Infectious Causes of Stillbirths: A Descriptive Etiological Study in Uganda.

BACKGROUND: Every year an estimated 2-3 million babies are stillborn, with a high burden in Africa. Infection is an important driver of stillbirth. There is a lack of data on the bacterial causes of stillbirth in Uganda, contributing to a lack of interventions such as effective prophylaxis and development of maternal vaccine options against the most implicated pathogens. METHODS: The PROGRESS study was an observational cohort study undertaken in Kampala, Uganda, between November 2018 and April 2021. If a woman delivered a stillborn baby, consent was sought for the collection of a heart-blood

Investigating two decades of Streptococcus pneumoniae bacteraemia in the Gelderland area, the Netherlands, using whole-genome sequencing.

In the Netherlands, the 7-valent pneumococcal conjugate vaccine (PCV) was introduced to the childhood immunization programme in 2006 and replaced by the 10-valent PCV (PCV10, GSK) in 2011. To describe invasive pneumococcal disease in the era of childhood PCV vaccination on pneumococcal bacteraemia across all ages, we collected and sequenced 979 pneumococcal blood isolates from consecutive patients with pneumococcal bacteraemia in the Gelderland area, the Netherlands, between 2000 and 2020. In total, 58% of the bacteraemia cases (n=563/979) occurred in the elderly population. Compared to the pr

Septic shock and purpura fulminans secondary to Streptococcus pneumoniae bacteraemia in an immunocompetent individual.

A woman in her 40s presented to the emergency department with a 3-day history of headache, vomiting and fever. Vital signs on arrival indicated septic shock, including fever of 40°C, tachycardia of 110 beats per minute and hypotension of 78/55 mm Hg. A retiform purpuric rash on her limbs, with mottling of skin on the tips of her nose and ears, developed subsequent to admission. This rash was identified as purpura fulminans in the context of disseminated intravascular coagulopathy. Blood cultures isolated Streptococcus pneumoniae (serotype 22F), confirming invasive pneumococcal disease (IPD) as

Death from S. pneumoniae in HbSC Disease: Was Expanded Pneumococcal Vaccination Too Late?

Despite proven efficacy, only 70% of eligible individuals in the U.S. are vaccinated against Streptococcus pneumoniae. This is especially concerning for patients with sickle cell disease (SCD), as they are susceptible to invasive pneumococcal disease owing to functional asplenia. Early and complete vaccination is crucial in preventing pneumococcal sepsis and its complications, including the rare but often fatal fat embolism syndrome. Here, we report a case of an adolescent male with Hemoglobin SC (HbSC) disease who presented to our pediatric emergency department (ED) with abdominal and back pa

Influenza A virus dissemination and infection leads to tissue resident cell injury and dysfunction in viral sepsis.

BACKGROUND: Severe respiratory viral infections can lead to viral sepsis (VS), a life-threatening condition characterized by lung and extrapulmonary organ dysfunction. However, the pathology of VS is not clear. Specifically, it is unknown how the cytokine storm and direct virus infection contribute to the damage of extrapulmonary organs. METHODS: In this study, we established survival and lethal mouse models of VS by intranasally administering different doses of PR8/H1N1 influenza virus in C57BL/6J male mice, as well as model of bacterial sepsis (BS) caused by Streptococcus pneumoniae as refe

The role of TLR4/NF-kB signaling axis in pneumonia: from molecular mechanisms to regulation by phytochemicals.

Pneumonia, a leading global health challenge, is characterized by inflammation driven by dysregulated immune responses. Central to its pathogenesis is the Toll-like receptor 4 (TLR4)/nuclear factor-κB (NF-κB) signaling axis, which orchestrates the recognition of pathogen-associated molecular patterns (PAMPs) and initiates cascades that mediate innate immunity. While this pathway is essential for bacterial clearance, its overactivation can lead to excessive inflammation, tissue damage, and severe complications, including acute respiratory distress syndrome (ARDS) and sepsis. This review examine

The ClpXP protease and the ClpX unfoldase control virulence, cell division, and autolysis in Streptococcus pneumoniae.

In all living cells, molecular chaperones and ATP-dependent proteases are essential for protein homeostasis. The ClpXP protease is a chaperone-protease complex conserved between bacteria and mitochondria. Proteolytic activity resides in the caseinolytic protease (ClpP) subunits that associate to form a tetradecameric serine protease that degrades substrates recognized and unfolded by the ClpX ATPase. In the important human pathogen Streptococcus pneumoniae (Spn), ClpX but not ClpP was proposed to be essential for viability. We here follow up on this finding by characterizing phenotypes associa

Association of time to positivity with disease severity in bloodstream infections-a population-based cohort study.

OBJECTIVES: Short time to positivity (TTP) has been proposed as a prognostic indicator in bloodstream infection (BSI) but results have been conflicting. The aim of this study was to explore the association between TTP and disease severity, using non-linear models. METHODS: This population-based retrospective study included all blood cultures in southern Sweden from 2021 to 2023. Using healthcare databases, BSI episodes were linked to information regarding prespecified disease severity markers at the time of culture (laboratory values and vital signs) as well as patient outcomes (intensive car

Utilizing large and diverse bacterial genome datasets to improve the detection and identification of Streptococcus pneumoniae via PCR-based diagnostics.

The accurate identification of Streptococcus pneumoniae (pneumococcus) is crucial for diagnostics and surveillance but is complicated by the use of molecular assays that may also detect non-pneumococcal Streptococcus (NPS) species. Therefore, the aim of this study was to use a combination of in silico and in vitro analyses to evaluate PCR assays for the molecular detection and identification of pneumococci. A diverse dataset of over 9,300 pneumococcal and NPS genomes was investigated in silico to determine the sensitivity and specificity of assays for seven recommended gene targets: lytA, piaB

Pediatric Invasive Pneumococcal Disease Spectrum Before Third-Generation Pneumococcal Conjugate Vaccine Implementation.

BACKGROUND: After the implementation of pneumococcal conjugate vaccines (PCVs), besides the decrease of invasive pneumococcal disease (IPD), the clinical spectrum of the remaining cases changed, with many differences among serotypes. This study aimed to describe the clinical profile of IPD and the serotype distribution 15 years after PCV13 implementation and before the implementation of third-generation PCVs. METHODS: From 2017 to 2022, 128 French pediatric wards prospectively reported IPDs in children. Data were collected on demographics and clinical data, underlying conditions increasing th

Influenza A and Streptococcus pneumoniae Co-infection Presenting With Cardiomyopathy and Acute Kidney Injury in a Previously Healthy Adult: A Case Report.

Cardiomyopathy is a serious complication of systemic infections, commonly linked to viral myocarditis and sepsis-induced myocardial dysfunction. Although influenza and Streptococcus pneumoniae are independently associated with cardiovascular complications, cases involving concurrent infection with both pathogens in the absence of bacteremia remain exceedingly rare. We describe a case of acute reversible cardiomyopathy in a previously healthy 31-year-old male presenting with shock, acute kidney injury, and left ventricular dysfunction. The patient tested positive for influenza A by reverse tran

Serotype 3 invasive pneumococcal disease in Tuscany across the eras of conjugate vaccines (2005-2024) and anthropic-driven respiratory virus fluctuations.

Serotype 3 Streptococcus pneumoniae (ser3) has emerged as a leading cause of invasive pneumococcal disease (IPD) despite targeted vaccination efforts. Preliminary evidence reported a differential impact of anti-ser3-pneumococcal-conjugate-vaccines (PCVser3) on different clinical presentations of ser3-IPD. Recently, a temporal association between respiratory syncytial virus (RSV) and IPD was observed in children, supporting a role of RSV in driving IPD dynamics. Ser3 IPD cases occurred in Tuscany from November 2005 to March 2024 were included. Comparisons of different clinical presentations (ba

Clinical Features and Outcomes of Pneumococcal Bacteremia in Children With Sickle Cell Disease in the Pneumococcal Conjugate Vaccine Era.

INTRODUCTION: Children with sickle cell disease (SCD) are at higher risk for invasive infection with Streptococcus pneumoniae compared with the general pediatric population. Penicillin prophylaxis, pneumococcal conjugate (PCV), and polysaccharide vaccines (PPSV) have reduced but not eliminated the risk of pneumococcal bacteremia. METHODS: A single institution cohort of children with SCD aged <19 years was reviewed over the 14-year period since PCV13 licensure (January 2010-December 2023) to identify and characterize the clinical features and outcomes of S. pneumoniae bacteremia, including ser
