Placement of Peripheral Nerve Stimulation Device in Single Limb Controls Long-Term Pain in Pediatric Patient With 4-Limb Amputation: A Case Report.

Septic shock caused by Group A Streptococcus (GAS) can cause multiorgan failure and limb ischemia, necessitating amputations. We present a 17-year-old girl who survived GAS-induced septic shock but required quadruple amputations. Postoperatively, she experienced significant stump and phantom pain primarily in her left leg, posing challenges for pain management. Two percutaneous peripheral nerve stimulation (PNS) leads were implanted, targeting the left femoral and posterior sciatic nerves. This resulted in excellent long-term pain control, allowing for rapid weaning off systemic opioids and ot

Pyogenic Liver Abscess as a Cause of Sepsis: A Case Report.

Pyogenic liver abscess (PLA) is a potentially life-threatening condition that can progress to sepsis if not treated promptly. The condition often presents with non-specific symptoms such as persistent fever and abdominal pain, which can delay diagnosis. Computed tomography plays a crucial role in identifying hepatic abscesses and guiding timely intervention. We report the case of a 54-year-old man with a one-week history of high fever and epigastric pain. Imaging confirmed the presence of a liver abscess, and due to haemodynamic instability, surgical drainage was performed, yielding purulent m

Post-Surgical Abdominal Myonecrosis: The Unusual Role of Candida albicans.

Necrotizing myonecrosis is a life-threatening infection of the skeletal muscle and soft tissues, predominantly caused by bacteria such as Staphylococcus aureus and Group A Streptococcus. In rare cases, fungal organisms, particularly Candida albicans, have been identified as the sole causative agent in these infections. We present a rare case of a 77-year-old female patient who underwent an elective hysterectomy that was complicated by an iatrogenic injury to her small bowel, which was missed in the preliminary surgery. Postoperatively, she developed signs of peritonitis and greenish-brown drai

Beyond Colonization: An Atypical Presentation of Maternal Group B Streptococcus Infection.

Group B Streptococcus (GBS), a commensal species that colonizes the genitourinary and GI tracts of many women, is well-studied as a leading cause of neonatal infection in the United States. Maternal colonization is the primary risk factor for neonatal sepsis, with vertical transmission from mother to fetus occurring during delivery. To reduce this risk, universal screening and intrapartum antibiotic prophylaxis have been implemented. While many women who test positive for colonization are asymptomatic, a small subset may develop invasive GBS infections such as bacteremia, meningitis, or pneumo

Antibiotic Susceptibility Report of Bacteria Isolated From Blood Cultures of Neutropenic Sepsis Patients Undergoing Chemotherapy.

Background Febrile neutropenia often occurs as a serious complication in patients receiving chemotherapy. It is regarded as an oncologic emergency due to its high morbidity and mortality, necessitating the prompt initiation of empirical antibiotic therapy. While gram-negative bacteria have historically been the primary cause of bloodstream infections in febrile neutropenic patients, recent shifts in bacterial epidemiology and antimicrobial resistance patterns underscore the need for regular evaluation of local data to inform treatment strategies. Methods This retrospective cohort study was con

A Rare Case of Parapharyngeal Abscess in a Healthy Adult: Importance of Timely Diagnosis and Intervention.

Parapharyngeal abscesses are rare infections of the neck fascia, most common in young male children. These bacterial infections are difficult to treat due to the complex vasculature in the parapharyngeal spaces. Common complications of untreated abscesses include airway obstruction, internal jugular vein thrombosis, and septic shock. A 43-year-old male presented to the emergency department with a new onset of fever, dyspnea, and sore throat with odynophagia. A bacterial infection was suspected after a physical exam indicated swelling, erythema, and tenderness over the submandibular/parotid reg

Streptococcus urinalis: A potential case of male urethritis caused by a novel pathogen.

Urethritis is a frequent lower urinary tract infection often linked to sexually transmitted infections (STIs). While Neisseria gonorrhoeae and Chlamydia trachomatis are the main pathogens, other microorganisms such as Mycoplasma genitalium, Trichomonas vaginalis, and Ureaplasma urealyticum may also be involved. In over half of non-gonococcal urethritis (NGU) cases, no causative agent is identified. We present the case of a 37-year-old man with scrotal pain, painful ejaculation, dysuria, and urethral discharge following a single unprotected sexual encounter. Initial STI PCR was positive for Ure

Undiagnosed Subacute Bioprosthetic Aortic Valve Abscess Leading to Complete Heart Block Requiring Permanent Pacemaker Insertion: A Case Report.

Aortic root abscess is a rare and serious complication of infective endocarditis (IE). The presentation can be vague, and a high degree of suspicion is usually required. We present the case of a 78-year-old woman with a previous history of aortic valve replacement (AVR) surgery eight months ago who presented to the Accident and Emergency (A&E) department after falling at home. After initial scrutiny, her electrocardiogram (ECG) showed complete heart block, for which she received a dual-chamber permanent pacemaker. At that time, due to suspected chest infection, her laboratory tests showed slig

Streptococcus gallolyticus Infection Associated With Colorectal Adenoma in a Patient With Type 1 Diabetes Mellitus: A Case Report.

We present a case of Streptococcus gallolyticus sepsis in a patient with type 1 diabetes mellitus (T1DM) who had poor glycemic control. Once the sepsis and infection had improved, we examined the patient for colorectal adenoma or carcinoma and detected a stalked polyp with colonoscopy. Therefore, this case underscores the importance of actively screening for malignant colonic diseases in patients with S. gallolyticus infection. We believe that clinicians should be aware of this association as it demonstrates how to perform gastroenterological follow-up for patients infected with S. gallolyticu

Effects of antibiotic prophylaxis during labour on maternal and neonatal outcomes in women planning vaginal birth.

RATIONALE: Maternal sepsis is the third leading cause of maternal mortality globally. However, the risk of maternal sepsis can be reduced by administering antibiotics prophylactically before infection occurs. Previous research has assessed the effects of azithromycin prophylaxis during pregnancy, but evidence is lacking on the effects of other types of antibiotics, and the potential for antimicrobial resistance. OBJECTIVES: To assess the effects of antibiotic prophylaxis in women in labour after 28 weeks' gestation on the prevention of maternal and neonatal infections and mortality. SEARCH M

Unexplained Intra-abdominal Abscess: A Case Report and Literature Review.

This is the case of a 65-year-old female patient with chronic obstructive pulmonary disease (COPD) who presented following a fall and was found to have a humeral fracture. Initial evaluation showed sepsis of unknown origin, leukocytosis, and elevated inflammatory markers. Persistent abdominal pain prompted imaging studies, which showed multiloculated intra-abdominal abscesses. Ultrasound-guided drainage yielded Streptococcus anginosus and Bacteroides fragilis. Antibiotics, along with percutaneous drainage, resulted in a good clinical response. Follow-up imaging revealed a decrease in the absce

Corynebacterium amycolatum: an underestimated pathogen in early-onset neonatal sepsis-a case report.

BACKGROUND: Early-onset neonatal sepsis (EOS) is a critical condition primarily caused by maternal-fetal transmission of bacterial pathogens during delivery, with Escherichia coli and Group B Streptococcus being the most prevalent. However, neonatal sepsis can also involve other rare bacteria, including Corynebacterium amycolatum, which was first described in 1988 and is widely recognized as an emerging pathogen in infectious diseases. CASE PRESENTATION: A male infant was admitted to the neonatal intensive care unit (NICU) due to premature birth and tachypnea. His mother had experienced prema

A Case of a Rapidly Infected Giant Pulmonary Cyst Complicated by Abscess Formation: Treatment and Management.

Pulmonary cysts are air- or fluid-filled parenchymal spaces surrounded by an epithelial or fibrous wall; diagnosing an infected cyst can be challenging. Herein, we report a case of a 44-year-old woman who presented with fever and a productive cough. Chest computed tomography revealed a giant cystic lesion in the left upper lobe with an air-fluid level, suggesting superimposed infection. Despite intravenous antibiotics, the lesion rapidly progressed, leading to pulmonary abscess formation. A lobectomy was performed due to poor response to medical therapy and risk of sepsis. Pathology confirmed

A CRISPR/Cas12a-based DNAzyme chemiluminescence platform for rapid detection of all Streptococcus suis and individually S. suis serotypes 7 and 9.

Streptococcus suis is a zoonotic pathogen that can infect pigs and humans with causing meningitis, sepsis, endocarditis, and arthritis. S. suis serotypes 7 and 9 cause substantial economic losses to the swine industry and pose a major threat to public health, thus, accurate and rapid detection is important for the prevention and control of epidemic disease. In this study, we developed a platform, combining recombinase polymerase amplification (RPA) with a CRISPR/Cas12a detection system to rapidly detect all S. suis serotypes and individually differentiates serotypes 7 and 9. This was achieved
