Implementing Continuous Nursing Care After Kasai Procedure in Infants with Biliary Atresia.

Biliary atresia (BA) is a severe cholangiopathy of infancy characterized by progressive obstruction of the extrahepatic bile ducts. Kasai portoenterostomy remains the standard initial treatment, but postoperative complications such as cholangitis, malnutrition, and progressive jaundice remain common and can compromise long-term outcomes. Conventional discharge guidance often lacks continuity, leading to poor family adherence and delayed recognition of complications once the infant returns home. To address this gap, we developed and implemented a standardized protocol for continuity of nursing

The effect of childhood body weight status on cardiovascular disease risk in adulthood: A narrative review.

The prevalence of obesity among children and adolescents is rising alarmingly worldwide; however, its long-term effect on adult cardiovascular health is still underestimated. A considerable body of evidence suggests a detrimental effect of childhood obesity on the risk of cardiovascular disease (CVD) in adulthood. This is mostly indicated by an increased prevalence of arterial hypertension and type 2 diabetes mellitus. Longitudinal cohort studies have shown that persistent obesity from childhood to adulthood markedly amplifies these risks, whereas normalization of body mass index (BMI) during

Type 5 diabetes: A disorder associated with undernutrition: The epidemiology, pathogenesis, clinical features and differential diagnosis.

In this review, we discuss the concept of Malnutrition Modulated Diabetes Mellitus, currently labelled as Type 5 diabetes mellitus, a distinct form of diabetes seen in lean, undernourished individuals, particularly in low- and lower middle-income countries. This review highlights the role of early-life and chronic undernutrition in impairing pancreatic β-cell development and diminished insulin secretion with preserved insulin sensitivity. Furthermore, we present epidemiological, experimental, and clinical evidence of undernutrition and its effects on glucose tolerance and discuss the different

Taiwanese consensus statement on the use of diabetes-specific formula: Scientific evidence.

INTRODUCTION: Medical Nutrition Therapy remains the cornerstone of diabetes management. Clinical evidence shows that early intervention with medical nutrition therapy can significantly improve cardiometabolic markers, thereby delaying or preventing diabetes-related complications. As part of medical nutrition therapy, diabetes-specific formula has proven to be an effective tool in clinical practice and a viable support alternative in special circumstances where consuming whole foods is challenging. However, Taiwan currently lacks consensus on diabetes-specific formula products, and many patient

In Utero Protein Deficiency Leads to Impaired Hepatic Lipid Metabolism in Adult Male Rats.

BACKGROUND: In utero exposure to a low-protein (LP) diet is a well-established model of developmental programming linked to increased risk of chronic metabolic diseases, including lean type 2 diabetes (T2D). OBJECTIVES: In this study, we investigated the long-term effects of maternal LP diet on hepatic lipid metabolism and mitochondrial dynamics in adult lean T2D male rat offspring. METHODS: Pregnant Wistar rats were fed either a control (20% protein) or isocaloric LP (6% protein) diet during gestation, and male offspring were examined in adulthood. Hepatic lipid accumulation, mitochondrial

Plant-based low-protein diet for preventing malnutrition‒inflammation complex syndrome in adults with CKD: a single-centre preliminary experience.

BACKGROUND: Chronic kidney disease (CKD) is a rising public health concern. Low protein intake reduces uremic toxin generation and improves the hemodynamic profile of the kidneys, postponing the need for eventual dialysis. Vegetarian diets have been shown to positively influence cardiovascular health and reduce uremic toxin levels and inflammation in CKD patients. The so-called “Malnutrition-Inflammation Complex Syndrome” (MICS) is a nontraditional cardiovascular risk factor that is associated with poor overall outcomes in CKD patients. The type of protein, not only the amount, may be relevant

Vitamin D Adequacy Conditions the Prolactin-Suppressive Effect of Metformin in Men Receiving Prolactin-Elevating Medications.

Background/Objectives: Metformin has been proposed as a potential treatment for hyperprolactinemia irrespective of etiology. Previous studies suggest that vitamin D deficiency attenuates the prolactin-lowering effect of metformin in women. This study examined whether vitamin D status modifies the effects of this agent on prolactin and other anterior pituitary hormones in men with iatrogenic hyperprolactinemia. Methods: Seventy-five adult men with antipsychotic-induced hyperprolactinemia and type 2 diabetes or prediabetes were enrolled. Participants were assigned to three equal groups based on

Vitamin B6 predicts poor outcomes in geographically distinct populations with primary sclerosing cholangitis.

BACKGROUND & AIMS: Primary sclerosing cholangitis (PSC) has a variable disease course, complicating patient counseling and the timing of liver transplantation. Vitamin B6 deficiency predicts reduced liver transplantation-free survival in Scandinavian PSC cohorts. Here, we aimed to validate this observation in US and German PSC cohorts and to expand our analyses to include hepatic decompensation as a clinical outcome. METHODS: Serum active vitamin B6 (pyridoxal 5'-phosphate [PLP]) was analyzed using liquid chromatography-tandem mass spectrometry in retrospective cohorts of people with PSC from

Vitamin K Biochemistry and Pharmacokinetics: The Basis of Late Vitamin K Deficiency Intracranial Bleeding in Early Infancy.

Vitamin K is a fat-soluble vitamin essential for the activation of vitamin K-dependent proteins involved in coagulation and other physiological processes. Neonates are particularly vulnerable to vitamin K deficiency due to limited placental transfer, low hepatic stores, immature liver function, and insufficient dietary intake, especially in exclusively breastfed infants. This review summarizes the biochemistry and pharmacokinetics of vitamin K, focusing on their role in the pathogenesis of late vitamin K deficiency bleeding (VKDB), including intracranial hemorrhage in early infancy. The limita

An Audit of the Use of Total Parenteral Nutrition: Are We Using It Correctly?

Background Total parenteral nutrition (TPN) provides essential nutritional support when the gastrointestinal (GI) tract is non-functional or inaccessible. Its use should be limited to situations where enteral feeding is not possible or contraindicated. Enteral nutrition remains the preferred option whenever feasible. Clinicians should ensure an informed discussion regarding the relative risks and benefits of parenteral versus enteral nutrition. Objectives To evaluate adherence to local and national guidelines in the use of TPN, including frequency of emergency TPN prescriptions, duration of th

Refractory Dysphagia From Stent-Induced Esophageal Ring Formation After Sequential Bariatric Surgery: A Case Report.

A 55-year-old female patient with prior sleeve gastrectomy and subsequent Roux-en-Y gastric bypass conversion presented with progressive dysphagia. Several years prior, a self-expanding metal stent had been placed to manage postbariatric dysphagia. The patient subsequently developed a proximal esophageal B-ring with critical luminal narrowing at the superior margin of the stent. This complication is described in the literature but may be underrecognized in postbariatric patients. Two esophageal balloon dilations provided only temporary relief before stricture recurrence. The clinical course wa

Effects of Combined Lipid-Based Nutrient Supplements and Malaria Preventive Treatment during Pregnancy on Malaria, Maternal Nutrition, and Birth Outcomes: A Scoping Review.

Malaria and malnutrition adversely affect maternal and neonatal health in Sub-Saharan Africa. While intermittent preventive treatment of malaria in pregnancy (IPTp) is widely used to reduce malaria incidence, the potential additional benefits of combining it with lipid-based nutrient supplements (LNS) remain unclear. The objective of this review was to systematically summarize the evidence on the effects of LNS in combination with IPTp on malaria incidence, maternal nutritional status, and birth outcomes. A comprehensive search of 4 databases-MEDLINE, EMBASE, Scopus, and CENTRAL-and the gray l
