Vitamin B12 in adults on vegetarian and vegan diets.

Plant-based dietary patterns are beneficial for cardiometabolic health and are associated with a lower risk of multiple diseases, including ischemic heart disease and several types of cancer. For these reasons, they are increasingly recommended by the main medical societies and dietary guidelines. However, if not properly planned, these dietary patterns can lead to insufficient intake of certain nutrients, including vitamin B12. Specifically, people who follow vegetarian or vegan diets usually have a low or no intake of vitamin B12, since the dietary sources of this vitamin are foods of animal

The impact of vitamin B12 deficiency on urinary profile of de novo purine synthesis intermediates: consequences of the methylfolate trap.

Vitamin B12 (B12) deficiency may cause severe clinical complications, partly attributable to the "methylfolate trap". When B12-dependent methionine synthase activity is impaired, 5-methyltetrahydrofolate (5-MTHF) becomes trapped because it cannot be used in the methionine synthase reaction or reconverted to 5,10-methylenetetrahydrofolate. This reduces the availability of folate cofactors, including tetrahydrofolate (THF), 5,10-methylene-THF, and 10-formyl-THF required for histidine catabolism, pyrimidine metabolism, and de novo purine synthesis (DNPS). We investigated whether this mechanism is

Clioquinol inactivates thiamine pyrophosphate by increasing cellular oxidative stress.

Clioquinol (CQ), a prescribed oral antibiotic, was withdrawn from use following its association with the incidence of subacute myelo-optic neuropathy (SMON) in Japan. The neurotoxicity associated with CQ may be linked to thiamine deficiency. However, their relationship and underlying mechanisms remain unclear. In this study, we identified thiamine pyrophosphate (TPP) as a key metabolite affected by CQ via metabolomics analysis. Analysis of related thiamine transport, TPP synthesis, and oxidative stress pathways revealed that CQ promoted TPP inactivation through oxidative modification. The Seah

Delusional parasitosis with shared psychotic features secondary to vitamin B12 deficiency and mild cognitive impairment: a case report.

BACKGROUND: Visual illusions are misperceptions of real external stimuli and, when persistent, may indicate underlying neurological or metabolic disorders. Delusional parasitosis is characterized by a fixed belief of infestation without medical evidence and may occur secondary to medical conditions. However, the relationship between visual illusion-related perceptual abnormalities and delusion formation, particularly in reversible metabolic states such as vitamin B12 deficiency, remains unclear. CASE PRESENTATION: An 80-year-old man presented with a persistent belief that mites infested his b

Apigenin ameliorates methionine-choline deficient diet-induced metabolic-associated fatty liver disease through modulation of gut microbiota-liver axis.

INTRODUCTION: Apigenin, a naturally occurring flavone widely present in the plant kingdom, possesses antioxidant, anti-inflammatory, and metabolic regulatory activities. Metabolic-associated fatty liver disease (MAFLD), a chronic liver disorder driven by metabolic stress, currently lacks effective pharmacological therapies. Given these pharmacological properties, apigenin may represent a promising candidate for MAFLD intervention. OBJECTIVE: This study aimed to evaluate the therapeutic potential of apigenin in MAFLD and to elucidate the underlying mechanisms linking the gut microbiota and hos

Progressive nonambulatory tetraparesis and cerebellar signs in a 6-year-old spayed female Shiba Inu dog with normal brain MRI.

A 6-year-old spayed female Shiba Inu dog was evaluated because of a 2-week history of progressive paraparesis that advanced to nonambulatory tetraparesis with cerebellar signs. The dog was fed a nutritionally unbalanced, commercial, freeze-dried, raw, fish-based diet. Thoracolumbar and brain MRI revealed no abnormalities, and CSF analysis showed albuminocytologic dissociation. Based on dietary history, clinical progression, and response to therapy, a presumptive diagnosis of thiamine deficiency was made. Treatment consisted of dietary modification and oral thiamine (vitamin B1) supplementation

Non-canonical ceRNA-independent mechanism of the lncRNA-H19/miR-212-5p axis targeting KLF4 in Hcy-triggered VSMCs dysfunction and atherogenesis.

Long non-coding RNAs (lncRNAs) and microRNAs (miRNAs) act as critical post-transcriptional regulators in the pathogenesis of atherosclerosis (As). However, the non-canonical regulatory mechanisms of lncRNAs and miRNAs, independent of the competing endogenous RNA (ceRNA) pathway, especially in homocysteine (Hcy)-induced vascular smooth muscle cells (VSMCs) dysfunction-a key event in As progression-remain largely unclear. In the present study, high-throughput small RNA sequencing, combined with bioinformatics analysis, identified miR-212-5p as a pivotal mediator of Hcy-induced VSMCs hyperprolife

Comparative Efficacy of Intranasal, Intramuscular, and Intravenous Vitamin B12 Therapy for Hematological Recovery in Vitamin B12 Deficiency Anemia: A Randomized Controlled Trial.

Vitamin B12 deficiency causes megaloblastic anemia and ineffective hematopoiesis. While intramuscular administration remains standard, intravenous and intranasal alternatives are increasingly used. Rigorous comparative data on hematological efficacy across routes remain limited. We thus aimed to compare intranasal, intramuscular, and intravenous vitamin B12 therapy for hematological recovery in vitamin B12 deficiency anemia, and to identify independent predictors of treatment response. In this prospective randomized controlled trial (IEC/2024/450; CTRI/2024/10/075521), 153 adults with vitamin

Paternal folate deficiency induces hepatic insulin resistance in offspring mice.

BACKGROUND: Folate (vitamin B9) is a water-soluble vitamin necessary for one-carbon metabolism, supporting the synthesis, repair, and methylation of DNA. While maternal folate status is well-studied for its role in fetal development and metabolic programming, the impact of inadequate folate intake in males on offspring development and metabolic diseases remains poorly understood. This study investigates the effects of folate deficiency in male parents on developing hepatic insulin resistance in offspring, focusing on molecular and metabolic disruptions within the liver. METHODS: Three-week-ol

Expert Perspectives on Diagnosis and Management of Vitamin B12 Deficiency in Adults.

There is no universally accepted definition of vitamin B12 deficiency despite the severity of its medical consequences if not properly diagnosed and treated. Vitamin B12 deficiency can arise either from inadequate dietary intake, particularly among vegans/vegetarians, or various causes of impaired absorption or availability. The latter include Helicobacter pylori and autoimmune gastritis, other causes of malabsorption, and drug side effects. Vitamin B12 deficiency can display diverse clinical signs and symptoms, including hematological, neurological, psychiatric, and thromboembolic manifestati

Modulating One-Carbon Metabolism with B-Vitamins to Protect the Retinal Barrier and Prevent Retinal Degeneration.

BACKGROUND/OBJECTIVES: Vitamin B12 deficiency is increasingly recognized as a contributor in both vascular and neurodegenerative aging-related disorders. Its deficiency disrupts one-carbon metabolism, leading to impaired homocysteine (Hcy) cycling. Elevated Hcy is a well-established risk factor for vascular dysfunction. Previously, we established that elevated Hcy contributes to aging retinal diseases and plays a central role in blood retinal barrier (BRB) dysfunction. Building on this foundation, the present study examines how B-vitamin deficiency disrupts one-carbon metabolism and whether re
