There is evidence that calcitriol is the only biologically active vitamin D metabolite. This review summarizes data on the regulation of renal and extrarenal synthesis of calcitriol by nutritional, physiologic, mechanical, genetic, and disease-related factors. Relatively low circulating calcitriol due to low substrate availability, i.e., low circulating 25-hydroxyvitamin D, has been reported in nutritional rickets, osteomalacia, obesity, and preeclampsia. In these situations, vitamin D supplementation can increase circulating calcitriol and, together with calcium, prevent rickets/osteomalacia

Proper bone development during the growth phase is essential for achieving normal skeletal length, mineral density, and strength, which is regulated by growth hormones, vitamin D, and calcium. Deficiencies in these elements can impair skeletal formation, increasing the risk of disorders such as rickets and osteoporosis. Therefore, natural compounds with bone-supportive effects are of growing interest. Sedum sarmentosum (S. sarmentosum), a plant traditionally used in Korea to treat liver disorders, contains abundant vitamin C and calcium, however, its effects on bone development remain unclear.

The formation of kidney stones is a complex biologic process involving interactions among genetic, anatomic, dietary, and environmental factors. Traditional lithogenic models were based on urine supersaturation in relation to the activity of crystallization promoters and inhibitors. However, modern research has added new principles such as the "renal epithelial cell response" and the role of inflammation and oxidative stress leading to the development of a "multi-hit hypothesis". A strong correlation between urinary stones and kidney damage has been well demonstrated by both cohort and case-co

First reported in 1948, hypophosphatasia (HPP) is a rare systemic disease caused by deficient activity of tissue-nonspecific alkaline phosphatase (ALP) enzyme. Patients with HPP experience skeletal and dental manifestations such as rickets/osteomalacia, fractures, pseudofractures, and premature tooth loss, as well as nonskeletal symptoms such as pain and muscle weakness, which result in impaired mobility and poor quality of life. For decades, no specific treatment was available for HPP and the disease was often fatal in infants. Asfotase alfa is a tissue-nonspecific ALP enzyme replacement ther

Phosphaturic mesenchymal tumor (PMT) is a rare bone or soft tissue tumor. It exhibits distinctive morphologic features, including bland spindled cells, highly vascularized stroma, "smudgy or grungy" calcified matrix, and osteoclast-like giant cells. It is characterized by paraneoplastic tumor-induced osteomalacia through producing phosphatonins, especially FGF23. Recent findings suggest that its tumorigenesis and FGF23 overproduction depend on the FGFR1 pathways, which may be activated via FN1::FGFR1 or FN1::FGF1 fusion, as well as by α-Klotho overexpression in the majority of fusion-negative

PURPOSE: X-linked hypophosphatemia (XLH) is the most common type of hereditary hypophosphatemic rickets caused by elevated fibroblast growth factor 23 (FGF23). Multiple cases have reported XLH had Fanconi syndrome (FS) with unidentified mechanism. We investigated the association between genetic polymorphisms of phosphate transporters in renal proximal tubules and XLH with FS for the first time.
METHODS: 25 Chinese XLH patients with FS (XLH-FS) and 33 patients without any urine abnormalities (XLH-nonFS) were included. We collected their clinical manifestations and laboratory results, screened

Both hypophosphatemic osteomalacia and primary hyperparathyroidism (PHPT) can lead to hypophosphatemia, but their simultaneous occurrence in the same patient is exceedingly rare. This article reports a case of a 43-year-old female patient whose primary clinical manifestations included pain in the lumbosacral and scapular regions, restricted mobility, and biochemical findings of decreased serum phosphate levels with normal parathyroid hormone (PTH) levels. The patient's symptoms improved after treatment with active vitamin D supplementation, although neutral phosphate supplements were not admin

Hereditary hypophosphatemic rickets with hypercalciuria (HHRH) is a rare genetic condition with autosomal recessive inheritance and a prevalence of 1 in 250000. It is due to mutation of the SLC4A3 gene. Correct diagnosis of this condition is important as treatment with active vitamin D metabolites is contraindicated. Evolution of the disease despite initial completely normal biochemistry has been observed, potentially creating diagnostic confusion. The first child presented at the age of 5.5 years with features of rickets. He had an abnormal bone profile with normal vitamin D levels. Urinary p

X-linked hypophosphatemic rickets (XLH) is a rare genetic disorder with a frequency of 1:20,000, caused by mutations in the PHEX gene, resulting in impaired phosphate metabolism and bone mineralization. There is an association between hypophosphatemia and dental issues, though this link is not definitively established. This study aims to evaluate the dental status, including oral hygiene, caries prevalence, and malocclusions, as well as parental awareness of dental complications, in children with XLH in Bulgaria, particularly those receiving or about to begin burosumab treatment, and to compar

PURPOSE OF REVIEW: In this review, we will examine the pathophysiology, anatomy, biochemistry, and genotype-phenotype correlation of femoral fractures in adult hypophosphatasia.
RECENT FINDINGS: Hypophosphatasia (HPP) is a rare genetic disease characterized by low activity of tissue-nonspecific alkaline phosphatase (TNAP). The disease presents a broad spectrum of clinical manifestations primarily determined by the degree of residual TNAP activity. Adults with HPP of moderate clinical severity may present with spontaneous femoral fractures that are like the atypical femoral fractures (AFF) of

Rare diseases, defined by the 2002 Rare Disease Act, affect fewer than 5 in 10,000 individuals. Rare metabolic bone diseases (MBDs), such as osteogenesis imperfecta, hypophosphatasia, osteopetrosis, and other unclassified disorders, can disrupt bone development and remodeling, posing diagnostic and management challenges. This study analyzed data from the rarembd.in registry (2010-2024), a 15-year database documenting only rare MBDs. Clinical presentation and demographic data of patients with rare MBDs were collated. Common MBDs (osteoporosis, primary hyperparathyroidism) were excluded. Genetic

BACKGROUND: In the second half of the 19th century, the Swiss autodidact Arnold Rikli developed his concept of the "atmospheric cure" as part of naturopathic efforts and implemented it in Veldes (today Bled, Slovenia).
OBJECTIVE: What concepts did Riki develop and what indications did he see. How is this to be assessed from today's perspective?
MATERIALS AND METHODS: Rikli's own writings are used to evaluate his "light air cure" as a form of phototherapy. Secondary literature is used to evaluate Rikli's status in his time as well as contradictions and ambivalences from a modern perspective.

Tumor-induced osteomalacia (TIO) is a rare disorder caused by a phosphaturic mesenchymal tumor (PMT) secreting fibroblast growth factor 23 (FGF23). The aim of this study was to analyze PMTs for their transcriptomic characteristics. We performed single-cell RNA (n = 3) alongside bulk RNA sequencing of PMTs (n = 5) and surrounding bone tissue (n = 4) obtained during tumor removal in 10 patients (age 44 (41;64), serum phosphate (Pi)- 0.54 (0.43; 0.59) mM/L, FGF23-113 (40; 205) pg/ml). We revealed a total of 22,449 cells divided into 13 different categories. We identified the heterogeneity of the

Burosumab, a human immunoglobulin G1 monoclonal antibody that targets fibroblast growth factor 23 (FGF23), was developed to treat X-linked hypophosphatemic rickets/osteomalacia (XLH). By binding to FGF23, burosumab promotes the reabsorption of phosphorus from the kidney, increases the production of active vitamin D, and promotes the intestinal absorption of phosphorus and calcium. Herein, we report a one-year evaluation of physical function in adult patients with XLH who were treated using burosumab. A 40-year-old male patient with a history of lower limb varus deformity was diagnosed with vit

Distal renal tubular acidosis (dRTA) is an uncommon disorder marked by defective hydrogen ion excretion in the distal nephron, leading to hyperchloremic metabolic acidosis, persistently alkaline urine, and hypokalemia. Chronic, untreated dRTA in children often presents with growth failure, refractory rickets, and nephrocalcinosis, yet is frequently misdiagnosed. We report a 13-year-old girl with severe stunting, bone pain, proximal muscle weakness, and polyuria for several years. Examination revealed significant short stature, malnutrition, widened wrists, a protuberant abdomen, and genu valgu
