Wernicke encephalopathy (WE) is a severe neurological condition caused by thiamine deficiency that remains underreported in pregnancy. We report a clinical diagnosis of WE in a 41-year-old woman, gravida 2, para 1, at 16 weeks of gestation. She presented with acute, painless visual impairment, severe dietary restriction, and marked weight loss secondary to severe hyperemesis gravidarum (HG). Examination revealed reduced visual acuity, horizontal nystagmus, psychomotor retardation, and an ataxic gait. The initial diagnosis was delayed due to her atypical presentation and the absence of MRI or b

BACKGROUND & AIMS: Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) have revolutionized the treatment of type 2 diabetes mellitus and obesity in recent years. While gastrointestinal adverse events are common, their association with nutritional deficiencies, including thiamine, has not been comprehensively investigated. This study aimed to evaluate whether treatment with GLP-1 RAs is associated with the occurrence of Wernicke encephalopathy (WE).
METHODS: We conducted a pharmacovigilance study using the FDA Adverse Event Reporting System (FAERS) and a narrative literature review. Dispropo

Thiamine (vitamin B1) deficiency is a recognized complication after bariatric surgery (BS) that can lead to severe neurological manifestations. Early identification is critical to prevent irreversible complications. We report a 23‑year‑old woman who presented two months after BS-Roux-en-Y gastric bypass (RYGB) with diplopia and limitation of ocular abduction in the right eye, consistent with isolated sixth cranial nerve palsy. She had no history of head trauma, infection, or prior neurological disease. Cranial CT angiography and brain MRI excluded ischemic, hemorrhagic, vascular, or demyelinat

Thiamine deficiency remains a clinically relevant yet frequently overlooked condition in modern emergency departments, particularly in aging societies such as Japan. Although traditionally regarded as a rare or historical disease, thiamine deficiency is not uncommon among emergency patients with high-risk backgrounds, including older adults, malnutrition, malignancy, chronic illness, diuretic use, gastrointestinal surgery, pregnancy-related vomiting, and social isolation. Despite its prevalence, diagnosis is often delayed or missed because of nonspecific presentations, reliance on incomplete c

Thiamine deficiency can lead to a spectrum of neurologic and cardiovascular manifestations, most notably beriberi and Wernicke encephalopathy. This condition is further separated into wet (cardiovascular manifestations) and dry (neurological manifestations) beriberi. Beriberi is often caused by a prolonged mild to moderate thiamine deficiency. In contrast, Wernicke encephalopathy occurs due to a severe, short-term thiamine deficiency that can present as the classic triad of mental status changes, ocular abnormalities, and gait ataxia. Since both conditions are due to the same cause of thiamine

INTRODUCTION: Deficiency of pyridoxal-5'-phosphate, the clinically measured and active form of pyridoxine (vitamin B6), is prevalent among those with alcohol use disorder. However, unlike thiamine, pyridoxal-5'-phosphate is not routinely repleted. Evidence is presented that its repletion would improve outcomes of alcohol withdrawal syndrome and promote long-term abstinence.
PYRIDOXAL-5′-PHOSPHATE DEFICIENCY CAN CAUSE A CLINICALLY CONSEQUENTIAL GLUTAMATERGIC/GABAERGIC SIGNALING IMBALANCE: Excessive excitatory glutamatergic signaling and inadequate inhibitory GABAergic signaling are central to

INTRODUCTION: Wernicke Encephalopathy (W.E.) is an acute neurological disorder induced by thiamine deficiency. Alcohol abuse is considered to be the leading cause of the disease; however, numerous other conditions, such as malnutrition or cancer, have been identified as potential risk factors.
CASE PRESENTATION: Clinical manifestations include a typical triad of mental status alteration, nystagmus, and ataxia and are attributed to damage in brain regions of high thiamine demand. The diagnosis is mainly clinical and further supported by the immediate response of neurological signs to parentera

Thiamine (vitamin B1) deficiency may be associated with serious neurological manifestations, including peripheral neuropathy, paralysis, confusion, paresthesia, pain, dysarthria, and nystagmus. The primary etiology of thiamine deficiency is consumption of a diet deficient in thiamine. In the United States, the most common cause of thiamine deficiency is alcohol use disorder due to the poor dietary intake that is often associated with this diagnosis. Thiamine deficiency also develops at an increased frequency in patients who have had gastric bypass surgery. Other medical conditions less commonl

Thiamine (vitamin B1), a critical cofactor for carbohydrate metabolism and neurotransmitter synthesis, plays an indispensable role in neuro-ophthalmic health. Deficiency of this crucial nutrient, while traditionally associated with a range of neuro-ophthalmic symptoms, can often lead to clinical presentations that are far from typical. In this case series, we delve into the unique experiences of three young men who developed uncommon neuro-ophthalmic manifestations due to thiamine deficiency following rapid weight loss due to strenuous exercises. Involvement of the optic nerve, presenting as e

Ovine gammaherpesvirus 2 (OvGHV2), is a member of the Macavirus genus, shares the 15 A antigenic epitope that is common to members of the malignant catarrhal fever virus (MCFV) complex, and induces sheep-associated malignant catarrhal fever (SA-MCF) in susceptible mammalian hosts. Sheep, the adapted hosts, rarely show pathological evidence of infection. This study describes the unusual SA-MCF lesions in a lamb, with concomitant polioencephalomalacia (PEM), bovine alphaherpesvirus 1 (BoAHV1)-associated pneumonia, and caseous lymphadenitis. Wernicke's encephalopathy (WE) is an acute neurological emergency resulting from severe thiamine deficiency, frequently mimicking acute cerebellar stroke due to its abrupt onset and neurological manifestations. Early recognition is crucial to prevent severe complications. Here, we report the case of a 52-year-old male with alcohol use disorder who presented with acute dizziness, vomiting, and gait instability. Neurological examination revealed left facial droop, dysarthria, and an ataxic gait with a positive Romberg sign. Given the presentation, an acute stroke was initially suspected (Nationa

Thiamine (vitamin B1) deficiency can occur secondary to malnutrition, which is associated with multiple etiologies, from imbalanced dietary habits to significant gastrointestinal nutritional losses such as recurrent vomiting or diarrhea. Thiamine deficiency manifests as dry or wet beriberi, Wernicke encephalopathy, or Korsakoff syndrome. We present a 26-year-old lady, three months post-laparoscopic sleeve gastrectomy with a complicated postoperative course due to poor oral intake and recurrent vomiting, who presented with bilateral limb weakness and vision loss of five days duration. Physical

Poorly controlled Crohn's disease is associated with several complications, including malabsorption of vitamins and minerals such as vitamin D, vitamin B12, and vitamin B1 (thiamine). Thiamine, among other vitamins, plays a crucial role in Adenosine Triphosphate formation as well as nerve propagation. Thiamine deficiency initially manifests as wet or dry beriberi but may lead to the devastating Wernicke-Korsakoff syndrome. In this report, we present a case of full-blown Wernicke encephalopathy associated with uncontrolled Crohn's disease. An 18-year-old young man was admitted with a chief comp

Wernicke's encephalopathy (WE) is a neurological emergency caused by thiamine (vitamin B1) deficiency and classically presents with a triad of confusion, ophthalmoplegia, and ataxia. Although classically associated with chronic alcohol misuse, it may also occur in non-alcoholic individuals due to causes such as persistent vomiting, malnutrition, or gastrointestinal disorders. We present a rare case of WE in a 35-year-old woman who developed internuclear ophthalmoplegia (INO), a highly unusual manifestation in this context. She presented with a one-week history of blurred and double vision, gai

Introduction Wernicke's encephalopathy (WE) is a serious yet potentially reversible neurological condition resulting from thiamine (vitamin B1) deficiency. Although traditionally linked to chronic alcohol consumption, an increasing number of cases are now being recognized in nonalcoholic individuals, where atypical clinical presentations often lead to diagnostic challenges and delays in treatment. This case series highlights a spectrum of WE cases with varied underlying etiologies, along with their clinical manifestations and characteristic neuroimaging findings. By emphasizing the importance

BACKGROUND: Wernicke encephalopathy (WE) is a severe neurological disorder caused by thiamine (vitamin B1) deficiency. Though often associated with chronic alcohol abuse, it can also arise from other conditions that impair thiamine intake or absorption. The classic triad of symptoms includes ophthalmoplegia, an abnormal mental state, and gait ataxia, although these may not be present in all patients, leading to underdiagnosis and undertreatment.
METHODS: This mini review synthesizes data from clinical studies, autopsy reports, and imaging findings to assess the prevalence, diagnostic challeng

<p>Beriberi is a nutritional disorder caused by thiamine deficiency. Classically, Beriberi presents in two primary clinical forms: "wet" Beriberi, which features heart and circulatory system impairment, and "dry" Beriberi, which causes polyneuropathy. Although it is an easily treatable condition, it is often misdiagnosed and can be life-threatening if not promptly recognized and managed. The diagnosis of Beriberi is performed by the signs and symptoms of the disease and can be confirmed by thiamine deficiency identification or by therapeutic testing. However, considering the costs and the limi

Inborn errors of metabolism are a prevalent cause of pediatric neurological abnormalities, often resulting from enzyme deficiencies that disrupt metabolic pathways. Understanding the radiological manifestations of these disorders is critical for timely diagnosis and therapy. This study aimed to elucidate the magnetic resonance imaging (MRI) brain characteristics and magnetic resonance spectroscopy (MRS) findings of specific treatable pediatric neurometabolic disorders through clinical vignettes, thereby enhancing diagnostic accuracy and informing therapeutic approaches. The study includes four

SUMMARY: Bariatric surgery and glucagon-like peptide-1 receptor agonist medications (GLP-1RAs) are common and effective methods for treating obesity. Since bariatric surgery is associated with an increased risk of malnutrition, there are extensive post-operative dietary recommendations and nutritional monitoring guidelines. There is, however, less evidence regarding the risk of malnutrition with GLP-1RAs and little research on the use of GLP-1RAs in patients post-bariatric surgery. We present a clinical case in which a 39-year-old male on semaglutide therapy presented with severe weakness and

Cardiac beriberi, a manifestation of thiamine deficiency, is an uncommon but reversible cause of heart failure that can mimic other forms of cardiomyopathy, such as dilated or tachycardia-induced cardiomyopathies. It is likely underrecognized or misdiagnosed. We report an elderly man with neurological symptoms and globally reduced left ventricular (LV) function in the context of prior gastrectomy, chronic alcohol consumption, and prolonged malnutrition. Early empiric thiamine administration led to rapid improvement in pulse rate, left ventricular function, and neurological symptoms, and the di
