Microbiological patterns of bacterial bloodstream infections and predictors of kidney dysfunction at a tertiary hospital in Zambia.

INTRODUCTION: bloodstream infections (BSIs) are a significant cause of morbidity and mortality and are frequently complicated by kidney dysfunction. Evidence on the prevalence, predictors, and associated microbiological patterns in Zambia remains scarce. This study evaluated the burden of kidney dysfunction, its laboratory predictors, and blood culture isolate patterns at Ndola Teaching Hospital (NTH). METHODS: a retrospective cohort study was conducted at NTH from January to December 2025. Data were extracted from the DISA laboratory database and patient registry records. Kidney dysfunction

[A delayed threat: S. aureus sepsis 4 years post-IUD insertion in a healthy young woman].

BACKGROUND: Intrauterine devices (IUDs) are highly effective long-acting reversible contraceptives with an established safety profile. Infectious complications are rare and usually occur shortly after insertion, typically involving polymicrobial pathogens. Staphylococcus aureus is an uncommon pathogen in this context, but its biofilm-forming ability may contribute to delayed and severe presentations. OBJECTIVES: A healthy 25-year-old primiparous woman developed septic shock four years after the insertion of a levonorgestrel-releasing intrauterine system (Mirena®). She was admitted to the inte

Spontaneous Renal Hemorrhage Secondary to MRSA Renal Abscess Requiring Nephrectomy After Failed Percutaneous Drainage: A Case Report.

INTRODUCTION: Spontaneous renal hemorrhage (SRH) caused by renal infection is extremely rare. Furthermore, sepsis caused by methicillin-resistant Staphylococcus aureus (MRSA) has a high mortality rate and requires prompt source control. CASE PRESENTATION: A 63-year-old man with severe diabetes presented to the emergency department in shock. Computed tomography revealed a left renal subcapsular hematoma. Based on his clinical course and history, infection-induced SRH was suspected. Blood and urine cultures yielded MRSA. Although percutaneous drainage was performed after the initial bleeding st

Case report: Septic shock associated with Granulicatella adiacens pneumonia in a polytrauma patient following road traffic accident: rare pathogen in critical care.

BACKGROUND: Ventilator-associated pneumonia (VAP) is a common and serious consequence of prolonged mechanical ventilation in critically ill polytrauma patients, especially those with underlying comorbidities such as diabetes mellitus. Although VAP is often caused by gram-negative bacilli and Staphylococcus aureus, infection due to fastidious organisms is uncommon and may delay microbiological diagnosis. Granulicatella adiacens, formerly categorized among the nutritionally variant streptococci (NVS), is a fastidious commensal of the oral and genitourinary tract that is most commonly associated

Torque Teno Virus Viremia in Complicated Staphylococcus aureus Infection: Prevalence, Viral Load, and Associations With Inflammatory Markers and Clinical Outcomes.

Torque teno virus (TTV) is a recognized marker of immune competence, but its relevance in severe bacterial infections is unclear. We evaluated prevalence, viral load, and clinical relevance of TTV viremia in complicated Staphylococcus aureus infection (CSAI). In this ancillary analysis of two prospective pharmacokinetic cohort studies at a Swiss university hospital, stored plasma samples were tested for TTV DNA using TTV R-GENE PCR, and cytokines were measured by multiplex immunoassay. Associations between TTV load, inflammatory biomarkers, and clinical outcomes (including 30-day mortality) we

Septic arthritis attributed to a spider bite - a case report and review of the literature.

Individuals who travel to tropical or subtropical areas are exposed to various infectious, environ-mental, and climatic risk factors, as well as traffic accidents, security problems, natural disasters, and direct contact with potentially dangerous plants or animals. Accidental bites by venomous spiders during long journeys are always dangerous to health and life, but it is not usually recognized that nonvenomous spiders also pose such a substantial threat to the lives of local inhabitants and travelers. Here, we present an unusual case of a 41-year-old male patient who was admitted to the Univ

Targeting the capsule of Klebsiella pneumoniae with a cationic CR3-binding protein enhances phagocytosis and promotes bacterial clearance and survival in a mouse sepsis model.

Platelet Factor 4 (PF4), a cationic antimicrobial peptide, serves as a ligand for the myeloid-specific phagocytic receptor CR3 (Mac-1, CD11b/CD18). We previously demonstrated that recombinant dimeric PF4 (rPF4) functions as a bacterial opsonin, enhancing phagocytosis of Gram-positive Staphylococcus aureus and facilitating clearance of both antibiotic-susceptible and methicillin-resistant S. aureus in a mouse model of infectious peritonitis. In this study, we examined whether rPF4 is pathogen-agnostic by assessing its effect on phagocytosis of Gram-negative encapsulated Klebsiella pneumoniae, a

Infection or contamination - Clinical attribution of blood culture-positive episodes in neonatal intensive care: an 11-year retrospective single-center study.

BACKGROUND: Positive blood cultures in neonatal intensive care do not necessarily indicate clinically relevant bloodstream infection. Interpretation is particularly challenging for coagulase-negative staphylococci (CoNS), which may represent either contamination or clinically relevant infection depending on the clinical context. We aimed to describe how treating teams attributed culture-positive episodes as infection or contamination in routine neonatal care and to examine which clinical, laboratory, and microbiological features were associated with this original clinical attribution. As a sec

Formulation, physicochemical characterization, and antimicrobial evaluation of drug-loaded essential oil microemulsions against Methicillin-resistant Staphylococcus aureus -associated diabetic foot ulcer infections.

Diabetes Mellitus, a leading endocrine disorder characterized by persistent hyperglycemia, ischemia, and neuropathy, causes ulcerative wounds that are often colonized by multidrug-resistant pathogens, such as Methicillin-resistant Staphylococcus aureus (MRSA), which exacerbate wound infections and can lead to complications such as sepsis and necrosis in Diabetic Foot Ulcers (DFU). This study aims to formulate phytochemical-based nanovesicle carriers for potential wound-healing applications to retain the therapeutic effects of antimicrobial compounds with their localized delivery. This investig

Chlorhexidine Versus Iodine-Based Skin Preparation Was Not Associated with a Significant Difference in 90-Day Infection Risk After Instrumented Degenerative Cervical or Lumbar Spine Surgery: A Propensity Score-Matched Analysis.

Background/Objectives: Chlorhexidine and iodine-based preparations are both widely used, and the existing literature suggests a possible advantage of chlorhexidine in clean elective surgery. Whether this extends to instrumented degenerative spine surgery is unknown. We asked whether chlorhexidine-based skin preparation reduces 90-day postoperative infection compared with iodine-based preparation after instrumented degenerative cervical or lumbar spine surgery. Methods: A retrospective TriNetX cohort study was performed among adults undergoing instrumented degenerative cervical or lumbar spine

AI-based clinical decision support systems for antibiotic therapy in common adult infections: a systematic review from development to clinical implementation.

BACKGROUND: Antimicrobial resistance is among the leading causes of global mortality, and artificial intelligence (AI)-based clinical decision support systems (CDSS) have been proposed to guide antibiotic prescribing. Several AI models for antibiotic decision-making have been developed and validated, but their actual deployment in clinical practice and effect on patient outcomes have not been systematically characterized. OBJECTIVE: To identify, classify, and appraise studies evaluating AI-based CDSS for antibiotic therapy in hospitalized adults with common infections, stratified by level of

Tricuspid Valve Infective Endocarditis Seen on Point-of-Care Ultrasound: A Case Report.

Intravenous drug use is a major risk factor for infective endocarditis (IE), particularly right-sided disease involving the tricuspid valve. Early diagnosis remains challenging as patients often present with nonspecific symptoms, while delays in recognition can result in severe complications including septic emboli, valvular dysfunction, and sepsis. Point-of-care ultrasound (POCUS) has emerged as a valuable bedside modality capable of rapidly identifying valvular abnormalities and expediting the diagnosis of IE in the emergency department. We present a case of tricuspid valve infective endocar

Impact of ethyl alcohol nasal treatment on hospital-onset Staphylococcus aureus bacteremia at a tertiary health system.

INTRODUCTION: Staphylococcus aureus is a common pathogen that can cause nosocomial infection. Ethyl alcohol has emerged as a potential alternative to mupirocin and povidone-iodine for nasal decolonization; however, data on real-world application are limited. METHODS: A quasi-experiment study was performed. Twice daily 62% ethyl alcohol nasal swabs were offered to inpatients at risk for MRSA acquisition. Compliance was monitored and reasons for non-adherence were evaluated. Rates of hospital-onset S. aureus bacteremia were compared pre and postintervention. RESULTS: A total of 274,999 doses o

MRSA nasal PCR as a prevalence-dependent stewardship tool in hospitalized children: a structured review.

INTRODUCTION: Empiric anti-methicillin-resistant Staphylococcus aureus (MRSA) therapy is common in hospitalized children with severe pneumonia or suspected sepsis. MRSA nasal polymerase chain reaction (PCR) is increasingly used to support de-escalation, but its central metric, the negative predictive value (NPV), is often quoted without the prevalence on which it depends. AREAS COVERED: PubMed, EMBASE, and the Cochrane Library were searched from January 2018 to April 2026. Twenty-seven sources were synthesized; the pediatric diagnostic-accuracy and stewardship studies and the in-window meta-a

Severe infections as collateral damage of decorative tattoos - A literature survey.

Tattooing is an ancient decorative cosmetic procedure with increasing popularity, but regulations concerning professional including hygienic practice are limited. Although mild inflammatory responses due to infections, for instance, are known complications, severe sequelae are beyond consideration. Here we summarize 40 cases of invasive and systemic infections as collateral damages of decorative tattooing. Reports from 14 countries revealed that most patients were male, with a median age of 28.5 years including a 6-week-old infant. Cases were categorized into deep abscesses and cellulitis, inf
