From Cysteamine to Pepstatin A: Targeting Cathepsin D To Improve Bone Health in Cystinosis.

Nephropathic cystinosis is a rare genetic lysosomal storage disorder caused by loss-of-function variants of the CTNS gene encoding for the lysosomal H+/cystine symporter cystinosin. The lack of cystinosin causes cystine accumulation in lysosomes and cell damage. Children with nephropathic cystinosis commonly exhibit skeletal abnormalities, including growth retardation, osteopenia and rickets. The only cystine-depleting drug available is cysteamine. However, several observations suggest that cysteamine, particularly at high doses, may have negative effects on the skeleton. In this study, we cha

Fibroblast growth factor 23 (FGF23): From synthesis to cleavage.

Fibroblast growth factor 23 (FGF23) maintains phosphate and vitamin D homeostasis. In the kidney, recognition of FGF23 is mediated by fibroblast growth factor receptors (FGFRs), α-Klotho, and heparan sulfate. Structural studies indicate that an FGF23-FGFR-α-Klotho recognition unit recruits a second FGFR with assistance from heparan sulfate to form an asymmetric 1:2:1:1 complex that activates Ras-mitogen-activated protein kinase (MAPK) signaling. Circulating intact FGF23 reflects transcription, post-translational processing, secretion, and clearance. In FGF23-producing cells, polypeptide N-acet

Cystinosis-associated metabolic bone disease: pathogenesis and outcome.

Cystinosis-associated metabolic bone disease (CMBD) is a severe complication in patients with infantile nephropathic cystinosis, which is a rare inherited lysosomal storage disease due to pathogenic variants in the CTNS gene that results in an accumulation of cystine in all organs. The kidneys are the organs primarily affected, resulting in renal Fanconi syndrome at around the age of 6-12 months. Without adequate treatment with the cystine-depleting agent cysteamine, this is followed by progressive chronic kidney disease (CKD) and kidney failure in the second decade of life. Patients usually p

Vitamin D-dependent Rickets Type 1A: Recognizing Clinical Clues to Avoid Delayed Diagnosis.

Vitamin D-dependent rickets type 1A (VDDR1A) is a rare, inherited form of rickets caused by biallelic mutations in the CYP27B1 gene, leading to 1α-hydroxylase deficiency and impaired 1,25-dihydroxyvitamin D (1,25(OH)₂D) production. We report the case of a 25-year-old woman who was initially diagnosed at 12 months of age with "vitamin D-resistant rickets" and presented with motor delay, skeletal deformities, and a history of multiple orthopedic surgeries. At 25 years of age, etiologic evaluation identified a homozygous CYP27B1 p.Thr325Met variant, classified as likely pathogenic by the testing

Health-Related Quality of Life in Children with X-Linked Hypophosphatemia Treated with Burosumab: Real-World Data from a German-Swiss Study.

Burosumab is effective in improving rickets in children with X-linked hypophosphatemia (XLH). Predictors of health-related quality of life (HRQoL) in pediatric XLH patients treated with burosumab are unknown. In this cross-sectional analysis of a prospective binational observational study, we investigated HRQoL in 64 pediatric XLH patients (36 female) on burosumab treatment, using the KIDSCREEN-52 questionnaire and qualitative interviews. Associations between HRQoL and clinical findings were analysed using multiple regression and receiver operating characteristics (ROC) curve analyses. Median

Noncanonical Splice Site Disruption: +4 Intronic Variant in Phosphate-Regulating Endopeptidase Homolog, X-linked (PHEX Gene) Supported by In Silico Analysis in X-linked Hypophosphatemic Rickets.

X-linked hypophosphatemic rickets (XLH) is the most common inherited cause of renal phosphate wasting. It is typically caused by pathogenic variants in the phosphate-regulating endopeptidase homolog, X-linked (PHEX gene), which lead to increased levels of fibroblast growth factor 23 (FGF23). However, noncanonical intronic variants represent a significant diagnostic challenge, as they may not be detected by conventional genetic studies and require interpretation supported by bioinformatic tools. We present the case of a pediatric patient with postnatal short stature and progressive genu varum.

Real-World Effectiveness of Burosumab Versus Oral Phosphate and Active Vitamin D in Adults with X-Linked Hypophosphatemia (XLH): A 3-Year Follow-Up of the XLH Disease Monitoring Program.

X-linked hypophosphatemia (XLH) is a progressive disease in which excess FGF23 causes renal phosphate wasting, leading to hypophosphatemia. Consequences in adults include osteomalacia, fractures, pseudofractures, joint dysfunction, pain, and impaired mobility. Treatments for XLH are the anti-FGF23 antibody burosumab and active forms of vitamin D alone or in combination with oral phosphate (Pi/D). Randomized studies comparing burosumab with Pi/D in adults with XLH are lacking. We present a 3-yr follow-up analysis evaluating real-world effectiveness of burosumab versus Pi/D based on biochemistry

Association of PHEX Gene Dosage With Meniere Disease and Related Audiovestibular Phenotypes in X-Linked Hypophosphatemia.

IMPORTANCE: Meniere disease (MD) is a heterogenous disorder whose underlying etiologies remain poorly understood. A subtype of MD characterized by endolymphatic sac (ES) underdevelopment (ES hypoplasia), frequent bilateral disease, and male predominance-termed MD-hp-has emerged as a promising model for genetic investigation. OBJECTIVE: To test whether the PHEX gene underlies the association of XLH with MD-hp. DESIGN, SETTING, AND PARTICIPANTS: This prospective, cross-sectional, study was conducted at tertiary academic centers in the US (Boston, Massachusetts) and Switzerland (Zurich) from Ja

Construction of a CKD-MBD osteoporosis animal model using adenine combined with high-phosphorus diet at different time intervals and gradient concentrations.

Recent advancements have led to the establishment of various animal models for chronic kidney disease-mineral and bone disorder (CKD-MBD), including 5/6 nephrectomy and drug induction. However, these models predominantly focus on kidney and vascular pathology, often neglecting skeletal lesions and exhibiting high mortality rates and unclear osteoporosis. Phosphorus plays a crucial role in bone metabolism, and prolonged high phosphorus intake can disrupt normal bone function. Our study introduces a novel CKD-MBD osteoporosis model using mice fed with adenine at three concentrations, combined wi

Recent insights into the role of dietary phosphorus in the development and progression of chronic kidney disease-mineral and bone disorder.

PURPOSE OF REVIEW: Dietary phosphorus is a central driver of chronic kidney disease-mineral and bone disorder (CKD-MBD). This review summarizes recent advances that clarify how differing forms and sources of dietary phosphorus influence bone and mineral metabolism, as well as related physiologic outcomes, in individuals with and without CKD. RECENT FINDINGS: Controlled feeding studies across CKD and non-CKD populations demonstrate that reducing intake of highly bioavailable phosphorus - particularly from food additives - leads to consistent improvements in urinary phosphorus excretion, fibrob

Lentivirus-based HPSC therapy provides effective and long-term treatment in hypophosphatasia mouse model.

Hypophosphatasia (HPP) is a multi-system metabolic disorder characterized by skeletal hypomineralization and perinatal lethality in severe cases and rickets, muscle weakness, and premature tooth loss in milder forms. It is caused by loss-of-function mutations in the ALPL gene encoding the tissue-nonspecific isozyme of alkaline phosphatase (TNALP). Currently, enzyme replacement therapy with asfotase alfa (STRENSIQ) is the only treatment approved for the most severe forms of the disease. It enhances survival and improves bone mineralization, but it is burdensome for the patients who must undergo

Gut-derived uremic toxin indoxyl sulfate and aryl hydrocarbon receptor in renal osteodystrophy.

Chronic kidney disease (CKD) affects 14% individuals worldwide. Changes in bone and mineral metabolism in CKD are ubiquitous (CKD- Mineral and Bone Disorder) including renal osteodystrophy with a fracture incidence of up to 5 times greater than the general population. While PTH lowering is the mainstay of therapy for decades this has not reduced fractures in patients or improve bone mechanics in pre-clinical models. This suggests additional factors involved in the pathogenesis of renal osteodystrophy such as uremic toxins due to either increased production or decreased renal excretion. One gro

Phlorizin attenuates chronic kidney disease induced bone loss by mitigating sclerostin mediated inhibition of Wnt/β-catenin signaling and ameliorating metabolic alterations.

CKD-MBD is a metabolic disorder characterized by disrupted mineral metabolism, leading to vascular calcification, renal osteodystrophy, and high fracture risk. Sclerostin, an osteocyte-derived Wnt inhibitor, is a key regulator of both bone loss and vascular calcification. This study explored the role of phlorizin (SGLT1 and SGLT2 inhibitor) on bone loss and metabolic profile in CKD-MBD. Calvarial osteoblasts were treated with phlorizin and evaluated for ALP activity, viability, mineralization, and gene/protein expression. Molecular docking and simulation studies were performed to check phloriz

Protective effect of the aqueous extract of Tabernaemontana stapfiana (Apocynaceae) stem bark against monosodium glutamate-induced neurotoxicity in Swiss albino mice.

BACKGROUND: Monosodium glutamate (MSG), a common food additive, is known to enhance taste and flavour. However, increasing evidence suggests that elevated MSG intake is linked to neuronal pathologies and death. Tabernaemontana stapfiana is a plant traditionally used in Cameroon for multiple purposes, including as a haemostatic, an anthelmintic, and in the treatment of rheumatism, kidney disorders, rickets, and conjunctivitis. Notably, traditional practitioners also use it to address nervous system disorders, drug addiction, and memory impairments, highlighting its potential as a neuroprotectiv

Vitamin D and Health Outcomes: State-of-the-Art Review of Triangulated Evidence and Ongoing Controversies.

PURPOSE OF REVIEW: Vitamin D is a pleiotropic hormone with an established role in skeletal integrity and broader actions in immune regulation, inflammation, cellular proliferation, and energy homeostasis. Despite decades of research, its extra-skeletal effects remain controversial, largely due to discordant findings across observational studies, Mendelian randomization studies (MRS), and randomized controlled trials (RCTs). Unlike many prior reviews, this state-of-the-art review synthesizes triangulated evidence across these study designs to clarify outcome-specific causal relationships and on
