The critical care nutrition landscape in sub-Saharan Africa: Field insights and clinical commentary from resource-limited clinical settings.

Critical care resources are very limited on the African continent. Within these limited resources, adequate and formalized critical care nutrition support is an even rarer clinical resource. While nutrition products for nasogastric feeding are specified in international consensus guidelines as essential requirements for critical care, these are desperately undersupplied in African Intensive Care Units (ICU). Malnutrition in ICUs in low and middle income countries is 65-78%, roughly double that of developed countries. Furthermore, malnutrition prevalence among severely ill oncology and infectio

Intertwined paths: exploring the link between oral diseases and eating disorders. A comprehensive narrative review.

Eating disorders (ED) are psychiatric conditions with profound impacts on physical health, emotional well-being and quality of life. They are associated with reduced employment participation and increased healthcare costs, representing a significant public health concern. Major ED, including anorexia nervosa, bulimia nervosa, binge-eating disorder and other specified feeding and eating disorders, are closely linked to oral health complications, which serve as both diagnostic markers and therapeutic targets in ED management. This narrative review explores twenty-two studies, organised around tr

Clinical trials of vitamin D supplementation and cardiovascular disease: A synthesis of the evidence.

The evidence linking vitamin D deficiency with increased risk of cardiovascular disease (CVD) extends back to 1970s when case control studies showed lower circulating concentrations of 25-hydroxyvitamin D [25(OH)D] in myocardial infarction cases compared with controls, which was strengthened by the identification of a vitamin D receptor in cardiac muscle in 1980s. Cohort studies published in the 2000s provided stronger evidence (by measuring 25(OH)D concentrations before the onset of CVD) and confirmed the inverse association between circulating 25(OH)D concentrations and CVD risk. However, co

Types of Vascular Access and Associated Clinical Outcomes in Dialysis Patients.

Background Vascular access in patients with end-stage renal disease (ESRD) plays a crucial role in determining both the quality of life and survival rates, as it directly affects dialysis effectiveness. It connects the patient to the dialysis machine and significantly impacts the dialysis dose by influencing blood flow efficiency. Despite advancements in technology, improved care, and collaborative efforts by healthcare providers and patients, complications related to vascular access continue to be a major cause of hospitalizations and mortality among patients with ESRD. Our study aims to eval

Prognostic Factors for Refractory Outcome in Localizing TIO: Experience in a Tertiary Center.

CONTEXT: Tumor-induced osteomalacia (TIO), a paraneoplastic disorder characterized by renal phosphate wasting, is cured by surgical removal of the culprit tumor. Despite correct localization, some remain refractory to intervention, resulting in substantial long-term medical complications. OBJECTIVE: We aim to identify risk factors associated with a refractory outcome. METHODS: This is a retrospective cohort of 44 patients with TIO diagnosed from 1998 to 2023 who underwent targeted intervention following successfully localization. Cure was defined as maintenance of normophosphatemia without s

Understanding the role of nutritional status on the outcomes of nonvariceal upper gastrointestinal bleeding: Findings from a retrospective cohort analysis.

BACKGROUND & AIMS: Nonvariceal upper gastrointestinal bleeding (NVUGIB) is a common cause of hospitalization worldwide. Malnutrition, on the other hand, varies in prevalence and closely correlates with age and comorbidities. Though malnutrition is linked to poor outcomes generally, its impact on NVUGIB patients remains unexplored. METHODS: Using the National Readmission Database (NRD) from 2016 to 2020, we employed the International Classification of Diseases, 10th revision, Clinical Modifications (ICD-10-CM) codes to identify adult patients (aged ≥18 years) admitted with a principal diagnosi

Treatment outcomes of pediatric patients on high dose pure fish oil-based fat emulsion: A retrospective study.

BACKGROUND: Pure fish oil-based fat emulsion (FO-ILE) dosed at 1 g/kg/day is FDA approved for the treatment of intestinal failure associated liver disease. However, this limited fat provision can lead to suboptimal weight gain and excessive caloric intake from dextrose, particularly in neonates. There is limited data on the use of FO-ILE at doses higher than 1 g/kg/day. This study describes our experience with pediatric patients receiving 1.5 g/kg/day of FO-ILE. METHODS: A retrospective chart review was performed on patients receiving parenteral nutrition aged 0-18 years and receiving FO-ILE

Micronutrient status among patients diagnosed with eating disorders.

OBJECTIVE: Micronutrients are essential for physiological functioning, and deficiencies may lead to adverse health outcomes. As micronutrient intake depends entirely on dietary consumption, individuals with eating disorders (EDs) are particularly vulnerable to developing deficiencies. Although numerous studies have investigated micronutrient status in individuals with EDs, findings remain inconsistent, sample sizes are often limited, and most research has focused primarily on anorexia nervosa (AN). This observational cross-sectional study aimed to examine micronutrient levels in blood across m

Postoperative nutritional support after pancreaticoduodenectomy in adults.

BACKGROUND: Resection of the head of the pancreas is most commonly done by a pancreaticoduodenectomy, known as a Whipple procedure. The most common indication for pancreaticoduodenectomy is malignancy, but can include benign tumours and chronic pancreatitis. Complete surgical resection, with negative margins, provides the best prospect of long-term survival. Pancreaticoduodenectomy involves specific and unique alterations to the digestive system and maintaining nutritional status (optimising outcomes and achieving resumption of a normal diet) in patients with cancer after major surgery is a ch

Hematological Correlations as Predictors of Disease Manifestations in Psychiatric Inpatients.

Background/Objective. Psychiatric disorders exhibit significant symptomatic and etiopathological heterogeneity, complicating diagnosis and treatment. Hematological parameters may serve as indicators of overall health and predictors of psychiatric symptom manifestation and remission, particularly in long-term hospitalized patients. This study evaluated hematological and biochemical markers, including vitamin B12, vitamin D, and glucose levels, to explore their potential role in psychiatric disorders and disease progression. Methods. This prospective observational study was conducted from 1 Janu

Determinants of underweight among HIV-Infected children in pastoral communities of Southern Ethiopia: a facility-based cross-sectional study.

BACKGROUND: The effect of HIV infection on the nutritional status of patients has been widely known for several years, as well as the synergy between the infection and the conditions of poverty in developing countries. However, particular communities like pastoralists in underdeveloped countries live in a unique crossroad where challenges related to HIV prevention and control, and vulnerability to the disease exacerbate each other. Thus information from such range of contexts is valuable in addressing the complicated medical condition resulting from the combination of HIV infection and malnutr

Malnutrition and Opioid Use Disorder Are Associated With Higher 30-day Readmission Rates in Patients Admitted for Ulcerative Colitis.

BACKGROUND AND GOALS: Ulcerative colitis (UC) readmissions to the hospital pose a burden to patients and health care systems. Preventing readmissions is important for the quality of care and quality of life of patients with UC. We aimed to identify the incidence of 30 and 90-day readmissions and its predictors in patients with UC. STUDY: Retrospective review of the 2018 National Readmission Database (NRD) of adult patients readmitted after an index admission (IA) of UC. ICD-10CM/PCS codes were used to identify patients with UC, other comorbidities, and procedures. We identified the most commo

Not All Maca Is Created Equal: A Review of Colors, Nutrition, Phytochemicals, and Clinical Uses.

Maca (Lepidium meyenii, Lepidium peruvianum) is part of the Brassicaceae family and grows at high altitudes in the Peruvian Andes mountain range (3500-5000 m). Historically, it has been used as a nutrient-dense food and for its medicinal properties, primarily in enhancing energy and fertility. Scientific research has validated these traditional uses and other clinical applications by elucidating maca's mechanisms of action, nutrition, and phytochemical content. However, research over the last twenty years has identified up to seventeen different colors (phenotypes) of maca. The color, hypocoty

The role of the nutrition in malnourished cancer patients: Revisiting an old dilemma.

BACKGROUND & AIMS: GLIM definition of malnutrition is recognised all over the world and, when is referring to cancer, it specifies that weight or muscle loss are associated with an inflammatory status. However, the real-world practice shows that GLIM definition cannot encompass all the wide and heterogenous clinical presentations of cancer patients with malnutrition, which involves many other drivers beyond inflammation. Moreover, placing an excessive emphasis on the inflammation can overshadow, in the clinical practice, the role of the nutritional support in malnourished cancer patients. The

Geriatric nutritional risk index and mortality from all-cause, cancer, and non-cancer in US cancer survivors: NHANES 2001-2018.

BACKGROUND: Malnutrition is strongly correlated with worsened treatment outcomes, reduced standard of living, and heightened mortality rates among individuals with cancer. Our research explores how the Geriatric Nutritional Risk Index (GNRI), a measure of nutritional status, relates to all-cause mortality, cancer-specific, and non-cancer mortality among middle-aged and older adult cancer patients. METHODS: We enrolled 3,253 participants aged 40 and above who were diagnosed with cancer. The data was obtained from the National Health and Nutrition Examination Survey (NHANES) dataset covering th

Evaluating Malnutrition Practices and Mother's Education on Children Failure to Thrive Symptoms Using Entropy-Weight and TOPSIS Method.

BACKGROUND/OBJECTIVES: Failure to thrive (FTT) is mostly caused by insufficient consumption of nutrient-rich food, recurrent infections like diarrhea and intestinal worms, substandard caregiving practices, and limited availability of health and other vital services. Furthermore, there was a correlation between the educational level of mothers and the occurrence of FTT in children aged 6-12 months. Thus, the objective of the current research is twofold: (i) to investigate other factors related to FTT and (ii) to evaluate the impact of them on FTT in Sub-Saharan African countries and their urban

Guidelines for Rigorous and Reproducible Research on Other Specified Feeding or Eating Disorder: Commentary on Dang et al. (2024).

Dang et al.'s review concludes that atypical anorexia nervosa (atypical AN), purging disorder (PD), and night eating syndrome (NES) are clinically significant and severe eating disorders (EDs). However, findings are unlikely to alter their status in future editions of the DSM due to limitations in the literature to date. Guidelines are offered to promote rigorous and reproducible research on other specified feeding or eating disorder OSFED. First, published research diagnostic criteria for atypical AN, PD, and NES should be consistently used to ensure findings across studies reflect the same c

An examination of adults with atypical anorexia nervosa at admission to treatment at higher levels of care: An attempt to increase diagnostic clarity.

OBJECTIVE: Anorexia nervosa (AN) is a serious illness with a high mortality rate and multiple physiological complications. The vague definition of atypical AN allows for subjective interpretation. This retrospective study aimed to focus future research on the operational definition of atypical AN by examining four factors associated with atypical AN at admission to higher level of care treatment. METHODS: Adults with atypical AN (n = 69) were examined within sample analyses among four groups: (1) >10% versus ≤10% weight loss; (2) weight loss within the previous 3 months versus >3 months; (3)
