Severe Central Nervous System Co-infection With Streptococcus pneumoniae and Varicella-Zoster Virus Presenting as Meningoencephalitis.

Prompt identification of bacterial and viral central nervous system (CNS) infections is crucial; however, co-infection remains rare and diagnostically challenging. We report the case of a 61-year-old woman who presented with altered mental status and was initially suspected of having a stroke but was later diagnosed with severe sepsis and meningitis. Cerebrospinal fluid (CSF) analysis revealed elevated protein levels, and nucleic acid amplification testing (NAAT) identified concurrent Streptococcus pneumoniae and varicella-zoster virus (VZV) infection. Blood cultures subsequently confirmed S.

Doxycycline versus azithromycin as combination therapy in the empirical treatment of community-acquired pneumonia in an intensive care unit - A retrospective propensity score-matched cohort study.

INTRODUCTION: Community-acquired pneumonia constitutes a significant burden to healthcare systems. While international guidelines recommend beta-lactam plus macrolide in treating severe community-acquired pneumonia, doxycycline instead of macrolide provides antimicrobial activity to macrolide-resistant organisms that are highly prevalent in East Asia and avoids its side effects. Yet, doxycycline's immunomodulatory effect is not as well studied. We investigated the comparative efficacy of doxycycline with azithromycin in the treatment of severe community-acquired pneumonia. METHODS: From 1 Jan

A natural product Ginsenoside C-K with Antimicrobial Activity against Streptococcus pneumoniae.

Streptococcus pneumoniae is a major cause of bacteremia and sepsis, and rising antimicrobial resistance necessitates new therapies. This study aimed to screen ginsenosides with antipneumococcal activity and to evaluate antibacterial effects of candidate compound. Among nine ginsenosides tested in vitro, only Ginsenoside C-K exhibited significant antibacterial activity with a minimum inhibitory concentration (MIC) of 4-8 µg·mL-1. Further investigation demonstrated that Ginsenoside C-K exerted rapid bactericidal effects and effectively inhibited both the formation and stability of biofilms. In a

Hepatocyte HNF4α Deficiency Protects Against Pneumococcal Sepsis Through a Neutrophil-Dependent Mechanism.

Sepsis induces profound alterations in liver function that contribute to disease progression. We previously demonstrated that peritoneal sepsis is associated with marked disruption of hepatic transcriptional programs, including loss-of-function of Hepatocyte Nuclear Factor 4 alpha (HNF4α), a master regulator of hepatic identity and metabolism. Using hepatocyte-specific HNF4α knockout mice, we further showed that loss of HNF4α is detrimental during peritoneal sepsis. Here, we investigated whether this response is conserved in pneumonia-induced sepsis and assessed its functional significance. Bu

Characterization of a novel monoclonal antibody candidate that targets bacterial GAPDH and protects neonatal mice from infections caused by Streptococcus pneumoniae or Streptococcus agalactiae.

Glyceraldehyde-3-phosphate dehydrogenase (GAPDH) is a highly conserved bacterial enzyme essential for glycolysis, yet it also plays important non-metabolic, "moonlighting" roles, including host immunomodulation. Here, we describe the generation and characterization of a monoclonal antibody (mAb01) that targets extracellular GAPDH from Streptococcus agalactiae (GBS) and Streptococcus pneumoniae. Using bio-layer interferometry (BLI) and surface plasmon resonance (SPR), we demonstrate that mAb01 binds with high affinity, exhibiting dissociation constants in the low nanomolar range for both antige

Vaccines preventing neonatal sepsis: focus on Group B Streptococcus, Klebsiella pneumoniae and Escherichia coli to combat rising antimicrobial resistance.

INTRODUCTION: Neonatal sepsis, a systemic infection occurring in infants within their first 28 days of life, is a leading cause of mortality globally. The burden is especially severe in low- and middle-income countries (LMICs), where incidence and mortality rates are higher than in high-resource settings. Antimicrobial resistance, driven by multidrug-resistant pathogens prevalent in LMICs, further complicates its effective management. Vaccination offers a promising strategy to prevent infections caused by common neonatal sepsis pathogens, potentially reducing sepsis incidence and mitigating re

Predictors, aetiology and outcome of early-onset neonatal bloodstream infection: a case-control study in a tertiary hospital in South Africa.

Difficulty identifying neonates at highest risk of early-onset bloodstream infection (EO-BSI) leads to high empiric antibiotic use. This retrospective case-control study analysed maternal and neonatal factors, pathogen profile, and outcomes of culture-confirmed EO-BSI (<72 hr of life) at a large neonatal unit in Cape Town, South Africa (1 January 2019-31 December 2021). Cases (neonates with culture-confirmed BSI) were matched 1:3 with randomly selected controls ('at risk' neonates with negative blood cultures, C-reactive protein < 10 mg/l and ≤4 days of antibiotics). Factors associated with EO

ypmS promotes capsular polysaccharide shedding and in vivo fitness of Group B Streptococcus.

Group B Streptococcus (GBS) is a leading cause of neonatal sepsis and meningitis worldwide. Capsular polysaccharide (CPS) is a major virulence factor that aids GBS in colonizing the gastrointestinal (GI) and vaginal tracts, surviving in whole blood and evading the immune system. CPS is also the basis for a GBS CPS-protein conjugate vaccine candidate. In addition to its function on the surface of the bacteria, CPS can also be released and shed into the environment. In Streptococcus pneumoniae, shed CPS has been shown to absorb antimicrobial peptides and CPS-specific antibodies, allowing the bac

The role of viridans streptococci and anaerobic bacteria of the oral cavity in paediatric parapneumonic pleural effusions/empyema-a nationwide hospital-based surveillance study.

OBJECTIVES: Parapneumonic pleural effusions/empyema (PPE/PE) are serious complications of community-acquired pneumonia in children. Although Streptococcus pneumoniae and Streptococcus pyogenes are established major pathogens, the clinical relevance of oral cavity bacteria (viridans streptococci and/or anaerobic bacteria [VS/AA]) in paediatric PPE/PE is largely unclear. METHODS: A nationwide hospital-based surveillance study in Germany recorded children aged <18 years with PPE/PE from October 2010 to June 2023, with bacteria detected from pleural fluid and/or blood. We compared clinical charac

Genome-informed metabolomic re-analysis identifies serum-associated amino acid signatures in sepsis-associated bacterial pathogens.

BACKGROUND: Antibiotic-resistant bacterial infections remain a major global health threat, particularly in hospital settings. Standard antimicrobial susceptibility testing provides essential clinical information, but it does not fully capture pathogen metabolic adaptation under host-like conditions. Integrating isolate whole-genome sequencing with metabolomics may help prioritize condition-associated metabolic signatures linked to bacterial physiology and antibiotic response. METHODS: We performed a secondary integrative re-analysis of publicly available isolate whole-genome sequencing and ma

RNA profiles in extracellular vesicles from severe sepsis and meningitis patients reveal pathogen-specific immune signatures in meningococcal versus pneumococcal infections.

BACKGROUND: This study investigates host-pathogen interactions by comparing RNA profiles in plasma extracellular vesicles (EVs) from patients with severe sepsis or meningitis caused by Neisseria meningitidis with profiles from patients with systemic Streptococcus pneumoniae infections. At hospital admission both bacteria may present with similar symptoms, making early differentiation difficult. We have focused on EVs, as they function as active mediators of intercellular communication in the circulation. METHODS: Plasma samples from patients with meningococcal septic shock, meningococcal meni

In vitro activity of the antimalarial, antibiotic drugs fosmidomycin and clindamycin against clinical isolates of bacterial bloodstream infections in febrile, hospitalized Ghanaian children.

BACKGROUND: The clinical and laboratory distinction between malaria and bacterial blood stream infections in patients with undifferentiated fever remains challenging. Misclassification may result in ineffective treatment with the risk of progression of disease and subsequent morbidity and mortality. A pragmatic solution is the use of multi-disease treatments effective against both malaria and the clinically most relevant bacterial pathogens. Fosmidomycin and clindamycin are two antibiotics that have demonstrated high efficacy in treating malaria. A multi-drug combination treatment of fosmidomy

Global, regional, and national burden of meningitis, its risk factors, and aetiologies, 1990-2023: a systematic analysis for the Global Burden of Disease Study 2023.

BACKGROUND: Meningitis remains the leading infectious cause of neurological disabilities globally, disproportionately affecting children younger than 5 years and populations in the African meningitis belt. Whereas previous global estimates focused on ten pathogen categories, this study presents the most comprehensive analysis to date, assessing the meningitis burden attributable to 17 causative pathogens based on the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2023 framework. METHODS: GBD is a systematic, scientific effort aimed at quantifying the comparative magnitude o

Pneumococcal serotype distribution and diagnostic yield of serotype-specific urinary antigen detection in adults with CAP in Switzerland, 2016-2021: a prospective cohort study.

BACKGROUND: Streptococcus pneumoniae remains the leading bacterial cause of community-acquired pneumonia (CAP) and a major source of morbidity and mortality in adults. Pneumococcal conjugate vaccines (PCVs) have substantially reduced invasive pneumococcal disease, but serotype replacement has led to shifts in pneumococcal CAP epidemiology. In addition, the identification of S. pneumoniae remains challenging. This study investigated pneumococcal serotype distribution, vaccine coverage, and the diagnostic yield of a serotype-specific urinary antigen detection assay (ssUAD) among adults with CAP
