Functional thiamine deficiency in patients with chronic kidney disease in Northeast Thailand.

BACKGROUND: Patients with chronic kidney disease (CKD) may be vulnerable to thiamine insufficiency due to dietary restrictions, altered metabolism, and renal replacement therapy. We aimed to determine the prevalence of functional thiamine deficiency, its distribution across CKD stages, and the associated clinical factors in a Thai population. AREAS COVERED: Thiamine status was assessed in red blood cells using the erythrocyte transketolase activity coefficient (ETKAC) assay. Participants (n = 605) were categorized as having normal thiamine status (ETKAC ≤1.25) or functional thiamine deficienc

Non-alcoholic Wernicke encephalopathy following pancreaticoduodenectomy (Whipple procedure): a case report and review of the literature.

INTRODUCTION AND IMPORTANCE: Wernicke encephalopathy (WE) is an acute neurological emergency caused by thiamine deficiency. It is uncommon after pancreaticoduodenectomy and may be overlooked when postoperative metabolic disturbances coexist. PRESENTATION OF CASE: A 66-year-old woman underwent pancreaticoduodenectomy for a duodenal neoplasm. Her postoperative course was complicated by pancreatic fistula, extended-spectrum beta-lactamase-producing Escherichia coli infection, impaired oral intake, and prolonged nutritional support. During the third to fourth postoperative weeks, she developed fl

Addressing micronutrient deficiencies and vitamin metabolism disorders in pediatric pulmonary hypertension: a strategic proposal for low- and middle-income countries.

BACKGROUND: Pediatric pulmonary hypertension (PH) is traditionally managed under a strict hemodynamic paradigm, often overlooking systemic metabolic and nutritional drivers. This review introduces the "Cellular Deprivation Model" to evaluate how specific micronutrient deficiencies and metabolic blocks accelerate pulmonary vasculopathy. METHODS: A systematic, PRISMA-compliant search was conducted in PubMed/MEDLINE for literature published between January 2000 and April 2026. Keywords intersected pediatric PH with malnutrition, avitaminosis, iron deficiency, and inborn errors of metabolism. Out

Acute Bilateral Visual Loss as an Atypical Manifestation of Wernicke Encephalopathy Secondary to Hyperemesis Gravidarum: A Case Report and Narrative Literature Review.

Wernicke encephalopathy (WE) is a severe neurological condition caused by thiamine deficiency that remains underreported in pregnancy. We report a clinical diagnosis of WE in a 41-year-old woman, gravida 2, para 1, at 16 weeks of gestation. She presented with acute, painless visual impairment, severe dietary restriction, and marked weight loss secondary to severe hyperemesis gravidarum (HG). Examination revealed reduced visual acuity, horizontal nystagmus, psychomotor retardation, and an ataxic gait. The initial diagnosis was delayed due to her atypical presentation and the absence of MRI or b

Genome-wide gene by sleepiness interaction analysis for sleep apnea.

STUDY OBJECTIVES: Excessive daytime sleepiness (EDS), influenced by environmental and social-behavioral factors, is reported by a subset of patients with sleep apnea-a group that may be at elevated cardiovascular risk. However, it is unclear whether sleep apnea with and without EDS have distinct genetic underpinnings. In this study, we perform gene-by-EDS interaction analyses for apnea hypopnea index, a diagnostic marker of sleep apnea severity, to understand EDS's influence on its underlying genetic risk. METHODS: Discovery interaction analyses for common variants and gene-based rare variant

Post-traumatic stress disorder impairs CD8+ T cell-dependent anti-tumor immune responses by downregulating thiamine.

Post-traumatic stress disorder (PTSD) is a severe and chronic psychiatric condition. However, whether PTSD influences tumor immunity remains poorly understood. In this study, we demonstrated that PTSD promotes the growth of subcutaneous transplant tumors in mice, an effect that can be reversed by anti-CD8 antibody treatment. Furthermore, we found that PTSD downregulates serum thiamine levels, which mediates the tumor-promoting effects observed in PTSD mice. In vitro, thiamine enhances CD8+ T cell activation and cytotoxic function. Mechanistically, thiamine augments tricarboxylic acid (TCA) cyc

Astrocytic TPK1 mitigates amyloid pathology via TFEB-mediated endocytosis.

Alzheimer's disease (AD), the leading cause of dementia, is characterized by amyloid-beta (Aβ) plaques, neurofibrillary tangles, and progressive neurodegeneration. Deregulation of glial cell activity plays an important role in the amyloid pathology. However, it is still unclear how changes in astrocytes contribute to Aβ deposition and clearance in AD. Here, we showed that deficiency of astrocytic thiamine pyrophosphokinase 1 (Tpk1), exacerbated Aβ burden leading to exacerbated spatial memory deficits in a mouse model of AD. While selective overexpression of Tpk1 in astrocytes ameliorated cogni

Thiamine Allocation and Deficiency Status Throughout the Life Cycle of Cod.

Several wild bird and fish species across the Northern Hemisphere have been shown to episodically be thiamine deficient. This may lead to mass-mortality events, especially in offspring. To understand the mechanisms underlying thiamine deficiency we need a better understanding of the dynamics and somatic allocation of the vitamin. Here we focus on a common, ecologically and economically important species, that is, Atlantic cod (Gadus morhua), which has been suggested to be sensitive to thiamine deficiency. We sampled cod of varying sizes and maturity stages in a system where thiamine deficiency

Deep Cerebral Venous Thrombosis Presenting with Korsakoff Syndrome.

Deep cerebral venous thrombosis (DCVT) is an uncommon cerebrovascular disease with diverse neurological deficits and a poor prognosis. Therefore, the early diagnosis and treatment of DCVT are needed. We herein present a case of DCVT with edematous lesions in the bilateral thalami with Korsakoff syndrome (KS) as the initial symptom. Although KS mainly occurs in patients with thiamine deficiency, it can also result from cerebrovascular disease. Because thalamic dysfunction may be associated with KS, DCVT with thalamic lesions should be considered a cause of KS to facilitate its early diagnosis a

Micronutrient and Nutritional Deficiencies Associated With GLP-1 Receptor Agonist Therapy: A Narrative Review.

Glucagon-like peptide-1 receptor agonists (GLP-1RA) such as semaglutide, liraglutide and tirzepatide are effective for obesity and type 2 diabetes mellitus (T2DM) but may predispose users to micronutrient deficiencies through appetite suppression, delayed gastric emptying and altered absorption. Given the rapid rise in GLP-1RA use, clarifying their nutritional impact is clinically important. We conducted a structured search of PubMed and the Cochrane Database (January 2019-May 2025) for adult studies evaluating nutritional or micronutrient outcomes during GLP-1RA therapy. Paediatric studies, t

The Gut Commensal Butyricimonas Virosa Modulates Gut Microbiota-Dependent Thiamine Metabolism and Attenuates Mouse Steatotic Liver Disease.

Metabolic dysfunction-associated steatotic liver disease (MASLD) is a common chronic liver disease. This study investigates the anti-MASLD effects of dietary prebiotic stachyose (STA) on disease progression identifying Butyricimonas virosa as a key bacterium boosted by STA supplementation. Oral gavage of B. virosa to high fat diet (HFD)-fed mice significantly suppresses the progression of MASLD and modulates gut microbiota composition. Integration of metagenomic and metabolomic data demonstrates that B. virosa treatment significantly enhances the production of thiamine monophosphate (TMP), as

Antifungal Mechanism of Biphenyl Phytoalexin Derivative Sorbauphylin A against Botrytis cinerea in Grapes.

Botrytis cinerea causes significant losses in crops, especially in grapes. The biphenyl derivative sorbauphylin A (4'-n-pentyl-[1,1'-biphenyl]-3,5-diol), derived from plant phytoalexins, exerts potent antifungal effects with low toxicity. This study demonstrated that sorbauphylin A significantly inhibited the growth of B. cinerea (EC50 = 15 μg mL-1), suppressed spore germination, and markedly reduced infection in grapes and leaves. The results of fluorescence staining and enzyme activity detection indicated that sorbauphylin A damaged cell membranes and walls and triggered a reactive oxygen sp

5-Fluorouracil-induced acute leukoencephalopathy in the absence of dihydropyrimidine dehydrogenase deficiency: A case report.

Introduction5-Fluorouracil (5-FU) is a chemotherapeutic agent used in various malignancies. 5-FU-induced leukoencephalopathy is a rare but reversible neurotoxic effect occurring within days of administration. Symptoms include confusion, agitation, and cognitive disturbances, with severe cases potentially causing coma. Diagnosis is supported by magnetic resonance imaging (MRI) showing characteristic brain changes. Treatment involves discontinuing 5-FU and initiating supportive care. Most patients recover within one week, but relapse may occur with repeated exposure, making early recognition cri

Visual outcomes in a series of patients with papilledema and comorbid nutritional deficiency.

BACKGROUND: Fulminant papilledema is a neuro-ophthalmologic emergency that can lead to permanent vision loss as a result of optic neuropathy. Despite appropriate treatment of intracranial hypertension and improvement of papilledema, some patients continue to experience worsening vision. In some cases, comorbid micronutrient deficiencies, which are an additional cause of optic neuropathy, may contribute to this decline. This case series highlights patients with papilledema whose vision worsened despite improvement in papilledema and who were found to have comorbid nutritional deficiencies. MET

Intravenous Thiamine-Induced Thrombocytopenia in a Patient With Chronic Adrenal Insufficiency.

Thiamine (vitamin B1) is generally considered safe, with rare adverse effects, including anaphylaxis. Although thrombocytopenia related to thiamine deficiency is known to improve with supplementation, thrombocytopenia occurring after thiamine administration has not been well described. We report the case of a 63-year-old female with adrenal insufficiency and malnutrition who developed severe thrombocytopenia shortly after initiation of intravenous thiamine for nutritional support. Platelet counts declined rapidly during therapy and recovered completely following thiamine discontinuation, with
