Resting energy expenditure-guided nutritional support in acute ischaemic stroke patients: a single-centre quasi-randomised controlled trial.

Acute stroke induces a hypermetabolic/hypercatabolic state, causing malnutrition and muscle wasting that hinder recovery. Weight-based predictive equations for nutritional support may fail to meet individual energy needs. This study compared resting energy expenditure (REE)-guided nutrition with weight-based calorie calculation in acute stroke patients. This single-centre, quasi-randomised controlled trial assessed seventy-four acute ischaemic stroke patients requiring enteral nutrition. Six patients were excluded before allocation, and sixty-eight were allocated by odd/even hospital admission

Malnutrition in Maintenance Dialysis: From Metabolic Disorder to Multisystem Complications.

Malnutrition and protein-energy wasting (PEW) are closely related yet distinct clinical entities. While malnutrition broadly refers to nutritional deficits arising primarily from inadequate dietary intake, PEW is a complex syndrome specific to chronic kidney disease characterized by the progressive loss of body protein mass and energy reserves driven by systemic hypercatabolism, uremic toxicity, chronic inflammation that cannot be corrected by dietary intake alone. Nutritional impairment remains one of the most prevalent and clinically significant complications of end-stage kidney disease (ESK

The complexity of malnutrition in people living with excess body weight.

PURPOSE OF REVIEW: Malnutrition in people living with excess body weight is increasingly recognized but remains frequently overlooked because nutritional screening and assessment commonly rely on body mass index (BMI). This review highlights emerging evidence on protein-energy malnutrition, skeletal muscle mass loss, and micronutrient deficiencies in individuals with excess adiposity, and examines the influence of contemporary obesity treatments on these nutritional disturbances. RECENT FINDINGS: Reduced skeletal muscle mass and micronutrient deficiencies are increasingly recognized in indivi

The impacts of energy and protein provision on anthropometric changes in infants with critical congenital heart disease.

OBJECTIVES: Malnutrition in patients with critical congenital heart disease (CCHD) is associated with worse outcomes, however, feeding challenges, and changes in metabolic demands obscure the relationship between nutrient intake and growth in this patient population. We examined the relationship between nutrition provision (caloric and protein intake) and anthropometric indices (weight-for-age z-score [WAZ], length-for-age z-score [LAZ], and occipital-frontal-circumference-for-age [OAZ]). METHODS: A single-center prospective cohort study of infants admitted to cardiac intensive care at a tert

Perioperative nutrition-oriented nursing for older women undergoing cancer surgery: frailty screening, enhanced recovery, and survivorship care.

Older women undergoing cancer surgery commonly enter the perioperative period with overlapping frailty, malnutrition, sarcopenia, obesity, multimorbidity, and treatment-related symptoms. These vulnerabilities reduce physiologic reserve and amplify the catabolic response to surgery, increasing the risk of complications, functional decline, delayed adjuvant treatment, and persistent nutritional impairment. Nutrition-oriented perioperative nursing provides a practical framework for translating frailty assessment and cancer nutrition guidance into coordinated care before surgery, during enhanced r

Machine Learning-based Prediction of Mortality Among Malnourished Patients Hospitalized With Inflammatory Bowel Disease.

BACKGROUND: Malnourished patients hospitalized with inflammatory bowel disease (IBD) have a high risk of morbidity and mortality. Risk stratification can help identify patients who are most in need of medical and nutritional intervention. GOAL: This study aimed to develop a machine-learning model that accurately predicts mortality in hospitalized IBD patients with protein-calorie malnutrition (PCM). STUDY: Hospitalized adults with IBD and PCM were identified in the 2016 to 2019 National Inpatient Sample (NIS). Random Forest Classifier (RFC) and Extreme Gradient Boosting (XGB) models were con

Perspectives on the Nutritional Paradox in Uremia: A "Sensitize-Then-Feed" Framework for Protein-Energy Wasting.

Protein-energy wasting (PEW) drives significant morbidity and mortality in advanced chronic kidney disease (CKD). Despite the implementation of comprehensive clinical guidelines and medical nutrition therapy, a subset of uremic patients may exhibit a defining "nutritional paradox." Unlike simple starvation, which responds to caloric and protein supplementation, CKD-associated cachexia is characterized by a potential systemic anabolic resistance. This renders standard nutritional interventions-including oral supplements and intradialytic parenteral nutrition-often insufficient. To elucidate the

Effects of Combined Lipid-Based Nutrient Supplements and Malaria Preventive Treatment during Pregnancy on Malaria, Maternal Nutrition, and Birth Outcomes: A Scoping Review.

Malaria and malnutrition adversely affect maternal and neonatal health in Sub-Saharan Africa. While intermittent preventive treatment of malaria in pregnancy (IPTp) is widely used to reduce malaria incidence, the potential additional benefits of combining it with lipid-based nutrient supplements (LNS) remain unclear. The objective of this review was to systematically summarize the evidence on the effects of LNS in combination with IPTp on malaria incidence, maternal nutritional status, and birth outcomes. A comprehensive search of 4 databases-MEDLINE, EMBASE, Scopus, and CENTRAL-and the gray l

Effectiveness of nutritional support team interventions in improving the nutritional status of in-hospital dental patients with decreased renal function.

Introduction: chronic kidney disease (CKD) is a major risk factor for end-stage renal disease, and owing to its increasing global prevalence, poses a serious public health challenge. Patients with CKD frequently experience protein-energy wasting, frailty, sarcopenia, and cachexia, necessitating complex dietary restrictions, including protein, salt, and potassium limitation. Although a nutrition support team (NST) can improve nutritional status in hospitalised patients, additional interventions such as swallowing evaluation, oral care, and medication adjustments are crucial for effective outcom

The potential of prehabilitation to enhance recovery in sarcopenic and frail older kidney transplant candidates: A narrative review.

BACKGROUND: Older adults represent the fastest-growing segment on kidney transplant (KT) waiting lists. In this population, the high prevalence of sarcopenia and frailty impairs the ability to adapt and respond to metabolic stress, contributing to poorer postoperative outcomes. Given that many adverse events occur in the early postoperative period, the optimal window for intervention is prior to surgery. Prehabilitation, defined as a structured intervention delivered before surgery, aims to enhance postoperative recovery. METHODS: In the context of older adults candidates to KT, we constructe

Nutritional status as a risk factor for adverse events associated with L-Asparaginase in children with acute lymphoblastic leukemia.

BACKGROUND: Acute lymphoblastic leukemia (ALL) is the most common pediatric cancer, and asparaginase (ASNase) is a key component of its treatment. However, ASNase is frequently associated with adverse events (AEs), such as pancreatitis and hypersensitivity reactions particularly with the Escherichia coli derived formulation (L-Asp) which may be influenced by nutritional status. OBJECTIVE: To evaluate the association between protein-energy malnutrition (PEM) and the incidence of L-Asp induced AEs in children with ALL. METHODS: A prospective observational cohort with a nested case-control comp
