Micronutrient trials in critical illness - new evidence since 2023.

PURPOSE: Numerous trials have tested the impact on outcome of single high to very high doses of micronutrients as adjuvant therapy in severe critical illness and particularly in septic shock. The present review will address the recent knowledge acquired since 2023 including mainly randomized controlled trials (RCTs), but also some quality observational studies. RECENT FINDINGS: Eighteen RCTs and nine meta-analyses were reviewed and globally confirm previous findings. Very high MN dose interventions delivered independent of a predisease deficiency in the early phase are not successful. The mos

Fatty acid profiles recorded in ocean prey and California salmonine eggs reveal maternal ocean diets linked to thiamine deficiency.

Following the principle 'you are what you eat', fatty acid signatures (FASs) of adult fish tissues, when compared with those of potential prey, can link diet to nutritional status. In 2020, thiamine (vitamin B1) deficiency was diagnosed for the first time in multiple anadromous salmon populations in California. However, direct information linking ocean diet to thiamine status in marine-feeding salmonines is typically unavailable. This research aimed to link diet composition and egg total thiamine concentration of anadromous Chinook salmon, coho salmon and steelhead using FAS analysis. From 202

Micronutrient risk with GLP-1 receptor and dual incretin agonists in obesity: Mechanistic pathways, clinical signals, and a monitoring framework.

BACKGROUND: Incretin-based pharmacotherapies, including GLP-1 receptor agonists and dual GIP/GLP-1 receptor agonists, have transformed obesity management by producing substantial weight loss through appetite suppression, reduced energy intake, and gastrointestinal effects. These mechanisms may also modify dietary intake, gastrointestinal physiology, and weight-loss-related metabolic adaptations, raising concern about micronutrient disturbances during long-term treatment. METHODS: This narrative clinical review synthesizes evidence on mechanistic pathways, clinical signals, and monitoring cons

PDC1 deficiency results in 2-deoxyglucose sensitivity through inhibition of Pdc2 activity in yeast.

Pyruvate decarboxylase (PDC) catalyzes the decarboxylation of pyruvate to yield acetaldehyde, using thiamine pyrophosphate as a coenzyme. Saccharomyces cerevisiae has two functional PDCs, Pdc1 and Pdc5, whose reactions constitute the first step in the alcoholic fermentation pathway. PDC1 is abundantly expressed in wild-type cells, whereas PDC5 is markedly repressed when cells are cultured in a glucose medium. PDC5 expression is induced in a Pdc1 deletion mutant, a phenomenon referred to as autoregulation, and this induction depends on the transcriptional regulator Pdc2. Therefore, it has been

Exploring the bioavailability and bioactivity of thiamine versus allithiamine: studies in Drosophila melanogaster and mice.

OBJECTIVE: Thiamine (vitamin B1) is an essential micronutrient involved in carbohydrate metabolism and mitochondrial function. Allithiamine, a lipid-soluble thiamine derivative, has been proposed to exhibit thiamine bioactivity. METHODS: Here, we conducted complementary studies in Drosophila melanogaster and laboratory mice to compare the bioavailability and bioactivity of thiamine and allithiamine. First Drosophila was established as a whole-organism model to assess thiamine activity. Flies were reared on a chemically defined, thiamine-free medium supplemented with thiamine, allithiamine or

Development and Genetic Monitoring of a Putatively Thiamine Deficiency Complex Tolerant Atlantic Salmon Broodstock.

The concept of assisted evolution suggests that hurdles to species restoration in modified environments might be overcome by selective breeding of individuals better-suited to the contemporary evolutionary landscape. Here, we introduce an experimental program aimed at developing an Atlantic salmon broodstock resilient to dietary thiamine deficiency complex (TDC). Dietary TDC is caused by overconsumption of lipid-rich prey items containing thiaminase, leading to insufficient maternal allocation of thiamine to eggs and high offspring mortality. Within Lake Champlain, USA, where TDC is widespread

Impact of thiamin supplementation on thiamin pyrophosphate effect and cardiac function in pediatric heart disease patients on diuretics: a randomized controlled trial.

Thiamin is a water-soluble vitamin essential for energy metabolism. Patients on long-term diuretics, particularly those with heart disease, are at risk of thiamin deficiency (TD) due to increased urinary loss, which may impact cardiac function. We evaluated changes in thiamin pyrophosphate effect (TPPE) values after 4 weeks of thiamin supplementation compared to placebo in pediatric heart disease patients receiving diuretics. The secondary objectives included assessing changes in left ventricular ejection fraction (LVEF) and identifying factors associated with TPPE changes. In this triple-blin

Severe Hypokalemia Complicated by Wernicke's Encephalopathy and Hypoxic-Ischemic Brain Injury in Pregnancy: A Case from a Resource-Limited ICU.

INTRODUCTION: Pregnancy is a metabolically vulnerable state in which electrolyte disturbances, nutritional deficiencies, and hypoxic events can rapidly lead to severe neurological complications. Wernicke's encephalopathy and hypoxic-ischemic brain injury are uncommon but potentially devastating conditions that are frequently underdiagnosed, particularly in resource-limited intensive care settings. To our knowledge, the coexistence of profound hypokalemia, Wernicke's encephalopathy, and hypoxic-ischemic brain injury during pregnancy has been rarely reported, particularly from resource-limited i

Pulmonary arterial hypertension in infancy: A case series.

Thiamine deficiency is a reversible cause of severe pulmonary arterial hypertension (PAH) in early infancy, particularly among exclusively breastfed infants of mothers consuming polished rice-based diets. This retrospective case series (July 2023-July 2025) included infants aged-12 months with echocardiographically confirmed severe PAH and no structural heart disease. Eight exclusively breastfed infants (median age 2.5 months) presented with respiratory distress. All mothers consumed polished rice-based diets. Echocardiography revealed severe PAH with right-sided cardiac dilation. Thiamine the

Status of non-thiamine B-vitamins in children with septic shock from a lower middle-income country: a case-control study.

The role of B-vitamins in septic shock is gaining attention, especially among patients with deficiencies. However, evidence beyond thiamine remains limited. This study aimed to evaluate the status of riboflavin, niacin, pyridoxine, folate, and cobalamin in children with septic shock in a low- and middle-income country (LMIC) setting, and to explore their potential association with patient outcomes. We enrolled 79 consecutive patients (aged 6 months-12 years) with fluid-refractory septic shock from July 2018 to December 2019. We also included 22 sibling controls and 64 external controls from an

Genetically Determined Loss-of-Function of the Organic Cation Transporter OCT1 Is Associated with Lower Liver Fat Content in Humans.

Fatty liver is associated with increased all-cause mortality. Organic cation transporter 1 (OCT1) is a polyspecific hepatic uptake transporter. OCT1 mediates hepatic uptake of vitamin B1 (thiamine); thus, OCT1 deficiency may impair thiamine availability and consequently reduce glucose-derived energy. Consequently, OCT1-knockout mice exhibit significantly reduced liver fat. In 2% of Europeans and White Americans, common genetic variants markedly reduce OCT1 function. In this study, we used these naturally occurring variants to investigate the influences of reduced OCT1 function on liver fat in

Testing-Yield Mismatch in Inpatient Micronutrient Assessment in Inflammatory Bowel Disease: A Real-World Implementation-Gap Analysis.

Background/Objectives: Micronutrient deficiencies are common in inflammatory bowel disease (IBD) and contribute to anemia, impaired wound healing, neurologic injury, and adverse disease outcomes. Major societies, including ESPEN, ACG, and ECCO, recommend nutritional and micronutrient assessment in patients with IBD, yet few studies describe how comprehensively these recommendations are applied during hospitalization. This study aimed to characterize inpatient testing practices for vitamins B1, B6, B12, and zinc in hospitalized IBD patients, quantify deficiency frequency among those tested, and

Micronutrients in adult critically ill patients: lessons learned and the path toward precision.

PURPOSE OF REVIEW: Despite strong biological plausibility, empiric high-dose micronutrient supplementation has failed to improve outcomes in critically ill patients. This review synthesizes evidence from the past 18 months and outlines the necessary transition toward biomarker-guided precision nutrition. RECENT FINDINGS: Recent evidence has further challenged routine high-dose micronutrient therapy. High-dose intravenous vitamin C failed to improve outcomes and was associated with potential harm in patients with sepsis, COVID-19, severe burns, and post-cardiac arrest syndrome. In contrast, a

Mitochondrial dysfunction in sepsis: nutritional strategies for restoring bioenergetic homeostasis.

BACKGROUND: Sepsis is increasingly recognized as a syndrome of maladaptive bioenergetic failure in which mitochondrial dysfunction-rather than being a secondary epiphenomenon-acts as a central driver of immune paralysis, endothelial incoherence, and multiple organ injury. Three interconnected molecular lesions are particularly consequential: impairment of the pyruvate dehydrogenase complex (PDC), excessive mitochondrial reactive oxygen species (mtROS) generation, and defective mitophagy. These mechanisms disrupt substrate oxidation, amplify oxidative injury, and prevent effective organelle tur

Nutritional support for severe malnutrition in newly diagnosed Crohn's disease: a case report.

Crohn's disease (CD) and ulcerative colitis are chronic inflammatory bowel diseases (IBDs) characterized by relapsing exacerbations related to a dysregulated immune response, resulting primarily in gastrointestinal symptoms and also frequently in signs of chronic systemic inflammation. Malnutrition is a possible complication of IBD with a prevalence ranging from 20 to 85%, varying with disease activity, location, and age, and is recognized as having a multifactorial pathogenesis. Malnutrition in patients with IBD should be diagnosed early and treated appropriately because it can heavily affect

Wernicke's Encephalopathy in a Young Non-alcoholic Woman Secondary to Prolonged Reduced Oral Intake: A Case Report.

Wernicke's encephalopathy (WE) is an acute neurological disorder resulting from thiamine deficiency and is classically associated with chronic alcohol misuse. However, it may also occur in non-alcoholic patients with prolonged malnutrition, persistent vomiting, or impaired nutritional intake, making diagnosis particularly challenging when the classical triad of ophthalmoplegia, ataxia, and altered mental status is incomplete or develops gradually. Delayed recognition can result in irreversible neurological injury, whereas early treatment with thiamine is associated with promising outcomes. We
