Cumulative Effects of Genetic Variants Detected in a Child with Early-Onset Non-Syndromic Obesity Due to SIM-1 Gene Mutation.

BACKGROUND: Single-minded homolog 1 (SIM1) gene mutations with autosomal dominant inheritance have been related to hyperphagia and early-onset severe obesity. SIM1 is implicated in the development of hypothalamic nuclei, which play a crucial role in energy homeostasis. The development of melanocortin neural circuits in the hypothalamus is promoted by other factors such as Semaphorine 3 (SEMA3) and its receptors, such as PLXNA1-4 and NRP1-2. Loss-of-function across multiple SEMA3/NRP/PLXNA genes can collectively contribute to obesity onset. CASE DESCRIPTION: A 3-year-old male was referred for

Combined Potential of Orlistat with Natural Sources and Their Bioactive Compounds Against Obesity: A Review.

Obesity represents a significant global public health issue, contributing to the rising prevalence of metabolic diseases. One treatment for obesity is orlistat, a drug that inhibits pancreatic lipase. It is widely used due to its efficacy in reducing dietary fat absorption. However, patient adherence to this drug is often hindered by its associated adverse effects. As a result, there is an increasing interest in exploring alternative therapeutic options derived from natural sources, such as plants and algae, particularly extracts and their bioactive compounds. These extracts and compounds have

Concurrent hiatal hernia repair and gastric bypass as an adjunct in the treatment of hiatal hernia in populations with obesity.

BACKGROUND: Minimally invasive hiatal hernia repair with fundoplication (HHR) is the standard of care for hiatal hernias but has a high risk of recurrence even in populations without obesity. Concomitant roux-en-y gastric bypass (RYGB) with HHR may mitigate the increased risk of hiatal hernia recurrence in patients with obesity while also addressing obesity-related comorbidities. There is a paucity of data on this procedure. It is hypothesized that a concomitant RYGB with HHR is safe and effective in patients with obesity. METHODS: This is a single institution retrospective review of adult pa

Improving the diagnosis of hyperphagia in melanocortin-4 receptor pathway diseases.

Characteristics of hyperphagia include heightened and prolonged hunger, longer time to satiation, shorter duration of satiety, severe preoccupation with food (i.e., hyperphagic drive), abnormal food-seeking behaviors, and distress or functional impairment when food is unavailable. Patients with melanocortin-4 receptor (MC4R) pathway diseases including those caused by variants in one of multiple key genes of the pathway often present with hyperphagia that results in early-onset, severe obesity because this pathway plays a critical role in regulation of hunger/satiation and energy balance. Association between weight-adjusted-waist index and retinopathy among American adults: a cross-sectional study and mediation analysis.

BACKGROUND: Retinopathy, a microvascular complication linked to obesity and metabolic dysfunction, is characterized by the absence of reliable anthropometric predictors. The weight-adjusted waist index (WWI), incorporating waist circumference and weight, may better capture visceral adiposity and its pathological impact. This study investigates the association between WWI and retinopathy in U.S. adults and explores the potential mediating role of glycated hemoglobin (HbA1c). METHODS: A cross-sectional analysis was conducted on 5,572 adults aged ≥40 years from the 2005-2008 National Health and

Sex and Gender Differences in Obesity: Biological, Sociocultural, and Clinical Perspectives.

Sex and gender differences significantly influence the prevalence of obesity, patterns of fat distribution, metabolic health outcomes, and responses to treatment. While women generally exhibit a higher overall prevalence of obesity, men are more susceptible to visceral fat accumulation, which increases the risk of cardiovascular disease (CVD), type 2 diabetes, and other obesity-related complications. This review examines the biological, genetic, and sociocultural foundations of sex-based differences in obesity. Estrogen plays a crucial role in regulating subcutaneous fat deposition and brown a

Relative Fat Mass: Refining Adiposity Measurement in the Era Beyond Body Mass Index.

To position relative fat mass (RFM) as a more accurate, physiologically grounded, and clinically useful alternative to body mass index (BMI) for assessing adiposity and predicting cardiometabolic risk, including heart failure.  RECENT FINDINGS: RFM estimates body fat percentage using a sex-specific formula based on waist circumference and height. RFM not only correlates more strongly with fat mass than BMI, but also shows a weaker correlation with muscle mass. This distinction helps reduce lean mass-related confounding in the assessment of adiposity. In clinical studies, RFM has emerged as a r

Obesity as a risk factor for early-onset colorectal cancer: Evidence from a nationally representative database.

BACKGROUND: Colorectal cancer (CRC) is the second leading cause of cancer-related deaths worldwide with an alarming rise in early-onset CRC (eoCRC) over the past several decades. Unlike late-onset CRC, the drivers behind eoCRC remain less clear. While certain risk factors such as obesity and smoking have demonstrated a relatively strong association with eoCRC in the literature, some studies have challenged these associations, emphasizing the need for additional studies. AIM: To investigate the impact of various risk factors on eoCRC with a special focus on obesity. METHODS: This cross-sectio

Weight adjusted waist index is a superior obesity index for predicting arterial stiffness in type 2 diabetes mellitus.

Obesity is a key risk factor for arterial stiffness (AS) and a strong predictor of cardiovascular diseases (CVD). In patients with type 2 diabetes mellitus (T2DM), the risk of AS is further exacerbated due to metabolic dysregulation and chronic inflammation, especially when both conditions coexist. Traditional obesity indices such as Body Mass Index (BMI) and Waist Circumference (WC) have limitations in assessing fat distribution and metabolic risk. New indices like Weight-adjusted Waist Index (WWI), A Body Shape Index (ABSI), and Body Roundness Index (BRI) have been proposed as supplementary
