Serum Vitamin D Levels at Birth Are Not Associated With the Formation of Interglobular Dentin in Deciduous Teeth of Modern New Zealand Children.

Interglobular dentin (IGD) has been used as a biomarker for vitamin D deficiency (VDD) in past populations. However, an evidence-based threshold for the levels of serum vitamin D associated with IGD occurrence has not been established. Here, we explore the association between perinatal serum vitamin D levels, presence of IGD, and biological sex in the deciduous teeth of modern children from Otago, New Zealand. Ground sections were created from the deciduous central or lateral mandibular incisors of 54 five to 6-year-old children (27 M, 27 F) with known perinatal blood vitamin D levels. Vitamin

[Reducing inappropriate vitamin D testing: an overview].

25-(OH)-vitamin D testing is an example of low-value care whose prevalence has increased dramatically in recent years. In Switzerland, testing rates doubled in less than a decade, resulting in annual costs of over 100 million Swiss francs, without any demonstrated clinical benefit. This article provides an overview of various national and international approaches aimed at reducing low-value care and overtesting. The analysis compares the effectiveness of informative approaches (smarter medicine - Choosing Wisely), decision support approaches (clinical alerts), restrictive approaches (reimburse

Growth Profile and Endocrine Dysfunction in Children Aged 2-10 Years With Transfusion-Dependent β-Thalassemia Major.

Background Advances in regular blood transfusion and iron chelation therapy have markedly improved survival in children with transfusion-dependent β-thalassemia major (TDT). As survival has increased, growth impairment and endocrine disorders related to chronic iron overload have become increasingly important causes of long-term morbidity. This study examined the prevalence of endocrine abnormalities and their relationship with growth parameters and iron overload in children with TDT. Methods A hospital-based cross-sectional study was conducted among 150 children aged 2-10 years with TDT atten

Association between serum vitamin D levels and pediatric distal radius fractures: a case-control study.

This study evaluated whether serum vitamin D levels are associated with distal radius fractures in children. In this case-control study, pediatric patients with confirmed distal radius fractures (cases) were compared to children visiting an orthopedic clinic for routine checks or minor muscular pain (controls). Serum vitamin D, phosphorus, and calcium levels were analyzed. We enrolled 98 children (mean age: 8.2 ± 2.6 years). Overall, 87.8% had serum 25-hydroxyvitamin D [25(OH)D] concentrations below 30 ng/ml, with a cohort mean of 22.04 ± 7.35 ng/ml. No statistically significant differences we

Vitamin D Supplementation in Granulomatous Disease: Why Individualized Assessment May Be Safer Than Universal Thresholds.

Vitamin D deficiency is commonly identified and treated according to serum 25-hydroxyvitamin D (25(OH)D) concentrations. In granulomatous disease, however, this approach may be insufficient because activated macrophages within granulomatous tissue convert 25(OH)D to the active metabolite 1,25-dihydroxyvitamin D (1,25(OH)2D) through extrarenal 1alpha-hydroxylase activity. A patient may therefore have a low 25(OH)D concentration while simultaneously having preserved or increased active vitamin D activity, creating a risk of hypercalcemia or hypercalciuria when vitamin D is supplemented. This edi

The Role of Vitamin D in Uveitis: Immunological Mechanisms, Clinical Evidence, and Therapeutic Potential.

Uveitis comprises a heterogeneous group of intraocular inflammatory disorders and remains a significant cause of visual impairment worldwide. Non-infectious uveitis is predominantly immune mediated, characterized by dysregulated innate and adaptive immune responses, breakdown of ocular immune privilege, and recurrent or chronic inflammation. Despite advances in immunosuppressive therapy, long-term disease control remains challenging, highlighting the need to identify additional modulatory factors in disease pathogenesis. Vitamin D has emerged as an important immunoregulatory hormone beyond its

Association Between Genetically Predicted 25-Hydroxyvitamin D and Type 2 Diabetes Risk in South Asians: An Exploratory Two-Sample Mendelian Randomization Study.

BACKGROUND: Vitamin D deficiency and Type 2 diabetes (T2D) are highly prevalent in South Asian populations. Observational studies report inverse associations between serum 25-hydroxyvitamin D [25(OH)D] and T2D risk, but these findings may reflect confounding or reverse causation. We used Mendelian randomization (MR) to assess the association between genetically predicted 25(OH)D and T2D risk in South Asians. METHODS: We conducted a two-sample MR analysis using publicly available OpenGWAS summary statistics. One variant (rs3755967) met the conventional genome-wide significance threshold (p < 5

Vitamin D Supplementation and Improvement in Insomnia Symptoms Among Malaysian Women With Vitamin D Deficiency: Protocol for a Randomized Controlled Trial.

BACKGROUND: Insomnia is a prevalent sleep disorder associated with adverse physical, psychological, and functional outcomes. Emerging evidence suggests that vitamin D deficiency may contribute to sleep disturbances. Women are more likely to have lower serum vitamin D (25-hydroxyvitamin D [25(OH)D]) levels than men, and they also report a higher prevalence of insomnia. Although several studies have proposed that vitamin D supplementation may improve insomnia symptoms, findings remain inconsistent, and evidence from Southeast Asian populations is limited. OBJECTIVE: This study aims to evaluate

Early parathyroid hormone elevation after denosumab therapy following parathyroidectomy: a diagnostic pitfall.

Persistently elevated parathyroid hormone (PTH) after parathyroidectomy is often attributed to residual disease but may reflect altered calcium (Ca) homeostasis. The impact of denosumab on perioperative PTH interpretation remains unclear. A 77-year-old woman presented with a hypercalcemic crisis and was initially managed at another hospital. During treatment with hydration and elcatonin (40 units twice daily), denosumab (60 mg) was administered, resulting in a rapid decline in corrected Ca (c-Ca). Eleven days later, at presentation to our institution, her c-Ca was 9.9 mg/dL (SI: 2.48 mmol/L; r

Trimester-specific associations of maternal vitamin D metabolic markers with risks of small vulnerable newborns: a prospective birth cohort study.

BACKGROUND: Vitamin D metabolites beyond total 25(OH)D may influence fetal development, yet their trimester-specific associations with small vulnerable newborns (SVN) remain underexplored. This study examined longitudinal associations of multiple vitamin D metabolic markers with SVN-related outcomes and identified key determinants during pregnancy. METHODS: In this prospective cohort of 2,505 mother-infant pairs from China, serum levels of five vitamin D metabolites 25(OH)D2, 25(OH)D3, total 25(OH)D, 3-epi-25(OH)D3, and 24,25(OH)2D3] were quantified using HPLC-MS/MS. The Alkaline Trap: Severe Calcium-Alkali Syndrome Secondary to Chronic Calcium Carbonate Ingestion.

Calcium-alkali syndrome is an increasingly recognised cause of parathyroid hormone (PTH)-independent hypercalcaemia resulting from excessive calcium and absorbable alkali ingestion, commonly through calcium-containing antacids or supplements. Its clinical presentation may mimic neurological or malignant disease, potentially leading to extensive investigation and delayed diagnosis. A 64-year-old woman presented following a fall with paraparesis and lower back pain. Investigations demonstrated severe hypercalcaemia (adjusted calcium 4.83 mmol/L), suppressed PTH of 1.4 pmol/L, metabolic alkalosis

Biochemical and clinical changes during enzymatic replacement therapy in hypophosphatasia: three real-world cases in Spain.

Hypophosphatasia (HPP) is a rare recessive or dominant autosomal disease caused by loss-of-function variants in the ALPL gene, which encodes tissue-nonspecific alkaline phosphatase (TNSALP). Molecules involved in the pathogenesis of HPP are alkaline phosphatase (ALP), inorganic pyrophosphate (PPi; an inhibitor of bone mineralization), pyridoxal-5'-phosphate (PLP; responsible for the neurological complications) and phosphoethanolamine. Asfotase alfa is a recombinant, bone-targeted, human TNSALP approved to treat patients with HPP. Its clinical effectiveness has been demonstrated in real-world s
