MEDICAL HISTORY: A 93-year-old sarcopenic female patient was admitted to the geriatric ward for further treatment and diagnostic evaluation following a fall and a sacral fracture that had been treated conservatively. Clinically, she primarily presented with loss of appetite accompanied by nausea and recurrent vomiting. A diagnosis of a traction diverticulum had already been made during a previous hospital stay. Subsequently, the patient experienced progressive weight loss with weakness, culminating in a fall and the resulting sacral fracture.

Children with Down syndrome (DS) may experience feeding difficulties arising from interactions among hypotonia, cardiorespiratory and gastrointestinal conditions, swallowing dysfunction, oral-motor and sensory differences, delayed feeding skill development, and psychosocial or environmental factors. These difficulties may compromise feeding safety and efficiency, restrict dietary diversity, and contribute to inadequate fluid or nutrient intake, faltering growth, micronutrient deficiencies, or excess adiposity. This narrative review synthesized the literature selected for its relevance to feedi

Background: Pediatric autoimmune disorders are increasingly common, yet outcomes remain poor due to delayed diagnosis and lack of appropriate actions. Immune system dysfunction frequently coincides with deficiencies in essential micronutrients and cofactors, including vitamin D. Objectives: This study evaluated the contribution of vitamin D and cofactor (e.g., magnesium) deficiencies to impaired endocrine secretion and autoimmune susceptibility, which may trigger or exacerbate autoimmunity in children. Methods: This systematic review examines the complex interactions between micronutrients-par

Major gynecologic oncology surgery produces an acute inflammatory and neuroendocrine stress response superimposed on cancer-associated inflammation, frailty, malnutrition, anemia, and treatment-related physiologic vulnerability. Although this response is necessary for tissue repair, excessive or poorly resolved activation can contribute to pain, postoperative nausea and vomiting, ileus, insulin resistance, endothelial dysfunction, infection, venous thromboembolism, functional decline, and delayed return to oncologic treatment. Enhanced recovery after surgery (ERAS) pathways reduce avoidable st

Median arcuate ligament syndrome (MALS) is atypical in elderly patients, in whom it frequently coexists with severe atherosclerotic disease. We report the case of a 77-year-old male with marked mitral regurgitation who presented with intestinal claudication and malnutrition. CT angiography demonstrated MALS associated with a fusiform aneurysm of the celiac trunk with dissection, along with 60% atherosclerotic stenosis of the superior mesenteric artery. Given the limiting ischemic presentation, the patient underwent laparoscopic release of the ligament. The procedure was complicated by avulsion

Vitamin D is a nutrient and prohormone obtained from diet and supplements and synthesized in the skin. It is essential for calcium and phosphate homeostasis and skeletal health and influences multiple extra-skeletal processes, yet deficiency remains common worldwide. This narrative review summarizes vitamin D sources, physiology, metabolism, pharmacokinetics, clinical relevance and safety and outlines the distinct pharmacokinetic profiles and clinical roles of vitamin D, calcidiol and calcitriol and its analogues. Evidence consistently supports vitamin D, combined with calcium, for preventing

UNLABELLED: Fractures in children are influenced by both skeletal biology and mechanical factors. While alterations in vitamin D and other metabolic markers have been proposed as contributors to fracture risk, their role in predicting fractures in otherwise healthy pediatric populations remains unclear. This study aimed to determine whether biochemical markers of bone metabolism and body mass are independently associated with fracture occurrence and fracture patterns in children after low-energy trauma and to assess their impact on fracture patterns and anatomical distribution. We conducted a

PURPOSE: Ectonucleotide pyrophosphate/phosphodiesterase family member 1 (ENPP1) deficiency is a rare genetic disorder caused by loss-of-function ENPP1 gene mutations. Characterized by abnormally low circulating inorganic pyrophosphate concentrations, bone hypomineralization, soft tissue calcification, and arterial stenosis, ENPP1 deficiency is associated with a phenotypic spectrum that includes generalized arterial calcification of infancy type 1 (GACI1) and autosomal recessive hypophosphatemic rickets type 2 (ARHR2). Despite phenotypic differences, patients with GACI1 and ARHR2 have a marked,

BACKGROUND: Vitamin B12 deficiency, commonly attributed to malabsorption, presents significant clinical consequences including hematological and neurological manifestations. Helicobacter pylori infection, a prevalent global pathogen, has been implicated in gastric pathology that may compromise vitamin B12 absorption through various mechanisms including gastric atrophy and impaired intrinsic factor production. This study aimed to evaluate the association between H. pylori infection and vitamin B12 deficiency.
METHODS: A retrospective study was conducted at Thumbay Hospital, Ajman, UAE, involvi

Older women undergoing cancer surgery commonly enter the perioperative period with overlapping frailty, malnutrition, sarcopenia, obesity, multimorbidity, and treatment-related symptoms. These vulnerabilities reduce physiologic reserve and amplify the catabolic response to surgery, increasing the risk of complications, functional decline, delayed adjuvant treatment, and persistent nutritional impairment. Nutrition-oriented perioperative nursing provides a practical framework for translating frailty assessment and cancer nutrition guidance into coordinated care before surgery, during enhanced r

Polycystic ovary syndrome (PCOS) is a common endocrine disorder and a leading cause of infertility in reproductive-aged women. Emerging evidence suggests that vitamin D deficiency may exacerbate PCOS-related reproductive and metabolic disturbances. This study aimed to assess the prevalence of vitamin D deficiency among infertile women with PCOS and explore its association with subfertility. This cross-sectional study was conducted at the Department of Infertility, Bangladesh Medical University (BMU), from November 2019 to April 2025. Total 155 infertile women aged 18-40 years; diagnosed with P

Iron deficiency is common in heart failure (HF) and particularly frequent in older adults, in whom it may contribute to fatigue, exercise intolerance, impaired quality of life and functional decline, even without overt anaemia. In HF, intravenous iron has shown consistent benefits on symptoms, functional capacity and health-related quality of life in selected populations, mainly with reduced ejection fraction, and some studies have suggested a reduction in recurrent HF hospitalisations. However, more recent large trials have shown less consistent effects on hard clinical outcomes and a mortali

BACKGROUND: Navigation of spinal instrumentation is heavily dependent on maintaining an accurate registration of the local anatomy to the navigation software. Any motion of the anatomy reduces accuracy. Historically, navigation in the cervical spine has been limited by the mobile nature of the anatomy. Here, we describe the development and testing of a novel stabilizer clamp designed to reduce motion and improve stability for navigated-instrumentation accuracy.
METHODS: Testing was completed both on a flat-top table and separately on a Jackson table. On both tables, the cadaveric specimen was

PURPOSE: Frailty is common among patients with acute ischemic stroke (AIS) and is associated with adverse health outcomes. Although hyperhomocysteinemia (Hhcy) is a recognized biomarker in AIS, its association with frailty remains unclear. This study aimed to examine the relationship between Hhcy and frailty and assess whether Hhcy is independently associated with frailty in AIS patients.
PATIENTS AND METHODS: We conducted a cross-sectional study involving 397 AIS patients admitted between April 2023 and April 2024. Frailty was assessed using the Tilburg Frailty Indicator (TFI). Clinical and

INTRODUCTION: Serum albumin reflects nutritional status and allostatic load, making it useful for surgical risk assessment in undernourished populations. Chronic socioeconomic and environmental stress can cause weathering, an accelerated decline in physiological reserve that may increase surgical vulnerability in disadvantaged patients. We aimed to define a locally optimized preoperative serum albumin cutoff for predicting 30-d postoperative mortality in our population.
METHODS: We performed a secondary analysis of ACS-NSQIP data (2019-2023) from a tertiary-care center in Karachi, Pakistan. A

A major challenge in Parkinson's disease (PD) research is the identification of non-invasive neurophysiological markers that can distinguish PD patients from healthy controls while providing interpretable information about cortical dysfunction. This study investigates whether aperiodic EEG spectral parameters extracted using Fitting Oscillations and One-Over-F decomposition can serve as candidate indicators for exploratory PD classification and medication-state analysis. Resting-state EEG data from 15 PD patients and 16 healthy controls were analyzed. For PD participants, both medicated and un
