INTRODUCTION: Bloodstream infections (BSIs) are a leading cause of healthcare-associated morbidity and mortality, with a significant portion being preventable. Despite this, BSIs remain common, particularly in Australian hospitals. Princess Alexandra Hospital (PAH) in Queensland has implemented a surveillance program that focuses on the preventability of BSIs, with continuous loop, real-time audit-feedback to teach clinicians about preventable factors and possible actions. This study evaluated the program's implementation and impact on reducing infections.
METHODS: A Type I hybrid implementat

Staphylococcus aureus is a leading cause of nosocomial infections, including sepsis, bacteraemia, pneumonia, and endocarditis, and continues to pose a major public health challenge due to escalating multidrug resistance and the absence of an effective vaccine. To address this unmet clinical need, we employed a machine learning-guided reverse vaccinology approach to systematically interrogate the S. aureus proteome for novel vaccine antigens. This strategy led to the identification of eight previously uncharacterized hypothetical proteins with predicted immunogenic properties. Among these, SAUS

MRSA infections give rise to chronic cutaneous and advance into profound tissue invasions, including osteomyelitis and sepsis-conditions for which existing clinical interventions offer only limited efficacy. The utility of broad-spectrum antibiotics has become increasingly compromised by escalating drug resistance, inadequate tissue targeting, and deleterious side effects. Neutrophil extracellular traps (NETs) have emerged as a compelling immunotherapeutic avenue for eradicating multidrug-resistant pathogens. Inspired by the bactericidal and immunoregulatory functions of NETs, we developed a p

Staphylococcus aureus is a pathobiont whose primary human reservoirs are nares, pharynx, intestines, and skin. When specific conditions in the host are altered, it can cause a wide variety of human diseases, including bacteremia and sepsis. Preclinical in vivo models mimicking the most severe S. aureus infections in humans have been used to develop treatments against this pathogen. This study aims to better characterize a murine model of S. aureus bacteremia and sepsis, offering a new and more comprehensive view of the complex interactions between S. aureus and the host while better reflecting

BACKGROUND: Immune checkpoint inhibitors (ICIs) modulate host immunity and may increase infection risk. The pathogen profile, outcomes, and predictors of bloodstream infections (BSIs) in ICI recipients are not well defined; we aimed to characterise these features and identify independent risk factors.
METHODS: We performed a retrospective nested case-control study within a cohort of 2,584 adult cancer patients who received ICIs at a tertiary centre from January 2018 to December 2023. We identified 185 BSI cases occurring during or within 90 days after the last ICI dose and matched each case 1

Blood culture contamination is common, causing diagnostic uncertainty and unnecessary antibiotic use. Analyzing growth patterns within culture sets might offer diagnostic value. We retrospectively analyzed peripheral blood culture sets from 2019 and 2024. Growth pattern (one bottle [discordant] vs both bottles [concordant]) was analyzed according to clinical significance (contaminant vs true pathogen). Overall, 38,216 blood culture sets were analyzed, including 1,491 (3.9%) discordant and 1,938 (5.1%) concordant sets (remaining 34,787 [91.0%] sets were sterile). Discordant sets grew 1,060/1,49

BACKGROUND: Myiasis is caused by the infestation of fly larvae and occurs more frequently in tropical, rural and low socioeconomic areas. We report a rare case of umbilical myiasis in a neonate, complicated by sepsis caused by Staphylococcus aureus .
CASE REPORT: The patient, born in a vulnerable family, presented on the fourth day of life with erythema in the umbilical region, accompanied by serous drainage and visible larvae. Immobilization and manual removal of the larvae were attempted 3 times but proved ineffective. On the 10th day of hospitalization, oral ivermectin (200 μg/kg) was admi

INTRODUCTION: Neonatal suppurative sialadenitis (NSS) is an uncommon but serious complication of salivary gland infection, with few cases reported in literature, most often affecting preterm infants. Typical cases involve the parotid gland and are caused by Staphylococcus aureus infection. Contributing factors include prematurity, dehydration, prolonged hospitalisation, and immature immunity. Sequential bilateral gland involvement through haematogenous spread is rare, and presents significant management challenges in prematurity.
PRESENTATION: We report the case of a preterm infant, born at 3

BACKGROUND: Our hospital observed a significant increase in nosocomial methicillin-resistant Staphylococcus aureus bacteremia with half of these cases occurring in the adult intensive care units (ICUs). We tested a set of implementation strategies to improve the mupirocin ordering rate for universal decolonization within the adult ICUs.
METHODS: There were 3 implementation phases followed by a washout phase and a final fourth phase. The desired goal was ≥ 85% of eligible patients ordered for mupirocin within 48 hours of admission; 2 consecutive weeks below this goal prompted movement to the n

BACKGROUND AND OBJECTIVE: Tunneled catheters (TC) have become an essential vascular access for hemodialysis (HD), despite their association with increased morbidity and mortality, particularly due to infections. Existing studies assessing the optimal combination of prophylactic measures to prevent TC-related infections are limited by small sample sizes and short follow-up periods. The objectives of this study were to describe the clinical and demographic characteristics of patients with TC in our healthcare area, determine the incidence and etiology of TC-related bloodstream infections (TC-BSI

Staphylococcus aureus is a major human pathogen responsible for severe infections, including infective endocarditis. While host factors influencing disease outcome are well characterized, lineage-specific virulence mechanisms remain poorly defined. Herein, we investigated eighty-nine bacteremic S. aureus isolates from nine globally distributed lineages using Galleria mellonella and rabbit infection models, alongside phenotypic and transcriptomic analyses. Despite marked genotypic and phenotypic heterogeneity, mortality in G. mellonella varied significantly between lineages and correlated with

Methicillin-susceptible Staphylococcus aureus (MSSA) necrotizing pneumonia can progress to severe disease, particularly when Panton-Valentine leukocidin (PVL) is involved. Here we report the case of a previously healthy 56-year-old man who developed severe necrotizing pneumonia due to MSSA despite the absence of major toxin genes including PVL. The patient reported a 10-day history of low-grade fever and was admitted for bilateral pneumonia with sepsis. Despite antibiotic therapy, his condition rapidly progressed, and chest computed tomography revealed extensive ground-glass opacities and mult

Klebsiella pneumoniae is a significant pathogen responsible for a range of infections, including pneumonia, urinary tract infections, and sepsis, in both human and veterinary medicine. The extracellular polysaccharide produced by this bacterium forms a capsule that protects the cell from immune responses and external influences, complicating treatment. The World Health Organization has placed this pathogen into the ESKAPE group (Enterococcus faecium, Staphylococcus aureus, Klebsiella pneumoniae, Acinetobacter baumannii, Pseudomonas aeruginosa, and Enterobacter spp.) of pathogens that require s

Drug use-associated infective endocarditis (DUA-IE) is an increasingly important clinical problem that affects younger patients and poses substantial diagnostic, therapeutic, and surgical challenges. We conducted a retrospective cohort study of adults with definite infective endocarditis treated at a tertiary referral center between 2017 and 2022, comparing patients with DUA-IE to those with non-drug use-associated infective endocarditis. Of the 189 patients, 43 (22.8%) had DUA-IE. These patients were significantly younger and had higher rates of HIV and hepatitis C coinfections. Staphylococcu

INTRODUCTION: MRSA bacteremia is associated with substantial clinical burden. Ceftaroline (CPT)-based combinations with vancomycin or daptomycin are increasingly utilized, but their added benefit remains uncertain. This meta-analysis compared the effectiveness and safety of CPT-based combination therapy versus vancomycin or daptomycin monotherapy.
METHODS: MEDLINE, Scopus, and Cochrane Central were systematically searched. Primary outcomes were all-cause mortality and microbiological recurrence. Data were pooled using Review Manager, and meta-regression analyses were performed in RStudio. Het
