Contemporary Outcomes Among Gynecologic Oncology Patients With Malignant Small Bowel Obstructions.

Objective: Malignant bowel obstructions (MBOs) are common sequelae of advanced gynecologic malignancies, accompanied by significant morbidity and mortality. Little evidence exists to guide optimal management. We aimed to describe contemporary outcomes for gynecologic oncology patients with MBOs to inform prospective management strategies. Methods: We conducted a retrospective chart review of gynecologic oncology admissions for malignant small bowel obstructions at a single academic institution from 1/2019 to 12/2023. Variables included cancer type, treatment phase, management interventions, go

A case of gastrointestinal tuberculosis in a 3-mo-old infant with profound immunodeficiency.

Gastrointestinal tract tuberculosis (GIT-TB) is a rare form of TB, but one that poses a serious challenge in diagnosis and management, particularly in the setting of severe immunosuppression. We present a case of intestinal TB in a 3-mo-old immunosuppressed infant whose chief complaints were progressive abdominal distension and failure to thrive with features of intestinal obstruction and a diagnosis of GIT-TB confirmed on histology and culture. The resulting serious complications despite surgery included short bowel syndrome, enterocutaneous fistulae, and cholestasis necessitating the use of

Antioxidant Supplementation Reduces In Vitro Oxidant Generation in Neonatal Total Parenteral Nutrition Solutions.

BACKGROUND: Newborns with gastrointestinal anomalies or premature birth often require lifesaving nutritional support in the form of total parenteral nutrition (TPN). However, TPN administration causes several complications in newborns. Several studies have suggested that it could be due to oxidant generation in the TPN admixture because of various factors. OBJECTIVES: This study aimed to reduce the peroxidation of the commercially available TPN for newborns by increasing concentrations of antioxidants, decreasing concentrations of prooxidants, and optimizing environmental conditions, leading

Successful treatment with plasmapheresis of severe hypertriglyceridemic acute pancreatitis complicated by fat overload syndrome caused by lipid injectable emulsion: a case report and review of the literature.

BACKGROUND: Hypertriglyceridemic acute pancreatitis (HTG-AP) and fat overload syndrome caused by lipid injectable emulsion (ILE) are rare in clinical practice. Although the treatment of HTG-AP with plasmapheresis has previously been reported, only a few reports describe the application of plasmapheresis to treat HTG-AP and fat overload syndrome caused by ILE. CASE PRESENTATION: We report a case diagnosed with severe HTG-AP and fat overload syndrome, based on laboratory findings and abdominal computed tomography, caused by intravenous infusion of ILE for about 3 weeks without monitoring of ser

Supporting Weight Management for Kidney Transplant Candidates in People With Obesity on Haemodialysis: The First National Survey of UK Kidney Healthcare Providers.

AIMS: Access to kidney transplantation (KTx) for people living with obesity and kidney failure is often challenging, especially given the inconsistent availability of publicly funded obesity services across the United Kingdom. This study explored UK kidney healthcare providers' views on weight management support for patients with obesity on haemodialysis before KTx, criteria for transplant waitlisting and the potential presence of weight stigma in clinical settings. MATERIALS AND METHODS: An online national UK cross-sectional survey was conducted (July-December 2024), using purposive and snow

Gut or Vein? Enteral Nutrition Versus Total Parenteral Nutrition in Acute Pancreatitis: A Systematic Review.

Acute pancreatitis is a common inflammatory condition with significant morbidity and mortality, particularly in patients who progress to moderately severe or severe disease. Nutritional support is an essential aspect of management, although the optimal route of nutrition remains debated. This systematic review assessed whether enteral nutrition (EN), compared with total parenteral nutrition (TPN), is associated with improved clinical outcomes in patients with acute pancreatitis. A thorough literature search was conducted across multiple electronic databases in accordance with Preferred Reporti

Cost-effectiveness of Taurolidine as secondary prevention of recurrent catheter-related bloodstream infections in patients with chronic intestinal failure: Findings from a randomized controlled trial.

BACKGROUND & AIMS: Catheter-related bloodstream infections (CRBSIs) are a major complication in patients with chronic intestinal failure (CIF) dependent on home parenteral support (HPS), contributing substantially to morbidity, mortality, and costs. Taurolidine-containing catheter lock solutions reduce CRBSIs, but data on cost-effectiveness remain limited. METHODS: This cost-effectiveness analysis used data from a double-blind, randomized controlled trial (2019-2022; ClinicalTrials.gov: NCT06660641) comparing 1.35% taurolidine with 0.9% saline as catheter lock solutions for secondary preventi

The Role of Anti-Inflammatory Nutrition in Facial Cosmetic Surgery Outcomes: A Scoping Review.

INTRODUCTION: Prolonged inflammatory responses are associated with impaired wound healing, increased surgical complications, and poor health outcomes. Additionally, anti-inflammatory diet and nutrition have been shown to reduce harmful inflammatory markers in the body. However, the relationship between anti-inflammatory diet and surgical outcomes remains poorly understood in cosmetic surgery populations. OBJECTIVES AND HYPOTHESES: This scoping review aims to evaluate the quality and quantity of the current literature regarding the impact of the anti-inflammatory diet on facial cosmetic surger

Parenteral nutrition and supplemental parenteral nutrition: should we use more?

PURPOSE OF REVIEW: This review examines when parenteral nutrition (PN) (inclusive of total (TPN), supplemental (SPN), or peripheral (PPN)) may be provided in intensive care unit (ICU). We discuss adapting PN across metabolic phases, including early and late acute phases in ICU, post-ICU, and ward recovery. RECENT FINDINGS: Given emerging data, it is more likely that historical negative sequelae associated with PN were driven by overfeeding than the feeding route itself. With conservative energy delivery and strict aseptic care, TPN/SPN is likely as safe as enteral nutrition (EN) in modern ICU

Patterns of parenteral nutrition use in patients with respiratory disease: indications, timing, and metabolic complications in a single-center study.

INTRODUCTION: Patients hospitalized with respiratory diseases are at high risk of nutritional deterioration due to increased metabolic demands and systemic inflammation. When enteral nutrition is not feasible, parenteral nutrition (PN) becomes necessary; however, evidence in respiratory referral centers remains limited. This study aimed to characterize the indications, timing, and clinical outcomes of PN, with emphasis on metabolic complications. METHODS: A retrospective analysis of prospectively collected data was conducted in a tertiary respiratory referral hospital between September 2023 a

Emphysematous gastritis: a systematic review of reported cases exploring predictors of mortality.

BACKGROUND: Emphysematous gastritis (EG) is a rare but life-threatening condition caused by gas-forming bacterial infection of the gastric wall. The rarity of this disease has resulted in limited consolidated data, making it essential to review reported cases to identify clinical predictors of mortality. This systematic review synthesizes current evidence on EG, focusing on epidemiology, clinical presentation, diagnostic modalities, treatment strategies, and factors associated with mortality. METHODS: A systematic literature search was conducted in PubMed, Embase, and Scopus from inception to

Management of congenital duodenal obstruction in neonates: a 10-year retrospective cohort study from a National Children's Hospital.

OBJECTIVE: Operative management is the definitive treatment for congenital duodenal obstruction (CDO). Currently, there is a paucity of large-scale comparative studies on laparoscopic procedure (LP) versus open procedure (OP). This study aims to present a 10-year, single-center experience in the management of CDO in neonates from a National Children's Hospital in China. METHODS: A single-center, retrospective cohort study with a comparative design was conducted. Neonates who underwent surgical intervention for CDO between January 2016 and July 2025 were included. Diagnosis and treatment of multiple postoperative fistulas following resection of a giant abdominal mesenteric fibromatosis: a case report and literature review.

Mesenteric fibromatosis (MF) is a rare, locally aggressive, borderline tumor with no metastatic potential, characterized by a high postoperative recurrence rate. This paper reports a complex case of aggressive MF. Despite receiving targeted therapy that was combined with immunotherapy, the tumor in our case progressed rapidly and symptoms of intestinal obstruction developed. Following a challenging radical resection, delayed and complex multiple complications ensued, including duodenal fistula, vaginal fistula, and ureterorectal fistula. Through personalized management under multidisciplinary

The updated recommendations for medical nutrition in the perioperative period: a focus on early oral intake.

PURPOSE OF REVIEW: Early oral feeding after gastrointestinal surgery is safe, promotes recovery, preserves gut integrity and aligns with enhanced recovery after surgery (ERAS) pathways. However, individualization and adaption of the pathway may be necessary postoperatively particularly after upper gastrointestinal procedures, where energy and protein requirements often remain unmet. The 2025 ESPEN guideline update advocates early oral/tube feeding while addressing supplementation strategies. The purpose of this review is to discuss the updated recommendations regarding the very recent literatu

Enterocutaneous Fistulas: Current Management.

Background: Enterocutaneous fistulas (ECFs) and enteroatmospheric fistulas (EAFs) are rare but highly morbid complications that most commonly arise after abdominal surgery. Outcomes have improved with advances in multidisciplinary care and with increasing research on how to best manage them; however, they remain associated with significant morbidity, high mortality, and prolonged hospitalization. Optimal timing of definitive repair is unknown, with many high-volume centers waiting 6-12 months, though emerging data suggest that earlier intervention may be feasible in carefully selected patients

Acute Esophageal Necrosis Secondary to Cardiogenic Shock: A Case Report and Review of the Literature.

Acute esophageal necrosis (AEN), also known as "black esophagus," is an uncommon clinical entity characterized by diffuse, circumferential, dark pigmentation of the esophageal mucosa. Its pathogenesis is explained by a "two-hit" mechanism involving systemic hypoperfusion and local chemical injury from gastric acid. We present the case of a 78-year-old man with a history of hypertension, diabetes, and a recent anteroseptal myocardial infarction complicated by cardiogenic shock, requiring triple stent placement and vasopressor support. One week after discharge, the patient was readmitted due to

Association of Early Supplemental Parenteral Nutrition with Clinical Outcomes in Patients with Traumatic Brain Injury.

OBJECTIVE: Early oral or enteral nutrition (EN) is recommended for critically ill patients; however, individuals with traumatic brain injury (TBI) often fail to meet their nutritional goals. Supplementary parenteral nutrition (SPN) in this instance may be of benefit, but evidence is lacking. This study examined the association between early SPN and nutritional adequacy and clinical outcomes in patients with TBI. METHODS: This retrospective, non-randomized, single-center cohort study analyzed adult patients with TBI admitted to a trauma intensive care unit (ICU) from January 2023 to December 2
