Choline and Choline-Related Metabolites in Pediatric Short Bowel Syndrome.

Background: Choline is an essential nutrient crucial for liver function. It is required for bile and lipoprotein secretion and the synthesis of both phosphatidylcholine (PC) to ensure tissue homeostasis and betaine as a methyl donor. Choline deficiency has been implicated in the pathogenesis of intestinal failure-associated liver disease (IFALD), with the strongest evidence for its contribution to hepatic steatosis in patients with short bowel syndrome (SBS). Contributing factors are (1) an impaired recycling of choline from bile PC, leading to fecal choline losses; (2) small bowel bacterial o

Vitamin B12 Supplementation: Is More Always Better?

Vitamin B12 supplementation among people without proven deficiency has become popularized, driven by perceptions of (i) frequent underdiagnosis of deficiency, (ii) promotion as a natural enhancer of well-being, and (iii) a favourable safety profile. Here, we examine whether these claims align with current evidence. We present guidance from major health authorities, which advises against routine testing in asymptomatic individuals without risk factors. The prevalence of B12 deficiency varies greatly, mainly because definitions of B12 deficiency are not standardized and may include clinical, bio

Caffeic Acid Ameliorates l-Methionine-Induced Hyperhomocysteinemia, Leading to Decreased Adult Neurogenesis and Oxidative Stress Status in Adult Rats.

Chronic l-methionine (l-met) treatment induces hyperhomocysteinemia, which impairs neurogenesis and memory by increasing oxidative stress. Caffeic acid (CA) possesses antioxidant properties that reduce brain oxidative damage and promote neurogenesis. This study investigated the neuroprotective effects of caffeic acid against the impact of l-met on hippocampal neurogenesis. Forty male Sprague-Dawley rats were randomly assigned to four groups (n = 10 per group): Control, l-met, CA, and CA+l-met. Rats received daily oral gavage of l-met (1.7 g/kg) and/or CA (40 mg/kg) for 28 days. The results dem

Predictors of functional disability in nitrous oxide induced myeloneuropathy: an observational study.

Recreational nitrous oxide (N₂O) use is an emerging cause of severe neurotoxicity due to vitamin B12 inactivation. This study analyzes the relationship between initial clinical, biochemical and neuroimaging findings and functional disability at discharge in a cohort of 10 patients recruited between 2024 and 2025. All patients presented with hyperhomocysteinemia, while only 40% showed vitamin B12 hypovitaminosis, confirming hyperhomocysteinemia as a more sensitive diagnostic marker. The most common clinical manifestations were acroparesthesia (90%) and unstable gait (80%). Electrodiagnostic tes

Homocysteine and S-adenosyl-L-homocysteine impair development and methylation in yeast and flies.

S-adenosyl-L-homocysteine (SAH), the product inhibitor of S-adenosyl-L-methionine-dependent methyltransferases, and its degradation product homocysteine (Hcy) are evolutionarily conserved master regulators of methylation metabolism, which is mediated by more than 200 methyltransferases in humans. Hyperhomocysteinemia (HHcy), characterized by elevated levels of Hcy in the blood, is an independent risk factor for atherosclerosis, a strong predictor of cardiovascular mortality and can cause associated pathology by interfering with methylation-dependent processes. Here, we developed a Drosophila m

Role of homocysteine in renal tubular epithelial-mesenchymal transition: N-homocysteinylation of β-catenin impairs FUNDC1-dependent autophagy and mitochondrial quality control.

Given the nephrotoxicity of hyperhomocysteinemia and the key role of mitophagy in the homeostasis of kidney, this study investigated the β-catenin/FUNDC1/mitochondrial quality control axis to elucidate the mechanisms underlying homocysteine-induced renal tubular fibrosis. A mouse model of hyperhomocysteinemia was established via continuous supplementation of homocysteine. Western blot quantified key proteins associated with autophagy (FUNDC1, LC3, p62), fibrosis (fibronectin, α-SMA, Snail1), β-catenin signaling, and kidney injury (KIM-1). Immunostaining assessed renal spatial distribution of β

Fenofibrate attenuates hyperhomocysteinemia-potentiated thrombosis by restoring platelet fatty acid β-oxidation.

Hyperhomocysteinemia (HHcy) is an independent risk factor for thrombotic cardiovascular events. We previously demonstrated that homocysteine (Hcy) amplifies platelet activation by promoting membrane remodeling and enhancing signaling through surface platforms such as integrin αIIbβ3 and G protein-coupled receptors. However, the mechanisms by which Hcy remodels platelet membrane lipid metabolism remain poorly understood. Here, using an integrated proteomic and lipidomic approach, we showed that Hcy disrupted platelet lipid homeostasis by impairing fatty acid β-oxidation (FAO), a metabolic pathw

Does preoperative homocysteine level influence the kidney functions after coronary artery bypass grafting?

BACKGROUND: Elevated homocysteine (Hcy) has been linked to endothelial dysfunction and adverse cardiovascular outcomes. However, its specific role as a risk factor for postoperative complications, especially acute kidney injury (AKI), in patients undergoing coronary artery bypass grafting (CABG), remains insufficiently defined. What's more we took into consideration presence of diabetes mellitus as an independent risk factor of AKI or the exacerbation of pre-existing chronic kidney disease (CKD), what can modify the prevalence of this complication. METHODS: We conducted a prospective analysis

First Revision of the Guidelines for the Diagnosis and Management of Remethylation Disorders.

This guideline summarizes diagnostic and therapeutic approaches based on a systematic literature review and evidence evaluation using the GRADE methodology. Given the limited high-quality data, expert consensus was additionally obtained through a modified Delphi process. Remethylation disorders are rare inherited conditions that disrupt the methionine-homocysteine cycle and consecutively impair essential methylation dependent metabolic pathways. Remethylation disorders are caused by defects in the cobalamin or folate metabolism. The disorders typically result in elevated homocysteine and often

An Amino Acid with Killer Instincts: Pulmonary Embolism Secondary to Hyperhomocysteinemia.

Pulmonary embolism (PE) is a life-threatening manifestation of venous thromboembolism, typically associated with identifiable risk factors such as immobilization, surgery, malignancy, or trauma. However, in young individuals without apparent triggers, underlying thrombophilic states must be investigated. Hyperhomocysteinemia is an often overlooked yet reversible metabolic risk factor contributing to thrombogenesis. We report a 31-year-old male electrician presenting with acute-onset breathlessness, pleuritic chest pain, and cough. Clinical evaluation revealed tachypnea, tachycardia, and hypoxe

Diagnostic challenge of high vitamin B12 levels: a case report and review of the literature.

BACKGROUND: Vitamin B12 (cobalamin) deficiency is a well-documented clinical condition, with several causes and important consequences. High serum B12 level (hypercobalaminemia) is less common and not easily explained, being caused by different conditions, such as blood disorders, autoimmune diseases, and solid tumors. Moreover, cases of increased B12 levels due to the presence of polymers formed by vitamin B12 and other proteins have been described to show falsely elevated serum levels when measured by conventional methods. CASE PRESENTATION: A 47-year-old white Italian man presented to an i

Plasma homocysteine and Endothelin-1 dysregulation: Insights into retinal endothelial dysfunction.

Hyperhomocysteinemia (HHcy) is increasingly recognised as a contributor to vascular dysfunction; however, its role in retinal disease remains incompletely defined. This review provides a structured, mechanism-focused synthesis of experimental and clinical evidence examining the interaction between homocysteine, endothelin-1 (ET-1), and vascular endothelial growth factor (VEGF) signalling in the context of retinal endothelial dysfunction. HHcy promotes oxidative and endoplasmic reticulum stress, leading to nitric oxide depletion, endothelial nitric oxide synthase uncoupling, and upregulation of

Oral vitamin B₁₂ after total gastrectomy for gastric cancer: mapping the evidence.

BACKGROUND: Patients who undergo total gastrectomy lose the ability to produce intrinsic factor and require lifelong supplementation to prevent vitamin B12 deficiency. While oral B12 has emerged as a potential alternative to parenteral therapy, data remain limited, and most clinicians still favor intramuscular injections. This scoping review aimed to map the current evidence on the efficacy, safety, and implementation of oral vitamin B12 supplementation in patients after total gastrectomy for gastric cancer. METHODS: We included peer-reviewed studies reporting outcomes of oral vitamin B12 sup
