatory syndrome in adults, resulting from uncontrolled immune activation and a cytokine storm, and is frequently misdiagnosed because its manifestations closely resemble severe infection or sepsis, particularly when triggered by bacterial pneumonia. CASE PRESENTATION: A 63-year-old woman presented with 1 week of fever, cough, and dyspnea despite outpatient antibiotic treatment. Investigations confirmed right-sided lobar pneumonia caused by Streptococcus pneumoniae. Despite targeted intravenous antibiotics, s

Extracellular pneumolysin enhances the activation of cytosolic pattern-recognition receptor NOD2 through its pore-forming activity.

Streptococcus pneumoniae is the leading cause of community-acquired pneumonia, meningitis, and sepsis. This bacterium produces pneumolysin, a cholesterol-dependent cytolysin that forms oligomeric transmembrane pores in the host cells. Although pneumolysin is known to exhibit proinflammatory properties, the mechanisms by which it activates innate immune responses remain to be investigated. Here, we show that extracellular pneumolysin enhances the activation of nucleotide-binding oligomerization domain 2 (NOD2), a cytosolic receptor that recognizes bacterial peptidoglycans. Platanosides, a Promising Class of Plant-Derived Antibiotics, Are Effective against a Suite of Clinically Derived MDR and Drug-Sensitive Staph and Strep spp.

Systemic multidrug-resistant bacterial infections have become a global public health priority due to the diminished efficacy of standard antibiotic regimens and can lead to sepsis, which is among the leading causes of death (11 million/year) and years of lost life worldwide. We report an effective and well-defined antibiotic equimolar mixture of the platanosides 1-4 that is as effective as its constituent parts and is active against multidrug-resistant Staphylococcus and Streptococcus spp. The platanosides can be easily and inexpensively manufactured in good purity from sycamore tree (Platanus

High-throughput antigen discovery using Functional Genomic Vaccinology (FGV) identifies protective Streptococcus pneumoniae vaccine candidates.

The discovery of protective antigens remains a major bottleneck in bacterial vaccine development. To overcome this limitation, we present Functional Genomic Vaccinology (FGV), a high-throughput antigen discovery platform integrating genome-wide antigen prediction, proteome-scale screening, and experimental immunogenicity validation to identify protective bacterial antigens. Using FGV, 222 conserved S. pneumoniae proteins are expressed in vitro, incorporated into a protein microarray, and coupled to magnetic beads for mouse vaccination. Protein array analysis shows significant IgG responses in

Pneumococcal pneumonia in adults - re-emergence of vaccine serotypes: a prospective multicenter cohort study in Germany, 2020-2023.

The main burden of non-invasive pneumococcal diseases in adults is largely due to community-acquired pneumonia (CAP). This study aimed to investigate the distribution and dynamics of pneumococcal vaccine serotypes and to determine the proportion of CAP cases attributable to serotypes covered by 13-valent conjugate vaccine (PCV13), the 20-valent conjugate vaccine (PCV20), and the 23-valent polysaccharide vaccine (PPV23) among adults in Germany from 2020 to 2023. In this prospective multicenter cohort study, we analyzed all adult patients with CAP enrolled between January 1, 2020, and December 3

Severe pneumococcal pneumonia with human rhinovirus/enterovirus coinfection leading to sepsis in a welder: a case report.

Human rhinovirus/enterovirus (HRV/HEV) members of the Picornaviridae family are traditionally associated with self-limiting upper respiratory tract infections. However, increasing evidence suggests their potential to involve the lower respiratory tract and contribute to severe pulmonary disease. HRV/HEV infections are frequently complicated by bacterial coinfection and the development of complicated pneumonia. This case report presents a case of a 64-year-old male welder admitted to the intensive care unit with severe right lobar pneumonia and profound hypoxemia. Microbiological investigations
