INTRODUCTION: Streptococcus pneumoniae is a human pathobiont that asymptomatically colonizes the upper respiratory tract but can cause severe diseases such as pneumonia, sepsis, and meningitis, as well as non-invasive infections like otitis media and sinusitis. It thrives in the nutrient-limited environment of the nasopharynx and has evolved mechanisms to manage host-induced stress and regulate protein levels accordingly.
METHODS: To investigate the molecular biology of S. pneumoniae under in vitro and infection-relevant conditions, a suitable cultivation medium is essential for reproducible

Campylobacter spp. bacteremia can be seen in immunocompromised patients. Nephrotic syndrome (NS) predisposes patients to serious bacterial infections (SBI). The most common SBIs in NS are spontaneous bacterial peritonitis (SBP), sepsis, and bacteremia due to Streptococcus pneumoniae or Escherichia coli. To date, despite several case reports in adults, there has been no report of Campylobacter bacteremia in the context of pediatric NS. A seven-year-old boy with NS receiving steroid treatment was admitted to our hospital, presenting with fever, diarrhea, and abdominal pain. Ceftriaxone was initi

OBJECTIVES: This study aimed to report the serotype distribution of Streptococcus pneumoniae isolates from UK and Irish patients with bacteraemia or community-associated lower respiratory tract infections (CA-LRTI). Depending upon the year, these were from 23 to 39 sentinel laboratories and were collected between 1999 and 2019, thus spanning the introduction of pneumococcal conjugate vaccines, PCV7 and PCV13.
METHODS: Pneumococcal identification, susceptibility testing and serotyping were undertaken by a central laboratory. Changes in serotype distributions and among the predominant types sho

Invasive pneumococcal disease, presenting as purulent pericarditis, is a rare but potentially fatal condition. Early recognition and targeted antimicrobial and supportive interventions are essential to improve outcomes, particularly in high-risk patients. A 62-year-old male with poorly controlled type 2 diabetes mellitus was admitted with community-acquired pneumonia, following a five-day history of respiratory symptoms prior to hospitalization. A thorax CT scan revealed left lower lobe consolidation and bilateral pleural effusion. Empiric antibiotic therapy was initiated with ceftriaxone and

BACKGROUND: Burn injuries are associated with significant morbidity and mortality, with infections being a major factor influencing outcomes. Given evidence of sex-related differences in infection risk, we investigated whether such disparities extend to bacteremia in critically ill burn patients.
METHODS: We conducted a single-centre cohort study of critically ill patients with severe burn injuries admitted to the Burn Center at the University Hospital Zurich between January 2017 and December 2021. The primary outcome was the association between sex and the development of a first bacteremia e

Group B Streptococcus (GBS; Streptococcus agalactiae) is a gram-positive colonizer of the healthy intestinal and genitourinary microbiota. During and shortly after birth, neonates and infants can be opportunistically infected leading to sepsis, pneumonia, or meningitis among other illnesses. GBS is the leading cause of neonatal meningitis globally, and while prophylactic treatments have been successful for reducing early-onset disease, no decrease in the incidence of late-onset disease has occurred and no vaccine is currently available. In this review, we describe GBS both from a clinical and

BACKGROUND: Presence of a prosthetic joint entails a life-long risk of haematogenous periprosthetic joint infection (hPJI) during bacteraemia. For bacteraemia with Staphylococcus aureus the hPJI frequency has been described to be up to 40% and for streptococci up to 20%, with large variation among reported frequencies.
OBJECTIVES: We aimed to investigate the hPJI frequency from bacteraemia with virulent gram-positive cocci in patients with hip and/or knee prosthesis.
METHODS: A population and register based approach was used. Through cross-matching of blood cultures positive for virulent gra

OBJECTIVES: Streptococcus agalactiae, or group B Streptococcus (GBS), is a significant pathogen associated with severe infections in neonates, particularly sepsis and meningitis. The increasing prevalence of antibiotic resistance among GBS strains is a growing public health concern, necessitating a comprehensive meta-analysis to evaluate the prevalence of this resistance globally.
METHODS: We conducted a comprehensive systematic search across four major scientific databases: Scopus, PubMed, Web of Science, and EMBASE, targeting articles published until December 13, 2023. This meta-analysis fo

BACKGROUND: Pyomyoma is a rare, but potentially fatal, complication of uterine leiomyoma. Most cases occur in postpartum or postmenopausal patients with vascular disease and arise due to infarction and infection of existing fibroids. The proposed diagnostic triad for pyomyoma includes 1) sepsis or bacteremia; 2) uterine leiomyomata; and 3) no other apparent source of infection. We present a case of a postmenopausal patient who presented with the aforementioned triad and was found to have a pyomyoma, positive for Streptococcus agalactiae.

INTRODUCTION: It is unclear whether routine testing of women for group B streptococcus (GBS) colonisation either in late pregnancy or during labour reduces early-onset neonatal sepsis, compared with a risk factor-based strategy.
METHODS AND ANALYSIS: Cluster randomised trial.
SITES AND PARTICIPANTS: 320 000 women from up to 80 hospital maternity units.
STRATEGIES: Sites will be randomised 1:1 to a routine testing strategy or the risk factor-based strategy, using a web-based minimisation algorithm. A second-level randomisation allocates routine testing sites to either antenatal enriched cult

Group B Streptococcus (GBS) is the most common cause of neonatal sepsis. Screening for vaginal-rectal colonization of the mother is important to prevent exposure. The current standard diagnostic method for GBS is to collect a vaginal-rectal swab and inoculate in LIM broth to increase sensitivity. Use of flocked swabs such as Eswabs has become the recommended collection device due to their increased recovery rates and potential for automation. Despite being the recommended collection device, there is a lack of adequate literature providing guidance on processing of Eswabs for GBS cultures. This

Group B Streptococcus (GBS) is a human pathogen and a leading cause of neonatal meningitis, pneumonia, and sepsis in newborns and adults. Group B Streptococcal meningitis is caused when blood-borne GBS interacts with and disrupts the blood-brain barrier, causing inflammation. The blood-brain barrier (BBB) is comprised of highly specialized brain endothelial cells (BECs) that serve as the interface between the circulation and the central nervous system (CNS), creating a tight endothelial cell barrier. Modeling the BBB using an induced pluripotent stem-cell (iPSC)-derived brain-like endothelial

Arcanobacterium haemolyticum (A. haemolyticum), a Gram-positive, anaerobic, rod-shaped bacterium, is typically associated with pharyngitis and skin infections but can also cause severe, life-threatening infections such as sepsis and necrotizing fasciitis (NF), particularly in immunocompromised patients. We report a case of severe NF of the lower extremity caused by a mixed infection with A. haemolyticum and Streptococcus agalactiae (S. agalactiae) in a patient with diabetes mellitus. A 51-year-old male with poorly controlled diabetes mellitus presented with extensive swelling, necrosis, and bu

Group B Streptococcus (GBS), or Streptococcus agalactiae, is a significant cause of neonatal morbidity and mortality, with late-onset disease (LOGBS) typically manifesting as bacteremia or meningitis. Osteoarticular infections, particularly septic polyarthritis, are exceedingly rare but potentially devastating complications of LOGBS. We report a rare case of LOGBS presenting as septic polyarthritis in a 28-day-old term male neonate. The infant presented with a 15-day history of progressive swelling in the left knee and right shoulder, accompanied by irritability and feeding difficulties. Ultra

Microbiome disruption is a proposed mechanism for the observed differences in child health outcomes by maternal HIV status, but the early neonatal microbiome of HIV-exposed (HE) newborns is not well-studied. We used 16S ribosomal ribonucleic acid sequencing to analyze the microbiome composition of nasal, skin, and rectal samples collected ≤72 h after birth from 57 hospitalized neonates in Botswana, 33% of whom were HE. Beta diversity differed by anatomic compartment (P = .001) and days since birth; however, interindividual differences were greater than those by anatomic site (P = .001). There

BACKGROUND: Group B Streptococcus (GBS), a commensal bacterium in the gastrointestinal tract, can cause invasive GBS (iGBS) infections, particularly in older adults with underlying comorbidities. We determined the relationship between mortality and host-related or virulence factors in older adults with iGBS infections.
METHODS: We retrospectively analyzed 105 cases of iGBS infection treated between January 2016 and October 2023 at Tokyo Medical University Hospital and two affiliated hospitals. The association between patient outcomes and demographics, underlying diseases, clinical manifestati

OBJECTIVE: Early-onset neonatal sepsis due to ascending infection is a potentially preventable complication of preterm premature rupture of membranes. Our objective was to determine whether the analysis of bacteria from vaginal swab samples is predictive of the risk of early-onset neonatal sepsis in preterm premature rupture of membranes.
STUDY DESIGN: In a prospective 3-center observational cohort, patients with preterm premature rupture of membranes were enrolled between 22 and 36 weeks of gestation + 6 days. Vaginal swab samples at delivery were analyzed using 2 different approaches, class
