Two horizons, one burden: estimating lifetime and decade-long medical costs of childhood obesity in Kazakhstan.

OBJECTIVE: To estimate the lifetime and cumulative direct medical burden associated with childhood overweight and obesity in Kazakhstan using a dynamic age-structured modeling approach. METHODS: A dynamic age-structured Markov cohort model was developed to simulate the progression of children aged 5-9 years through successive age groups and weight states (normal weight, exclusive overweight, and obesity). Transition probabilities were reconstructed using a synthetic cohort approach based on repeated cross-sectional prevalence data and were calibrated to reproduce observed epidemiological patt

Pancreaticoduodenectomy after Laparoscopic Sleeve Gastrectomy with Preservation of Gastric Arterial Inflow: A Case Report.

INTRODUCTION: Obesity is an established risk factor for pancreatic ductal adenocarcinoma. As laparoscopic sleeve gastrectomy (LSG) has become increasingly prevalent, pancreatic surgeons may encounter malignancies in patients with a surgically altered gastric anatomy. Pancreaticoduodenectomy (PD) after an LSG is rare and requires meticulous preoperative planning to prevent ischemic and anastomotic complications. CASE PRESENTATION: A 47-year-old man with morbid obesity (BMI 59.1 kg/m2) underwent LSG. Twenty-three days later, obstructive jaundice led to the diagnosis of pancreatic head adenocarc

Pretreatment body mass index as a prognostic factor for clinical course and survival in primary surgically treated oral squamous cell carcinoma - a retrospective study.

INTRODUCTION: In oral squamous cell carcinoma (OSCC), body mass index (BMI) is an available but imperfect marker of nutritional reserve. Evidence on pretreatment BMI in surgery-based European OSCC cohorts remains limited, especially regarding perioperative outcomes. This study evaluated associations of pretreatment BMI with overall survival (OS), recurrence-free survival (RFS), and perioperative outcomes after curative-intent R0 resection. METHODS: This retrospective single-center cohort included adult patients with previously untreated primary OSCC, available pretreatment BMI, and curative-i

Ubiquitous Sirt1 overexpression is associated with preserved cardiac diastolic function and mitochondrial respiratory capacity in mice on a high-fat diet.

BACKGROUND: Chronic high-fat diet (HFD) induces obesity, insulin resistance, and mitochondrial dysfunction, serving as a critical driver of diabetic cardiomyopathy. In turn, the NAD + -dependent deacetylase Sirt1 regulates metabolic flexibility and cellular survival, making it a promising therapeutic target to counteract this diet-induced cardiovascular damage. METHODS: Here, we investigated the impact of a moderate, systemic Sirt1 overexpression (∼3-fold via a BAC transgenic mouse model; Sirt1OE) on HFD-induced metabolic and cardiac remodeling. Sirt1OE and wild-type (WT) male mice were fed a

Dynapaenic Abdominal Obesity Is Associated With a Poorer Quality of Life in Women Diagnosed With Breast Cancer Awaiting Surgery.

BACKGROUND: Lower quality of life (QoL) in cancer can negatively influence survival and treatment course. We have investigated the co-occurrence of dynapaenia and overweight or obesity in a sample population of women diagnosed with breast cancer admitted for cancer surgical treatment and how those chronic conditions influence QoL. METHODS: Ninety-seven overweight or obese women admitted to hospital for scheduled lumpectomy or mastectomy were recruited. Correlative analyses of clinical, epidemiological, anthropometric, nutritional, and handgrip strength assessments were carried out. Dynapaenic

Obesity and work: HEAF study of older workers, a 6-year follow-up.

BACKGROUND: Obesity impacts work ability. Previous longitudinal research in the Health and Employment After Fifty (HEAF) cohort found that obesity reduced work participation to older ages, with effects only partially explained by comorbidities. AIMS: To test the hypothesis that obesity impacts working to older ages through either depressive symptoms and/or function, this study was conducted using 6-year prospective data. METHODS: HEAF is a population-based cohort study of 8000+ adults aged 50-64 years recruited 2013-14. Six-year data from HEAF were used to construct Cox proportional hazard m

Gut microbiome composition and cardiometabolic risk among healthcare workers: findings from the EXPONIT study.

BACKGROUND AND AIMS: The gut microbiome plays an important role in metabolic regulation and has been linked to cardiometabolic disorders. Circadian-related factors (night shift work, sleep quality, chronotype) may influence gut microbiome characteristics and cardiometabolic health. We aimed to investigate the associations between gut microbiome diversity and composition and cardiometabolic outcomes, and evaluate whether these associations differ by work schedule, sleep quality, and chronotype. METHODS AND RESULTS: Fecal samples were taken on a working day, and gut microbiota composition was a

[Weigh your words: standardizing terminology in obesity management and metabolic bariatric surgery].

The substantial increase in scientific publications and accumulated knowledge in obesity management and metabolic bariatric surgery has been accompanied by lack of uniformity in professional terminology, both in English and in its translation into Hebrew. This inconsistency may lead to stigmatization, ambiguity, limited comparability between studies, and interpretation challenges among clinicians and researchers. In response, the International Federation for the Surgery of Obesity and Metabolic Disorders (IFSO) published recommendations for a standardized terminology system to improve scientif

Virtual Diabetes Optimization Program (VDOP) for Veterans With Uncontrolled Diabetes.

OBJECTIVE: More than 25% of U.S. veterans have diabetes, and 78% have a BMI ≥25 kg/m2. Despite access to lifestyle interventions and specialty endocrine care through the Department of Veterans Affairs (VA), many veterans with uncontrolled diabetes and obesity do not achieve sustained glycemic control or weight loss. We developed the Virtual Diabetes Optimization Program (VDOP), a quality improvement initiative targeting veterans with uncontrolled diabetes (glycated hemoglobin A1c [HbA1c] ≥8% [≥64 mmol/mol]) with or without obesity. RESEARCH DESIGN AND METHODS: VDOP used a multidisciplinary te

Physical Activity Patterns in Adults Taking Glucagon-Like Peptide 1 Receptor Agonists: An All of Us Cross-sectional Study.

OBJECTIVE: Treatment with a glucagon-like peptide 1 receptor agonist (GLP-1RA) is an effective pharmacotherapy for obesity and diabetes care, but physical activity patterns among adults taking this class of medication have not been well described. Thus, the objective of this study was to characterize physical activity patterns in U.S. adults taking a GLP-1RA and identify sociodemographic and clinical factors associated with low activity levels. RESEARCH DESIGN AND METHODS: We conducted a cross-sectional study using data from the National Institutes of Health All of Us Research Program (2018-2

Evaluating the safety of glucagon-like peptide-1 agonist use in non-diabetic overweight patients undergoing shoulder arthroplasty.

BACKGROUND: Initially indicated for diabetes mellitus (DM), glucagon-like peptide 1 (GLP-1) agonists are now indicated for obesity management in isolation of DM. While prior studies have examined post-operative outcomes after total shoulder arthroplasty (TSA) in GLP-1 users with DM, there is a paucity of literature evaluating its use in non-diabetic patients for weight loss rather than glucose control. Therefore, our analysis compared 90-day and 2-year outcomes following TSA between non-diabetic obese GLP-1 users and a matched cohort of obese GLP-1 non-users. METHODS: The PearlDiver Mariner d

Not one enclave: Disaggregation and cardiometabolic health in Asian ethnic enclaves.

Many Asian American (AA) subgroups experience disproportionate rates of cardiometabolic (CMB) conditions, yet the contextual drivers of these disparities remain unclear. Little is known about the role of Asian residential segregation, often conceptualized as Asian enclaves, with limited prior work largely ignoring region of origin and nativity. Using six years of cross-sectional survey data from New York City (N = 6686 AAs) linked with multiple sources of community data, we examine how residence in ethnicity-specific enclaves relates to CMB risks (obesity, hypertension, and diabetes), whether

Long-term voluntary exercise partially reverses obesity-associated transcriptomic remodeling in brown adipose tissue.

BACKGROUND: Although exercise may counteract some changes caused by obesity-induced functional and molecular remodeling of brown adipose tissue (BAT), the extent to which long-term voluntary exercise reverses obesity-associated BAT transcriptional changes remains unclear. METHODS: Male C57BL/6J mice were assigned to the control, high-fat diet (HFD), or HFD plus voluntary exercise (HFD + VE) groups for four months (n = 4 per group). Interscapular BAT transcriptomic profiles were analyzed using the Clariom S Mouse Array. Differential expression was explored using eBayes ANOVA, and transcript cl

Body mass index is associated with muscle strength, functional capacity, balance, and quality of life in patients with type 2 diabetes: A cross-sectional study.

BACKGROUND: Type 2 diabetes mellitus (T2DM) is accompanied by obesity and elevated body mass index (BMI), which may be associated with poorer physical function and quality of life (QOL). However, associations of BMI with muscle strength, functional capacity, balance, and QOL have not been comprehensively evaluated together in T2DM. PURPOSE: To evaluate associations of BMI with muscle strength, functional capacity, balance, and QOL in patients with T2DM. METHODS: Patient characteristics were recorded. Body composition, muscle strength, functional capacity, balance, and QOL were assessed using

High-fiber diet altered gut microbiota, improved glycemic control, and reduced body weight in newly diagnosed type 2 diabetes and obesity.

Type 2 diabetes and obesity commonly coexist, making glycemia and body weight both central treatment goals. Few trials have directly compared a high-fiber dietary intervention with metformin. The factors associated with individual responses to high-fiber diet also remain unclear. In this single-center, open-label, head-to-head clinical trial, 48 adults with newly diagnosed type 2 diabetes and overweight or obesity were randomized to a high-fiber dietary intervention without glucose-lowering medication (n = 36) or metformin (n = 12) monotherapy for 84 days; 46 completed the trial. Exercise, Dietary, and Combined Exercise-Dietary Interventions for Metabolic Syndrome: A Bayesian Network and Component Network Meta-Analysis of Randomized Controlled Trials.

Existing reviews have not clarified how exercise only, dietary only, and combined exercise dietary interventions compare across individual metabolic syndrome (MetS) components. We aimed to compare structured exercise-only, dietary-only, and combined exercise-dietary interventions in adults with MetS and overweight or obesity with an inactive or minimal-intervention control or other active interventions for waist circumference (WC), systolic blood pressure (SBP), diastolic blood pressure (DBP), fasting blood glucose (FBG), triglycerides (TG), and high-density lipoprotein cholesterol. Association Between Lipid Accumulation Product and Visceral Adiposity Index, as Markers of Visceral Adiposity and Metabolic Dysfunction, and Stroke: NHANES 1999-2018.

AIMS: Lipid accumulation product (LAP) and visceral adiposity index (VAI) are surrogate markers of visceral adiposity-related endocrine and metabolic dysfunction, including insulin resistance, rather than simple obesity indices. This study aimed to evaluate their associations with stroke prevalence using nationally representative data. MATERIALS AND METHODS: This cross-sectional study analyzed data from the National Health and Nutrition Examination Survey (NHANES) 1999-2018. Stroke status was determined through standardized questionnaires. LAP and VAI were calculated using established gender-

Heavy inheritance: Genetic pathways from excess weight to biological aging and health in old age.

In this paper, we investigate the impact of excess body weight on biological aging and health in old age. Drawing on a large sample of U.S. adults from the Health and Retirement Study, we construct a frailty index, defined as the proportion of age-related health deficits present in a person. Our findings reveal a positive association between BMI (and other indicators of excess weight) and the frailty index. To identify a causal relationship, we use a polygenic risk score (PRS) for BMI as an instrumental variable for excess weight. We further demonstrate that the effect of the PRS on BMI streng
