An anesthesiological complication related to bone cement use during reconstructive osteoplasty using the Masquelet technique in a pediatric patient: A case report.

A 16-year-old male (ASA II, BMI 31) underwent reconstructive osteoplasty with insertion of a polymethylmethacrylate (PMMA) spacer. Shortly after cement application, he developed sudden hypotension, tachycardia, and decreased end-tidal CO₂, consistent with Grade II BCIS. Supportive management with fluids, ketamine, and norepinephrine stabilized the patient. Postoperatively, he experienced recurrent hypotension, acute kidney injury, and elevated liver enzymes and creatine kinase. Intensive care treatment resulted in recovery, and he was discharged from the ICU on day five. Bone cement implantati

Case Report: SMART syndrome mimicking Todd's paralysis after meningioma irradiation.

Stroke-like Migraine Attacks after Radiation Therapy (SMART) syndrome is a rare, delayed complication of cerebral irradiation characterized by stroke-like symptoms, seizures, and/or headaches, with mean latency period of 11.2 years. It is most associated with gliomas and medulloblastomas and diagnosis is challenging due to symptom overlap with recurrent neurologic deficits. We present a 59-year-old male with history of right frontal convexity anaplastic meningioma complicated by seizure disorder and Todd's paralysis, status post tumor resection, adjuvant radiation to 6000cGy, craniotomy and cr

Longitudinal EEG, Seizure, and Developmental Patterns in Children With CDKL5 Deficiency Disorder in the First 2 Years of Life.

BACKGROUND AND OBJECTIVES: Although there is near-universal early-onset epileptic encephalopathy in cyclin-dependent kinase-like 5 (CDKL5) deficiency disorder (CDD), it has been observed clinically that EEGs are initially only mildly abnormal and worsen in the first 2 years of life. This study aimed to identify specific EEG patterns, ictal and interictal, in individuals with CDD during the first 2 years of life and to explore their relationship with clinical and developmental outcomes. METHODS: This multicenter study included 48 infants and young children with CDD (75% female) aged 0-24 month

Emergency endovascular management of oropharyngeal tumor hemorrhage via aberrant vertebral collateral pathways: illustrative case.

BACKGROUND: Acute oropharyngeal hemorrhage in advanced head and neck squamous cell carcinoma (SCC) is a life-threatening oncological emergency. Concomitant cerebrovascular occlusive disease can severely complicate endovascular access. OBSERVATIONS: The authors report the case of a 66-year-old male with a cT4N1M0 p16-positive SCC of the right base of the tongue who presented with massive oropharyngeal hemorrhage during concurrent chemoradiotherapy. Angiography revealed a chronic right common carotid artery occlusion. The right external carotid artery (ECA) was reconstituted via a large hypertr

Diagnosis of sleep-disordered breathing using few-shot learning.

Sleep-Disordered Breathing (SDB) is a common and clinically significant disorder characterized by recurrent airflow limitation and oxygen desaturation during sleep, which can lead to serious cardiovascular and metabolic complications. Accurate and early diagnosis of SDB is crucial for timely clinical intervention and risk stratification, yet diagnostic results are often influenced by substantial variations in physicians' clinical experience and diagnostic skills across different regions, particularly in large-scale screening and real-world medical settings. However, existing diagnostic methods

Retrospective Cohort Study of Peripheral Blood miR-486-3p Levels in Patients with Acute Cerebral Infarction and Obstructive Sleep Apnea Syndrome.

Acute cerebral infarction (ACI) complicated with obstructive sleep apnea syndrome (OSAS) confers a high risk of stroke recurrence and death, yet effective biomarkers for this comorbid condition remain limited. This retrospective cohort study enrolled 270 patients with ACI between November 2020 and December 2023, who were classified into the ACI group (n = 100, apnea‑hypopnea index [AHI] < 5) and the ACI with OSAS group (n = 170, AHI ≥ 5), with the latter further stratified into mild (5 ≤ AHI < 15, n = 60), moderate (15 ≤ AHI < 30, n = 75), and severe (AHI ≥ 30, n = 35) subgroups. Serum miR‑486

Stroke-heart syndrome outcomes in pre-existing vs. new-onset dementia: a report from a global federated research network.

The prognostic significance of dementia onset in patients with stroke-heart syndrome (SHS) remains uncertain. We compared clinical outcomes between patients with pre-existing and new-onset dementia after ischemic stroke complicated by SHS. This retrospective cohort study used the TriNetX Global Collaborative Network, comprising de-identified electronic health records from more than 130 healthcare organizations between 2000 and 2024. Adults with ischemic stroke and subsequent SHS, defined as acute cardiac complications within 30 days after stroke, were identified and categorized as having pre-e

Suspected shrinking lung syndrome: a rare pulmonary manifestation of systemic lupus erythematosus-a case-based review of an underrecognized condition.

Shrinking lung syndrome (SLS) is a rare pulmonary manifestation of systemic lupus erythematosus (SLE). It is characterized by unexplained dyspnea, restrictive ventilatory defect, and diaphragmatic elevation without interstitial lung disease (ILD). A 32-year-old man with SLE presented with progressive dyspnea initially attributed to pleural and infectious processes. As systemic features evolved, SLE was confirmed. Pulmonary function tests (PFTs) showed severe restriction (forced vital capacity-FVC-28%, total lung capacity-TLC-37%), with elevated hemidiaphragm and no alternative pulmonary pathol

KILT syndrome revealed by right iliac fossa pain in an 11-year-old boy: A case report.

Kidney and Inferior Vena Cava abnormalities with Leg Thrombosis syndrome, an acronym for kidney and inferior vena cava abnormalities with leg thrombosis, is a rare congenital condition that predisposes young patients to deep venous thrombosis. We report on the case of an 11-year-old boy with a recent diagnosis of pulmonary tuberculosis who presented with right iliac fossa pain and inability to walk because of a painful flexed right lower limb. Initial ultrasound was inconclusive for appendicitis because the appendix was not visualized due to bowel gas; it showed no peritoneal fluid or intraper

TARM1 may contribute to acute respiratory distress syndrome progression via regulating M1 macrophage polarization.

BACKGROUND: The underlying pathophysiology of acute respiratory distress syndrome (ARDS), a potentially fatal condition, remains poorly understood. In this work, we screened important genes associated with ARDS and M1 macrophage polarization using publicly available bioinformatics datasets and experimental validation in an attempt to identify potential candidate genes for further investigation in ARDS. METHODS: Bulk transcriptomic data from three independent murine ARDS models were analyzed to identify common differentially expressed genes. Single-cell RNA sequencing data from ARDS lungs were

Content validity of the Familial Chylomicronemia Syndrome Symptoms and Impacts Scale (FCS-SIS) for severe hypertriglyceridemia (sHTG).

OBJECTIVE: Severe hypertriglyceridemia (sHTG) and familial chylomicronemia syndrome (FCS) are metabolic conditions associated with an increased risk of adverse health outcomes. The FCS Symptoms and Impacts Scale (FCS-SIS) was developed to assess the symptoms and impacts of FCS. This study assessed the content validity of the 4 FCS-SIS symptom items in adults with sHTG. METHODS: Two iterative rounds of qualitative interviews were conducted with adults (aged ≥ 18 years) who had clinician-confirmed sHTG (triglycerides ≥ 500 mg/dL) and recently experienced ≥ 2 symptoms of sHTG. Participants descr

Three FBN1 pathogenic variants in Chinese patients with Marfan syndrome: Two novel variants including a splicing-altering deletion with functional verification.

BACKGROUND: Marfan syndrome (MFS) is an autosomal dominant connective tissue disorder. The clinical manifestations are pleiotropic, predominantly affecting the cardiovascular system (atrioventricular valve prolapse and regurgitation and aortic complications), the skeletal system (disproportionate elongation of long bones), and the ocular system (ectopia lentis and myopia). Pathogenic variants in the fibrillin-1 gene (FBN1) are the primary genetic cause of MFS. METHODS: This study enrolled three Han-Chinese patients from three unrelated families, all of whom underwent comprehensive clinical as

Acute Necrotizing Pancreatitis Secondary to Adhesive Afferent Loop Syndrome Six Years After Total Gastrectomy With Roux-en-Y Reconstruction: A Case Report.

Acute pancreatitis is an uncommon presentation of afferent loop syndrome, and progression to necrotizing pancreatitis is rarely described in the literature. A 65-year-old man presented with severe epigastric pain, nausea, and bilious vomiting six years after total gastrectomy with Roux-en-Y reconstruction for gastric adenocarcinoma. Laboratory tests showed marked elevations of amylase and lipase. Initial contrast-enhanced computed tomography (CT) demonstrated afferent loop obstruction with focal pancreatic hypoenhancement. Repeat CT performed approximately 72 hours after symptom onset confirme

The impact of supernormal lung function on mortality risk in adults with and without sleep-disordered breathing.

BACKGROUND: In the general population, supernormal lung function is associated with a lower risk of all-cause mortality. RESEARCH QUESTION: It remains unclear whether sleep-disordered breathing (SDB) affects this relationship. METHODS: This cohort analysis included 4,839 adults. Lung function was categorised as supernormal (FEV1 > ULN), normal (LLN ≤ FEV1 ≤ ULN), and below normal (FEV1 < LLN). SDB severity was classified using apnoea-hypopnoea index categories: no SDB (<5 events/hour), mild SDB (5-<15 events/hour), moderate SDB (15-<30 events/hour), and severe SDB (≥30 events/hour). The asso

Trained immunity in autoinflammatory diseases: Cellular reprogramming across the monogenic-polygenic spectrum.

Trained immunity, an innate immunological memory induced by epigenetic and metabolic reprogramming, has changed the paradigm of host defense and pathogenesis of chronic inflammatory disease. Unlike adaptive immunological memory, trained immunity is characterized by the ability of innate immune cells and their progenitors to respond more robustly or differently to subsequent stimulations and contributes to chronic inflammatory conditions. Emerging data suggests that this process might be essential in autoinflammatory and immune-mediated inflammatory illnesses by enhancing sterile inflammation,
