Clinical concerns and considerations for leucovorin use in autism spectrum disorder.

PURPOSE OF REVIEW: To provide pediatric clinicians with an overview of current research on leucovorin use in children with autism spectrum disorder (ASD) and a guide to patient evaluation and treatment. RECENT FINDINGS: An association between cerebral folate deficiency (CFD) and ASD has been suggested in some studies. Autoantibodies that block folate entry into the brain are a cause of CFD and have been detected in 71% of patients with ASD. Leucovorin is a synthetic drug that increases folate concentrations in the brain despite the presence of autoantibodies. Certain studies have indicated re

Vitamin D deficiency selectively exacerbates mitochondrial dysfunction in receptor-rich soleus muscle during disuse-induced atrophy.

This study investigated the role of vitamin D (VD) in disuse skeletal muscle atrophy, with a particular focus on muscle type-specific differences. Male C57BL/6 J mice were fed either a standard or VD-deficient diet and subjected to hindlimb immobilization to induce disuse atrophy. The expression of vitamin D receptor (VDR) was analyzed across several skeletal muscles, and muscle fiber atrophy and mitochondrial function were evaluated. In addition, cultured C2C12 myotubes were used to assess the direct effects of VD on oxidative stress-induced mitochondrial dysfunction. VDR expression was marke

Registered Dietitians' perspectives on nutrition management of persons living with amyotrophic lateral sclerosis.

AIM: Persons with amyotrophic lateral sclerosis (PALS) are at high risk for malnutrition, which is a negative prognostic factor for disease progression and survival. This study explored the perspectives of Registered Dietitians (RDs) providing nutrition care to PALS, focusing on barriers and facilitators to optimal patient care. METHODS: RDs from U.S. multidisciplinary ALS clinics participated in six virtual focus groups. Recordings were transcribed verbatim and independently coded by two researchers for deductive content analysis. RESULTS: RDs highlighted the importance of using clinical ju

Management strategies in older adults with diabetes mellitus and polypharmacy; a narrative review.

Diabetes mellitus (DM) is a growing public health concern that is affecting a significant proportion of older adults aged ≥ 65 years. Diabetes-related complications often lead to other comorbidities necessitating the use of multiple medications. Consequently, polypharmacy, defined as the concurrent use of five or more medications, is highly prevalent among older adults with DM. While sometimes necessary, greater medication counts increase the risk of medication nonadherence, potentially inappropriate medication use, drug-drug interactions, and adverse drug reactions. Thus, polypharmacy is cons

Maternal nutrition practices and its implications for child growth and development in Jimma Town a community based cross sectional study.

Maternal nutrition is crucial for optimal child growth and development, reducing pregnancy complications like low birth weight and premature birth. Adequate nutrition during pregnancy and breastfeeding helps prevent nutrient deficiencies with long-lasting effects. However, poor maternal nutrition can lead to adverse child growth outcomes, such as intrauterine growth restriction and developmental issues. The main objective of this study was to investigate the factors influencing maternal nutrition practices and its implication for child growth and development in the communities of Jimma Town. A

Impact of Severe Vitamin D Deficiency on Antibiotic Use in Hospitalized COPD Patients with Exacerbations: A Retrospective Analysis.

BACKGROUND: Exacerbations of chronic obstructive pulmonary disease (ECOPD) often require antibiotic treatment. Vitamin D deficiency has been implicated in influencing the outcomes of ECOPD; however, its role in antibiotic usage during hospitalization remains unclear. PATIENTS AND METHODS: We conducted a retrospective analysis of 125 hospitalized ECOPD patients, stratified into two groups based on the presence of severe vitamin D deficiency (SVDD, 25-hydroxyvitamin D < 10 ng/mL) and non-severe vitamin D deficiency (NSVDD). Clinical outcomes, including duration of antibiotic treatment, cumulati

Nutritional Management in Patients With Hematological Malignancies: An Underestimated Supportive Care Issue.

Advances in the management of hematological malignancies have significantly improved survival outcomes; however, supportive care remains a critical determinant of treatment tolerance and overall prognosis. Malnutrition is highly prevalent among patients with hematological cancers and is associated with increased treatment-related toxicity, infectious complications, prolonged hospitalization, and reduced quality of life. In Morocco, nutritional management in hematology is frequently under-recognized and inconsistently implemented. Limited availability of specialized clinical nutrition services,

Early-life infectious and nutritional exposures and cardiovascular risk in early adulthood in Uganda: protocol for a new round of data collection in the Entebbe Mother and Baby Study birth cohort at 21 years (EMaBS@21).

INTRODUCTION: Non-communicable diseases, particularly cardiovascular diseases (CVDs), have become major contributors to morbidity and mortality in sub-Saharan Africa (SSA) and are projected to surpass infectious diseases as the leading cause of death among adults by 2030. Although CVDs have traditionally been associated with older age and obesity, adverse cardiovascular phenotypes are increasingly being observed in younger and leaner individuals in SSA. This pattern suggests that pathways to CVD risk in SSA may differ from those described in high-income countries. Early-life infectious exposur

The Geriatric Nutritional Risk Index Is an Independent Predictor of Prosthesis Aseptic Loosening in Patients Undergoing Total Joint Arthroplasty.

BACKGROUND: The geriatric nutritional risk index (GNRI) is a simple index for evaluating malnutrition, but whether it contributes to the development of prosthesis aseptic loosening (PAL) after total joint arthroplasty (TJA) is controversial. The aim of this study was to determine whether the GNRI is independently associated with PAL. METHODS: We retrospectively analyzed the medical data of 13,265 patients who underwent primary TJA at our institution between 2012 and 2023. After matching the patients to one another at a 1:2 ratio on the basis of clinical demographic variables, we compared the

Celiac Disease: A Comprehensive Review of Epidemiology, Pathogenesis, and Therapeutic Strategies.

Celiac disease (CD) is a T-cell-immune mediated enteropathy triggered by gluten ingestion in genetically susceptible individuals. The estimated global prevalence is 1% (range 0.7-1.4%), and the clinical manifestation results from a complex interplay of genetic, immunologic, and environmental factors. Although it occurs predominantly in predisposed individuals with the genetic haplotypes HLA-DQ2, HLA-DQ8, or DQA1*05 haplotypes, several other predisposing genes have been described. Clinical presentations are variable and include both gastrointestinal and extraintestinal manifestations, contribut

Gastrostomy Tube Placement for Enteral Nutrition Among Hospitalized Patients With Cirrhosis Is Associated With Increased Mortality.

OBJECTIVE: Gastrostomy tubes may be associated with increased morbidity and mortality in patients with cirrhosis. However, as liver transplantation becomes more accessible, clarifying the feasibility of using these devices to overcome malnutrition and sarcopenia is increasingly relevant. We aim (1) to show whether gastrostomy tube placement increases mortality in patients with cirrhosis and (2) to examine which additional factors influence the odds for procedure-related complications and death. METHODS: The National Inpatient Sample was queried from 2016 to 2019 for patients with cirrhosis. A

Effect of RYGB Limb Lengths on HbA1c in Patients with Obesity and Type 2 Diabetes.

INTRODUCTION: Roux-en-Y gastric bypass (RYGB) is one of the most commonly performed procedures in metabolic surgery, known for its beneficial effects on type 2 diabetes (T2D). However, the optimal choice of small bowel limb lengths remains a matter of debate. Recent studies suggest that a longer biliopancreatic limb (BPL) could improve T2D more effectively than a longer alimentary limb (AL). The aim of this randomized controlled trial was to compare the effects of a long BPL-RYGB to a long AL-RYGB on T2D in adults with body mass index (BMI) between 27 and 60 kg/m2. METHODS: 131 patients were r

Successful Closure of Multiple Post-bariatric Surgery Fistulas Using Modified Internal and External Endoscopic Vacuum Therapy: A Case Report.

Leaks and fistulas following bariatric surgery are serious complications associated with sepsis, malnutrition, and high morbidity. Endoscopic vacuum therapy (EVT) has emerged as an effective minimally invasive alternative for the management of transmural defects of the upper gastrointestinal tract. Recently, low-cost modifications using conventional hospital supplies have been developed to expand their applicability in resource-limited settings. We present the case of a 46-year-old female patient with a history of multiple bariatric surgeries: sleeve gastrectomy (2017), Roux-en-Y gastric bypas
