Ascorbate deficiency increases quiescence and self-renewal in hematopoietic stem cells and multipotent progenitors.

Ascorbate (vitamin C) limits hematopoietic stem cell (HSC) function and suppresses leukemia development, partly by promoting the function of the Tet2 tumor suppressor. In humans, ascorbate is obtained from the diet, whereas in mice, it is synthesized in the liver. In this study, we show that deletion of the Slc23a2 ascorbate transporter from hematopoietic cells depleted ascorbate to undetectable levels in HSCs and multipotent hematopoietic progenitors (MPPs) without altering the plasma ascorbate levels. Slc23a2 deficiency increased HSC reconstituting potential and self-renewal potential upon t

Gene-dose effect of the glutathione biosynthesis gene on ascorbate deficiency in mice.

Glutathione (GSH) and ascorbate (vitamin C) are key antioxidants with well-established biochemical and clinical interplay in protecting against oxidative stress. Glutamate-cysteine ligase is the rate-limiting enzyme in GSH biosynthesis; its modifier subunit (GCLM) regulates tissue GSH levels. L-gulono-γ-lactone oxidase (GULO) catalyzes a critical step in ascorbate biosynthesis; Gulo-knockout (GuloKO) mice, like humans, require dietary vitamin C. Previous work using double-knockout (GclmKO/GuloKO) mice revealed an essential role of GSH-ascorbate interaction in brain function. Herein, we report

Exploring barriers and enablers of vitamin C nutrition care in General Medicine patients: A mixed methods study.

BACKGROUND: Vitamin C deficiency (VCD) is common in adult inpatients in high-income countries and is associated with moderate to severe frailty, cognitive impairment, and scurvy signs and symptoms. However, clinical practice associated with vitamin C status has not been explored in this setting. This study aims to describe current vitamin C clinical practices and explore barriers and enablers that influence nutrition care delivered to adult hospitalized patients within routine clinical practice. METHOD: This study was conducted across General Medicine units at a tertiary health service in Mel

[Vitamin C in the modulation of skin aging: molecular mechanisms and innovative delivery strategies.].

Vitamin C (ascorbic acid) is a key nutrient whose multifunctionality in anti-aging processes remains a subject of intensive research. The relevance of this study stems from the need to systematize data on its dual role (antioxidant/pro-oxidant), mechanisms of action on collagenogenesis, autophagy, melanogenesis, and epigenetic regulation, as well as the development of strategies to overcome its deficiency in the skin-an organ critically vulnerable to age-related and photo-induced changes. The article comprehensively analyzes the causes of localized vitamin C deficiency in cutaneous tissues, wh

Are the UK's vitamin C recommendations evidence-based? A critical comment.

There is substantial international variation in recommended vitamin C intake levels. In the USA, the recommendation is 90 mg/d for men and 75 mg/d for women, while in the UK, the current recommendation – established in 1991 – is only 40 mg/d for adults. This UK level was based on the 1953 Sheffield study, which found that 10 mg/d prevents scurvy, with 40 mg/d chosen as the recommended level for yielding somewhat higher plasma levels. In this commentary, we argue that the UK recommendation overlooked key evidence available at the time. Specifically, at least six controlled trials published befo

A Guinea Pig Model of Pediatric Metabolic Dysfunction-Associated Steatohepatitis: Poor Vitamin C Status May Advance Disease.

Background/Objectives: Children and teenagers display a distinct metabolic dysfunction-associated steatohepatitis (MASH) phenotype, yet studies of childhood MASH are scarce and validated animal models lacking, limiting the development of treatments. Poor vitamin C (VitC) status may affect MASH progression and often co-occurs with high-fat diets and related metabolic imbalances. As a regulator of DNA methylation, poor VitC status may further contribute to MASH by regulating gene expression This study investigated guinea pigs-a species that, like humans, depends on vitC in the diet-as a model of

Curious case of severe scurvy.

Scurvy, the clinical manifestation of vitamin C deficiency, exhibits the characteristic cutaneous findings of perifollicular petechiae, corkscrew hairs, gingivitis, swollen gums, and easy bruising. Syncope is a less reported presenting symptom. Although considered a rare nutritional deficiency not commonly seen in the developed world, it should remain on the differential for those with a history of malnutrition or malabsorption. We present a challenging and severe case of scurvy in a patient secondary to malnutrition and possible undiagnosed Crohn's disease. Recognizing classic dermatologic se

Vitamin C Deficiency Megaloblastic Anemia Mimicking Hemolytic Anemia.

A 31-year-old female presented at our hospital with fatigue and subcutaneous bleeding. Blood tests revealed hemolytic anemia. The patient had Coombs-negative normocytic anemia and thrombocytopenia; therefore, thrombotic thrombocytopenic purpura was ruled out. On day two, we noticed that the eating habits of the patient, hypersegmented neutrophils, and megaloblastic changes suggested malnutrition; therefore, we initiated vitamin supplementation. On day six, the vitamin C levels were <0.2 μg/mL, and the patient was therefore diagnosed with scurvy. Scurvy can mimic hemolytic anemia by causing nor

Pulmonary hypertension secondary to vitamin C deficiency: A case report.

Scurvy is a rare disease caused by exogenous ascorbic acid deficiency. It should be considered in atrisk groups, such as patients with neurodevelopmental disorders who present restrictive diets due to food selectivity. Although pulmonary hypertension associated with vitamin C deficiency is extremely rare, its occurrence is possible. Signs and symptoms such as edema, tachycardia, palpitations, and dyspnea should raise suspicion about the diagnosis. In most cases, this condition is transient and can be reversed with early diagnosis and adequate supplementation with ascorbic acid. We present a ca

Ascorbic acid deficiency promotes metabolic remodeling and pulmonary fibrosis that leads to respiratory failure in Sod1 and Akr1a double-knockout mice.

We recently reported that mice with a double knockout (DKO) of Sod1 encoding superoxide dismutase 1 (SOD1) and Akr1a encoding aldehyde reductase survived more than one year when supplemented with ascorbic acid (Asc) (1.5 mg/ml in drinking water), and that the withdrawal of Asc resulted in premature death in only two weeks due to oxidative damage-associated pneumonia. SOD1 is known to disable the radical electrons of superoxide, which suppresses the subsequent formation of highly reactive oxygen species (ROS). Akr1a encodes aldehyde reductase, which catalyzes the biosynthesis of Asc, which is a

When a diet is followed too strictly. Scurvy - An old disease in a modern gut: A case report.

RATIONALE: This case highlights the importance of considering a wide range of possible diagnoses when faced with unexplained hemorrhagic symptoms. When standard investigations fail to identify a clear cause, it is essential to conduct a detailed dietary history. This can lead to the diagnosis of scurvy, a reversible vitamin C deficiency that is often overlooked in populations at risk. PATIENT CONCERNS: The patient is a 55-year-old man with well-controlled Crohn disease who presented with unexplained bilateral leg pain and extensive hematomas on his lower limbs, significantly affecting his mob

Vitamin C deficiency in allogeneic hematopoietic cell transplantation recipients in the peri-transplant period.

PURPOSE: Hypovitaminosis C is observed in patients with hematologic malignancy, including those receiving hematopoietic cell transplantation (HCT). This study examines longitudinal vitamin C levels and inflammatory markers in patients receiving allogeneic HCT and determines the impact of treated hypovitaminosis C prior to conditioning on clinical outcomes, including mucositis. METHODS: Adults were prospectively enrolled at the time of admission for allogeneic HCT. Clinical outcomes were compared between patients with normal vitamin C, hypovitaminosis C, and indeterminate vitamin C due to acut

Vitamin C status across the spectrum of chronic kidney disease and healthy controls: a cross-sectional study.

BACKGROUND: Patients with chronic kidney disease (CKD) are at risk for vitamin C (VitC) deficiency. OBJECTIVES: We aimed to investigate VitC status and its determinants across the spectrum of CKD and healthy individuals. METHODS: In this cross-sectional study, we measured plasma VitC concentrations (<10 μmol/L = deficient, 10‒35 μmol/L = inadequate) in 62 individuals on dialysis, 41 with CKD stage 4/5, 42 kidney transplant recipients (KTRs) (together: CKD population), and in 447 living kidney donors, and 385 healthy controls (together: healthy population). Impaired healing following implant placement surgery: A case report of a modern-day manifestation of scurvy.

BACKGROUND: Delayed wound healing following implant placement surgery is an uncommon complication, usually attributed to systemic (smoking and alcohol), local (severely inflamed tissues), or technical (poor suturing) factors. However, delayed wound healing after implant placement caused by severe hypovitaminosis C has not been previously documented. METHODS: This report describes the case of a 40-year-old systemically healthy male who developed delayed wound healing after implant placement, despite having quit smoking 10 months prior to the uneventfully completed surgery and having received s
