Gastrointestinal involvement in systemic sclerosis: A spotlight on small bowel involvement, complications and management.

Small-bowel involvement in systemic sclerosis is frequent but often underdiagnosed, largely due to the lack of standardized diagnostic protocols for its evaluation. Small bowel dysmotility manifests with symptoms such as abdominal pain, diarrhea, bloating, and distension, and complications including small intestinal bacterial overgrowth, reduced oral intake, malnutrition, and, in severe cases, intestinal pseudo-obstruction. These complications are associated with significant morbidity, dependence on parenteral nutrition, and poor prognosis. In this review, we summarize the current evidence on

Optimizing clinical nutrition practices: impact of a formulary prescription-based nutrition therapy protocol in surgical patients.

BACKGROUND: Parenteral nutrition (PN) is a crucial clinical therapy, particularly in surgical patients. However, due to the complex composition of PN formulations, the inappropriate use of PN remains widespread in clinical practice to date. This study established a formulary prescription-based nutrition therapy protocol and assessed the effectiveness of its implementation. METHOD: The formulary prescription-based nutrition therapy program included the development of formulary prescriptions for PN and the establishment of a perioperative nutrition medication pathway based on the formulary pres

Spatulated umbilical cord technique embracing Wharton's jelly for gastroschisis repair.

The purpose was to prospectively analyze outcomes of gastroschisis closure using the Spatulated Umbilical Cord technique (SUCT). In SUCT, the umbilical cord is split open and used to cover the abdominal defect. The procedure involved opening the amnion layer from the base, exposing Wharton’s jelly and ‘spatulating’ using longitudinal incisions and securing it as a patch over the defect. A pilot interventional study was conducted prospectively at 3 centres from 2016 to 2023 with follow-up till 2024. The inclusion criteria included uncomplicated gastroschisis, where primary fascial closure was a

Parenteral Nutrition Management from the Clinical Pharmacy Perspective: Insights and Recommendations from the Saudi Society of Clinical Pharmacy.

Parenteral nutrition (PN) is essential for patients who are unable to tolerate oral or enteral feeding, providing them with necessary nutrients intravenously, including dextrose, amino acids, electrolytes, vitamins, trace elements, and lipid emulsions. Clinical pharmacists (CPs) play a critical role in PN management by ensuring proper formulation, monitoring therapy, preventing complications, and optimizing patient outcomes. In Saudi Arabia, limited literature exists on CPs' involvement in total parenteral nutrition (TPN) administration, health information management (HIM) systems, and pharmac

Management of Complex Gallstone Disease With Bouveret's and Mirizzi Syndrome: A Case Report With Literature Review.

Gallstone disease affects up to 20% of adults, but rare complications include Bouveret's syndrome, occurring in approximately 0.05% of patients with gallstones, and Mirizzi syndrome, with Type Va involving concurrent biliary and enteric fistulae. We report the case of a 64-year-old man with concurrent Bouveret's syndrome and Csendes Type Va Mirizzi syndrome associated with a cholecystoduodenal fistula. An initial laparoscopic cholecystectomy was abandoned due to concern for malignancy. During the same admission, the patient developed gastric outlet obstruction, and imaging confirmed Bouveret's

Appropriate selection of vascular access devices in the hospitalized patient: a prospective observational pilot study.

INTRODUCTION: Vascular access devices (VADs) play a critical role in clinical practice, facilitating the administration of medications, fluid therapy, and parenteral nutrition. Nevertheless, improper selection of VADs can result in various complications, escalate healthcare costs, and significantly impact patients' experience. This study seeks to evaluate the appropriateness of VAD selection in contemporary clinical practice. METHODS: A prospective observational pilot study was conducted between September 1st and December 31st, 2023, in two medical wards at Careggi University Hospital, Floren

Pilot trial: 8.4% sodium bicarbonate locks for infection prevention in pediatric intestinal failure.

BACKGROUND: Pediatric patients with intestinal failure (IF) require long-term central vascular catheters (CVCs) for parenteral nutrition, placing them at high risk for catheter-related bloodstream infections (CRBSIs). Sodium bicarbonate (NaHCO₃) locks have shown potential in reducing CRBSIs in adult populations, but data in pediatric patients with IF remain limited. METHODS: We conducted an open-label prospective cohort study at a tertiary medical center to evaluate the impact of 8.4% NaHCO₃ locks on CRBSI and CVC replacement rates in pediatric patients with IF (ClinicalTrials.gov NCT05927484

An Audit of the Use of Total Parenteral Nutrition: Are We Using It Correctly?

Background Total parenteral nutrition (TPN) provides essential nutritional support when the gastrointestinal (GI) tract is non-functional or inaccessible. Its use should be limited to situations where enteral feeding is not possible or contraindicated. Enteral nutrition remains the preferred option whenever feasible. Clinicians should ensure an informed discussion regarding the relative risks and benefits of parenteral versus enteral nutrition. Objectives To evaluate adherence to local and national guidelines in the use of TPN, including frequency of emergency TPN prescriptions, duration of th

Effectiveness of olanzapine for hyperemesis gravidarum in individuals without a psychiatric history: a case report.

BACKGROUND: Hyperemesis gravidarum (HG) is characterized by severe nausea and vomiting during pregnancy, and can markedly impair quality of life as well as cause maternal and fetal complications. Although a stepwise treatment strategy with antiemetics and nutritional support is commonly used, some cases remain refractory. Olanzapine, an atypical antipsychotic, has shown potential benefit in HG, although most reports involve patients with psychiatric comorbidities. We describe a patient with persistent HG in late gestation without psychiatric history, in whom adjunctive olanzapine was temporall

Superior mesenteric artery syndrome in a 26-year-old male presenting as acute pancreatitis and portal venous gas: a case report and review of the literature.

BACKGROUND: Superior mesenteric artery syndrome is a rare form of duodenal obstruction that usually requires nonoperative management. While individual complications such as acute pancreatitis, portal venous gas, and gastric emphysema have been reported, the simultaneous occurrence of this triad in a single patient with superior mesenteric artery syndrome has never been documented. This case is novel as it highlights the successful nonoperative management of superior mesenteric artery syndrome despite the presence of these severe, potentially life-threatening complications. CASE PRESENTATION:

Comparison of catheter-related bloodstream infection between peripherally inserted central catheters and tunneled central venous catheters in patients receiving home parenteral nutrition: a meta-analysis.

BACKGROUND: Catheter-related bloodstream infection (CRBSI) remains one of the most severe complications in patients receiving home parenteral nutrition. Tunneled central venous catheters (CVCs) and peripherally inserted central catheters (PICCs) are the most commonly used devices for home parenteral nutrition. However, the relative risks of CRBSI from these devices remain controversial. This meta-analysis aimed to compare the incidence of CRBSI between PICCs and tunneled CVCs in patients receiving home parenteral nutrition. METHODS: A systematic search of PubMed, Embase, and the Cochrane Libr

Artificial intelligence-based analysis of thrombotic and infectious risk factors in peripherally inserted central catheters (PICCs).

INTRODUCTION: Peripherally inserted central catheters (PICCs) are increasingly used in France for prolonged intravenous therapies such as chemotherapy, parenteral nutrition, or antibiotics. They are easier to place than traditional central venous catheters and carry fewer immediate risks, but remain associated with delayed complications, mainly infections, thromboembolic events, and mechanical issues. METHODS: This literature review aimed to identify risk factors for PICC-related infections and thrombosis. The study used BIBOT, an artificial intelligence program for natural language processin
