COVID-19 and long-term risk of incident laboratory-defined micronutrient deficiency in patients with type 2 diabetes: a multicenter propensity score-matched cohort study.

BACKGROUND: COVID-19 has been linked to broad post-acute sequelae, but delayed micronutrient deficiency remains insufficiently characterized. Whether COVID-19 increases long-term risks of laboratory-defined vitamin D, magnesium, and vitamin B12 deficiencies among patients with type 2 diabetes mellitus is unclear. METHODS: Using the predominantly US-based TriNetX network, we identified adults with type 2 diabetes mellitus (T2DM) during 2020-2022 who had COVID-19 and matched controls without COVID-19. A 12-month landmark design excluded pre-existing deficiencies, skeletal outcomes, and early de

Hypohidrosis, electrolyte imbalance, lacrimal gland dysfunction, ichthyosis and xerostomia syndrome caused by a novel claudin-10 variant in an adult with hypokalaemic metabolic alkalosis.

We report a man in his mid-30s who presented with persistent hypokalaemia and was initially suspected to have Gitelman syndrome because of hypokalaemic metabolic alkalosis, hypocalciuria and normal blood pressure. However, the absence of hypomagnesaemia was atypical, and a urine osmolality that was low relative to a concurrent serum osmolality of 285 mOsm/kg prompted further evaluation, which confirmed a partial urinary concentrating defect on formal testing. Genetic testing identified a novel homozygous frameshift variant in CLDN10 NM_006984.5(CLDN10):c.113del, p.(Thr38Argfs*46) confirming HE

Molecular Mechanisms of Obesity-Associated Oxidative Stress and Therapeutic Strategies.

Obesity is a chronic metabolic disorder characterized by excessive adipose tissue accumulation and associated with systemic oxidative stress, mitochondrial dysfunction, and chronic low-grade inflammation. Adipose tissue dysfunction promotes the overproduction of reactive oxygen species through multiple mechanisms, including NADPH oxidase activation, mitochondrial electron transport chain leakage, and endoplasmic reticulum stress via mitochondria-associated ER membranes. The resulting oxidative stress damages lipids, proteins, and DNA, while activating redox-sensitive signaling pathways that pe

Adult-onset pseudohypoparathyroidism type 1B diagnosed by methylation analysis: A case report and diagnostic considerations.

RATIONALE: Pseudohypoparathyroidism type 1B (PHP1B) is a rare endocrine disorder caused by epigenetic defects at the GNAS locus, leading to isolated renal resistance to parathyroid hormone (PTH). Although typically identified in childhood, adult-onset cases are uncommon and easily overlooked. This case highlights the diagnostic challenge of late-onset PHP1B and the critical role of methylation-specific testing. PATIENT CONCERNS: A 33-year-old man was referred for evaluation of incidentally detected hypocalcemia (serum calcium 5.9 mg/dL) during a routine health examination. He complained of mi

Residential radon in Peru: challenges for health surveillance and lung cancer prevention.

The Journal of Genetics, started by William Bateson in 1910, played a distinguished role in the early years of genetics. However, it stopped publishing in 1978. The Indian Academy of Sciences revived it in 1985, and has published it regularly since then. To commemorate this landmark, I highlight one of the 17 articles published that year. 'The isolation and genetic analysis of a Caenorhabditis elegans… X-chromosome balancer' by András Fodor and Péter Deak, of the Institute of Genetics, Biological Research Centre, Hungarian Academy of Sciences, Szeged, Hungary. Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Magnesium protects against anaphylactic shock and cardiac myolysis in guinea-pigs.

The pathophysiological responses to immune stress (IS) include activation of several processes which are dependent on cytosolic Ca2+ elevation. Magnesium frequently acts as a natural Ca2+ antagonist. In this study we have observed that Mg2+ can protect guinea-pigs against IS. Antigen-sensitized guinea-pigs, which had been fed a magnesium-deficient diet, were given a single dose (15 mg) of MgCl2 intraperitoneally 1 h before antigen challenge. The development of anaphylactic shock (AS) was observed during the next 2 h, and the hearts were subsequently examined histologically for signs of cardiac

Severe Hypomagnesemia and Hypocalcemia Linked to Semaglutide in Type 2 Diabetes: A Case Report.

BACKGROUND Although hypomagnesemia is common in type 2 diabetes, clinical presentations with severe hypomagnesemia are rare. A number of oral blood glucose-lowering medications can reduce serum magnesium concentrations, and several severe cases have been reported in the presence of marked glucagon-like peptide-1 receptor agonist (GLP-1RA)-associated gastrointestinal adverse effects. In the present case, an acute presentation with severe hypomagnesemia was likely due to polypharmacy including semaglutide, albeit with a delayed relationship to discontinuation of this GLP-1RA, due to nausea and v

Adult-Onset Hypomagnesemia With Secondary Hypocalcemia Caused by a Novel Variant in TRPM6 Gene: A Case Report.

Hereditary hypomagnesemia with secondary hypocalcemia (HSH) is a rare autosomal recessive disorder caused by biallelic variants in the transient receptor potential melastatin 6 (TRPM6) gene, typically presenting in infancy. Currently, there is a lack of reports in the literature on adult-onset cases. This case report describes a 51-year-old male with adult-onset HSH, presenting with limb weakness, muscle spasms, and electrolyte imbalances, including severe hypomagnesemia (0.28 mmol/L). Genetic testing revealed a novel heterozygous variant in the TRPM6 gene (c.4914del, p.E1638Dfs*8), classified

Higher magnesium depletion score increases the risk of all‑cause and cardiovascular mortality in US adults with diabetes.

BACKGROUND: Both dietary magnesium and serum magnesium are associated with the prognosis of diabetic patients. However, the impact of the magnesium depletion score (MDS), which assesses systemic magnesium deficiency, on the prognosis of diabetic patients remains unclear. This cohort study aims to explore the potential association between the MDS and all-cause and cardiovascular mortality in diabetic patients. METHODS: In this study, we analyzed data from 5,219 diabetic individuals from National Health and Nutrition Examination Survey (NHANES) 2003-2018. Participant mortality information was s

Magnesium Depletion Score and Mortality in Individuals with Metabolic Dysfunction Associated Steatotic Liver Disease over a Median Follow-Up of 26 Years.

UNLABELLED: Metabolic dysfunction associated steatotic liver disease (MASLD) has been associated with increased risks of all-cause and cardiovascular disease (CVD) mortality. Identification of modifiable risk factors that may contribute to higher risks of mortality could facilitate targeted and intensive intervention strategies in this population. This study aims to examine whether the magnesium depletion score (MDS) is associated with all-cause and CVD mortality among individuals with MASLD or metabolic and alcohol associated liver disease (MetALD). METHODS: A total of 3802 participants with

Unraveling the link between magnesium and diabetic neuropathy: Evidence from in vitro to clinical studies.

Diabetic neuropathy (DN) is one of the major complications of diabetes and the most common cause of neuropathic pain. Although the underlying pathological mechanisms remain unclear, several studies have produced conflicting results regarding the link between magnesium (Mg) concentration and DN. This ambiguity raises questions about the potential benefits of Mg supplementation in individuals with DN. Therefore, this comprehensive review summarizes and discusses the evidence from clinical, in vitro, and in vivo studies on the association between Mg and DN. Several findings indicate that Mg deple

The impact of undernutrition on the association between hypomagnesemia and dynapenia in older women.

OBJECTIVES: To determine the association between hypomagnesemia and dynapenia in older women with different nutritional status. METHODS: This cross-sectional study included older women who attended one outpatient geriatric clinic. Undernutrition was defined according to the Mini Nutritional Assessment score (MNA) (< 23,5), and handgrip strength of < 16 kg on dynamometer was defined as dynapenia. The association between hypomagnesemia (serum magnesium < 1.7 mg/dL) and dynapenia was determined by logistic regression analysis. RESULTS: Among the 933 older women (mean age 81 ± 8), the prevalence
