B vitamins in dermatology.

B vitamins play a pivotal role in cellular metabolism and erythrocyte synthesis, serving as essential water-soluble nutrients. Rapid excretion through the urinary system prevents their accumulation, thus mitigating potential toxicity. With minimal storage in the body, the levels of these vitamins are primarily influenced by recent dietary intake, especially in healthy individuals. The B vitamin group comprises eight distinct vitamins: thiamine (B1), riboflavin (B2), niacin (B3), pantothenic acid (B5), pyridoxine (B6), biotin (B7), folic acid (B9), and cobalamin (B12). In developed nations, B-c

Susceptibility to columnaris disease in Chinook salmon Oncorhynchus tshawytscha offspring from thiamine-deficient and thiamine-replete females.

Nutrient deficiency can cause increased susceptibility to infectious diseases in fish, thus leading to high rates of morbidity and mortality. Thiamine deficiency complex (TDC) in fish can lead to low reproductive success and high mortality rates. Columnaris disease in salmonids, caused by Flavobacterium columnare, has resulted in devastating losses in aquaculture production and wild populations of Pacific salmon particularly associated with climate change and high water temperatures. There is growing awareness that both TDC and columnaris are emerging diseases of salmonids on the west coast of

Thiamine-responsive megaloblastic anemia syndrome with novel compound heterozygous SLC19A2 mutations and thrombotic events: a case report.

BACKGROUND: Thiamine-responsive megaloblastic anemia syndrome represents a rare autosomal recessive condition originating from mutations in the SLC19A2 gene. It is characterized by a classical triad of megaloblastic anemia, insulin-dependent diabetes mellitus, and sensorineural hearing loss. We present the case of a woman diagnosed with thiamine-responsive megaloblastic anemia, with no history of consanguinity, in whom genetic testing revealed novel SLC19A2 mutations and an unreported clinical manifestation. CASE PRESENTATION: This case describes an 18-year-old mestizo female patient who pres

Thiamine status of whitefish (Coregonus maraena) in the Baltic Sea.

Many coregonine species have declined drastically across the Northern Hemisphere, including populations of Coregonus maraena (whitefish) in the Baltic Sea, and the mechanisms leading to these declines are not well investigated. An abrupt population crash occurred in the 1990s, coinciding with heavy declines in salmonid recruitment, also known as thiamine deficiency syndrome. Thiamine, i.e., vitamin B1, is an essential micronutrient needed for a functional metabolism. Offspring with thiamine deficiency have a high mortality posing significant negative impact on populations. Here, we aim to dete

Reassessing Transketolase Assays: Methodological Considerations for Detecting Functional Thiamine Deficiency.

Thiamine (vitamin B1) deficiency due to degradation, inadequate intake, impaired absorption, or increased metabolic demand remains prevalent in both human and animal populations. In its diphosphate form (thiamine diphosphate, TDP), thiamine serves as an essential cofactor for metabolic enzymes, including transketolase (TKT), a rate-limiting enzyme of the pentose-phosphate pathway. Measurement of TKT activity with and without exogenous TDP provides a functional assessment of thiamine utilization that serves as a surrogate for or complements direct quantification of thiamine forms. Beyond its hi

Thiamine and Micronutrient Deficiencies in Hospitalized Veterans Without Alcohol Use Disorder.

BACKGROUND: Micronutrient deficiencies (MiDs) can increase medical complexity in hospitalized adults, but the prevalence in those without alcohol use disorder (AUD) is unknown. Our objectives were to prospectively determine the prevalence of thiamine, cobalamin, folate, magnesium, and cholecalciferol deficiencies in hospitalized veterans without AUD. METHODS: Newly hospitalized veterans without AUD were recruited. Plasma thiamine, cholecalciferol, cyanocobalamin, folate, magnesium, C-reactive protein, albumin, and prealbumin were obtained. Interviews, physical exams, and medical record review

Hyperemesis gravidarum revisited: from GDF15 biology to precision multimodal therapy.

Hyperemesis gravidarum (HG) is a severe and potentially life-threatening form of nausea and vomiting in pregnancy, characterized by intractable vomiting, dehydration, electrolyte imbalance, nutritional deficiencies, and significant weight loss. Despite being a leading cause of early pregnancy hospitalization, its management remains inconsistent and often empirical. Emerging evidence suggests a paradigm shift in the understanding of HG pathophysiology, moving beyond traditional hormonal theories toward an integrated model involving genetic susceptibility and metabolic signaling pathways, partic

Delayed Recognition of Wernicke's Encephalopathy Following the Interruption of Thiamine in an Alcohol-Dependent Patient With Status Epilepticus: A Case Report.

Wernicke's encephalopathy (WE) is an acute neurological disorder precipitated by thiamine deficiency, most commonly associated with chronic alcohol misuse. WE is underdiagnosed and can lead to significant neurological morbidity if treatment is delayed. We report the case of a 35-year-old man with alcohol dependence and a history of seizures who was admitted for the management of status epilepticus requiring intubation and intensive care. During his sedation, his regular thiamine supplementation was inadvertently withheld for four days. Following extubation and seizure stabilization, the patien

Description of a novel organic acid-degrading bacterium with biotechnological potential, Denitratimonas organivorans sp. nov., genomic assessment of the genus Denitratimonas, and proposal of a new species Denitratimonas yunnanensis sp. nov. named following the rules of the SeqCode.

Anaerobic digestion of food waste supports a highly diverse microbial community, constituting an underexplored reservoir of functional taxa. A noval Gram-stain negative, facultatively anaerobic bacterial strain, CY0512T, was isolated from anaerobic digestion liquid of a food waste treatment plant. Phylogenetic analysis based on 16S rRNA gene sequences positioned CY0512T within the genus Denitratimonas, sharing 96.79% similarity with Denitratimonas tolerans E4-1T (=KACC 17565T). Genomic comparisons revealed significant divergence, with average nucleotide identity (ANI, 78.8%), amino acid identi

A stable Cd(II) metal-organic framework as a fluorescence sensor for nitrofuran antibiotics and vitamin B1/B9 in living cells.

The escalating threat of antibiotic resistance, fueled by drug misuse, coupled with the health risks posed by vitamin deficiencies, underscores a critical demand for efficient detection technologies. Herein, a novel V-shaped organic compound containing 2,1,3-benzothiadiazole (BTD) group, 4,4'-(benzo[c][1,2,5]thiadiazol-5,6-diyl)dibenzoic acid (H2BTDA), has been designed and synthesized. Further, a Cd(II) metal-organic framework (MOF) has been successfully prepared with the formula {[Cd(BTDA)]∙0.5DMF}n, (JXUST-61, where JXUST denotes Jiangxi University of Science and Technology). Interestingly,

Nitrous Oxide-Induced Subacute Degeneration of the Spinal Cord Requiring Spinal Rehabilitation: A Case Report.

BACKGROUND: Nitrous oxide is a widely available anaesthetic agent whose chronic use causes irreversible inactivation of vitamin B12, leading to subacute combined degeneration (SACD) of the spinal cord. While high-dose intramuscular B12 replacement often halts disease progression, complete neurological recovery is not always achieved. CASE PRESENTATION: A 19-year-old Chinese male with a 3-month history of nitrous oxide use presented to the emergency department with acute lower-limb weakness and encephalopathy. Examination revealed mild upper-limb paresis, diminished deep tendon reflexes, and m

Medical nutrition in the glucagon-like peptide-1 (GLP-1) era: Protein strategies, micronutrient monitoring, and lean mass preservation.

Glucagon-like peptide-1 receptor agonists (GLP-1RAs) and dual glucose-dependent insulinotropic polypeptide (GIP)/GLP-1 agents produce substantial, sustained weight loss primarily by suppressing appetite and lowering ad libitum energy intake. While fat mass loss predominates, randomized trials with body-composition substudies indicate a clinically relevant reduction in absolute lean mass. Concurrently, baseline micronutrient inadequacies are common in people with obesity and may be exacerbated by reduced intake, nausea, or vomiting during therapy. This narrative review synthesizes evidence on e

Wernicke's Encephalopathy Secondary to Hyperemesis Gravidarum in Pregnancy: A Case Report.

Wernicke's Encephalopathy, although classically associated with chronic alcoholism, can also occur during pregnancy as a complication of hyperemesis gravidarum and is often underdiagnosed. We report a case of a 34-year-old pregnant woman at 16 weeks of gestation who presented with prolonged vomiting, altered mental status, nystagmus, and gait ataxia. Magnetic Resonance Imaging of the brain revealed characteristic bilateral thalamic and periaqueductal hyperintensities. Pregnancy increases thiamine requirements, and persistent vomiting in hyperemesis gravidarum can rapidly precipitate thiamine d

Unexpected development of Wernicke's encephalopathy in a patient with non-dialysis-dependent chronic kidney disease.

Wernicke's encephalopathy (WE) is a life-threatening neuropsychiatric syndrome resulting from thiamine deficiency. Alcohol misuse is the main cause. However, non-alcoholic cases, including those associated with hyperemesis gravidarum and cancer, have also been reported. We herein report a case of WE in a patient with G4 stage, non-dialysis-dependent chronic kidney disease (CKD). The patient strongly believed that an extremely-low-protein diet would prevent the development of CKD. His estimated daily thiamine intake (0.37 mg) was less than one-third of the recommended level (1.3 mg). This case

Epidemiological distribution of MRI-based presumptive imaging diagnoses in a large general population of cats with suspected epileptic seizures.

BACKGROUND: The reported prevalence of feline idiopathic epilepsy (IE) and presumptive imaging diagnoses based on magnetic resonance imaging (MRI) varies depending on factors such as differences between primary care versus referral study populations. METHODS: This retrospective, imaging-based study reviewed MRI data from cats with suspected epileptic seizures. Cases were categorised as follows: no lesions detected (IE, referring to cats aged 6 months‒7 years with recurrent epileptic seizures, and epilepsy or epileptic seizure of unknown aetiology [EuA] [i.e., cats not fulfilling IE criteria])

Degradation study of thiamine hydrochloride and analysis of expired ampoules using a novel, validated HPLC-UV method.

Thiamine is a vital vitamin and one-sided diet carries a high risk of deficiency. Supplementation is simple with appropriate pharmaceutical products. Unfortunately, the availability of medicines is still not guaranteed everywhere in the world, while at the same time, perfectly good medicines are being disposed of after their expiry date. For this reason, the shelf-time has often been critically questioned in recent years. Analysing finished pharmaceutical products, stored for decades under uncontrolled conditions, offers a rare insight in real-world stability of pharmaceuticals. Hence, this st

Thiamine deficiency in outpatients at a regional cancer centre in Japan: cross-sectional observational study.

OBJECTIVE: To examine the prevalence of thiamine deficiency and its potential risk factors in cancer outpatients. METHODS: A cross-sectional study of cancer outpatients aged ≥20 years at a regional hospital in Kyoto, Japan (January-February 2022). Thiamine deficiency was defined as ≤23 ng/mL by liquid chromatography-tandem mass spectrometry (LC-MS/MS). Multivariable logistic regression was used to estimate ORs and 95% CIs for potential risk factors for thiamine deficiency. RESULTS: A total of 106 patients were included in this study (mean age 68.3 years, 61% female). Of the 106 patients, 27

Role of Thiamine in Pediatric CHD: Effects on LV Function and MMP-9/TIMP-1.

Congenital Heart Disease (CHD) represents the predominant form of congenital anomaly and occurs in approximately 0.8% of live births. Surgical correction is the definitive method used to treat CHD; however, postoperative complications can increase the risk of mortality and morbidity. Transcatheter closure has emerged as an alternative therapy for some CHD with left-to-right shunts, providing outcomes comparable with those of surgical correction. This narrative review was conducted to synthesize existing literature on the effect of thiamine supplementation in pediatric patients undergoing trans

Alpha-Lipoic Acid and Benfotiamine in Diabetic Peripheral Neuropathy: A Critical Review of Mechanistic Rationale and Clinical Evidence Within a Nutritional Therapeutic Framework.

BACKGROUND: Diabetic peripheral neuropathy (DPN) affects up to 50% of diabetes patients and is driven by hyperglycemia-induced oxidative stress, mitochondrial dysfunction, polyol pathway activation, advanced glycation end-product formation, and inflammation. Current management is largely symptomatic, prompting interest in metabolic/nutritional therapies. This review critically evaluates the mechanistic rationale and clinical evidence for alpha-lipoic acid (ALA) and benfotiamine as adjunctive treatments for DPN. METHODS: A structured narrative review of PubMed/MEDLINE was conducted using prede
