Pachymic acid alleviates circadian rhythm disorders in high-fat diet-induced obesity mice via the sphingolipid pathway.

BACKGROUND: Obesity caused by a high-fat diet (HFD) is known to disrupt metabolic homeostasis and circadian rhythms. Pachymic acid (PA), a bioactive triterpenoid, exhibits anti-inflammatory, antihyperglycemic, antihyperlipidemic, and sedative-hypnotic properties, though its role in circadian regulation remains unexplored. METHODS: We assessed PA's impact on metabolic dysfunction (glucose/lipid profiles), systemic inflammation using biochemical assays, ELISA, Oil Red O staining. Circadian parameters were evaluated via 24-h serum melatonin and core body temperature. Hepatic circadian gene oscil

Metabolic dysfunction and insulin resistance in depression: mechanisms, evidence, and therapeutic implications.

Major depressive disorder (MDD) remains one of the most frequently diagnosed mental health disorders and a leading cause of disability worldwide. The discovery of new treatments for MDD is necessary given up to 30% of individuals with MDD are resistant to current antidepressant treatments. The significant physiological interplay between MDD and metabolic conditions is crucial to consider for future therapeutics given the current obesity epidemic and the increased prevalence of treatment-resistant depression in individuals with metabolic dysfunction. The aim of this review is to discuss the rel

Screening for High Predictors of Cardiovascular Risk among Medical Students in Coastal Area of Andhra Pradesh.

BACKGROUND: Cardiovascular diseases are the leading cause of global mortality, with a rapidly increasing burden in low- and middle-income countries, including India. Early-life exposure to modifiable risk factors significantly contributes to the development of atherosclerosis and future cardiovascular events. Medical students, owing to academic stress, sedentary lifestyle, and dietary irregularities, represent a potentially high-risk yet underevaluated group. AIMS: This study aimed to assess the prevalence and distribution of cardiovascular risk factors among medical students and to compare t

Primary keratinizing squamous cell carcinoma of the renal pelvis with pancreatic tail invasion and five-year disease-free survival: A case report.

Primary squamous cell carcinoma (SCC) of the renal pelvis (<1% of upper urinary tract tumors) was diagnosed in a 58-year-old woman with anemia. MRI showed infiltrative renal pelvis mass with hydronephrosis and pancreatic tail invasion (pT4N0M0). Radical nephroureterectomy with bladder cuff, lymph node dissection, and en bloc pancreatic resection achieved complete resection despite morbid obesity. Well-differentiated keratinizing SCC with perineural invasion was confirmed externally. No adjuvant therapy was given. Five-year PET/CT showed no recurrence, demonstrating excellent long-term survival

Association of body mass index with complications in cardiac implantation electronic devices: A retrospective single-center cohort study.

Cardiac implantable electronic devices (CIEDs) are increasingly used for managing arrhythmias and advanced heart failure, but implantation procedures carry risks of bleeding, infection, and mechanical complications. Patient-related factors such as obesity and comorbidities, alongside perioperative anticoagulation, may influence outcomes, yet evidence from South Asian populations is limited. We conducted a single-center observational study of 1925 patients who underwent CIED procedures between January 2010 and October 2025 at a tertiary heart center in Pakistan. Patients were categorized by bod

Psychosocial aspects and mental health burden of pediatric MASLD.

Metabolic dysfunction-associated steatotic liver disease (MASLD) is the most common chronic liver disease of childhood and the hepatic component of a systemic metabolic and endocrine disorder linked to obesity and insulin resistance. Its psychological and social dimensions, however, remain under-recognized relative to its metabolic features. Affected children carry a substantial mental-health burden: elevated rates of depression, anxiety, disordered eating, and impaired quality of life that both reflect and aggravate the underlying disease. This mini review uses the biopsychosocial model to su

Beyond Glycated Hemoglobin (HbA1c): A Systematic Review of Metabolic Interventions, Insulin Resistance, and Cognitive Function Across the Obesity-to-Diabetes Continuum.

Insulin resistance may contribute to cognitive vulnerability before persistent hyperglycemia becomes apparent; however, its clinical relevance as a predictor of cognitive change or therapeutic target remains uncertain. This systematic review with narrative synthesis evaluated the effects of metabolically targeted interventions on insulin resistance and cognitive function in middle-aged and older adults across the obesity-to-type 2 diabetes continuum. MEDLINE via PubMed, Embase, Scopus, and the Cochrane Central Register of Controlled Trials (CENTRAL) were searched for studies published from Jan

Association of triglyceride-glucose and obesity-derived indices with the risk of aortic stenosis among individuals in cardiovascular-kidney-metabolic syndrome stages 0-3: a prospective cohort study from the UK Biobank.

BACKGROUND: Cardiovascular-kidney-metabolic (CKM) syndrome emphasizes the interplay between metabolic dysfunction, kidney injury, and cardiovascular abnormalities. The triglyceride-glucose (TyG) and its obesity-related derivatives have emerged as simple biomarkers for insulin resistance, yet their association with aortic stenosis (AS) risk in CKM syndrome stages 0-3 remains unclear. METHODS: This prospective cohort study included 363,907 participants with CKM stages 0-3 from the UK Biobank. We calculated six TyG-related indices (TyG, TyG-BMI, TyG-Waist, TyG-WHR, TyG-WHtR, and TyG-ABSI) and ev

Hematopoietic-Specific DARC Deficiency Is Associated with Adipose Tissue Inflammation and Impaired Glucose Tolerance During Diet-Induced Obesity in Mice.

OBJECTIVE: Adipose tissue inflammation in obesity promotes insulin resistance and metabolic disease. The Duffy Antigen Receptor for Chemokines (DARC), a non-signaling receptor expressed on erythrocytes and on other cell types, modulates inflammation by regulating chemokine levels. Gene variants that affect DARC expression on erythrocytes are common in people of African descent. Here, we disrupted DARC expression in hematopoietic cells and adipocytes to determine the impact on obesity. METHODS: DARC floxed mice were bred with vav1-Cre mice to disrupt DARC expression in hematopoietic cells (DAR

Long-term diet-induced obesity disrupts testicular activity in Wistar rats: biochemical, hormonal, and structural evidence.

The increasing prevalence of obesity has been closely associated with male reproductive disorders. Studies on diet-induced obesity provided valuable insights into the factors involved in the decline of male reproductive potential in obese subjects. The aim of the present study was to investigate the consequences of long-term exposure to cafeteria diet, particularly on testicular activity, and pinpoint the most affected components by this condition. The study was conducted on male Wistar rats fed either a standard chow or a cafeteria diet during 16 weeks. A model of diet-induced obesity was ass

Prediction analysis for delayed discharge following gastric bypass surgery.

BACKGROUND: Growing rates of obesity and an increased demand for bariatric surgery have led to shorter post-operative hospital stays. While next-day discharge is becoming increasingly common, limited data exists to identify patients requiring longer stays. This study aims to develop a prediction model using preoperative factors to predict the successful discharge within 24hours (single-day discharge) following Roux-en-Y gastric bypass. METHODS: A retrospective chart review was performed. Patient age, gender, preoperative weight, body mass index (BMI), medication use, comorbidities and dischar

Impact of aquatic versus land exercise on glycated hemoglobin and blood pressure in older adults with multimorbidity: A randomized controlled trial protocol.

INTRODUCTION: Multimorbidity is defined as the prese of two or more chronic conditions in the same individual, being a growing global health concern, particularly among older adults. This condition is associated with decreased quality of life, increased healthcare utilization, and functional decline. Although physical exercise is widely recommended for managing chronic diseases, such as hypertension and type 2 diabetes, evidence remains limited regarding the most effective exercise modality for individuals with multimorbidity. This study protocol outlines a randomized controlled trial designed

Glucagon-Like Peptide-1 Signaling in Learning and Memory: Evidence, Mechanisms, and Therapeutic Implications.

Glucagon-like peptide-1 (GLP-1) is primarily known for its role in glucose homeostasis and food intake control, and GLP-1 analogs (either as monotherapy or dual agonists) are commonly used for type 2 diabetes and obesity treatment in humans. Beyond these functions, the GLP-1 receptor (GLP-1R) is widely expressed throughout the brain, including in the hippocampus (HPC) and interconnected regions that contribute to learning and memory processes. Here we review emerging evidence supporting a role for GLP-1 signaling in promoting learning and memory function, particularly in dementia and other con
