Empirical Antibiotic Activity and Outcomes in Pediatric Gram-negative Bloodstream Infections: A Rank-based Composite Outcome Analysis.

BACKGROUND: Timely active empirical therapy (AET) may improve outcomes following bloodstream infection. In high-income settings, low pediatric mortality following gram-negative bloodstream infection (GNBSI) limits its value as a sole endpoint. Composite measures that incorporate multiple clinically relevant outcomes may detect differences more readily. We assessed the effect of AET on clinical outcomes in pediatric GNBSI. METHODS: We analyzed data from a prospective surveillance study of hospitalized Australian children with GNBSI. A ranked composite endpoint included 30-day all-cause mortali

Beyond Bioremediation: A Rare Case of Comamonas testosteroni Bloodstream Infection in a Patient With Stage 5 Chronic Kidney Disease on Maintenance Hemodialysis.

Comamonas testosteroni is a gram-negative aerobic bacillus commonly found in environmental sources and known for its bioremediation potential. Although it is widely distributed in nature, human infection is rare, but it is increasingly being recognized as a nosocomial pathogen. A 68-year-old female with stage 5 chronic kidney disease (CKD V) on maintenance hemodialysis since 2017 presented with fever and decreased consciousness. On examination, a tunneled cuffed catheter (TCC) was noted in the right internal jugular vein. Two sets of blood cultures, one drawn from the TCC and the other from a

Protective efficacy of a PcrV-based vaccine formulated with chitosan nanoparticles against Pseudomonas aeruginosa infection in burn and sepsis mouse models.

Pseudomonas aeruginosa is an opportunistic Gram-negative pathogen and a major clinical concern due to its multidrug resistance and extensive virulence repertoire. In this study, recombinant PcrV protein was formulated with Alum and chitosan nanoparticles (CS-NPs) as co-adjuvants to enhance immunogenicity. CS-NPs were prepared via ionic gelation and characterized for morphology and charge. BALB/c mice were immunized with formulations containing r-PcrV + Alum + Nanochitosan, r-PcrV + Alum, r-PcrV + Nanochitosan, or Nanochitosan + Alum or PBS (control). This was followed by burn induction, after

Microbiological Profile and Antimicrobial Resistance in Intensive Care Unit Patients With Sepsis and Septic Shock: A Prospective Single-Center Study From a Tertiary Care Intensive Care Unit.

Background Sepsis and septic shock remain the major causes of mortality in intensive care units (ICUs), increasingly complicated by antimicrobial resistance. However, microbiological profiles and resistance patterns vary widely across regions, and data from North African ICUs remain limited. This study aimed to describe the distribution of pathogens according to infection site, to analyze antimicrobial susceptibility patterns, and to assess the burden of multidrug-resistant organisms among ICU patients with sepsis and septic shock. Methods We conducted a prospective observational study in a mi

Clinical Management and Outcomes of Urosepsis in Relation to Diagnostic Complexity and Microbiological Profile.

Background and Objectives: Urosepsis is a common cause of sepsis in adults and is associated with substantial morbidity and mortality, particularly when urinary obstruction delays timely source control. The roles of diagnostic uncertainty at presentation, microbiological phenotypes (including multidrug resistance), and biomarkers in shaping management pathways and outcomes warrant further evaluation. Materials and Methods: This retrospective, single-center, observational study included 154 consecutive adult patients hospitalized for urosepsis. Sepsis was defined according to the Sepsis-3 crite

Fulminant Chromobacterium violaceum Infection With Suspected Septic Arthritis and a Concurrent Dengue Virus Infection: A Case Report.

Chromobacterium violaceum is a rare but highly virulent Gram-negative organism found in tropical and subtropical environments, associated with rapidly progressive sepsis and high mortality. Musculoskeletal involvement, particularly septic arthritis, is exceedingly uncommon. We report a case of a previously healthy 23-year-old male who presented with progressive left ankle pain, swelling, and fever. Initial evaluation revealed thrombocytopenia, markedly elevated inflammatory markers, and positive serology for Dengue virus infection, suggestive of a possible concurrent infection. Blood cultures

Assessment of TIMP1, Transgelin-2, and Procalcitonin as Protein Biomarkers for Diagnosis and Prognosis of Bloodstream Infection in a Critically Ill Cohort.

BACKGROUND: Bloodstream infections (BSI) are a significant contributor to morbidity and mortality among hospitalized patients. Rapid diagnosis of BSI is critical for timely and appropriate clinical management, but current diagnostic strategies have a prolonged turnaround time or only identify select pathogens with minimal prognostic information. METHODS: We examined serial serologic samples from 77 intensive care unit (ICU) patients at time points before, during, and after diagnosis of presumptive BSI to identify host-derived protein biomarkers associated with specific organisms and with clin

RNA modifications shape innate immunity and cellular adaptation during bacterial respiratory infection.

Gram-negative bacteria are clinically significant pathogens responsible for life-threatening infections, including respiratory infections. These can be acute or persistent and can exacerbate existing chronic diseases, such as cystic fibrosis, COPD and lung cancer. In this review, we use Pseudomonas aeruginosa as a model organism that demonstrates the molecular complexity of host-pathogen interactions during lower airway infection. Specifically, we focus on RNA modifications and show that they, on the one hand, regulate bacterial fitness and pathogenicity, and on the other control the execution

Colonization with extended-spectrum and AmpC β-lactamase-producing Enterobacterales and bacteremia risk in patients with acute leukemia who receive induction chemotherapy without fluoroquinolone prophylaxis.

Neutropenic patients colonized with extended-spectrum β-lactamase-producing Enterobacterales (ESBL-E) frequently develop ESBL-E bacteremia while receiving fluoroquinolone prophylaxis. However, the impact of colonization with ESBL-E and AmpC β-lactamase-producing Enterobacterales (AmpC-E) in neutropenic patients who do not receive fluoroquinolone prophylaxis is unclear. To investigate this, we conducted a prospective study of patients with acute leukemia or myelodysplastic syndrome (MDS) receiving induction chemotherapy without antibacterial prophylaxis between November 2015 and January 2020. Hemodynamic Assessment in Newborn Infants With Sepsis: A Prospective Observational Study.

BACKGROUND AND OBJECTIVE: Sepsis is defined as SIRS (systemic inflammatory response syndrome) in the presence of confirmed or suspected infection. Neonatal sepsis remains a major cause of morbidity and mortality and is frequently complicated by septic shock, resulting in impaired tissue perfusion and organ dysfunction. Hemodynamic alterations in septic neonates are heterogeneous and often under-recognized. The primary objective of this study was to characterize the hemodynamic profile of neonates with sepsis for early detection of circulatory dysfunction using functional echocardiography. The

Tularemia-associated septic shock with pneumonia, lung abscesses, and pleural effusion leading to multiorgan failure: Is Francisella tularensis still a deadly pathogen in the antibiotic era? A case report.

OBJECTIVES: Francisella tularensis is a Gram-negative coccobacillus causing tularemia. Due to its high infectivity and potential for aerosol transmission, it is considered a potential biological weapon. Although tularemia is rare in most regions, it can cause severe community-acquired infections, particularly in individuals with environmental exposure, and may be life-threatening. PATIENTS AND METHODS: We report a confirmed case of F. tularensis infection diagnosed despite diagnostic difficulties. The patient developed pulmonary tularemia complicated by sepsis and lung abscess formation. The

Clinical features, risk factors and 28-day mortality in neonates with carbapenem-resistant sepsis or meningitis in a South African tertiary neonatal unit: a prospective cohort study.

INTRODUCTION: Carbapenem-resistant, Gram-negative bacteria (GNB) are a significant cause of neonatal sepsis, threatening survival. There is a scarcity of robust data on their burden and outcomes in low- and middle-income countries (LMICs). We aimed to describe clinical and microbiological characteristics, outcomes and mortality risk factors in neonates infected with GNB and assess the impact of carbapenem resistance. METHODS: Prospective observational cohort study from 1 February 2021 to 30 June 2023 conducted in a tertiary neonatal unit in Johannesburg, South Africa, in infants <90 days with

Vertical Transmission of Extended-Spectrum β-Lactamase (ESBL)-Producing Escherichia coli Infection in Premature Neonates: An Emerging Concern in Developed Countries.

Early-onset neonatal sepsis (EONS) is predisposed by maternal colonisation and neonatal risk factors. We present two cases of preterm neonates with vertically transmitted extended-spectrum β-lactamase (ESBL)-producing Escherichia coli sepsis. In the United Kingdom (UK), the common organisms causing EONS episodes are susceptible to the routinely used first-line empirical antibiotics, i.e., benzylpenicillin and gentamicin. These cases observed in a local neonatal unit highlight the growing concern of drug-resistant E. coli infections in this highly vulnerable age group. Gram-negative bacterial s

Longitudinal and adjusted prognostic assessment of calprotectin in Gram-negative sepsis with acute respiratory failure.

Sepsis caused by Gram-negative bloodstream infection is characterized by dynamic host responses, but the longitudinal and incremental prognostic value of calprotectin remains uncertain. We conducted a multicenter retrospective observational study of 82 adults with Gram-negative bloodstream infection, sepsis, and acute respiratory failure. Serum calprotectin, routine biomarkers, cytokines, and growth factors were measured at diagnosis before antibiotic therapy (T0), day 3 (T1), and day 7 (T2), and compared according to 28-day survival. Sixty-four patients survived, and 18 died. Calprotectin dec

A call for minimum standards in methodological reporting for studies using animal-borne accelerometers.

Fault-tolerant connectivity labelings are schemes that, given an n-vertex graph G = ( V , E ) and a parameter f, produce succinct yet informative labels for the elements of the graph. Given only the labels of two vertices u, v and of the elements in a faulty-set F with | F | ≤ f , one can determine if u, v are connected in G - F , the surviving graph after removing F. For the edge or vertex faults models, i.e., F ⊆ E or F ⊆ V , a sequence of recent work established schemes with poly ( f , log n ) -bit labels for general graphs. This paper considers the color faults model, recently introd
