Effective Implementation Strategies for Delivering Nutritional Interventions through the Health System to Prevent Malnutrition during Pregnancy: A Systematic Review and Meta-analysis.

Malnutrition during pregnancy is a public health concern. Interventions implemented through the health sector can prevent maternal malnutrition. Our aim was to identify implementation strategies for delivering nutritional interventions through primary health care to prevent malnutrition in all its forms during pregnancy. We followed the Cochrane Handbook for Systematic Reviews and PRISMA guidelines. A search strategy was developed for 5 databases. The information was systematized using the Template for Intervention Description and Replication. Meta-analyses were performed using a random-effect

Machine learning predicts postoperative mortality in a Latin American cardiovascular surgery multicenter cohort: Toward precision risk stratification.

OBJECTIVE: A total of 28% of global cardiac surgeries are performed in Latin America; however, surgeons there are faced with many preventable deaths, partly as the result of the lack of accurate, region-specific risk assessment tools. We developed an explainable machine learning (ML) model tailored to a multicenter Latin American cohort that outperformed European System for Cardiac Operative Risk Evaluation II and Society of Thoracic Surgeons scores in predicting postoperative mortality. METHODS: We analyzed 8680 adult cardiovascular surgery cases (2010-2025) from 5 Latin American countries.

Malnutrition in Enterostomy Infants in the Neonatal Intensive Care Unit: Prevalence, Risk Factors and Impact on Clinical Outcomes.

BACKGROUND: Infants living with an enterostomy were commonly reported as underweight. However, the nutritional status of enterostomy infants after stoma formation has not been given due attention, and there is limited evidence to demonstrate infant growth failure before stoma closure. AIM: To identify the prevalence, risk factors and the effect on clinical outcome of malnutrition in infants undergoing enterostomy in the neonatal intensive care unit (NICU). STUDY DESIGN: The design was a retrospective cohort study. We retrospectively analysed surgical infants with gastrointestinal disease fro

IL-1β neutralization ameliorates cognitive deficits and tau pathology in a mouse model of Alzheimer's disease with hyperhomocysteinemia.

BACKGROUND: Hyperhomocysteinemia (HHcy) is an important risk factor for Alzheimer's disease (AD), but its differential effects on tau pathology and beta-amyloid (Aβ) deposition, as well as the key mediating molecules involved, remain unclear. This study investigates how HHcy influences AD pathology and examines whether interleukin-1β (IL-1β) neutralization can mitigate HHcy-accelerated neurodegeneration. METHODS: Female 12-month-old 3 × Tg-AD mice were supplemented with methionine water for 7 weeks to induce HHcy. Brain tissues were analyzed for Aβ deposition, tau phosphorylation, oligomeriza

CARE study: prospective cohort study on supportive care among paediatric oncology patients in western Kenya-a study protocol.

INTRODUCTION: Childhood cancer presents significant challenges in low- and middle-income countries (LMICs), as survival rates remain substantially low. Supportive care, including nutritional support and infection prevention plus management, is crucial in improving outcomes of childhood cancer patients. To develop evidence-based interventions improving supportive care and survival, insight is needed into local prevalences of malnutrition, colonisation and infections, their association with clinical outcomes and the attitude of parents or legal guardians towards nutritional care and infection pr

[First intestinal transit bipartition at the Mexican Institute for Social Security].

BACKGROUND: Obesity is a highly prevalent disease that significantly impacts quality of life and life expectancy. Over the past decades, bariatric surgery has become the gold standard for treating obesity and its comorbidities. In 2006, Santoro et al. described the technique of intestinal transit bipartition (ITB) to enhance neuroendocrine mechanisms of weight loss while minimizing nutritional deficiencies common in other procedures. The objective was to describe the first case of ITB performed in a high specialty center as a surgical treatment for obesity. CLINICAL CASE: A 44-year-old male p

Covert hepatic encephalopathy as a multi-organ syndrome: the gut-liver-muscle-brain axis, diagnosis, treatment, and multidisciplinary care.

Covert hepatic encephalopathy (CHE) is a highly prevalent complication of liver cirrhosis. Despite the absence of overt symptoms, CHE is strongly associated with impaired quality-of-life, overt hepatic encephalopathy, and mortality. Over the past two decades, evidence regarding the pathophysiology, diagnosis, and treatment of CHE has accumulated considerably, and clinical guidelines recommend screening in patients with cirrhosis. Nevertheless, diagnostic and therapeutic algorithms have not been fully implemented in real-world practice, and many patients remain undiagnosed and untreated. Unders

Statistical and experimental approaches to discriminate between substance-specific reproductive effects and those mediated by maternal undernutrition - A case study with Rosin and derivatives.

It is well-accepted that undernutrition lowers fertility of rodents and retards growth of their offspring. However, accounting for undernutrition's confounding effects in reproductive toxicology studies remains challenging. Rosin (EC 232-475-7), a substance derived from pine trees, and fourteen of its derivatives were tested in OECD test guideline 422 studies. Dietary exposure to several of the substances lowered ovulation rate and retarded offspring growth. However, the substances proved unpalatable and rats consumed lower amounts of diet. The resulting undernutrition was suspected to be the

Prehabilitation in Cardiovascular Surgery: Concepts, Evidence, and Future Direction.

Patients undergoing cardiovascular surgery are increasingly older and multimorbid, with a high prevalence of frailty, sarcopenia, malnutrition, and psychological vulnerability, all of which are strongly associated with adverse postoperative outcomes yet remain incompletely captured by conventional risk models. Prehabilitation has emerged as a proactive, multimodal strategy to address these modifiable vulnerabilities in the preoperative period. This narrative review synthesizes contemporary evidence on prehabilitation in adult cardiovascular surgery, focusing on its core components, reported cl

Controlling Nutritional Status Scores Predict Postoperative Acute Kidney Injury in Living Donor Liver Transplantation.

BACKGROUND: Acute kidney injury (AKI) is a frequent complication after liver transplantation (LT), and malnutrition is a known risk factor. The Controlling Nutritional Status (CONUT) score, based on serum albumin, lymphocyte count, and cholesterol, has been associated with postoperative AKI in other surgeries, but its relevance to LT remains unclear. This study aimed to evaluate the utility of the CONUT score for predicting AKI in living donor LT (LDLT) recipients. METHODS: We retrospectively analyzed 480 patients who underwent LDLT from January 2012 to June 2023. The CONUT score was calculat

Pediatric Health at the Crossroads of Climate Change, Food Insecurity, and Malnutrition.

Climate change (CC) poses a major global threat to the health of current and future generations, disproportionately affecting pediatric populations. Investigating the links between CC and pediatric diseases is crucial to inform research and prevention strategies aimed at breaking the transgenerational cycle of social inequalities. This narrative review explores the complex interactions between early-life exposures to CC, food insecurity, and malnutrition, and their impact on infectious and noncommunicable diseases (NCDs) in pediatric populations. Data reveal a concerning global scenario: half

Factors associated with depression among people with cancer: Systematic umbrella review.

INTRODUCTION: Depression in cancer patients is a common condition that poses significant challenges for prognosis, treatment adherence, and quality of life. Its onset reflects the interplay of diverse biological, psychological, and social factors, which has been the focus of numerous studies. METHODS: We identified both systematic and non-systematic reviews examining cross-sectional or prospective studies reporting associations between DAFs and depression. We extracted data relative to DAFs, as well as the direction and statistical significance of the reported association. Consistency of find

Psychometric Validation of the Bedford Alzheimer Nursing-Severity Scale in Community-Dwelling Persons With Severe Dementia.

OBJECTIVES: Measuring disease severity in persons with severe dementia is essential for clinical care and research. Most instruments encounter issues, for example floor effects in characterising persons with severe dementia. We aimed to evaluate the psychometric properties of the Bedford Alzheimer Nursing-Severity Scale (BANS) and its short version, BANS-6, in community-dwelling persons with severe dementia. METHODS: We used baseline data from a multi-centre prospective longitudinal study. 215 caregivers of community-dwelling persons with severe dementia (≥ FAST stage 6c) were recruited (mean

Tumor-Induced Osteomalacia Localized to an Abdominal Wall Phosphaturic Mesenchymal Tumor by 68Ga-DOTATATE PET/CT.

A ⁶⁸Ga-DOTATATE PET/CT scan was performed on a 40-year-old woman with suspected tumor-induced osteomalacia (TIO), who presented with progressive bone pain, multiple fractures, and hypophosphatemia. The scan revealed a solitary focus of intense somatostatin receptor avidity within the right lower abdominal wall musculature. No other suspicious lesions were identified. On the basis of this finding, the tumor was surgically resected, and histopathology confirmed a phosphaturic mesenchymal tumor (PMT). Postoperatively, the patient's serum phosphate levels normalized. This case underscores the para

Perspectives on the Nutritional Paradox in Uremia: A "Sensitize-Then-Feed" Framework for Protein-Energy Wasting.

Protein-energy wasting (PEW) drives significant morbidity and mortality in advanced chronic kidney disease (CKD). Despite the implementation of comprehensive clinical guidelines and medical nutrition therapy, a subset of uremic patients may exhibit a defining "nutritional paradox." Unlike simple starvation, which responds to caloric and protein supplementation, CKD-associated cachexia is characterized by a potential systemic anabolic resistance. This renders standard nutritional interventions-including oral supplements and intradialytic parenteral nutrition-often insufficient. To elucidate the
