Bone Turnover-Guided Treatment of Low Bone Density in Haemodialysis Patients Using Bone Biopsy and Fluoride Positron Emission Tomography Imaging.

AIM: Chronic kidney disease-mineral and bone disorder (CKD-MBD) contributes to skeletal fragility through abnormalities in bone turnover. This study evaluated turnover-guided therapy using bone histomorphometry and 18F-NaF PET/CT in haemodialysis patients with low bone mineral density (BMD). METHODS: Adult haemodialysis patients at a single centre (2018-2024) underwent turnover assessment by bone biopsy (n = 31) or PET/CT (n = 27). Patients were grouped according to whether treatment followed an institutional turnover-guided protocol introduced in 2020. Outcomes included changes in BMD, bone

Dysregulated neutrophils contribute to bone loss in renal osteodystrophy by enhancing osteoclastogenesis: Insights from integrated bioinformatics and experimental validation.

Renal osteodystrophy (ROD) is a major debilitating skeletal complication of chronic kidney disease characterized by dysregulated bone remodeling. Neutrophils are essential for bone homeostasis. However, their functionality in the ROD osteoimmune microenvironment remains unclear. To investigate the crosstalk between neutrophils and bone metabolic imbalance in ROD, we firstly performed transcriptomic profiling in patients with ROD, revealing a pronounced disruption of bone homeostasis. These findings were corroborated in a murine model of ROD, which demonstrated an osteoporotic phenotype and inc

Is guided growth a viable treatment option for angular deformity correction in distal renal tubular acidosis patients?

BACKGROUND: Distal renal tubular acidosis (dRTA) is a rare disorder marked by impaired hydrogen ion secretion in the distal nephron, leading to metabolic acidosis and skeletal manifestations resembling rickets. While medical treatment with alkali, calcium, and vitamin D supplementation is essential, severe coronal plane knee deformities may necessitate surgical correction. Traditional osteotomy is an invasive modality, and data on less invasive options such as guided growth with eight-plate hemiepiphysiodesis in dRTA is limited. METHODS: This retrospective study included seven children (6 mal

Beyond Nutritional Deficiency: Persistent Rickets as a Manifestation of Vitamin D-Dependent Rickets Type II in a Toddler.

Vitamin D-dependent rickets type II (VDDR-II) is a rare hereditary disorder caused by mutations in the vitamin D receptor gene, resulting in resistance to active vitamin D and impaired calcium absorption. We report a 2-year and 6-month-old female toddler presenting with persistent rickets, delayed motor milestones, dental abnormalities, and recurrent hypocalcemia despite standard vitamin D and calcium supplementation. Initial assessment of nutritional rickets was reconsidered due to the development of pathological fractures and persistent biochemical abnormalities-including hypocalcemia, eleva

Meta-organismal tryptophan metabolism: an appealing therapeutic target in CKD-MBD.

Despite important advances over the past few decades, chronic kidney disease-mineral and bone disorder remains a major clinical therapeutic challenge. Traditional interventions targeting hyperphosphatemia, impaired vitamin D metabolism, and secondary hyperparathyroidism overall failed to meet expectations. This calls for a paradigm shift. The 2023 Madrid Chronic Kidney Disease-Mineral and Bone Disorder Kidney Disease: Improving Global Outcomes (KDIGO) controversies conference advocated a holistic approach to replace the current parathyroid hormone-calcium phosphate-centric approach. In this co

A new metabolic path in type 3 rickets.

Rickets, a disorder of bone formation, was originally known as nutritional rickets due to vitamin D deficiency. Advances in science have since identified various genetic forms, typically involving loss-of-function mutations in vitamin D activation or other mineral metabolism pathways. Recently, type 3 rickets was identified as a previously undescribed gain-of-function mutation in CYP3A4 (Ile301Thr). This mutant enzyme leverages the unique features of cytochrome P450 to produce an inactive vitamin D metabolite, 11α,25(OH)2D3, resulting in insufficient active vitamin D. The discovery of this uni

Burosumab in Chinese adults with X-linked hypophosphatemia: a phase 4 study.

X-linked hypophosphatemia (XLH) is a rare genetic disorder, for which burosumab treatment may be beneficial. This study evaluated the efficacy, pharmacokinetics, and safety of burosumab in Chinese adults with XLH. In this open-label, multicenter, postmarketing phase 4 study, 18 Chinese adult outpatients with XLH received conventional oral phosphate (30 mg/kg/d in 5 doses) and calcitriol (0.50 μg/d in 2 doses) during a 14-wk run-in period. Patients then received subcutaneous burosumab 1.0 mg/kg every 4 wk for 48 wk (12 doses). The main outcome measures were changes from baseline in mean serum p

Complex Febrile Seizures As the Initial Presentation of Nutritional Rickets Due to Severe Vitamin D Deficiency.

Simple febrile attacks are common in young children and are typically benign. However, complex febrile seizures are characterized by recurrence, focal features, or longer duration and require assessment to determine underlying causes, such as metabolic issues like hypocalcemia. Vitamin D deficiency rickets, a preventable yet re-emerging disease, can also present with seizures. We report a previously healthy, exclusively breastfed eight-month-old boy who developed three episodes of general tonic seizures, initially thought to be triggered by an upper respiratory infection. Further workup reveal

Reconsidering the Widespread Use of Active Vitamin D Analogues for Osteoporosis in Japan: A Call for Evidence-Based Prescription Practices.

In Japan, active vitamin D has been widely prescribed for osteoporosis due to its regulatory approval, in contrast to international practice where it is not considered standard therapy. However, growing evidence indicates that these agents offer limited efficacy in fracture prevention and may pose significant risks. Mechanistically, active vitamin D increases both serum calcium and phosphate, which, in the absence of bone-forming signals such as growth, fracture healing, or high mechanical loading, may promote ectopic calcification rather than bone strengthening. Osteoporosis, particularly in

ETA guidelines for the use of levothyroxine sodium preparations in monotherapy to optimize the treatment of hypothyroidism.

Bones form the skeletal framework, enabling movement, mineral balance, and hematopoiesis. Bone remodelling - tightly controlled by hormones, cytokines, and growth factors - is a dynamic process involving different cell types: osteoclasts, osteoblasts, and osteocytes; disrupted remodelling leads to disorders such as osteoporosis, arthritis, rickets, and osteomalacia. This review examines how osteoblast differentiation and bone metabolism are influenced by important hormonal regulators, including parathyroid hormone (PTH), which is the key regulator, followed by the effect of calcitonin, vitamin

PLAIN ENGLISH SUMMARY: Parents of children with physical disabilities do a lot to support their child in daily life. In doing this they are faced with many challenges. These parents have a wide range of unmet needs, especially for information, on different topics. It is sometimes hard for them to get the right information at the right moment, and to ask the right questions to physicians and other healthcare professionals. In order to develop a digital tool to help parents formulate questions and find information, we thought it would be crucial to work together in a process of co-creation with

Hereditary Rickets: A Quick Guide for the Pediatrician.

With the increased discovery of genes implicated in vitamin D metabolism and the regulation of calcium and phosphate homeostasis, a growing number of genetic forms of rickets are now recognized. These are categorized into calciopenic and phosphopenic rickets. Calciopenic forms of hereditary rickets are caused by genetic mutations that alter the enzymatic activity in the vitamin D activation pathway or impair the vitamin D receptor action. Hereditary forms of phosphopenic rickets, on the other hand, are caused by genetic mutations that lead to increased expression of FGF23 hormone or that impai

Revisiting the significance of nano-vitamin D for food fortification and therapeutic application.

OBJECTIVE: Vitamin D (a prohormone) is an important micronutrient required by the body for skeletal homeostasis and a range of non-skeletal actions. Calcitriol, the active form of vitamin D, regulates a variety of cellular and metabolic processes through both genomic and nongenomic pathways. Often prescribed for treating rickets and osteoporosis, vitamin D deficiency can exacerbate various other medical conditions. SIGNIFICANCE, METHODS, AND RESULTS: Despite its multifunctional uses, the sensitivity of vitamin D makes formulating an efficient drug delivery system a challenging task, which is

Successful Burosumab Treatment in an Adult Patient with X-Linked Hypophosphatemia and Chronic Kidney Disease Stage 3b.

X-linked hypophosphatemic rickets (XLH) is a genetic cause of renal hypophosphatemia due to inactivation of the PHEX gene, with an inappropriate concentration of fibroblast growth factor 23 (FGF23). Burosumab, an anti-FGF23 monoclonal antibody, is a validated treatment for XLH, but its use in patients with chronic kidney disease (CKD) has not been validated. A 61-year-old man with XLH developed CKD during follow-up. Conventional treatment (phosphate salts and active vitamin D analogs) was poorly tolerated. Treatment with burosumab was decided at a multi-professional meeting. Before burosumab i

The role of irisin in kidney diseases.

Irisin is a hormone that is produced mainly by skeletal muscles in response to exercise. It has been found to have a close correlation with obesity and diabetes mellitus for its energy expenditure and metabolic properties. Recent research has revealed that irisin also possesses anti-inflammatory, anti-oxidative and anti-apoptotic properties, which make it associated with major chronic diseases, such as chronic kidney disease (CKD), liver diseases, osteoporosis, atherosclerosis and Alzheimer s disease. The identification of irisin has not only opened up new possibilities for monitoring metaboli

FGF23-related hypophosphatemic rickets preceding the onset of systemic lupus erythematosus: A juvenile case.

KEY CLINICAL MESSAGE: This case report describes the clinical course of a juvenile female with FGF23-related hypophosphatemic rickets preceding the onset of SLE. Our study demonstrates the possibility of hypophosphatemic rickets as an early symptom of SLE. ABSTRACT: Fibroblast growth factor 23 (FGF23)-related hypophosphatemic rickets is observed in both genetic and acquired disorders. Various reports describe FGF23-related hypophosphatemia with systemic lupus erythematosus (SLE), although FGF23-related hypophosphatemia preceding the onset of SLE has never been described. Here, we report the c

Disease Manifestations and Complications in Dutch X-Linked Hypophosphatemia Patients.

X-linked hypophosphatemia (XLH) is the most common monogenetic cause of chronic hypophosphatemia, characterized by rickets and osteomalacia. Disease manifestations and treatment of XLH patients in the Netherlands are currently unknown. Characteristics of XLH patients participating in the Dutch observational registry for genetic hypophosphatemia and acquired renal phosphate wasting were analyzed. Eighty XLH patients, including 29 children, were included. Genetic testing, performed in 78.8% of patients, showed a PHEX mutation in 96.8%. Median (range) Z-score for height was - 2.5 (- 5.5; 1.0) in

A patient-centred and multi-stakeholder co-designed observational prospective study protocol: Example of the adolescent experience of treatment for X-linked hypophosphataemia (XLH).

The importance of patient centricity and keeping the patient at the heart of research design is now well recognised within the healthcare community. The involvement of patient, caregiver and clinician representatives in the study design process may help researchers to achieve this goal and to ensure robust and meaningful data generation. Real-world data collection allows for a more flexible and patient-centred research approach for gaining important insights into the experience of disease and treatments, which is acutely relevant for rare diseases where knowledge about the disease is more like

X-linked hypophosphatemia: The value of feedback focus groups to assess patient and caregiver needs.

X-linked hypophosphatemia (XLH) is a rare, multi-systemic, invalidating disease requiring a multi-disciplinary approach. No specific action in XLH, neither for the patients' specific needs nor for the methodology for the evaluation of these were found. Thus, to identify the needs of XLH patients and their caregivers, we organised focus groups in our reference centre with a view to build educational sessions. Focus groups including either XLH children, XLH adults, or caregivers ran in parallel. Each group was led by a person trained in therapeutic education (nurse, paediatric nephrologist) with
