MEDICAL HISTORY: A 93-year-old sarcopenic female patient was admitted to the geriatric ward for further treatment and diagnostic evaluation following a fall and a sacral fracture that had been treated conservatively. Clinically, she primarily presented with loss of appetite accompanied by nausea and recurrent vomiting. A diagnosis of a traction diverticulum had already been made during a previous hospital stay. Subsequently, the patient experienced progressive weight loss with weakness, culminating in a fall and the resulting sacral fracture.

Acute stroke induces a hypermetabolic/hypercatabolic state, causing malnutrition and muscle wasting that hinder recovery. Weight-based predictive equations for nutritional support may fail to meet individual energy needs. This study compared resting energy expenditure (REE)-guided nutrition with weight-based calorie calculation in acute stroke patients. This single-centre, quasi-randomised controlled trial assessed seventy-four acute ischaemic stroke patients requiring enteral nutrition. Six patients were excluded before allocation, and sixty-eight were allocated by odd/even hospital admission

Sarcopenia is a progressive skeletal muscle disorder associated with impaired physical function and adverse outcomes in older adults. Burn injury imposes profound hypermetabolic and catabolic stress that may compromise muscle mass, strength, and physical function, particularly in individuals with reduced baseline muscle reserve. This narrative review integrates current evidence on sarcopenia and muscle dysfunction in older adults with burn injuries, addressing baseline vulnerability, burn-related muscle deterioration, diagnostic challenges, clinical implications, and therapeutic strategies. Children with Down syndrome (DS) may experience feeding difficulties arising from interactions among hypotonia, cardiorespiratory and gastrointestinal conditions, swallowing dysfunction, oral-motor and sensory differences, delayed feeding skill development, and psychosocial or environmental factors. These difficulties may compromise feeding safety and efficiency, restrict dietary diversity, and contribute to inadequate fluid or nutrient intake, faltering growth, micronutrient deficiencies, or excess adiposity. This narrative review synthesized the literature selected for its relevance to feedi

Malnutrition and protein-energy wasting (PEW) are closely related yet distinct clinical entities. While malnutrition broadly refers to nutritional deficits arising primarily from inadequate dietary intake, PEW is a complex syndrome specific to chronic kidney disease characterized by the progressive loss of body protein mass and energy reserves driven by systemic hypercatabolism, uremic toxicity, chronic inflammation that cannot be corrected by dietary intake alone. Nutritional impairment remains one of the most prevalent and clinically significant complications of end-stage kidney disease (ESK

Background: Pediatric autoimmune disorders are increasingly common, yet outcomes remain poor due to delayed diagnosis and lack of appropriate actions. Immune system dysfunction frequently coincides with deficiencies in essential micronutrients and cofactors, including vitamin D. Objectives: This study evaluated the contribution of vitamin D and cofactor (e.g., magnesium) deficiencies to impaired endocrine secretion and autoimmune susceptibility, which may trigger or exacerbate autoimmunity in children. Methods: This systematic review examines the complex interactions between micronutrients-par

Major gynecologic oncology surgery produces an acute inflammatory and neuroendocrine stress response superimposed on cancer-associated inflammation, frailty, malnutrition, anemia, and treatment-related physiologic vulnerability. Although this response is necessary for tissue repair, excessive or poorly resolved activation can contribute to pain, postoperative nausea and vomiting, ileus, insulin resistance, endothelial dysfunction, infection, venous thromboembolism, functional decline, and delayed return to oncologic treatment. Enhanced recovery after surgery (ERAS) pathways reduce avoidable st

PURPOSE OF REVIEW: Malnutrition in people living with excess body weight is increasingly recognized but remains frequently overlooked because nutritional screening and assessment commonly rely on body mass index (BMI). This review highlights emerging evidence on protein-energy malnutrition, skeletal muscle mass loss, and micronutrient deficiencies in individuals with excess adiposity, and examines the influence of contemporary obesity treatments on these nutritional disturbances.
RECENT FINDINGS: Reduced skeletal muscle mass and micronutrient deficiencies are increasingly recognized in indivi

OBJECTIVES: Malnutrition in patients with critical congenital heart disease (CCHD) is associated with worse outcomes, however, feeding challenges, and changes in metabolic demands obscure the relationship between nutrient intake and growth in this patient population. We examined the relationship between nutrition provision (caloric and protein intake) and anthropometric indices (weight-for-age z-score [WAZ], length-for-age z-score [LAZ], and occipital-frontal-circumference-for-age [OAZ]).

Median arcuate ligament syndrome (MALS) is atypical in elderly patients, in whom it frequently coexists with severe atherosclerotic disease. We report the case of a 77-year-old male with marked mitral regurgitation who presented with intestinal claudication and malnutrition. CT angiography demonstrated MALS associated with a fusiform aneurysm of the celiac trunk with dissection, along with 60% atherosclerotic stenosis of the superior mesenteric artery. Given the limiting ischemic presentation, the patient underwent laparoscopic release of the ligament. The procedure was complicated by avulsion

Vitamin D is a nutrient and prohormone obtained from diet and supplements and synthesized in the skin. It is essential for calcium and phosphate homeostasis and skeletal health and influences multiple extra-skeletal processes, yet deficiency remains common worldwide. This narrative review summarizes vitamin D sources, physiology, metabolism, pharmacokinetics, clinical relevance and safety and outlines the distinct pharmacokinetic profiles and clinical roles of vitamin D, calcidiol and calcitriol and its analogues. Evidence consistently supports vitamin D, combined with calcium, for preventing

UNLABELLED: Fractures in children are influenced by both skeletal biology and mechanical factors. While alterations in vitamin D and other metabolic markers have been proposed as contributors to fracture risk, their role in predicting fractures in otherwise healthy pediatric populations remains unclear. This study aimed to determine whether biochemical markers of bone metabolism and body mass are independently associated with fracture occurrence and fracture patterns in children after low-energy trauma and to assess their impact on fracture patterns and anatomical distribution. We conducted a

PURPOSE: Ectonucleotide pyrophosphate/phosphodiesterase family member 1 (ENPP1) deficiency is a rare genetic disorder caused by loss-of-function ENPP1 gene mutations. Characterized by abnormally low circulating inorganic pyrophosphate concentrations, bone hypomineralization, soft tissue calcification, and arterial stenosis, ENPP1 deficiency is associated with a phenotypic spectrum that includes generalized arterial calcification of infancy type 1 (GACI1) and autosomal recessive hypophosphatemic rickets type 2 (ARHR2). Despite phenotypic differences, patients with GACI1 and ARHR2 have a marked,

BACKGROUND: Vitamin B12 deficiency, commonly attributed to malabsorption, presents significant clinical consequences including hematological and neurological manifestations. Helicobacter pylori infection, a prevalent global pathogen, has been implicated in gastric pathology that may compromise vitamin B12 absorption through various mechanisms including gastric atrophy and impaired intrinsic factor production. This study aimed to evaluate the association between H. pylori infection and vitamin B12 deficiency.
METHODS: A retrospective study was conducted at Thumbay Hospital, Ajman, UAE, involvi

Older women undergoing cancer surgery commonly enter the perioperative period with overlapping frailty, malnutrition, sarcopenia, obesity, multimorbidity, and treatment-related symptoms. These vulnerabilities reduce physiologic reserve and amplify the catabolic response to surgery, increasing the risk of complications, functional decline, delayed adjuvant treatment, and persistent nutritional impairment. Nutrition-oriented perioperative nursing provides a practical framework for translating frailty assessment and cancer nutrition guidance into coordinated care before surgery, during enhanced r

Polycystic ovary syndrome (PCOS) is a common endocrine disorder and a leading cause of infertility in reproductive-aged women. Emerging evidence suggests that vitamin D deficiency may exacerbate PCOS-related reproductive and metabolic disturbances. This study aimed to assess the prevalence of vitamin D deficiency among infertile women with PCOS and explore its association with subfertility. This cross-sectional study was conducted at the Department of Infertility, Bangladesh Medical University (BMU), from November 2019 to April 2025. Total 155 infertile women aged 18-40 years; diagnosed with P
