Biochemical and clinical changes during enzymatic replacement therapy in hypophosphatasia: three real-world cases in Spain.

Hypophosphatasia (HPP) is a rare recessive or dominant autosomal disease caused by loss-of-function variants in the ALPL gene, which encodes tissue-nonspecific alkaline phosphatase (TNSALP). Molecules involved in the pathogenesis of HPP are alkaline phosphatase (ALP), inorganic pyrophosphate (PPi; an inhibitor of bone mineralization), pyridoxal-5'-phosphate (PLP; responsible for the neurological complications) and phosphoethanolamine. Asfotase alfa is a recombinant, bone-targeted, human TNSALP approved to treat patients with HPP. Its clinical effectiveness has been demonstrated in real-world s

Locomotor Health in Modern Broiler Chickens: Risk Factors, Major Disorders, Assessment Methods, and Implications for Welfare and Production Efficiency.

Genetic selection for rapid growth, improved feed efficiency, and increased breast-muscle yield has substantially enhanced broiler production; however, it has also increased the functional demands placed on the locomotor system. This review critically examines locomotor health in modern broiler chickens in relation to welfare and production efficiency. Particular attention is given to the multifactorial interactions among genotype, growth rate, body weight and conformation, skeletal development, nutrition, physical activity, housing conditions, age, and infectious factors. Major locomotor diso

Alkaline phosphatase in paediatric practice: a practical approach with a proposed diagnostic algorithm.

This review summarises the clinical relevance of alkaline phosphatase (ALP) in paediatrics, placing emphasis on its biochemical heterogeneity, age-dependent physiology and diagnostic value in bone and hepatobiliary disorders. ALP is a group of membrane-bound enzymes, predominantly represented in serum by tissue-nonspecific alkaline phosphatase, with major contributions from bones and the liver. In children, interpretation of ALP concentration is complicated due to marked physiological variation occurring in growth and puberty, as well as by method-dependent differences in laboratory reference

Single cell multi-omics guided hydrogel enables closed-loop therapy for renal osteodystrophy.

Fracture repair in renal osteodystrophy remains challenging because impaired bone quality compromises fixation stability, while current systemic therapies regulating bone metabolism do not sufficiently address the disease-specific mechanisms underlying defective repair. An integrated strategy combining early fracture stabilization with mechanism-guided local treatment is therefore needed. Here, integrated single-cell RNA sequencing and multi-omics analyses identify inflammatory macrophages as a key pathogenic population, characterized by glycolytic reprogramming driven by hypoxia-inducible fac

Study of biallelic variants in CYP2R1 causing vitamin D hydroxylation deficiency rickets type 1B.

Vitamin D hydroxylase deficiency rickets type 1B (VDDR1B) is a rare hereditary bone disease, due to a defect in vitamin D-25-hydroxylation. It is caused by mutations in CYP2R1, consistent with an autosomal recessive pattern. We aim to access the phenotype and investigate the genetic etiology of a pedigree with suspected rickets. The probands were a pair of identical twin girls, who had leg curvature and delayed independent ambulation with abnormal gait. X-rays showed valgus ribs and genu varum. The probands had taken vitamin D and calcium supplements since the age of 6 months. Their serum 25(O

The developmental remodeling front: resorptive cell-vascular coupling at the chondro-osseous junction.

The chondro-osseous junction is commonly described as a site where cartilage is removed and bone is deposited, but this sequence does not explain how matrix topology is relayed into primary trabecular architecture. This review frames the junction as a developmental remodeling front in which cartilage-derived signals, endothelial advance, matrix-processing cell states, temporary scaffold retention, and osteoid deposition overlap in space and time. Mouse perturbation studies support vascular endothelial growth factor A (VEGF-A)-dependent entry, endothelial matrix metalloproteinase 9 (MMP-9)-medi

Exploring the Association of Angiotensin-(1-7) with Left Ventricular Mass Index in Youth with Hereditary Hypophosphatemia: A Pilot Study.

INTRODUCTION: Fibroblast growth factor 23 (FGF23) regulates phosphate homeostasis and is associated with cardiovascular-kidney-metabolic syndrome in patients with chronic kidney disease in part mediated by angiotensin II. X-linked hypophosphatemia (XLH) is characterized by phosphate wasting due to excess FGF23 activity, but its associations with angiotensin peptides and cardiovascular disease remain unclear. We evaluated cross-sectional associations between angiotensin II, angiotensin-(1-7), and left ventricular mass index (LVMI) in youth with XLH. METHODS: A pilot prospective cross-sectional

Biochemical phenotype of hypophosphatasia in asymptomatic individuals carrying ALPL variants.

Hypophosphatasia (HPP) is the rare metabolic disorder caused by variants in the ALPL gene, resulting in deficient activity of tissue-nonspecific alkaline phosphatase (ALP). This leads to accumulation of substrates contributing to impaired bone mineralization. Hypophosphatasia manifests with a broad clinical spectrum; however, an increasing number of individuals with ALPL variants have been identified presenting the hallmark biochemical feature of HPP of low serum ALP activity, with or without elevated serum pyridoxal-5-phosphate (PLP) or urine phosphoethanolamine (PEA), while remaining asympto

Vitamin D and Fracture Healing.

Maintaining bone health involves complex interactions among nutrients, hormones, and physiological mechanisms, with vitamin D playing a pivotal role. Vitamin D is essential for the absorption of calcium and phosphate, which are critical minerals for bone development, maintenance, and repair. Disruptions in mineral ion homeostasis can lead to conditions such as hypocalcemia, osteoporosis, osteomalacia, and rickets, which are prevalent worldwide. Inadequate vitamin D impairs bone health, increasing the risk of fractures and catabolic bone diseases. This chapter elaborates on the synthesis, activ

The growth plate: Zonal architecture, plasticity, and endocrine control of linear growth.

The growth plate (physis) is a highly specialized cartilaginous organ that drives longitudinal bone growth and ultimately determines adult stature. Its zonal architecture, including the resting, proliferative, hypertrophic, and calcification zones, integrates stem-like progenitor activity, clonal chondrocyte expansion, matrix remodeling, vascular invasion, and replacement by bone. Here, we review how the structural organization of the growth plate emerges from the interplay among extracellular matrix composition, sulfation pathways, and canonical paracrine signaling pathways, including Ihh-PTH

Structural and functional insights into Vitamin D receptor mutations: An in-silico investigation of polymorphism-induced resistance.

Vitamin D is critical for calcium homeostasis, bone health, and immune regulation via the Vitamin D Receptor. Mutations in the ligand-binding domain and DNA-binding domain can disrupt ligand interactions, causing biologically active metabolite of vitamin D [1α,25-dihydroxyvitamin D3 (calcitriol)] resistance and clinical complications such as hereditary rickets and immune dysregulation. This study explored the structural and functional effects of VDR missense mutations using computational approaches. An AlphaFold-generated VDR model incorporated selected mutations from 503 reported variants, of

Heterozygous CYP27B1 c.262delG pathogenic variant and its impact on vitamin D metabolites and phosphocalcic profile in humans.

BACKGROUND/AIMS: Vitamin D-dependent rickets type 1A is a rare autosomal recessive disorder caused by pathogenic variants in the CYP27B1 gene. CYP27B1 encodes for the 1α-hydroxylase enzyme catalyzing the conversion of 25-hydroxyvitamin D (25(OH)D) to calcitriol, the last step of vitamin D activation. In the Saguenay-Lac-Saint-Jean (SLSJ) region (Quebec, Canada), the CYP27B1 c.262delG variant has a carrier rate of 1 in 27 due to a founder effect. This study aimed to characterize the impact of heterozygosity for the CYP27B1 c.262delG variant on vitamin D metabolites and the phosphocalcic profile

Adolescents' experience of living with X-linked hypophosphataemia (XLH): a mixed-methods analysis of those who continued and discontinued burosumab treatment after end of skeletal growth.

BACKGROUND: X-linked hypophosphataemia (XLH) is a rare, genetic, phosphate-wasting disorder caused by excess fibroblast growth factor 23 (FGF23). Children experience skeletal abnormalities, pain and impaired health-related quality of life (HRQL). The FGF23 inhibitor burosumab improved growth, decreased rickets severity and improved symptoms and HRQL in paediatric phase 3 trials. Using a mixed-methods approach, the MyXLH study aims to describe the lived experience of adolescents with XLH and to compare the experiences of adolescents who did and did not continue burosumab for the 26 weeks immedi

Autoimmune osteomalacia: a novel FGF23-related hypophosphatemic osteomalacia.

BACKGROUND: Fibroblast growth factor 23 (FGF23)-related hypophosphatemic rickets/osteomalacia arises from excessive FGF23 activity, with X-linked hypophosphatemia (XLH) and tumor-induced osteomalacia (TIO) as the most common congenital and acquired forms, respectively. However, in a substantial subset of patients with acquired FGF23-related hypophosphatemic osteomalacia, phosphaturic mesenchymal tumors (PMTs) remain undetectable despite extensive imaging studies. A recent study identified autoantibodies against PHEX, the gene responsible for XLH, in 5 of 13 patients with acquired FGF23-related

Impact of oral phosphate supplements and active vitamin D treatment on dentoalveolar features of X-linked hypophosphatemia.

X-linked hypophosphatemia (XLH), the most common form of genetic rickets (1/20000 births), results in the disruption of skeletal and dental mineralization. Oral features include dentinomalacia, spontaneous dental abscesses, and a high susceptibility to periodontitis. The association of phosphate supplementation and active vitamin D analogs (PO4/VitD) aims to counteract the consequences of FGF23 excess and the impaired production of active vitamin D. Despite a significant improvement in the long bone phenotype, the impact on dentoalveolar tissues remains poorly documented. Here, we aimed to det

Expanding the Autosomal Recessive Hypophosphatemic Rickets Type I Carrier Phenotype and Adult Treatment with Burosumab.

Autosomal recessive hypophosphatemic rickets type 1 (ARHR1) is a rare, inherited cause of fibroblast growth factor 23 (FGF23)-mediated renal phosphate-wasting that results from homozygous inactivating pathogenic variants in the dentin matrix acidic phosphoprotein-1 (DMP1) gene. A variable clinical spectrum has been described in this condition, including features of rickets (in children), hyperostotic osteomalacia (in children and adults), bone pain, short stature, lower limb deformities, recurrent dental abscesses, and hearing defects. The purpose of this report is to describe the clinical evo

Vitamin D Receptor Signaling and Ligand Modulation: Molecular Mechanisms and Therapeutic Implications.

Vitamin D, a fat-soluble vitamin functioning as a hormone via the vitamin D receptor (VDR), is critical for calcium homeostasis and bone health. Vitamin D deficiency is linked to nutritional rickets, osteomalacia, and increased risk of non-communicable diseases such as cancer and diabetes. While serum 25(OH)D3 is used to assess vitamin D status, its active form, 1α,25(OH)2D3, exerts context-dependent effects on calcium metabolism. Nonetheless, the therapeutic utility of native vitamin D is limited in certain pathologies. In chronic kidney disease (CKD), the renal conversion of 25(OH)D3 to acti

The lived experience of adolescents with X-linked hypophosphataemia treated with burosumab at end of skeletal growth: a mixed-methods analysis.

X-linked hypophosphataemia is a rare, genetic, lifelong disorder caused by phosphate-regulating endopeptidase homologue X-linked pathogenic variants and, if left untreated, is associated with a progressive accumulation of musculoskeletal manifestations. Burosumab is a fully human monoclonal antibody that targets circulating fibroblast growth factor 23 and directly inhibits its activity, thereby correcting the abnormal phosphate homoeostasis in people with X-linked hypophosphataemia (XLH). The efficacy and safety of burosumab has been demonstrated in a programme of clinical trials in children a
