Maternal vaccination to prevent neonatal infections and combat antimicrobial resistance.

Maternal vaccination during pregnancy is emerging as a powerful strategy in protecting newborns from infectious diseases, improving neonatal outcomes, and potentially reducing antimicrobial use and resistance. Maternal immunisation works by eliciting protective antibodies in the mother that are transferred to the fetus transplacentally and through breastmilk postnatally to provide the infant with passive immunity during the first vulnerable months of life. There is sufficient evidence to support the role of maternal vaccination in averting many neonatal infections that would otherwise require

Cardiac Tamponade in Disguise: Purulent Pericarditis as a Deadly Turn in Pneumococcal Sepsis.

Invasive pneumococcal disease, presenting as purulent pericarditis, is a rare but potentially fatal condition. Early recognition and targeted antimicrobial and supportive interventions are essential to improve outcomes, particularly in high-risk patients. A 62-year-old male with poorly controlled type 2 diabetes mellitus was admitted with community-acquired pneumonia, following a five-day history of respiratory symptoms prior to hospitalization. A thorax CT scan revealed left lower lobe consolidation and bilateral pleural effusion. Empiric antibiotic therapy was initiated with ceftriaxone and

Endothelial cells retain inflammatory memory through chromatin remodeling in a two-hit model of infection-induced inflammation.

Sepsis survivors face a heightened risk of secondary infections following discharge, yet the underlying mechanisms remain poorly defined. Our study identifies a novel mechanism of endothelial inflammatory memory, wherein inflammatory exposure induces durable chromatin remodeling in endothelial cells (ECs), priming them for exaggerated responses to a subsequent infection. Utilizing a clinically relevant two-hit mouse model, cecal ligation and puncture (CLP) followed by mild Streptococcus pneumoniae (Sp) infection in CLP survivors, we reveal transcriptional activation in endothelial cells (ECs)

The metabolic, microbial and immunological demands of pneumococcal colonisation.

Streptococcus pneumoniae (the pneumococcus) causes a range of life-threatening diseases including pneumonia, sepsis and meningitis. Despite widespread vaccine deployment, pneumococcal disease remains a leading cause of global mortality. The human nasopharynx is its principal ecological niche, and all pneumococcal disease originates from this reservoir of organisms. Acute infections are, however, an evolutionary dead-end for the pneumococcus. What sustains the pneumococcus within human populations are cycles of colonisation and transmission. To persist within the nasopharynx, it must overcome t

Angiotensin-(1-7) treatment improves pneumonia and prevents sepsis caused by pneumococcal infection.

BACKGROUND: Pneumonia caused by Streptococcus pneumoniae remains a major global health concern, leading to significant morbidity and mortality. Angiotensin-(1-7) [Ang-(1-7)] is an endogenous peptide with known anti-inflammatory and pro-resolving properties, but its therapeutic potential in bacterial pneumonia is not fully understood. PURPOSE: This study aimed to investigate whether Ang-(1-7) could reduce inflammation, improve bacterial clearance, and enhance survival in a murine model of pneumococcal pneumonia, alone or in combination with the antibiotic ceftriaxone. METHODS: A mouse model o

[Pneumococcal disease without apparent pulmonary involvement].

Spinal epidural abscess (SEA) is a classically monomicrobial infection most commonly caused by Staphylococcus aureus (∼65%), followed by Gram-negative bacilli (∼16%), and less frequently by Gram-positive diplococci such as Streptococcus pneumoniae (∼7%), which typically reach the epidural space hematogenously from another focus, mainly respiratory. SEA can originate from hematogenous seeding or direct inoculation, and in approximately one-third of cases, the source of infection cannot be identified. We present a clinical case of SEA associated with pneumococcal bacteremia. A 65-year-old woman

Baseline oral microbiome associated with post-living donor liver transplant early complications within 6 months.

Oral dysbiosis is noted in liver cirrhosis. This may play a role in post-liver transplantation (LT) outcomes. This study aimed to assess the baseline oral microbiome and its association with early complications (ECs) within 6 months after LT. This prospective longitudinal study included 94 cirrhotic recipients (CRs) and their living donors. Patients were managed with a standardized immunosuppressive regimen and monitored for 6 months post-LT EC rejection, infection, biliary complications (BCs), and death. Pre-LT and post-LT saliva samples were processed for bacterial sequencing, qPCR validatio

Metabolomic analysis of Streptococcus pneumoniae: uncovering key metabolic pathways.

BACKGROUND: Pneumococcal infection, caused by Streptococcus pneumoniae, is a prevalent cause of community-acquired pneumonia and a major pathogen responsible for illnesses such as meningitis, sepsis, and pharyngitis. The complex etiology of S. pneumoniae infection poses significant challenges in elucidating the molecular mechanisms underlying its pathogenesis. METHODS: In this study, twenty serum samples from individuals infected with S. pneumoniae and fifteen serum samples from normal controls were analyzed using liquid chromatography/mass spectrometry (LC-MS) to identify metabolites. Two decades of bacteraemia in Norway: an ecological study of incidence and shifts in microbial epidemiology, 2005-2024.

This study aimed to describe temporal changes in the incidence and microbial composition of bacteraemia in Norway from 2005 to 2024. Using national surveillance data from all microbiology laboratories, combined with contextual indicators such as population size, hospital activity, immunosuppression, and cancer incidence, we conducted an ecological analysis of annual aggregated data. The number of isolates from blood cultures increased from 10,964 in 2005 to 22,679 in 2024, or from 238.0 to 408.6 per 100,000 people, while the estimated blood culture positivity rate remained stable at approximat

Impact of sex on the development of bacteremia in critically ill burn patients: A retrospective cohort study.

BACKGROUND: Burn injuries are associated with significant morbidity and mortality, with infections being a major factor influencing outcomes. Given evidence of sex-related differences in infection risk, we investigated whether such disparities extend to bacteremia in critically ill burn patients. METHODS: We conducted a single-centre cohort study of critically ill patients with severe burn injuries admitted to the Burn Center at the University Hospital Zurich between January 2017 and December 2021. The primary outcome was the association between sex and the development of a first bacteremia e

Early onset neonatal bloodstream infections in South African hospitals.

BACKGROUND: Neonatal sepsis is a leading cause of death in low- and middle- income countries (LMIC). Increasing antibiotic resistance in early onset (< 72 h of life) bloodstream infection (EO-BSI) pathogens in LMIC has reduced the effectiveness of the recommended empiric antibiotic regimen (ampicillin plus gentamicin). METHODS: We retrospectively analysed blood culture-confirmed EO-BSI episodes at nine neonatal units from three central and six peripheral hospitals in the Western Cape Province, South Africa between 1 January 2017 and 31 December 2018.
