Superior mesenteric artery syndrome in a child with scurvy and long-standing growth failure: a case report.

BACKGROUND: Superior mesenteric artery syndrome is a rare cause of small intestine obstruction, characterized by the compression of the third segment of the duodenum between the aorta and the superior mesenteric artery due to a decreased angle between these vessels, typically attributed to a reduction in mesenteric adipose tissue. CASE PRESENTATION: A 14-year-old Middle Eastern boy presented with scurvy and failure to thrive, accompanied by constipation, intractable vomiting, abdominal pain, and severe bloating. Upper gastrointestinal endoscopy and multi-slice computed tomography confirmed th

Relationship Between Hypovitaminosis C, Obesity, and Features of Metabolic Syndrome-A Narrative Review.

This narrative review aimed to review plausible mechanisms for the role of vitamin C (ascorbic acid [AA]) in the maintenance of healthy weight and energy metabolism; examine the evidence for inadequate vitamin C (plasma AA <50 µmol/L), hypovitaminosis C (≤23 µmol/L), and vitamin C deficiency (≤11.4 µmol/L) in the disrupted homeostasis of obesity and metabolic syndrome; and ascertain whether vitamin C supplementation or dietary intervention could potentially treat obesity and the associated features of metabolic syndrome. Vitamin C hypovitaminosis and deficiency are prevalent in developed count

High-dose vitamin C supplementation in patients undergoing allogeneic hematopoietic stem cell transplantation: A pilot randomized, triple-blind, placebo-controlled trial.

PURPOSE: Allogenic hematopoietic stem cell transplantation (allo-HSCT) is a curative treatment for hematological disorders but often results in micronutrients deficiency and complications. Vitamin C, a potent antioxidant, may improve endothelial function, tissue protection, and immune recovery. This study evaluated the effects of early high-dose vitamin C supplementation on plasma vitamin C levels and post-HSCT complications. METHODS: In this pilot, triple-blind, placebo-controlled trial, 31 adult allo-HSCT patients were randomized to receive intravenous vitamin C (50 mg/kg/day) or placebo fr

Imaging hematomas in unforeseen sites - unveiling 2 cases of pediatric scurvy.

BACKGROUND: Scurvy, a nutritional disorder resulting from chronic deficiency in vitamin C, is now a rare occurrence in contemporary times. Nevertheless, instances can still arise in pediatric patients, typically linked to inadequate nutrition and socioeconomic factors. Diagnosing scurvy proves challenging due to its infrequency and nonspecific symptoms. Clinical and radiographic findings, coupled with low serum vitamin C levels, are crucial for diagnosis. Pediatric scurvy often exhibits prominent musculoskeletal manifestations with varying presentations. CASE PRESENTATION: We present two cases

Red rooibos tea mitigates recognition memory deficits in female rats fed a low vitamin D and calcium diet.

Low vitamin D status is associated with cognitive decline, while polyphenols may support brain health. This study investigated whether 3 months of supplementation with red rooibos (RR) tea, as a source of polyphenols, could mitigate recognition memory deficits in female Sprague-Dawley rats fed a low vitamin D and calcium diet. Results showed that RR tea attenuated memory impairments induced by this diet (p = 0.0034), suggesting its potential as a strategy to support recognition memory under conditions of low vitamin D and calcium status.

Vitamin D deficiency and depression: A Neuroimmune review of complement-associated synaptic remodeling and VDBP-Megalin signaling.

Depression is increasingly understood as a disorder of neuroimmune and synaptic dysregulation. Vitamin D deficiency (VDD), common in patients with depression, has been associated with inflammatory imbalance and poorer clinical outcomes, but the mechanisms linking VDD to circuit-level pathology remain incompletely defined. In this review, we examine evidence that VDD may influence depression-relevant neurobiology through two interacting routes. First, reduced vitamin D signaling may weaken complement restraint, favoring C3-associated microglial synaptic remodeling and pathological pruning. Seco

Interpretable machine learning model for predicting the risk of dysphagia in patients after cardiac surgery.

BACKGROUND: Dysphagia is recognized as one of the most common severe complications following cardiac surgery, with the potential to result in adverse clinical outcomes, including aspiration pneumonia, malnutrition, and prolonged hospitalization. Early identification of patients at high risk for postoperative dysphagia is critical for implementing targeted preventive interventions and improving clinical outcomes. However, traditional risk assessment approaches have inherent limitations in accurately predicting this complication. Accordingly, this study was designed to develop and validate a mac

Pharyngeal electrical stimulation for postextubation dysphagia after stroke: a randomized trial on hospitalization costs from a health insurance perspective.

Dysphagia is a frequent and serious complication after stroke, associated with pneumonia, malnutrition, prolonged intensive care unit stay, and higher healthcare costs. Pharyngeal electrical stimulation (PES) is a novel therapy that may support recovery from postextubation dysphagia. This study assessed the cost-effectiveness of PES in stroke patients from a health insurance perspective. We performed a secondary analysis of a randomized controlled trial including 60 stroke patients with postextubation dysphagia, randomized to PES (n = 30) or sham stimulation (n = 30). Sham treatment involved i

Vitamin D deficiency and supplementation in pregnancy: a clinical trial using an irregular fuzzy cellular automata model.

BACKGROUND: Pregnant women need enough vitamin D for healthy pregnancies, but many women in Iran's Khuzestan province don't get sufficient sunlight to produce vitamin D naturally. In this study, we aimed to determine how many pregnant women have low vitamin D levels and to test different methods of providing vitamin D supplements. We also sought to assess whether our new algorithm model, called Irregular Fuzzy Cellular Automata (IFCA), performs better than older methods for predicting pregnancy complications. METHODS: We examined 2,481 pregnant women who participated in this study in two Iran

Percutaneous endoscopic gastrostomy in atypical parkinsonian syndromes: survival and aspiration outcomes from a retrospective international cohort.

INTRODUCTION: Dysphagia frequently occurs in movement disorders, leading to malnutrition and aspiration. Percutaneous endoscopic gastrostomy (PEG) provides nutrition directly into the stomach, bypassing the dysfunctional swallow. However, PEG insertion is a complex decision, both clinically and ethically. Although PEG outcomes are reported in other neurological disorders, there is limited research in atypical parkinsonian syndromes such as multiple system atrophy (MSA), progressive supranuclear palsy (PSP) and corticobasal degeneration (CBD). Insertion rates remain variable, reflecting a pauci

Perception and reality: assessing the accuracy of self-assessed dysphonia and dysphagia risk in people with cardiovascular disease.

BACKGROUND/OBJECTIVES: Cardiovascular disease (CVD) is a chronic condition influenced by aging and lifestyle, and its effects extend to systems involved in voice and swallowing. Age-related changes in these functions are common, and when worsened by CVD, they can lead to isolation, malnutrition, and complications such as aspiration pneumonia. This study investigated how individuals with CVD perceive their risk of dysphonia and dysphagia compared with clinician-administered screening-based assessments. METHODS: Seventy-three participants (mean age 65.2 years; 86.3% male) completed structured s

Revisional Surgery After Bilio-Pancreatic Diversion: Indications, Outcomes, and Complications in a High-Volume Center.

BACKGROUND: Bilio-pancreatic diversion (BPD), once a prominent bariatric procedure, has seen a marked decline in use due to high rates of nutritional deficiencies and frequent need for surgical revisions. Despite this, BPD remains in practice in select centers, leading to a persistent cohort of patients requiring revisional surgery. METHODS: We conducted a retrospective review at a high-volume bariatric surgery center to identify all patients who underwent revisional surgery following primary BPD between January 2015 and December 2025. Inclusion criteria were age ≥ 18 years, documented primary

Type 5 diabetes mellitus: nutritional-imprinted β-cell insufficiency, diagnostic gaps, and emerging therapeutic strategies.

Historical descriptions of malnutrition-related diabetes mellitus (MRDM) have regained attention in contemporary discourse, with the term type 5 diabetes mellitus (T5DM) increasingly used as a harmonized research construct to describe insulin-deficient diabetes associated with early-life undernutrition. Although the International Classification of Diseases, 11th Revision (ICD-11) includes categories related to malnutrition-associated diabetes, the pathophysiological interpretation, diagnostic boundaries, and therapeutic implications of T5DM remain incompletely defined, resulting in variable cl
