Chronic oxidative stress leads to the accumulation of reversibly oxidised thiamine derivatives in tissues: effects of thiamine and cholecalciferol.

There is limited data on oxidised thiamine (vitamin B1) derivatives in animal and human tissues. Some studies suggest that oxidative stress (OS) leads to their formation. This study aimed to estimate the proportion of reversibly oxidised (disulfide) thiamine derivatives in the total vitamin B1 pool in rat tissues exposed to various chronic OS forms, and to investigate the possibility of reducing these derivatives to a cyclic form. Rat models of thiamine deficiency and chronic alcoholism were used in experiments. The thiochrome method was used to analyse thiamine, with pretreatment of samples w

Correlates of serum thiamine levels among acute decompensated heart failure patients.

BACKGROUND: Acute decompensated heart failure (ADHF) contributes adversely to health-related outcomes. Anorexia, early satiety, high metabolic rates, malnutrition, advanced age, and diuretic use characterise ADHF. This necessitated evaluating the correlates of low serum thiamine levels among ADHF patients in our facility. METHODS: This was a case-control study. One hundred and ten participants (55 AHF patients with 55 age and sex-matched healthy controls) were recruited consecutively. All participants had a physical examination, resting 12-lead ECG, 2D transthoracic echocardiography and basel

Clioquinol inactivates thiamine pyrophosphate by increasing cellular oxidative stress.

Clioquinol (CQ), a prescribed oral antibiotic, was withdrawn from use following its association with the incidence of subacute myelo-optic neuropathy (SMON) in Japan. The neurotoxicity associated with CQ may be linked to thiamine deficiency. However, their relationship and underlying mechanisms remain unclear. In this study, we identified thiamine pyrophosphate (TPP) as a key metabolite affected by CQ via metabolomics analysis. Analysis of related thiamine transport, TPP synthesis, and oxidative stress pathways revealed that CQ promoted TPP inactivation through oxidative modification. The Seah

Progressive nonambulatory tetraparesis and cerebellar signs in a 6-year-old spayed female Shiba Inu dog with normal brain MRI.

A 6-year-old spayed female Shiba Inu dog was evaluated because of a 2-week history of progressive paraparesis that advanced to nonambulatory tetraparesis with cerebellar signs. The dog was fed a nutritionally unbalanced, commercial, freeze-dried, raw, fish-based diet. Thoracolumbar and brain MRI revealed no abnormalities, and CSF analysis showed albuminocytologic dissociation. Based on dietary history, clinical progression, and response to therapy, a presumptive diagnosis of thiamine deficiency was made. Treatment consisted of dietary modification and oral thiamine (vitamin B1) supplementation

Case Report: a case of prolonged post-traumatic duodenal obstruction complicated by traumatic pancreatitis and Wernicke's encephalopathy: a multidisciplinary challenge.

BACKGROUND: Post-traumatic gastrointestinal injuries, especially those involving the duodenum and proximal jejunum, present significant management challenges. Complications such as retroperitoneal abscess, high intestinal obstruction, and secondary pancreatitis can lead to prolonged fasting and severe nutritional deficiencies. We report a complex case where a patient developed a rare triad of post-operative combined abscess (retroperitoneal abscess and intra-abdominal abscess)-induced high intestinal obstruction, traumatic pancreatitis, and subsequent Wernicke's encephalopathy (WE), highlighti

Effects of Consumption of Fortified Complementary Food and Promotion of Responsive Feeding on the Hemoglobin and Micronutrient Status of Young Children in a Controlled, Cluster Randomized Trial in the District of Amparafaravola in Madagascar.

BACKGROUND: Micronutrient deficiencies are a serious public health problem in early childhood in Madagascar. OBJECTIVES: We aimed to evaluate the effects of responsive feeding (RF) and the consumption of fortified complementary foods (FCFs), either alone or combined, on hemoglobin (Hb) concentrations and micronutrient status of young children. METHODS: A 2 × 2 factorial randomized controlled intervention trial in infants (n = 706), recruited at 6-7 mo of age, was conducted. The control group received counseling on water, sanitation, and hygiene (WASH) practices, whereas the 3 intervention gr

Thiamine deficiency in patients with chronic migraine: A case-control study.

OBJECTIVES/BACKGROUND: Thiamine deficiency is well recognized in several neurological disorders, and subclinical deficiency has been associated with nonspecific symptoms, including headache. However, thiamine deficiency is not currently recognized as a cause of headache in standard headache classifications. Chronic migraine (CM) is often accompanied by symptoms such as nausea, vomiting, and reduced appetite, which may influence nutritional intake and micronutrient status, including thiamine depletion. These factors raise the possibility of an interaction between migraine and thiamine status. O

Motor and cognitive development is associated with anthropometry and severe zinc deficiency in infants, in rural Madagascar.

Globally, more than 250 million children under five fail to reach their developmental potential, with malnutrition representing a major contributing factor. In Madagascar, as in many low- and middle-income countries, malnutrition remains a serious public health concern. Although the high prevalence of micronutrient deficiencies, stunting, and wasting is well documented, their consequences for early motor and cognitive development, which can be assessed reliably by six months, are less studied. In 2019, a baseline data collection prior to intervention as part of a randomized controlled trial co

Severe Perioperative Lactic Acidosis and Hyperglycemia Responsive to Thiamine in a Patient With Prior Semaglutide Use and Restrictive Dieting: A Case Report.

Thiamine deficiency is a rare but potentially fatal cause of type B lactic acidosis, classically associated with alcoholism or severe malnutrition. Restrictive dieting and weight-loss regimens, including glucagon-like peptide-1 (GLP-1) receptor agonist use, may unmask thiamine deficiency under perioperative stress. We report a young, nondiabetic woman on a restrictive diet who developed severe lactic acidosis and hyperglycemia after total abdominal hysterectomy, with worsening lactate despite hemodynamic stability and adequate hydration. Empiric intravenous thiamine produced rapid normalizatio

Severe thiamine deficiency in Baltic salmon coincides with low wild recruitment, increasingly so at long migration distances.

Thiamine deficiency is a condition implicated as a driver of population declines across continents and taxa. In salmonids, symptoms of this deficiency are often observed in compensatory hatcheries, where large parts of the clutches display severe neurological disorders and die. However, whether hatchery-quantified thiamine deficiency is associated with reduced wild salmonid recruitment has rarely been tested. Here, we combine two long-term monitoring datasets: (i) a 30+ year time series of thiamine deficiency status in eight Atlantic salmon (Salmo salar) rivers draining into the Baltic Sea (ca

The risk of thiamine deficiency in common eider ducklings increases when females consume large prey.

Episodic mass mortality due to thiamine (vitamin B1) deficiency is a major threat to marine top-consumers worldwide. Recent reports have shown that thiamine deficiency can lead to high mortality rates of common eider (Somateria mollissima) ducklings in the Baltic population. It is unclear whether the drastic reduction in offspring recruitment is linked to the changes in egg thiamine levels. This would have an additional negative impact on the Baltic population of the common eider, which is mainly driven by increased predation pressure. As thiamine concentration decreases with the size of organ

Assessment of thiamine status in humans.

Thiamine (vitamin B1) is an essential nutrient with established adverse health consequences associated with acute deficiency. Emerging evidence also suggests long-term neurocognitive deficits due to sub-clinical deficiency in early life, and there is a growing appreciation of a potential role for thiamine in Alzheimer's disease. Therefore, the assessment of thiamine biomarkers, particularly in large, population-based studies, is critical to grasp the true scope of this public health problem. The most reliable biomarkers of thiamine status are whole blood concentrations of thiamine diphosphate

Wernicke's encephalopathy unmasking progressive supranuclear palsy and Alzheimer's pathology: a diagnostic challenge in an older adult.

This case highlights the rare coexistence of Wernicke's encephalopathy, probable progressive supranuclear palsy and probable Alzheimer's disease. The patient presented with acute confusion, repeated falls, postural instability, bradyphrenia, apathy and vertical gaze palsy. Clinical examination raised suspicion of overlapping neurological syndromes. Complementary investigations included brain MRI which revealed periventricular hyperintense regions on T2 and on fluid-attenuated inversion recovery (FLAIR) sequences, suggestive of Wernicke's encephalopathy. A subsequent ioflupane (123I) single-pho

[Wernicke Encephalopathy In Refeeding Syndrome].

This report describes a patient with a history of recurrent depressive disorder and restrictive anorexia nervosa. In the context of a depressive episode, increased appetite led to refeeding syndrome with consecutive Wernicke encephalopathy following previous food deprivation due to anorexia. The pathophysiology is based on increased thiamine consumption during a sudden transition to an anabolic metabolic state. Wernicke's encephalopathy should be considered even in cases of nonspecific symptoms associated with any type of malnutrition, especially since the encephalopathy can be triggered by re

Zinc-associated Copper Deficiency Myelopathy: A Case Report.

OBJECTIVE: Copper deficiency myelopathy is an uncommon but potentially reversible neurologic disorder characterized by sensory ataxia, gait impairment, and dorsal column dysfunction. Acquired copper deficiency most commonly results from malabsorption, prior gastrointestinal surgery, or excess zinc exposure. Zinc induces intestinal metallothionein, which preferentially binds copper and reduces its systemic absorption, potentially leading to hypocupremia and neurologic dysfunction. Although zinc-containing nutritional supplements are widely used, clinically significant neurologic complications r

Alcohol-Related Metabolic Emergencies.

Patients with alcohol use disorders are commonly identified and managed in the emergency department. Although the alcohol-intoxicated patient has a high risk for significant injury and diseases, the majority will be allowed to sober in the emergency department and can be discharged without incident. However, there are metabolic derangements in these patients, such as alcoholic ketoacidosis, Wernicke-Korsakoff, and potomania that very commonly present similar to intoxication and can be misdiagnosed by emergency clinicians.

Thiamine for altered mental status: a high-value strategy beyond alcohol.

BACKGROUND: Altered mental status (AMS) prompts rapid diagnostic evaluation in hospitalized patients, yet a low-cost, reversible cause - thiamine deficiency - often receives inconsistent attention outside alcohol-associated contexts. Although classically linked to alcohol misuse, contemporary evidence links thiamine depletion to a broad range of medical, surgical, oncologic, obstetric, and critically ill populations. DISCUSSION: In these settings, AMS may be the earliest clinical manifestation, while standard diagnostic tools such as MRI and serum thiamine levels frequently lack sensitivity e

Wernicke Encephalopathy Associated with Malabsorption in Degos Disease.

A 40-year-old female with systemic Degos disease presented with 4 days of confusion, somnolence, and ataxia. She was dependent on total parenteral nutrition (TPN) following ileocecectomy for Degos-related gastrointestinal complications and had recently transitioned from intravenous (IV) to oral thiamine supplementation. Although central nervous system involvement of Degos was initially suspected, magnetic resonance imaging findings, low thiamine levels, and rapid improvement with IV thiamine confirmed Wernicke's encephalopathy. This case highlights the importance of recognizing nutritional def
