Bactericidal effect of photodynamic inactivation against methicillin-resistant and methicillin-susceptible Staphylococcus aureus is strain-dependent.

Staphylococcus aureus is one of the most important etiological factors responsible for nosocomial infections. Some of them may be life-threatening, especially in the case of immuno-compromised patients, causing bacteremia, endocarditis, sepsis or toxic-shock syndrome. Their multiresistance to antibiotics produces many therapeutic problems, and for this reason the development of a method alternative to antibiotic therapy is needed. It seems that photodynamic inactivation (PDI) may be an effective and alternative therapeutic option against both methicillin resistant (MRSA) and methicillin sensit

[Panton-Valentine leukocidin positive Staphylococcus aureus isolated from blood in Korea].

BACKGROUND: Panton-Valentine leukocidin (PVL) is a pore-forming toxin secreted by some Staphylococcus aureus strains and associated with skin and soft tissue infections; these strains are epidemiologically associated with current outbreaks of community-acquired methicillin-resistant S. aureus (MRSA) and with necrotizing pneumonia in healthy adults in USA and Europe. This study was performed to investigate the presence of PVL-positive S. aureus and the significant infections known to be caused by this organism. METHODS: A total of 573 strains of S. aureus blood isolates at university-affiliate

Optimizing therapy for methicillin-resistant Staphylococcus aureus bacteremia.

Methicillin-resistant Staphylococcus aureus bacteremia in patients in intensive care units is associated with significant morbidity and mortality. Prompt clinical attention is essential to ensure good outcomes, including identification and management of the source of infection and any associated complications. Foreign-body sources of infection should be removed or replaced in the majority of cases, and debridement of infectious foci should be undertaken. Particular attention should be given to evaluation for the presence of cardiac involvement because inadequately managed S. aureus endocarditi

Diminished vancomycin and daptomycin susceptibility during prolonged bacteremia with methicillin-resistant Staphylococcus aureus.

An elderly patient with methicillin-resistant Staphylococcus aureus (MRSA) bacteremia was treated sequentially with vancomycin plus rifampin then daptomycin plus gentamicin. The MRSA strain developed diminished susceptibility to vancomycin (MIC increase and tolerance), daptomycin, and gentamicin, and resistance to rifampin during therapy.

[Total hip arthroplasty after hip arthrodesis performed for septic arthritis].

PURPOSE OF THE STUDY: Revision total hip arthroplasty (THA) after hip arthrodesis is an uncommon and challenging operation. The task would appear to be even more difficult if the arthrodesis was performed because of septic arthritis due to the theoretical risk of recurrent infection. We report our fifteen-year experience. MATERIAL AND METHODS: This retrospective study concerned 17 procedures performed in 17 patients (11 women, 6 men) between 1988 and 2003 on 5 right and 12 left hips. All of the patients had arthrodesis for sepsis: eight subsequent to tuberculosis and nine subsequent to septic

[Piercing--medical problems from otorhinolaryngological point of view].

Piercing is defined as puncturing an organ in order to place a jewel in the perforated site. There is hardly any external organ in the human body that has escaped piercing. The origin of piercing traces back to the dawn of human history. Piercing is performed for decorative or symbolic purposes. Many different medical specialists are confronted with the increasingly popular practice of body piercing in their daily practice. Until recently body piercing was mainly confined to the ears and/or nose. In the last few years there has been a significant increase in the prevalence of body piercing. Th

Four years of monitoring antibiotic resistance in microorganisms from bacteremic patients.

From the second semester of 2002 to the end of the first semester of 2005, a total of 2544 bacterial strains were isolated from the blood stream of patients with clinical sepsis and bacteremia hospitalized in six University Hospitals in the Slovak Republic. Almost 30% of strains were coagulase-negative staphylococci (CONS), about 14% were Staphylococcus aureus and, of the Gram-negative bacteria, up to 9% were Klebsiella pneumoniae. All CONS, S. aureus and Enterococcus spp. strains were found to be still susceptible to vancomycin, but the resistance of CONS and/or S. aureus to macrolides and fl

Community-acquired pneumonia and bacteremia due to methicillin-resistant Staphylococcus aureus carrying Panton-Valentine-leukocidin gene in Greece: two case reports and literature review.

We report the first two cases of community-acquired necrotizing pneumonia and bacteremia complicated by acute respiratory distress syndrome (ARDS) due to Panton-Valantine leukocidin-producing methicillin-resistant Staphylococcus aureus (MRSA-PVL) in Greece, together with a short literature review. Diagnosis was made by culture and broad spectrum PCR of respiratory secretions and blood. One patient received appropriate therapy and recovered fully. The other one died rapidly due to septic shock and life-threatening hemoptysis. Clinicians should be suspicious of community-acquired pneumonia due t

Hepatic iron content and the risk of Staphylococcus aureus bacteremia in liver transplant recipients.

Iron is a critical nutrient source and contributes to staphylococcal pathogenesis. We assessed the role of hepatic explant iron overload as a risk factor for Staphylococcus aureus bacteremia in liver transplant recipients. Seven of 13 cases with S aureus bacteremia (53.8%) had hepatic explant iron concentrations that exceeded normal limits (grade > or = 2). Length of posttransplant intensive care unit stay (P= .013) and hepatocellular carcinoma as underlying liver disease (P = .04), but not hepatic explant iron concentration, correlated with a higher risk of S aureus bacteremia after transplan

Hospitalizations and deaths caused by methicillin-resistant Staphylococcus aureus, United States, 1999-2005.

Hospital-acquired infections with Staphylococcus aureus, especially methicillin-resistant S. aureus (MRSA) infections, are a major cause of illness and death and impose serious economic costs on patients and hospitals. However, the recent magnitude and trend of these infections have not been reported. We used national hospitalization and resistance data to estimate the annual number of hospitalizations and deaths associated with S. aureus and MRSA from 1999 through 2005. During this period, the estimated number of S. aureus-related hospitalizations increased 62%, from 294,570 to 477,927, and t

Epidemiology and outcomes for Staphylococcus aureus bacteraemia in Australian hospitals, 2005-06: report from the Australian Group on Antimicrobial Resistance.

The Australian Group on Antimicrobial Resistance studied the epidemiology and outcomes of Staphylococcus aureus bacteraemia in selected Australian hospitals in 2005-06. Seventeen hospital-based laboratories collected basic demographic, susceptibility and patient outcome data on all cases of S. aureus bacteraemia for 5 to 24 months during the study period. There were 1,511 cases of bacteraemia documented, of which 66% occurred in males and 32% originated from vascular access devices. Bacteraemia had a community onset in 60% of cases, although 31% of these were health-care associated. Overall, 5

Outcomes of bacteremia in burn patients involved in combat operations overseas.

BACKGROUND: Burn patients constitute approximately 5% of casualties injured in support of US military operations in Iraq (Operation Iraqi Freedom [OIF]) and Afghanistan (Operation Enduring Freedom [OEF]). Since the onset of these conflicts, there have been numerous casualties infected with multidrug-resistant bacteria. It is currently unclear if bacteremia with these multidrug-resistant organisms in OIF/OEF burn casualties is associated with increased mortality. STUDY DESIGN: We performed a retrospective cohort study of all patients admitted to the US Army Institute of Surgical Research burn

Is reduced female survival after mating a by-product of male-male competition in the dung fly Sepsis cynipsea?

BACKGROUND: In a number of species males damage females during copulation, but the reasons for this remain unclear. It may be that males are trying to manipulate female mating behaviour or their life histories. Alternatively, damage may be a side-effect of male-male competition. In the black scavenger or dung fly Sepsis cynipsea (Diptera: Sepsidae) mating reduces female survival, apparently because males wound females during copulation. However, this damage does not seem to relate to attempted manipulation of female reproduction by males. Here we tested the hypothesis that harming females duri
