Oronasal blood, intrathoracic petechiae and pulmonary hemorrhages differentiate prone separate-bed from bed-sharing SIDS cases.

PURPOSE: To compare autopsy findings in sudden infant death syndrome (SIDS) cases found prone in a separate bed with those occurring during bed-sharing, and to contrast these with confirmed asphyxia and violent death controls. METHODS: We conducted a retrospective review of 313 SIDS cases from Southeast Norway (1984-2021) to assess forensic autopsy findings and death scene circumstances. A subset of cases was selected for blinded re-evaluation, with standardized microscopic examination of lung sections and semi-quantitative scoring of intra-alveolar hemorrhage. RESULTS: As SIDS incidence dec

A Crunch That Spoke: An Atypical Presentation of Spontaneous Pneumomediastinum in an Elderly Male.

Spontaneous pneumomediastinum is a rare, usually self-limiting, condition characterized by the presence of free air in the mediastinal cavity without any identifiable precipitating factor. We report the case of a 63-year-old man who presented with chest pain, chest tightness, and shortness of breath. The patient had no known comorbidities and no history of smoking or alcohol intake. On examination, the patient was found to have subcutaneous emphysema around the neck and chest progressing to the face and upper limbs and extending to the groin, with no external injuries. Crepitus was present, ex

Cognitive disengagement and emotional-behavioral profiles in children with voiding dysfunction: a controlled clinical study.

Voiding dysfunction (VD) is a common condition in childhood that is associated with lower urinary tract symptoms and bowel dysfunction. It may also adversely affect patients' emotional, behavioral, and social functioning. This cross-sectional case-control study evaluated emotional and behavioral problems, attention-related symptoms, and cognitive disengagement syndrome (CDS) in children with VD and examined their relationship with lower urinary symptoms severity and constipation. VD was diagnosed according to International Children's Continence Society criteria using voiding diaries, imaging f

Superior vena cava reconstruction using autologous femoral vein graft.

Chronic superior vena cava obstruction is a recognized complication of transvenous pacemaker and implantable cardioverter-defibrillator leads. A 39-year-old woman with genetically confirmed left ventricular non-compaction cardiomyopathy and a transvenous implantable cardioverter-defibrillator placed for primary prevention developed progressive superior vena cava syndrome. Computed tomography angiography demonstrated chronic occlusion of the superior vena cava and left brachiocephalic vein with extensive collateralization. Endovascular recanalization was attempted from femoral, bilateral intern

Clobazam and Valproate, but Lamotrigine, a Sodium Channel Inhibitor, Reduce the Incidence of Hyperthermia-Induced Clonic Seizures in Dravet Syndrome Mice: Assessment of Anti-Seizure Effects in Mice Using a Stabilized Ambient Temperature System.

BACKGROUND: Dravet syndrome (DS) is a severe and treatment-resistant epileptic encephalopathy, most commonly caused by de novo mutations in the sodium voltage-gated channel alpha subunit 1 (SCN1A) gene. Patients with DS often present with febrile seizures. It has been reported that Scn1a mutant mice have the risk for hyperthermia-induced clonic seizures similar to those observed in patients with DS. Using our heating protocol with an incubator, we evaluated the effects of antiseizure medications on the incidence of hyperthermia-induced clonic seizures in mutant Scn1a knock-in (KI)/+ mice. MET

Association of cardiovascular-kidney-metabolic syndrome with blinding eye diseases: insights from a UK Biobank cohort analysis.

BACKGROUND: Cardiovascular-kidney-metabolic (CKM) syndrome's overall impact on major blinding eye diseases remains unclear. METHODS: We included participants from the UK Biobank. Baseline CKM syndrome at study entry was classified into five stages according to the American Heart Association criteria. We calculated the absolute incidence rates of cataract, glaucoma, and age-related macular degeneration (AMD) across CKM stages. The associations between CKM syndrome and these eye diseases were examined using Cox proportional hazards models, with further sensitivity and subgroup analyses conducte

Associations between Primary Hyperhidrosis and Psychiatric Conditions: A Matched Case-Control Study of 8,508 Patients.

INTRODUCTION: Primary hyperhidrosis (PHH) is defined by excess sweating not attributable to medications or underlying medical conditions. It is associated with significant burden of subsequent psychiatric comorbidities, but psychiatric conditions preceding PHH diagnosis are not well characterized. We assessed associations of PHH with preceding psychiatric conditions using a matched case-control approach. METHODS: Overall, 1,418 participants with PHH and 7,090 matched controls were assessed using the All of Us Research Program database. Multivariable logistic regressions calculated odds ratios

Straatsma syndrome: the diagnostic value of dilated funduscopy in anisometropic amblyopia.

Straatsma syndrome is a rare condition defined by the triad of unilateral high axial myopia, amblyopia and myelinated retinal nerve fibre layer (MRNFL). Visual prognosis is often guarded when myelination is extensive, the macula is involved, or anisometropia is high. We present a case of classic Straatsma syndrome in a boy in his middle childhood from the Arabian Gulf region. In the right eye, he had extensive type 2 MRNFL involving the superior macula, high axial myopia and amblyopia. Given his age and extensive MRNFL, retinoblastoma was excluded. We describe his longitudinal clinical course

Post-Tracheostomy Fistula Formation Between the Trachea and Left Common Carotid Artery in a Pediatric Patient with Trisomy 18.

The long-term survival of individuals with trisomy 18 has improved, and the need for tracheostomy in this population has thus increased. We describe the case of a 3-year-old Japanese boy with trisomy 18 who developed a tracheoarterial fistula involving the left common carotid artery (LCCA). He underwent an emergency tracheostomy for respiratory failure secondary to pneumonia. Massive hemorrhage from the tracheostoma occurred on postoperative day 31, leading to his death. The autopsy revealed that the LCCA originated from the brachiocephalic trunk and coursed medially, forming a fistulous conne

The development of the digital coercion scale (DCS) and its association with suicide crisis syndrome (SCS).

As internet and social network use have risen, rates of anxiety, depression, and suicide have increased. Loss of autonomy and difficulty disengaging from technology may contribute to suicide vulnerability, but no measure currently captures this experience. To address this gap, we developed the Digital Coercion Scale (DCS) to measure perceived loss of autonomy related to ubiquitous technology. Eighty-nine psychiatric inpatients completed the DCS. Reliability was evaluated using Cronbach's α and item-total correlations. Construct validity was assessed through correlations with established scales

Case Report: Transjugular liver biopsy-confirmed lymphoma presenting as hemophagocytic lymphohistiocytosis with severe thrombocytopenia: a case series of three patients.

BACKGROUND: Hemophagocytic lymphohistiocytosis (HLH) is a life-threatening hyperinflammatory syndrome that can obscure underlying malignancies, particularly aggressive B-cell lymphoma. Severe cytopenias and coagulopathy often preclude conventional biopsy, delaying diagnosis. OBJECTIVES: To assess the diagnostic utility of transjugular liver biopsy (TJLB) in adult patients presenting with HLH, transfusion-refractory thrombocytopenia, and predominant hepatic involvement, and to explore mechanisms of thrombocytopenia. METHODS: Three adult patients with HLH, hepatic lesions, and severe thrombocy

Treating hematologic immune dysregulation in inborn errors of immunity: a real-life multicenter study.

BACKGROUND: Hematologic immune dysregulation (H-ID) - including autoimmune cytopenia (AIC), lymphoproliferation (LPD), and hemophagocytic lymphohistiocytosis (HLH) - is a potentially life-threatening manifestation of inborn errors of immunity (IEI). Despite the availability of targeted therapies, its management remains challenging, with no standardized treatment strategies established. OBJECTIVE: To evaluate the efficacy and safety of the available treatments for H-ID, with a specific focus on the treatment indications and outcome differences between targeted versus non-targeted therapies. A case of Lemierre-like syndrome with mediastinal-pulmonary communication managed without lung resection.

Lemierre-like syndrome, an atypical variant of Lemierre's syndrome, is characterised by septic thrombophlebitis without classical features and may be complicated by descending necrotising mediastinitis (DNM). Contrast-enhanced computed tomography revealed extensive thrombosis of the right internal jugular, external jugular, and subclavian veins, along with multiple cervical and mediastinal abscesses, consistent with DNM. A cystic lesion in the right upper lobe suggested a lung abscess. Emergency cervical drainage was performed, followed by thoracoscopic mediastinal drainage. Communication betw

[When radiology does not rule out intracranial hypertension].

This case report presents a 43-year-old female with a ventriculoperitoneal shunt. She had symptoms of increased intracranial pressure: headache, nausea, vomiting and blurred vision. A CT showed unchanged narrow ventricles, but an invasive intracranial pressure monitor showed severely elevated intracranial pressure. Elevated intracranial pressure can, in select cases, elude typical radiological findings in those with cerebral ventricular shunts, especially those with long-standing shunts, typically from birth.

Venous Air/Gas Embolism During Surgical Hysteroscopy: An Underestimated, Potentially Lethal Complication.

Hysteroscopic surgery is generally considered low risk; however, venous air/gas embolism is a potentially lethal complication with high mortality in severe cases. We summarize the pathophysiology and clinical features to support timely emergency management. A 46-year-old woman with a uterine myoma underwent hysteroscopic surgery under general anesthesia. Approximately 50 minutes after the start of the procedure, a sudden decrease in end-tidal CO₂, bradycardia, hypotension, and ST-segment elevation were observed. Transthoracic echocardiography revealed air bubbles in the right ventricle, confir
