We report a case of a previously healthy woman presenting with septic shock unresponsive to seven litres of intravenous fluid resuscitation, broad-spectrum antimicrobials, necessitating admission to intensive care. The history and clinical presentation suggested streptococcal toxic shock syndrome (STSS), and group A beta-hemolytic streptococcus (GAS) was isolated from blood cultures. Persisting leg weakness, neck and spinal pain led to an MRI, which showed an inflammatory process on the right side of the neck, confirmed by CT angiogram as right internal jugular venous thrombosis. The triad of

Currently, the pathogenesis of sepsis has not been fully elucidated. This study aimed to explore the role of cytochrome P450 oxidoreductase (POR) in this disease. Specifically, two-sample Mendelian randomization, multivariate Mendelian randomization, and summary data-based Mendelian randomization methods were employed to assess the genetic impact of peripheral POR protein and POR genes from multiple tissue sources on the risk of streptococcal sepsis (ICD-10 A40) and other sepsis (ICD-10 A41). Subsequently, a mouse model of sepsis (based on lipopolysaccharide) was constructed, and the expressio

Pancreatico-enteric fistulas are rare complications of chronic pancreatitis, with colonic involvement carrying particularly high morbidity and mortality. Early recognition and prompt surgical management are critical to improving outcomes. We present a case of a patient with necrotizing pancreatitis who developed a pancreatico-colonic fistula five months following initial hospitalization. A male in his 40s with a history of poorly controlled diabetes, hypertension, and gallstone pancreatitis (status post-cholecystectomy) was transferred to our tertiary care hospital in septic shock. Serratia (S.) marcescens is an opportunistic, gram-negative pathogen most commonly associated with respiratory tract, urinary tract, and soft-tissue infections. Reports of S. marcescens as a causative agent of infective endocarditis are rare, especially when compared with the more frequently implicated pathogens such as Staphylococcus and Streptococcus species. The clinical presentation, diagnostic challenges, and therapeutic considerations of S. marcescens endocarditis remain poorly characterized in the literature. This case report aims to provide a detailed description of an unusual presenta

BACKGROUND: Universal group B streptococcus (GBS) screening in pregnant people and maternal antibiotic prophylaxis have reduced the incidence of early-onset group B streptococcus (EOGBS) infection. Despite a lower incidence, there is still a high risk of morbidity and mortality without treatment, and there is limited knowledge about late-onset group B streptococcus (LOGBS) infection. Using statewide neonatal sepsis surveillance and hospital admission data, this study describes factors associated with EOGBS and LOGBS.
METHODS: This retrospective cohort study combined statewide surveillance dat

There is a positive association between bacteraemia with Streptococcus bovis-Streptococcus equinus complex (SBSEC) and colorectal cancer (CRC). However, the relationship between the timing of SBSEC bacteraemia and CRC is not well-established. Associations with other gastrointestinal cancers have also been suggested. Using national registries, we retrospectively examined the incidence of CRC and other gastrointestinal cancers after SBSEC-bacteraemia in Sweden 2010-2019, and analysed the timing, characteristics, and prognosis of diagnosed CRC. Individuals with SBSEC-bacteraemia were matched to r

Microbiome perturbations are associated with advanced chronic liver disease (ACLD), but how microorganisms contribute to disease mechanisms is unclear. Here we analysed metagenomes of paired saliva and faecal samples from an ACLD cohort of 86 individuals, plus 2 control groups of 52 healthy individuals and 14 patients with sepsis. We identified highly similar oral and gut bacterial strains, including Veillonella and Streptococcus spp., which increased in absolute abundance in the gut of patients with ACLD compared with controls. These microbial translocators uniquely share a prtC gene encoding

We present a rare case of Streptococcus (S.) equi subspecies zooepidemicus (SEZ) bacteremia in a 73-year-old male with a history of Parkinson's disease, pulmonary embolism, and recent left knee total arthroplasty who developed fever and sepsis complicated by septic arthritis of a prosthetic knee and deep vein thrombosis (DVT). Blood cultures grew S. equi in four out of four bottles, and history revealed frequent horse exposure, suggesting zoonotic transmission with hematogenous seeding of the prosthetic joint. The diagnosis was further challenged by systemic computed tomography (CT) abnormalit

Term prelabor rupture of membranes (PROM) is a common obstetric event associated with an increased risk of maternal and neonatal infectious morbidity. The use of antibiotic prophylaxis to prevent these complications is widespread, yet its efficacy remains debated due to conflicting evidence and concerns regarding antimicrobial resistance. This systematic review aimed to synthesize the existing evidence on the impact of antibiotic prophylaxis on maternal and neonatal outcomes in term PROM. A systematic literature search was conducted following Preferred Reporting Items for Systematic Reviews an

Since 2020, Streptococcus gallolyticus infection has emerged as one of the leading causes of disease in commercial turkey poults, causing fatal bacterial sepsis without any prior clinical signs except in unusual cases involving central nervous system signs. In this study, eight S. gallolyticus strains-S. gallolyticus subsp. pasteurianus (n = 7) and S. gallolyticus subsp. gallolyticus (n = 1)-were used in a turkey embryo lethality assay that were isolated from outbreaks involving commercial poults in the U.S. Midwest. API 20 STREP V8.0 and API RAPID ID 32 STREP V4.0 were used to identify these

BACKGROUND: Streptococcus pneumoniae is an important cause of pneumonia, sepsis, and meningitis, which are leading causes of child mortality. Pneumococcal conjugate vaccines (PCVs) protect against disease and nasopharyngeal colonization with vaccine serotypes, reducing transmission to and among unvaccinated individuals. Mozambique introduced 10-valent PCV (PCV10) in 2013. In 2017-2019, 13-valent PCV (PCV13) replaced PCV10, and in September 2019 the schedule changed from three primary doses to two primary doses and a booster; the booster-containing schedule may increase indirect effects. BACKGROUND: The dysregulated immune response during sepsis involves endothelial injury, which may be augmented by infusion of clear fluids such as crystalloids. Plasma has been suggested as an alternative resuscitation fluid but it is unclear whether previously observed benefits were due to the type of fluid, or due to less volume required to restore tissue perfusion. We hypothesized that resuscitation with plasma reduces endothelial injury, inflammation, and organ injury compared to similar and higher volumes of crystalloids in a rat pneumosepsis model.
METHODS: Rats were intratracheally ino

Background/Objectives: Continuous monitoring blood culture systems (CMBCSs) are revolutionary automated instruments that facilitate the rapid identification of pathogens in blood samples from patients with sepsis. However, with only a few CMBCSs being widely used as references, user dependency on these limited options has grown. In response, a new CMBCS was developed and compared with existing systems to evaluate microbial growth. Methods: HubCentra84 was compared to BacT/Alert® 3D and BACTEC™ FX. BACKGROUND: Every year an estimated 2-3 million babies are stillborn, with a high burden in Africa. Infection is an important driver of stillbirth. There is a lack of data on the bacterial causes of stillbirth in Uganda, contributing to a lack of interventions such as effective prophylaxis and development of maternal vaccine options against the most implicated pathogens.
METHODS: The PROGRESS study was an observational cohort study undertaken in Kampala, Uganda, between November 2018 and April 2021. If a woman delivered a stillborn baby, consent was sought for the collection of a heart-blood

BACKGROUND: Bloodstream Infection(BSI) are one of the leading causes of infection-related mortality worldwide. However, epidemiological data related to BSI in China remain very limited.
METHODS: Based on the Global Burden of Disease(GBD) database, a systematic analysis was conducted on the epidemic trends, pathogen spectrum, and the current status of Antimicrobial Resistance(AMR) related to BSI in China for the year 2021. Additionally, an Autoregressive Integrated Moving Average(ARIMA) time series model was constructed to predict the trend of the disease burden associated with BSI in China fr

This case report presents Streptococcus pneumoniae bacteremia in a 60-year-old male with a history of intravenous drug use (IDU), presenting with acute respiratory distress syndrome (ARDS). The patient experienced fever, malaise and myalgia for about a week. Chest imaging revealed diffuse bilateral infiltrates, and laboratory tests showed elevated inflammatory markers. His condition deteriorated abruptly, rapidly progressing to respiratory failure, shock, and ultimately death. Both sputum and blood cultures confirmed Streptococcus pneumoniae infection and revealed resistance to commonly used a

BACKGROUND: After the implementation of pneumococcal conjugate vaccines (PCVs), besides the decrease of invasive pneumococcal disease (IPD), the clinical spectrum of the remaining cases changed, with many differences among serotypes. This study aimed to describe the clinical profile of IPD and the serotype distribution 15 years after PCV13 implementation and before the implementation of third-generation PCVs.
METHODS: From 2017 to 2022, 128 French pediatric wards prospectively reported IPDs in children. Data were collected on demographics and clinical data, underlying conditions increasing th

Cardiomyopathy is a serious complication of systemic infections, commonly linked to viral myocarditis and sepsis-induced myocardial dysfunction. Although influenza and Streptococcus pneumoniae are independently associated with cardiovascular complications, cases involving concurrent infection with both pathogens in the absence of bacteremia remain exceedingly rare. We describe a case of acute reversible cardiomyopathy in a previously healthy 31-year-old male presenting with shock, acute kidney injury, and left ventricular dysfunction. The patient tested positive for influenza A by reverse tran

BACKGROUND: Streptococcus pneumoniae is a common mortality and morbidity causing pathogen worldwide which can lead to both invasive and noninvasive pneumococcal disease. Invasive pneumococcal disease (IPD) is a severe form of pneumococcal infection manifesting, for example, as bacteremia, sepsis, or meningitis. This study evaluated the clinical and economic burden caused by laboratory confirmed IPD in the population aged ≥18 years in Finland 2016-2022.
METHODS: IPD episodes were identified from the National Infectious Disease Register. Healthcare resource use (HCRU) was obtained from the Finn

INTRODUCTION: Children with sickle cell disease (SCD) are at higher risk for invasive infection with Streptococcus pneumoniae compared with the general pediatric population. Penicillin prophylaxis, pneumococcal conjugate (PCV), and polysaccharide vaccines (PPSV) have reduced but not eliminated the risk of pneumococcal bacteremia.
METHODS: A single institution cohort of children with SCD aged <19 years was reviewed over the 14-year period since PCV13 licensure (January 2010-December 2023) to identify and characterize the clinical features and outcomes of S. pneumoniae bacteremia, including ser
