Laboratory-based case finding using phosphate and alkaline phosphatase to identify genetic and nutritional rickets in children.

INTRODUCTION: Hypophosphatemic rickets is a rare disease characterized by renal phosphate loss, leading to chronic hypophosphatemia, elevated alkaline phosphatase, and skeletal symptoms that may remain unnoticed until advanced stages. Early diagnosis is essential because effective treatment is available to reduce complications. OBJECTIVES: To evaluate a laboratory-based case-finding strategy for identifying oligosymptomatic patients with hypophosphatemic rickets using phosphorus and alkaline phosphatase results in a pediatric population. METHODS: A descriptive observational study was conduct

The complexity of malnutrition in people living with excess body weight.

PURPOSE OF REVIEW: Malnutrition in people living with excess body weight is increasingly recognized but remains frequently overlooked because nutritional screening and assessment commonly rely on body mass index (BMI). This review highlights emerging evidence on protein-energy malnutrition, skeletal muscle mass loss, and micronutrient deficiencies in individuals with excess adiposity, and examines the influence of contemporary obesity treatments on these nutritional disturbances. RECENT FINDINGS: Reduced skeletal muscle mass and micronutrient deficiencies are increasingly recognized in indivi

The impacts of energy and protein provision on anthropometric changes in infants with critical congenital heart disease.

OBJECTIVES: Malnutrition in patients with critical congenital heart disease (CCHD) is associated with worse outcomes, however, feeding challenges, and changes in metabolic demands obscure the relationship between nutrient intake and growth in this patient population. We examined the relationship between nutrition provision (caloric and protein intake) and anthropometric indices (weight-for-age z-score [WAZ], length-for-age z-score [LAZ], and occipital-frontal-circumference-for-age [OAZ]). METHODS: A single-center prospective cohort study of infants admitted to cardiac intensive care at a tert

Median Arcuate Ligament Syndrome in an Elderly Patient: A Symptomatic Post-stenotic Aneurysm and Systemic Atherothrombotic Disease.

Median arcuate ligament syndrome (MALS) is atypical in elderly patients, in whom it frequently coexists with severe atherosclerotic disease. We report the case of a 77-year-old male with marked mitral regurgitation who presented with intestinal claudication and malnutrition. CT angiography demonstrated MALS associated with a fusiform aneurysm of the celiac trunk with dissection, along with 60% atherosclerotic stenosis of the superior mesenteric artery. Given the limiting ischemic presentation, the patient underwent laparoscopic release of the ligament. The procedure was complicated by avulsion

Vitamin D: Physiology, Pharmacology and Clinical Evidence for Supplementation-A Narrative Review.

Vitamin D is a nutrient and prohormone obtained from diet and supplements and synthesized in the skin. It is essential for calcium and phosphate homeostasis and skeletal health and influences multiple extra-skeletal processes, yet deficiency remains common worldwide. This narrative review summarizes vitamin D sources, physiology, metabolism, pharmacokinetics, clinical relevance and safety and outlines the distinct pharmacokinetic profiles and clinical roles of vitamin D, calcidiol and calcitriol and its analogues. Evidence consistently supports vitamin D, combined with calcium, for preventing

Vitamin D status and adiposity in children with low-energy fractures: a prospective case-control study.

UNLABELLED: Fractures in children are influenced by both skeletal biology and mechanical factors. While alterations in vitamin D and other metabolic markers have been proposed as contributors to fracture risk, their role in predicting fractures in otherwise healthy pediatric populations remains unclear. This study aimed to determine whether biochemical markers of bone metabolism and body mass are independently associated with fracture occurrence and fracture patterns in children after low-energy trauma and to assess their impact on fracture patterns and anatomical distribution. We conducted a

Optimizing diagnosis and management of patients with ENPP1 deficiency: an expert opinion.

PURPOSE: Ectonucleotide pyrophosphate/phosphodiesterase family member 1 (ENPP1) deficiency is a rare genetic disorder caused by loss-of-function ENPP1 gene mutations. Characterized by abnormally low circulating inorganic pyrophosphate concentrations, bone hypomineralization, soft tissue calcification, and arterial stenosis, ENPP1 deficiency is associated with a phenotypic spectrum that includes generalized arterial calcification of infancy type 1 (GACI1) and autosomal recessive hypophosphatemic rickets type 2 (ARHR2). Despite phenotypic differences, patients with GACI1 and ARHR2 have a marked,

Perioperative nutrition-oriented nursing for older women undergoing cancer surgery: frailty screening, enhanced recovery, and survivorship care.

Older women undergoing cancer surgery commonly enter the perioperative period with overlapping frailty, malnutrition, sarcopenia, obesity, multimorbidity, and treatment-related symptoms. These vulnerabilities reduce physiologic reserve and amplify the catabolic response to surgery, increasing the risk of complications, functional decline, delayed adjuvant treatment, and persistent nutritional impairment. Nutrition-oriented perioperative nursing provides a practical framework for translating frailty assessment and cancer nutrition guidance into coordinated care before surgery, during enhanced r

Development and Testing of a Novel Stabilizer Clamp to Reduce Motion During Instrumented Cervical Fusions: The Caption System.

BACKGROUND: Navigation of spinal instrumentation is heavily dependent on maintaining an accurate registration of the local anatomy to the navigation software. Any motion of the anatomy reduces accuracy. Historically, navigation in the cervical spine has been limited by the mobile nature of the anatomy. Here, we describe the development and testing of a novel stabilizer clamp designed to reduce motion and improve stability for navigated-instrumentation accuracy. METHODS: Testing was completed both on a flat-top table and separately on a Jackson table. On both tables, the cadaveric specimen was

Undernourished and Under the Knife: How Preoperative Albumin Exposes Surgical Risk.

INTRODUCTION: Serum albumin reflects nutritional status and allostatic load, making it useful for surgical risk assessment in undernourished populations. Chronic socioeconomic and environmental stress can cause weathering, an accelerated decline in physiological reserve that may increase surgical vulnerability in disadvantaged patients. We aimed to define a locally optimized preoperative serum albumin cutoff for predicting 30-d postoperative mortality in our population. METHODS: We performed a secondary analysis of ACS-NSQIP data (2019-2023) from a tertiary-care center in Karachi, Pakistan. A

Aperiodic EEG spectral features as consistent indicators of Parkinson's disease: a machine learning approach utilizing fitting oscillations and one-over-F decomposition.

A major challenge in Parkinson's disease (PD) research is the identification of non-invasive neurophysiological markers that can distinguish PD patients from healthy controls while providing interpretable information about cortical dysfunction. This study investigates whether aperiodic EEG spectral parameters extracted using Fitting Oscillations and One-Over-F decomposition can serve as candidate indicators for exploratory PD classification and medication-state analysis. Resting-state EEG data from 15 PD patients and 16 healthy controls were analyzed. For PD participants, both medicated and un

Body roundness Index on the clinical outcome of peritoneal dialysis patients: Analysis of two cohorts.

BackgroundCentral obesity is common among patients undergoing peritoneal dialysis (PD) and may have important metabolic consequences. The body roundness index (BRI) is a convenient anthropometric measurement that represents central adiposity and visceral fat. However, its prognostic value in PD patients has not yet been explored.MethodWe conducted a retrospective study involving two cohorts of PD patients: an exploratory cohort of 249 prevalent PD patients and a validation cohort of 162 incident ones. In addition to BRI, we performed bioimpedance spectroscopy and other routine clinical and bio
