Research on vitamin D metabolic regulation in the pathogenesis of related diseases.

Vitamin D is a group of fat-soluble vitamins that plays critical roles in calcium-phosphate homeostasis, bone health, and immune regulation. The metabolic pathway of vitamin D involves two key enzymatic steps: hepatic 25-hydroxylation to produce 25-hydroxyvitamin D [25(OH)D] and renal 1α-hydroxylation to generate the biologically active form 1,25-dihydroxyvitamin D [1,25(OH)2D]. 25(OH)D serves as the gold standard biomarker for assessing vitamin D status due to its high circulating concentration, long half-life, and stable levels. This metabolic process is precisely regulated by parathyroid ho

Markers and regulators of osteoblast and osteoclast activity in children with X-linked hypophosphatemia treated with burosumab.

Burosumab is effective for treatment of rickets in children with X-linked hypophosphatemia (XLH). The effects of burosumab on markers and regulators of osteoblast and osteoclast acitivity are largely unknown. In this cross-sectional observational study, we investigated 7 key markers and regulators of osteoblast and osteoclast activity in 107 burosumab-treated children with XLH. We calculated z-scores for bone-specific alkaline phosphatase (BAP), procollagen-1-N-terminal propeptide (P1NP), carboxy-terminal telopeptide of type 1 collagen (CTX), tartrate-resistant acid phosphatase type 5b (TRAP5b

Renal Fanconi syndrome and vitamin D deficiency: chicken or egg?

BACKGROUND: Renal Fanconi syndrome (RFS) refers to a generalized dysfunction of the proximal tubule, including impaired 1-α hydroxylation of vitamin D. Consequently, rickets is a typical complication. Clinical observations in children with severe nutritional vitamin D deficiency sometimes include proximal tubular dysfunction, raising the possibility that lack of vitamin D could not only be a consequence of RFS but also a cause of it, although this has never been confirmed. Observations in Mendelian disorders with their genetically defined pathophysiology can provide clearer insights. METHODS:

Tumor induced-osteomalacia caused by a phosphaturic mesenchymal tumor in the sartorius muscle: a case report.

Tumor-induced osteomalacia (TIO) is a rare paraneoplastic syndrome resulting from excessive secretion of fibroblast growth factor-23 (FGF23) by phosphaturic mesenchymal tumors (PMTs). We report the case of a 35-year-old man with a three-year history of progressive musculoskeletal symptoms and severe mobility impairment, who had not received a correct diagnosis during this period. Laboratory evaluation revealed severe hypophosphatemia and elevated FGF23 levels. Magnetic resonance imaging (MRI) and 68Ga-DOTA-TATE PET/CT identified a metabolically active lesion in the sartorius muscle. Surgical r

Burosumab for X-linked Hypophosphataemia and Tumour-induced Osteomalacia in Adults.

X-linked hypophosphataemia and tumour-induced osteomalacia are rare, distinct metabolic conditions that can affect both children and adults, and result in elevated fibroblast growth factor-23 levels and subsequent low phosphate levels. This leads to severe musculoskeletal symptoms and complications, and these conditions carry significant disease burden and high levels of morbidity. In this article, we examine the conditions and analyse the use of burosumab, a recently developed monoclonal antibody treatment, in the care of adults with these conditions.

X-linked Hypophosphatemic Rickets Revealed by Exome Sequencing: A Pediatric Case Report of a PHEX Pathogenic Variant.

X-linked hypophosphatemic rickets (XLH) is the most common form of hereditary vitamin-resistant rickets, caused by inactivating mutations of the PHEX (phosphate-regulating endopeptidase homolog, X-linked) gene, leading to elevated fibroblast growth factor 23 (FGF23) and chronic hypophosphatemia secondary to renal phosphate wasting. We report the case of a 3.5-year-old boy presenting with bilateral varus deformity of the lower limbs, frontal bossing, rachitic rosary, and growth retardation (-2.5 SD). Laboratory investigations revealed hypophosphatemia (0.88 mmol/L) with severely reduced urinary

Insight into Natural History and Phenotype in Untreated Adults with X-Linked Hypophosphatemia.

X-linked hypophosphatemia (XLH) is a rare genetic rachitic/osteomalacic and dental disorder caused by pathogenic variants in PHEX gene resulting in fibroblast growth factor 23 (FGF23) excess leading to hypophosphatemia by renal phosphate wasting and decreased 1,25-dihydroxyvitamin D production. The aim of the study was to provide insight into natural history and phenotype in untreated adults with XLH (no phosphate supplements and active vitamin D metabolites or burosumab). Clinical, biochemical, skeletal features, co-morbidities, and patient-reported outcomes (PROs) were examined in 52 patient

Vitamin D in the UK: an urgent call to redefine the threshold for deficiency.

ACT NOW Vitamin D is a multidisciplinary collective tackling persistent public health concerns in the UK, including widespread vitamin D deficiency, nutritional rickets in children, and osteomalacia across the lifespan. National Diet and Nutrition Survey data show significant declines in vitamin D intake - especially among lower intake percentiles - and rising rates of severe vitamin D deficiency (serum 25-hydroxyvitamin D <25 nmol/L) among people aged 4-64 years. Contributing factors include reduced meat, fish and seafood consumption, increasing obesity, and growing high-risk ethnic minority

Generalized Developmental Enamel Hypoplasia of the Permanent Dentition Associated with Early Childhood Vitamin D Deficiency Rickets: A Case Report.

Background: Vitamin D deficiency rickets is a metabolic bone disorder caused by impaired calcium and phosphate homeostasis resulting from insufficient vitamin D. In children, severe vitamin D deficiency can disturb the mineralization of growing bones and teeth. Although the skeletal manifestations are well recognized, reports describing generalized developmental enamel defects affecting nearly all permanent teeth remain limited. Methods: A 6-year-9-month-old Japanese boy with a history of vitamin D deficiency rickets diagnosed at 2 years 5 months of age was referred to our department for evalu

Phenotypic Variability in Vitamin D-Dependent Rickets Type 1a: A Case Report of Two Children With the Same CYP27B1 Mutation.

BACKGROUND: Vitamin D-dependent rickets Type 1A (VDDR1A) is a rare autosomal recessive disorder caused by pathogenic variants in CYP27B1, resulting in impaired renal 1α-hydroxylation of 25-hydroxyvitamin D. The consequent deficiency of active vitamin D disrupts calcium-phosphate homeostasis and causes rickets. Although VDDR1A is monogenic, clinical severity may vary considerably even among patients with the same mutation. CASE PRESENTATION: We describe two unrelated Saudi boys with genetically confirmed VDDR1A who both harbored the same homozygous CYP27B1 c.1286G > C (p.Arg429Pro) variant but

Calcium Supplementation in the Indian Context: A Consensus Statement by the Endocrine Society of India Skeletal Health Taskforce.

Optimum intake of calcium is fundamental for skeletal health and additionally plays a role in cardiovascular health, haemostasis, synaptic transmission, and several enzyme-driven metabolic reactions. In several countries, including India, dietary calcium intake is poor, and it is necessary to ensure adequate calcium intake for optimizing musculoskeletal health. Not many guidelines focus on regions where calcium in the diet is suboptimal, and supplementing calcium may be specially relevant. In view of this, the Endocrine Society of India (ESI) Skeletal Task Force has developed clinical practice

FGF23 - A hormone produced by bone and has many faces.

Fibroblast growth factor 23 (FGF23) was identified as an etiological factor for hypophosphatemic rickets/osteomalacia approximately a quarter-century ago. Since then, FGF23 has been shown to be a bone-derived hormone that regulates phosphate and vitamin D metabolism by binding to the α-KLOTHO-FGF receptor complex. Dysregulation of FGF23 activity results in hypophosphatemic and hyperphosphatemic disorders. Medical therapy targeting FGF23 is now available for hypophosphatemic diseases caused by FGF23 excess. Furthermore, elevated FGF23 levels have been associated with various adverse clinical ou

Two variants in SLC34A3 in a patient with X-linked hypophosphatemia: a diagnostic and therapeutic dilemma.

X-linked hypophosphatemia (XLH) is the most prevalent form of hereditary rickets due to pathogenic variants in the PHEX gene. Since 2018, a treatment with burosumab, which inhibits the activity of fibroblast growth factor 23 (FGF23), has been approved for XLH patients. In contrast, hereditary hypophosphatemic rickets with hypercalciuria (HHRH), caused by pathogenic variants in the SLC34A3 gene, exhibits a similar increase in urinary phosphate excretion due to abnormal function of the renal phosphate transporter; however, FGF23 levels are elevated in XLH but low in HHRH, whereas the levels of b
